The purpose of this study was to identify factors related to daily activities of post myocardial infarction patients. Data were collected from Aug. 1 to Aug. 31, 1988 through an interview schedule lasting for about 30~60 minutes by the investigators. Tools for this study were a daily activities scale and a subjective physical symptom scale developed by the researchers, and Zung's self rating depression scale. The subjects were 45 men 18 women post myocardial infarction patients who were receiving follow up care at SNU Hospital. The data were analyzed by percentage, mean t-test ANOVA, the Pearson moment Correlation Coefficient test, and Cronbach's $\alpha$ reliability test. The results were as follows. \circled1 Reliability of the daily activities scale was 0.91 by Cronbach's $\alpha$. In the daily activities scale, items about transfer, exercise, and job related activities were most highly rated as meaningful to the post myocardial infarction patients. \circled2 The average daily activities score of these patients was 3.30 (maximum point ; 4) all scores were high except for exercise and job related activities. \circled3 There was a significant difference in daily activities according to sex ; men had a higher daily activities score than women (T=2.32, p<0.05). \circled4 There was a significant difference in daily activities according to job status. Subjects having a job had a higher daily activities score. \circled5 The lower the depression score, the higher the daily activities score(${\gamma}$=-0.5748, P<0.05). \circled6 The lower the subjective physical symptoms score, the higher the daily activities score(${\gamma}$= -0.6015, p<0.05).
During the period from November 1981 through June 1986, 18 cases of coronary arterial bypass graft were performed at Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital. They consisted of 13 males and 5 females with the mean age of 49 [range: 28-69 years]. History of myocardial infarction was noted in 50% of the patients and cardiomegaly on chest PA in 2 patients with preserved LV function. On resting EKG, except the evidences of old myocardial infarction, the findings of LVH were noted in 7 cases, acute myocardial infarction in 2, diffuse myocardial ischemia in 1, and significant ventricular arrhythmia in 2 cases. The angina by type of presentation is stable in 3 patients, unstable in 15 patients with resting, postinfarction and progressive angina as the criteria of unstability. The patterns of involvement of significant disease were single vessel involvement [5 cases] double vessel involvement [8 cases], and triple vessel involvement [5 cases] including 5 cases of left main coronary arterial diseases. The pattern of coronary arterial disease in individual patient was one or more stenosis of the proximal left coronary arterial system with or without right coronary involvement, in every case. We performed 9 cases of double bypass and 9 cases of triple bypass with great saphenous vein using single anastomosis technique except in 4 cases, One of the 4 cases is our first case, sequential anastomosis between LAD and diagonal was performed due to shortage of the prepared vein graft. In the other 3 cases, our latest experience, we adopted the left internal mammary artery for the left anterior descending coronary revascularization. The distribution of sites of distal anastomosis revealed more striking predilection to LAD, showing our attention on the significance of the revascularization of LAD system. The ischemic time was 35 minutes per graft and mean number of grafts per patient was 2.5. Of the 18 patients, 13 [77.2%] had complete revascularization, and incomplete in 5 cases with the causes of incompleteness as presented. The early results of operation were as followed: surgical death in 2 [11%], perioperative infarction 2 [11%], need of inotropic support 5 [28%], arrhythmia 2 [11%], wound problem, bleeding, and emotional dysfunction. The actuarial anginal free survival during the period of 6 months through 2 years was 85.2% with excellent symptomatic control according to the angina classification of Canadian Cardiovascular Society.
The purpose of this study was to examine the effects of the intensive teaching program on compliance of health behavior for patients with myocardial infarction. The subjects were 47 patients and twenty-three patients were assigned to the experimental group and twenty-four to the control. Data were collected through questionnaire surveys. As for data analyses. $\chi^2$ test, unpaired t-test. Repeated measures ANOVA were adopted using the SAS program. The results were as follows : 1. After 4 weeks 1) The compliance scores of health behavior were significantly higher in the experimental group than in the control group. 2) The experimental group was at a higher compliance score for smoking cessation. dietary health behavior, doing regular exercise, avoiding physical stress and managing mental stress than the control group. 2. After 12 weeks 1) The compliance score of health behavior was significantly higher in the experimental group than in the control group. 2) The experimental group was at a higher compliance score in smoking cessation. dietary health behavior (except abstaining from alcoholic beverages), doing regular exercise. avoiding physical stress and managing mental stress than the control group. The above findings show that the intensive teaching care program was effective In increasing compliance of health behavior in the patients with myocardial infarction.

Background/Aims: Due to limited real-world evidence on the association between time to presentation (T2P) and outcomes following acute myocardial infarction and diabetes (AMI-DM), we investigated the characteristics of patients with AMI-DM and their outcomes based on their T2P. Methods: 4,455 patients with AMI-DM from a Korean nationwide observational cohort (2011-2015) were divided into early and late presenters according to symptom-to-door time. The effects of T2P on three-year all-cause mortality were estimated using inverse probability of treatment weighting (IPTW) and survival analysis. Results: The incidence of all-cause mortality was consistently higher in late presenters than in early presenters (11.4 vs. 17.2%; p < 0.001). In the IPTW-adjusted dataset, the incidence of all-cause mortality was numerically higher in late presenters than in early presenters (9.1 vs. 12.4%; p = 0.072). In the survival analysis, the cumulative incidence of all-cause mortality was significantly higher in late presenters than in early presenters before and after IPTW. In the subgroup with ST-elevation myocardial infarction, late presenters had a higher incidence of cardiac death than early presenters before (4.8 vs. 10.5%; p < 0.001) and after IPTW (4.2 vs. 9.7%; p = 0.034). In the initial glycated hemoglobin (HbA1c)-stratified analysis, these effects were attenuated in patients with HbA1c ≥ 9.0% before (adjusted hazard ratio [HR]: 1.45, 95% confidence interval [CI]: 0.80-2.64) and after IPTW (adjusted HR: 0.82, 95% CI: 0.40-1.67). Conclusions: Late presentation was associated with higher mortality in patients with AMI-DM; therefore, multifaceted and systematic interventions are needed to decrease pre-hospital delays.

Purpose: This study was performed to investigate the clinical significance of reverse redistribution(RR) phenomenon detected on delayed Tc-99m tetrofosmin myocardial single photon emission computed tomography(SPEG) in patients with acute myocardial infarction after revascularization. Materials and Methods: A Tc-99m tetrofrosmin myocardial SPECT was performed in 67 consecutive patients after revascularization for acute myocardial infarction. Myocardial SPECT imaging was performed for early imaging at 40 min and for delayed imaging at 180 min after reinjection at myocardial stress. Regional myocardial uptakes were scored by 4-point scoring in the left ventricular wall divided into 17 segments. Reverse redistribution was defined as an increase of more than 2 point in the activity score on the delayed image. Follow-up myocardial SPECT and coronary angiography(CAG) were performed 9 months later. Results: On myocardial SPECT performed following revascularization, RR was observed in 100 of all 319 segments(31%) and in 43 patients(64%). The abnormalities of perfusion and regional wall motion were more severe in the patients with RR compared to those without RR(p<0.05). On follow-up myocardial SPECT, the myocardial perfusion, regional wall motion, and myocardial thickness were significantly improved in the patients with RR(p<0.05) however, these changes were not significant in those without RR. There was no significant difference between the patients with RR and those without RR in the occurrence of restenosis on CAG. Conclusions: In patients with acute myocardial infarction, the regions showing the RR phenomenon on delayed Tc-99m tetrofosmin SPECT may reflect viable myocardium and indicate recovery of salvaged myocardium.
Purpose: This study was to investigate the relationship among the symptom recognition, health behavior compliance, and the hospital arrival time to identify factors influencing the hospital arrival time in patient with acute myocardial infarction (AMI). Methods: The subjects of this study were 200 patients with AMI in C hospital in D city. Data were analyzed using descriptive statistics, independent t-test, One way ANOVA, Pearson's correlation coefficients, and stepwise multiple liner regression tests. Results: Level of symptom recognition and health behavior compliance was low. The median value of hospital arrival time was 4.48 hours (ST-segment Elevation Ml was 2.43 hours and Non ST-segment Elevation MI was 7.83 hours). Among the studied factors, only symptom recognition had a statistically significant positive correlation with health behavior compliance (r=0.38, p<.001). Factors influencing the hospital arrival time were MI classification, diabetes mellitus (DM) and transport vehicle to the 1st hospital, and they accounted for 13% of the variance for hospital arrival time in AMI patients. Conclusion: To prevent the delay of hospital arrival time in MI patients, a more robust nursing strategic intervention according to MI classification and DM is necessary; further education on the importance of transportation utilization is also mandated.
The purpose of this study was to investigate the effects of cardiac rehabilitation teaching program on knowledge level and compliance of health behavior for the patients with myocardial infarction. Method: The subjects were 47 patients 23 were assigned to the experimental group and 24 were for the control. The cardiac rehabilitation teaching program is a individualized teaching program which was delivered to the experimental group during hospitalization period by present researcher. Data were collected through questionnaire surveys for knowledge level and compliance of health behavior from September 15, 1999 to December 31, 2000. The collected data was analyzed by using the SAS program. Results: 1. With regard to the knowledge scores 1) The total knowledge level in the experimental group was significantly higher than in the control group. 2) As to the knowledge domains, nature of disease, risk factors, diet, medication, exercise, and daily activities were significantly higher in score in the experimental group than in the control group. 2. With regard to the compliance of health behavior 1) The average compliance with good health behavior was significantly higher in the experimental group than in the control group. 2) As to the health behavior domains smoking cessation, diet, stress management, regular exercise, and other measures for lifestyle modification were significantly higher in score in the experimental group than in the control group. 3. The pre-treatment knowledge score was positively correlated to the post-treatment knowledge score and post- treatment knowledge score was positively correlated to the post-treatment compliance of health behaviors. Conclusion: The above findings indicate that the cardiac rehabilitation teaching program for the experimental group was effective in increasing level of knowledge and improvement of compliance with good health behavior of patients with myocardial infarction.
In this study, we developed the severity-adjusted length of stay (LOS) model for acute myocardial infarction patients using data from the hospital discharge survey and proposed management of medical quality and development of policy. The dataset was taken from 2,309 database of the hospital discharge survey from 2004 to 2006. The severity-adjusted LOS model for the acute myocardial infarction (AMI) patients was developed by data mining analysis. From decision making tree model, the main reasons for LOS of AMI patients were CABG and comorbidity. The difference between severity-adjusted LOS from the ensemble model and real LOS was compared and it was confirmed that insurance type and location of hospital were statistically associated with LOS. And to conclude, hospitals should develop the severity-adjusted LOS model for frequent diseases to manage LOS variations efficiently and apply it into the medical information system.

In patients with acute myocardial infarction (AMI), the delay from symptom onset to hospital arrival has a critical effect on morbidity and mortality. This study examined to find out the determinants of the prehospital delay in patients with AMI. The study sample consisted of 597 patients hospitalized with AMI between Jan and Dec 2009. Demographic, medical history, and clinical data were abstracted from the hospital medical records of patients with confirmed AMI, the prehospital delay was categorized as less than or greater than 6 hours. Older age, low socioeconomic status(medical aid), and low use of Emergency medical system were associated with delays in seeking emergency care for Acute myocardial infarction. Education programs to improve patient knowledge of acute coronary syndrome symptoms and promote patient responsiveness with regard to seeking medical care should be used to reduce the prehospital delay time, especially in the low socioeconomic group.

This study aimed to compare the Korean Activity Status Index (KASI) with the cardiopulmonary exercise test (CPET) among patients with acute myocardial infarction. A total of 2,268 patients (85.4% male; mean age, 59.3±10.2 years; range, 23-90 years) diagnosed with acute myocardial infarction were enrolled in the Regional Center Myocardial Infarction Registry between July 2016 and June 2019. The KASI is a tool used to measure functional capacity by asking patients about their ability to perform specific activities and then scoring their responses. In contrast, CPET is the gold standard for assessing the objective functional capacity in patients undergoing cardiac rehabilitation. Peak oxygen uptake (VO2peak) was used to analyze the correlation. Patients who completed two consecutive KASI and CPET evaluations during their first (KASI_1, VO2peak_1) and second visits (KASI_2, VO2peak_2) for cardiac rehabilitation were included in the study. The mean KASI_1 and KASI_2 scores were 43.3±14.3 and 49.8±13.9, respectively, and the mean VO2peak_1 and VO2peak_2 scores were 25.9±8.0 and 28.5±8.3, respectively. Both the KASI scores were significantly correlated with the measured VO2peak during each visit, with correlation coefficients of 0.385 (P<0.001) and 0.346 (P<0.001), respectively. Moreover, the KASI score and VO2peak had a linear relationship (VO2peak_1=0.22×KASI_1+16.5, P<0.001; VO2peak_2=0.21×KASI_2+18.2,VO2peak_2=0.21×KASI_2+18.2, P<0.001). This study revealed that the KASI is a valid measure for the follow-up evaluation of the functional capacity of patients. These findings suggest that VO2peak can be predicted using the KASI score in patients who do not undergo CPET.
