Chromogenic In Situ Hybridisation Test for Breast Cancer Patients with Equivocal IHC Results - a Study from Iran

  • Background: HER2/neu overexpression on cell membranes of breast cancer cells is due to HER2/neu gene amplification and it is important to identify potential candidates for anti HER2 therapy with trastuzumab. IHC, FISH and CISH are standard FDA approved assays currently used to determine HER2 status in routine practice. The aim of this study was to determine HER2 gene amplification, using the CISH method in breast carcinoma samples which had IHC +2 reactions. Materials and Methods: This study was conducted from 2008-2010 using 334 consecutive breast carcinoma samples referred from local laboratories to Mehr Hospital. CISH assays were performed for all cases, and IHC tests were also done for determining efficacy and accuracy of local labs. HER2 status in local IHC tests was compared with central IHC and CISH results. Results: Of 334 breast cancer patients, 16 were negative for HER2 IHC (0, +1), 201 cases were equivocal (+2), and 31 positive (+3). Of 334 referral cases, 88 were CISH positive (26.3%) and 246 were CISH negative (73.7%). Of 201 IHC +2 cases, HER2 gene amplification was observed in 42 cases (kappa: 0.42). A 29.9% concordance was found between local IHC and central IHC. Sensitivity and specificity of local IHC were 90% and 53.8%, respectively. Conclusions: Low accuracy of IHC results in local labs was associated with the following factors: using former FDA-approved criteria for HER2 interpretation, utilizing non-validated kits, and lack of any quality assurance program. Therefore, following the new 2014 ASCO/CAP guideline and comprehensive quality assurance should be implemented to ensure accuracy of HER2 testing.

유체기계에 있어서 부의 음원 임피던스의 원인에 관한 고찰 (A Review of the Possible Causes of Negative Source Impedance in Fluid Machines)

  • 대부분의 유체기계는 일정 조건하에서는 주기적인 소음 원들로 간주될 수 있으며, 소음원 및 관련 덕트의 음향계를 주파수영역으로 표현이 가능하게 되는데, 음원은 주파수의 함수인 음원강도와 음원 임피던스로 표현된다. 이들 변수를 알아내기 위하여는 다양한 이론적, 실험적 방법이 있으나, 고속, 고온, 고강도의 유체 기계와 같은 단일 연결구를 갖는 음원에 대하여는 다양한 길이의 많은 관들을 부하로 사용하는 다부하법만이 적용 가능하다. 문제는 이 다부하법을 적용했을 때, 음원 임피던스의 실수부가 부의 값을 갖는 경우가 많다는 점이다. 본 논문에서는 이 문제의 명확한 원인분석을 위한 일환으로서, 다양한 이론적 실험적 결과에 대한 분석을 바탕으로 하여 가능한 여러 가지 원인에 대한 물리적 조사 결과를 보이고, 분석 결과에 대한 새로운 해석을 통해 기본 가정의 위배와 함께 다부하법 자체의 문제점을 제시한다 유체 기계 덕트에서는 다른 무엇보다도 음원의 시변성이 부의 음원저항을 낳는데 가장 큰 영향을 미치게 되며, 부하 임피던스가 최종 결과에 지대한 영향을 미치게 된다.

GENOTYPIC AND PHENOTYPIC CORRELATIONS IN A SOYBEAN CROSS

  • In a plant breeding program, an efficient selection of desired characters in a population is important. Generally, many agronomic characters in a given population are determined by polygenes and quantitatively inherited. In practice, the genetic relationship between two observed characters which are undoubtedly subjected to the environmental influence is difficult to identify. In recent years, many workers have attempted to understant the genetic relationship between characters in terms of genotypic correlation, and the knowledge thus gained should furnish many important and useful information for the planning of breeding, selection, and interpretation of the result. The genotypic correlation is the result of pleiotropy, linkage of genes(2, 3, 5, 6, 8) and natural or artificial selection(4). The purposes of this study were to estimate genotyric and phenotypic correlations between all possible pairs of nine characters. and to seek certain characters which may be useful as indicators of certain important agronomic characters. Weber and Moorthy(10), Johnson et al. (5) and Sheth(7) found that in general, the genotypic correlations were higher than the phenotypic correlations. Weiss et al. (11) obtained significant positive correlations between maturity and oil content, maturity and low protein content, and high protein content and low oil content. Weber and Moorthy(10) reported the positive genotypic correlations between flowering and maturity, yield and maturity, yield and plant height, yield and seed weight, and negative genotypic correlations between maturity and oil content, and oil content and seed weight. Johnson et al. (5) studied the genotypic and phenotypic correlations among 24 characters and concluded that selection based entirely on a long fruiting period, lateness, heavy seed, low protein, high oil and resistance to lodging would be effective in increasing yield. Sheth(7) found the following positive associations among characters; height and maturity, yield and lodging, low protein content and high oil content, and yield and low protein content. Hanson et al.(1) also reported high negative correlation between seed yield and protein content.

마이크로웨이브 플라즈마 CVD에 의한 나노결정질 다이아몬드 박막 성장 시 DC 바이어스 효과 (Effect of DC Bias on the Growth of Nanocrystalline Diamond Films by Microwave Plasma CVD)

  • The effect of DC bias on the growth of nanocrystalline diamond films on silicon substrate by microwave plasma chemical vapor deposition has been studied varying the substrate temperature (400, 500, 600, and $700^{\circ}C$), deposition time (0.5, 1, and 2h), and bias voltage (-50, -100, -150, and -200 V) at the microwave power of 1.2 kW, working pressure of 110 torr, and gas ratio of Ar/1%$CH_4$. In the case of low negative bias voltages (-50 and -100 V), the diamond particles were observed to grow to thin film slower than the case without bias. Applying the moderate DC bias is believed to induce the bombardment of energetic carbon and argon ions on the substrate to result in etching the surfaces of growing diamond particles or film. In the case of higher negative voltages (-150 and -200 V), the growth rate of diamond film increased with the increasing DC bias. Applying the higher DC bias increased the number of nucleation sites, and, subsequently, enhanced the film growth rate. Under the -150 V bias, the height (h) of diamond films exhibited an $h=k{\sqrt{t}}$ relationship with deposition time (t), where the growth rate constant (k) showed an Arrhenius relationship with the activation energy of 7.19 kcal/mol. The rate determining step is believed to be the surface diffusion of activated carbon species, but the more subtle theoretical treatment is required for the more precise interpretation.

두 층 관측 기상인자의 주성분-다중회귀분석으로 도출되는 고농도 미세먼지의 부산-서울 지역차이 해석 (Interpretation and Comparison of High PM2.5 Characteristics in Seoul and Busan based on the PCA/MLR Statistics from Two Level Meteorological Observations)

  • In this study, two-step statistical approach including Principal Component Analysis (PCA) and Multiple Linear Regression (MLR) was employed, and main meteorological factors explaining the high-PM2.5 episodes were identified in two regions: Seoul and Busan. We first performed PCA to isolate the Principal Component (PC) that is linear combination of the meteorological variables observed at two levels: surface and 850 hPa level. The employed variables at surface are: temperature (T2m), wind speed, sea level pressure, south-north and west-east wind component and those at 850 hPa upper level variables are: south-north (v850) and west-east (u850) wind component and vertical stability. Secondly we carried out MLR analysis and verified the relationships between PM2.5 daily mean concentration and meteorological PCs. Our two-step statistical approach revealed that in Seoul, dominant factors for influencing the high PM2.5 days are mainly composed of upper wind characteristics in winter including positive u850 and negative v850, indicating that continental (or Siberian) anticyclone had a strong influence. In Busan, however, the dominant factors in explanaining in high PM2.5 concentrations were associated with high T2m and negative u850 in summer. This is suggesting that marine anticyclone had a considerable effect on Busan's high PM2.5 with high temperature which is relevant to the vigorous photochemical secondary generation. Our results of both differences and similarities between two regions derived from only statistical approaches imply the high-PM2.5 episodes in Korea show their own unique characteristics and seasonality which are mostly explainable by two layer (surface and upper) mesoscale meteorological variables.

Assessment of a Deep Learning Algorithm for the Detection of Rib Fractures on Whole-Body Trauma Computed Tomography

  • Objective: To assess the diagnostic performance of a deep learning-based algorithm for automated detection of acute and chronic rib fractures on whole-body trauma CT. Materials and Methods: We retrospectively identified all whole-body trauma CT scans referred from the emergency department of our hospital from January to December 2018 (n = 511). Scans were categorized as positive (n = 159) or negative (n = 352) for rib fractures according to the clinically approved written CT reports, which served as the index test. The bone kernel series (1.5-mm slice thickness) served as an input for a detection prototype algorithm trained to detect both acute and chronic rib fractures based on a deep convolutional neural network. It had previously been trained on an independent sample from eight other institutions (n = 11455). Results: All CTs except one were successfully processed (510/511). The algorithm achieved a sensitivity of 87.4% and specificity of 91.5% on a per-examination level [per CT scan: rib fracture(s): yes/no]. There were 0.16 false-positives per examination (= 81/510). On a per-finding level, there were 587 true-positive findings (sensitivity: 65.7%) and 307 false-negatives. Furthermore, 97 true rib fractures were detected that were not mentioned in the written CT reports. A major factor associated with correct detection was displacement. Conclusion: We found good performance of a deep learning-based prototype algorithm detecting rib fractures on trauma CT on a per-examination level at a low rate of false-positives per case. A potential area for clinical application is its use as a screening tool to avoid false-negative radiology reports.

해석수준과 메시지 프레이밍에 따른 자율주행택시의 사용의도에 관한 연구 (A Study on the Construal Level and Intention of Autonomous Driving Taxi According to Message Framing)

  • 본 연구는 최근 4차산업 혁명의 산물로 대두되고 있는 자율주행차가 대중교통 수단인 택시로 이용될 때 해석수준과 메시지 프레이밍에 따른 사용 의도의 차이를 분석하는 것이다. 해석수준이란 가까운 미래에 발생한 일과 먼 미래에 발생할 일을 가정하여 어떤 제품이나 서비스를 해석하는 것이 다르다는 것을 말한다. 메시지 프레이밍이란 긍정 혹은 부정의 표현 또는 혜택, 손실의 양극단의 메시지를 구성한 것을 말한다. 즉, 기존연구에서는 이 두 개념에 따라 제품이나 서비스의 가치를 다르게 해석한다고 한다. 본 연구는 자율주행차가 택시로 출시될 때 두 개념을 적용할 경우 사용 의도의 차이를 보이는지 살펴보고자 한다. 결과를 요약하면 우선, 메시지 프레이밍 구성에서 자율주행 택시를 이용할 경우 혜택(Gain)과 왜(Why) 사용해야 하는지를 설명한 메시지 형식과 자율주행 택시를 이용하지 않았을 경우 손실(Loss)와 어떻게(How)를 강조한 메시지를 구성하여 비교해 보았다. 두 메시지 프레이밍은 차이를 보였으며(t= 3.063) 혜택(Gain)과 왜(Why)를 설명한 메시지 형태가 더 높은 사용 의도를 보였다. 또한, 해석수준에 따른 결과를 요약하면 다음과 같다. 혜택(Gain) 및 손실(Loss)에 대한 먼 미래와 가까운 미래에 발생할 것을 가정한 경우 사용 의도에 차이가 있었으며, 구체적으로 혜택(Gain)을 설명한 메시지와 먼 미래에 발생할 것을 가정한 경우가 사용 의도가 높았다. 요약하면 자율주행 택시의 사용 의도를 높이기 위해서는 긍정의 메시지(Gain)와 먼 미래에 일어날 수 있는 것을 가정하여 사람들에게 메시지를 전달해야 한다는 결론이다. 또한, 본 연구를 통하여 향후 신기술의 출현에 대한 사용 의도 연구 시 연구방법을 활용할 수 있을 것이다.

도시 이미지 분석 기법에 관한 연구 : MDS(Multidimensional Scaling)에 의한 도시 간 이미지 비교 (A Study on the Analysis Method of City Image : Focusing on the Image Comparison between Cities by MDS)

  • Rapid economic development in Korea caused functions of city functions such as concentration of population, deterioration of the quality of living environment and traffic congestion. Korean cities have lost their identity becausr they are merged functionally and physically with neighboring cities, forming one mesa-city. Unified shape and disorganized streets of cities often cause confusion among foreigners and visitors. It is very difficult for them to find their image in strange cities. It is, however, important to correctly analyze the image and meaning of cities for understanding its identity. The purpose of this study is to develop a method to analyze the city image by focusing on some of the main cities in Korea. For this purpose, the adjective questionnaire and multi-dimension scaling (MDS) are applied to the analysis of city image. Image analysis graph by MDS can visually present the general and integrate images. The results of this study are summarized as follows: The important factors for interpretation of city image are historical and industrial character. Seoul, Taegu and Pusan have industrial and complex city images. Kongju has historical city image, while Changwon has a modern image. Chuncheon belongs to a soft and small image. Each city has an alternative solution against a negative image, according to the image analysis graph.

사회정보처리 구성요소와 정서요인이 유아의 외현적 공격성과 관계적 공격성에 미치는 영향 (The Effects of Components of Social Information Processing and Emotional Factors on Preschoolers' Overt and Relational Aggression)

  • The present study examines the sex differences in 5-year-old preschoolers' aggression according to the type of aggression (overt, relational) and the effect of components of social information processing (SIP : interpretation, goal clarification, response generation, response evaluation) and emotional factors (emotionality, emotional knowledge, emotion regulation) on their aggression. The subjects were 112 5-year-olds (56 boys, 56 girls) and their 11 teachers recruited from 9 day-care centers in Seoul and Kyung-Ki province. Each child's SIP and emotional knowledge were individually assessed with pictorial tasks and teachers reported on children's aggression, emotionality, and emotion regulation by questionnaires. Results indicated that there was a significant sex difference only in the preschoolers' overt aggression. Overtly aggressive response generation in SIP was the strongest predictor of preschoolers' overt aggression while anger of negative emotionality in emotional factors was the strongest predictor of preschoolers' relational aggression.

Identifying Suspended Particulate Matters in an Urban Coastal System: Significance and Application of Particle Size Analysis

  • In situ particle size spectra are obtained from two sequent cruises in order to evaluate the physical consequences of suspended particulate matters caused by episodic storm runoff from the Santa Ana River watershed, an urbanized coastal watershed. Suspended particles from various sources including surface runoff, near-bed resuspension, and phytoplankton are identified in empirical orthogonal function (EOF) analysis and an entropy-based parameterization (Shannon entropy). The first EOF mode is associated with high turbidity and fine particles as indicated by the elevated beam attenuation near the Santa Ana River and Newport Bay outlets, and the second EOF mode explains the suspended sediment dispersal and particle coarsening at the near-surface plume. Chlorophyll particles are also distinguished by negative magnitudes of the first EOF mode, which is supported by the relationship between fluorescence and beam attenuation. The integrated observation between the first EOF mode and the Shannon entropy index accentuates the characteristics of two different structures and/or sources of sediment particles; the near-surface plumes are originated from runoff water outflow, while the near-bottom particles are resuspended due to increased wave heights or mobilizing bottom turbidity currents. In a coastal pollution context, these methods may offer useful means of characterizing particle-associated pollutants for purposes of source tracking and environmental interpretation.
