목적 : 본 연구의 목적은 음악을 통한 청각자극이 뇌졸중 환자의 편측무시에 미치는 효과를 알아보는 것이다. 연구방법 : 본 연구는 편측무시를 보이는 뇌졸중 환자 1명을 대상으로 하였다. 본 연구는 ABA 설계를 사용하였으며, 연구기간 동안 기초선A 5회, 중재기B 10회, 기초선A'5회로 총 20회기 실시하였다. 각 중재기 동안에는 대상자가 좋아하는 음악 5곡을 선택하게 하여 회기 당 60분씩 음악을 감상하도록 하였다. 기초선과 중재기간에는 편측무시 평가를 위해 별 지우기 검사(Star cancellation test), 선 나누기 검사(Line bisection test)를 시행하였다. 또한 중재 전, 후로 일상생활에서의 편측무시를 평가하기 위해 Catherine Bergego Scale(CBS)와 정서 상태를 평가하기 위해 Positive Affect and Negative Affect Schedule(PANAS)를 시행하였다. 결과 : 중재기 동안 별 지우기 검사, 선 나누기 검사를 통해 편측무시가 감소되는 추세를 확인하였으며, 중재 후에도 중재효과가 유지되었다. 또한 중재 후, CBS를 통해 일상생활에서도 편측무시가 감소되었음을 알 수 있었으며 PANAS를 통해 긍정적 감정이 증가하였음을 알 수 있었다. 결론 : 본 연구 결과, 음악을 통한 청각자극 뇌졸중 환자의 편측무시 감소에 긍정적인 영향을 준 것으로 확인되었다.
본 연구는 자폐스펙트럼장애(Autism Spectrum Disorder, 이하 ASD) 아동과 비장애 형제를 대상으로 협력적 악기연주 프로그램을 실행하여 형제간 상호작용에 긍정적인 영향을 미치는지를 알아보기 위한 사례연구이다. 대상자는 만 7-12세 ASD 아동 3명과 만 6-11세 비장애 형제 3명으로 ASD 아동과 비장애 형제로 구성된 각 그룹이 40분씩, 주 2회, 총 8회기의 중재에 참여하였다. 중재는 동시적 행동 수행의 단계, 상호보완적 행동 수행의 단계, 협력을 통한 공동의 목표 성취의 단계 총 3단계로 구성되었다. 매회기 중재 시 ASD 아동과 비장애 형제에게 나타난 상호작용 시작·반응행동, 연쇄적 상호작용 행동 및 협력적 연주 행동을 관찰하여 측정하였으며 중재 전후 비장애 형제는 형제관계척도를 보호자는 보호자가 지각한 형제관계척도를 평정하도록 하였다. 연구 결과, 상호작용 시작·반응행동, 연쇄적 상호작용 행동, 협력적 연주 행동이 중재 회기가 진행됨에 따라 점차 증가하는 양상을 보였다. 보호자와 비장애 형제가 평정한 형제관계척도의 평정 점수 또한 프로그램 후 증가하였으나 형제 갈등 및 경쟁의식 하위 영역에서는 뚜렷한 변화가 나타나지 않았는데 이는 형제간 접촉이 증가하면서 갈등과 경쟁의식을 경험하는 기회 역시 증가할 수 있음을 나타낸다. ASD 아동과 비장애 형제가 함께 참여하는 중재 연구의 필요성이 증가하는 시점에서 본 연구 결과는 공동의 목표를 가지고 협력적으로 연주하는 중재가 형제간 상호작용 시도 및 친밀감 증가에 효과적일 수 있음을 시사한다.
Purpose: This study was conducted to examine the effects of music therapy on anxiety, sedation, and stress responses of patients during surgery with spinal anesthesia. Methods: A quasi-experimental design with a non-equivalent control group pre-post test was used. There were 55 adults over 19 years of age scheduled for a lower leg surgery with spinal anesthesia: 27 in the treatment group and 28 in the comparison group. Each subject in the treatment group identified their music preference which was used during the period of surgery which usually lasted, 61 minutes. Data were analyzed using chi-square, Fisher's exact test, independent t-test and repeated measured ANOVA. Results: There were significant reported differences in levels of anxiety, sedation, and stress responses which were measured by plasma cortisol levels, heart rate, and respiratory rate. The findings of all the variables were effectively decreased among those in the treatment group than the comparison group. Conclusion: The preference based music therapy may be useful as a non-pharmacological intervention.
The aim of this article is to argue that rhythm of janggu may have meaningful effects as a form of vitality for the elderly. Janggu is the most popular and soft pitched instrument in the percussion family in Korean music. It is warm and soft makes very deep and exquisite sounds. Janggu is a source of Korean national pride because of unique, manifestation of yin-yang five-elements represented in Korean music. Music therapy has been employed as a therapeutic intervention to facilitate healing across a many of clinical populations. A cheerful rhythm of janggu promotes the vitality of the elderly. This joining has contributed to the vitality, health improvement and the whole body for the elderly. Instead of visiting hospital, they sit in front of the janggu what could be better than a daily dose of exercise. Therefore, the author thinks this might help them forget their anxieties, this very moment, janggu music rhythm therapy lays the foundation of a more vital lifestyle.
Purpose: The purpose of the study was to test the effects of music therapy on postoperative pain in patients with total hip replacement. Method: The research design was a nonequivalent control group pretest-posttest design. The subjects were composed of thirty patients with total hip replacement. Fifteen of them were assigned to the experimental group and fifteen to the control group. Fifteen minutes tailored music therapy was given to the experimental group during five consecutive days. The instruments used for this study were pain NRS(numerical rating scale). The data were analyzed using percent, mean, standard deviation, ${\chi}^2-test$ and repeated measure ANOVA using SPSS WIN 11.0. Results: Hypothesis 1 "The score of pain NRS of experimental group will be lower than those of control group" was accepted(F=15.945, p<.001). Hypothesis 2 "The frequency of PCA analgesics of experimental group will be fewer than those of control group" was accepted (t=-2.312, p=.028). Hypothesis 3 "The vital signs(pulse, systolic BP and diastolic BP) of experimental group will be different from those of control group" was rejected. Conclusion: This music therapy can be recommended as an efficient nursing intervention to reduce postoperative pain in patients with total hip replacement.
Purpose: This study was conducted to investigate the effect of music therapy as auditory stimulus on the heart rate and behavioral state of premature infants. Method: The study design was a non-equivalent control group pretest-posttest design. Forty premature infants who were admitted at NICU were selected and assigned to two groups, experimental and control. Data were collected from April 20 to July 31, 2003 and analysed using Wilcoxon rank sum test and ANCOVA. Infants in experimental group were given music therapy for 20 minutes daily in 7 days at the average level of 56dB. Results: The first hypothesis that 'Infants in the experimental group will be lower in heart rate than those in the control group' was rejected(F= .05, ρ= .816). The 2nd hypothesis that 'Infants in the experimental group will be lower in behavioral state score than those in the control group, was supported(F=7.40, ρ= .010). Conclusion: The music therapy in this study was an effective nursing intervention in decreasing the heart rate and behavioral state score of premature infants.
Purpose: Music therapy is a non-pharmacological treatment of the behavioral and psychological symptoms. This study was conducted to contribute to the rehabilitation of patients with idiopathic Parkinson's disease in the areas of functionality, emotionality and sociability. Methods: The study included 35 participants, aged between 52 and 77 years, who were diagnosed with idiopathic Parkinson's disease (IPD) and reported suffering from depression, chronic illness anticipated stigma, and worrying about quality of life. The patients were recruited from the department of outpatient neurology at School of Medicine, D University in B Metropolitan City. Group music therapy was performed for nine weeks. The results were analyzed by repeated measures ANOVA. Results: In terms of stigma, there were significant differences between two groups and time (p<.05). Analysis of the interaction between time and service method revealed significant differences (p<.05). Between time, and the interaction between time and service method revealed significant differences in terms of quality of life (p<.05). Conclusion: The results suggest that group music therapy will be effective on the psychosocial well-being of patients with Parkinson's disease living in assisted and independent living communities.
본 연구의 목적은 치료적 음악 만들기가 여성 한부모 가족의 의사소통에 어떠한 변화가 나타나는지 알아보고자 함에 있다. 이를 위해 본 연구에서는 이혼으로 인한 여성 한부모 2가족을 대상으로 노래 만들기, 즉흥 모방하기, 오스티나토 창작하기, 합주하기로 구성된 치료적 음악 만들기 프로그램을 14회기 동안 진행하였다. 본 연구의 결과는 다음과 같다. 첫째, 프로그램의 사전, 사후에 측정한 대상자들의 부모-자녀 간 의사소통 점수가 대상자 B는 59에서 69.5로 상승하였으나, 대상자 A는 잦은 결석 및 음악 적용의 차이로 인해 81에서 75.5로 감소하였다. 둘째, 대상자들의 언어적 비언어적, 음악적 반응을 분석한 결과 대상자 B는 의사소통 유형에 따른 모-녀의 역기능적 의사소통이 후기로 갈수록 감소하고, 기능적 의사소통이 증가하는 변화가 있었다. 대상자 A는 언어적 비언어적 의사소통에서 초기에는 역기능적 의사소통이 많이 나타나다가 8회기 이후 기능적 의사소통이 증가하였고, 음악적 의사소통에서는 언어적 비언어적 의사소통과 다르게 초기부터 기능적 의사소통을 주로 보였으며, 프로그램 후반까지 유지하였다.
자폐범주성장애 아동은 사회적 의사소통 발달에 있어 넓은 범주의 개별적 변이성을 보이기 때문에 이에 대한 중재 개입 시 효과적인 전략이 매우 중요하며, 이를 정확하게 평가할 수 있는 사회적 의사소통 평가도구의 중요성이 지속적으로 강조되고 있다. 본 연구에서는 음악 중재 문헌 내에서 사용된 사회적 의사소통 측정 도구에 대해 고찰하고 이를 분석하였다. 전자데이터베이스 및 음악치료 학술지 검색을 통해 자폐범주성장애 아동의 사회적 의사소통을 변인으로 한 중재 연구 중 1980년에서 2015년까지 출간된 통제된 디자인의 연구가 검색되었고, 최종 분석에는 21개의 연구가 포함되었다. 분석 결과, 측정 도구 유형으로는 행동 직접 관찰이 가장 많이 사용되었고, 측정 도구 유형과 그에 따른 목표 영역 간 조합에 있어 변이성이 크게 나타났다. 또한 선정된 연구 중 90.4%를 차지하는 연구에서 측정 도구 사용의 적절성 검증을 위한 평가자 간 신뢰도가 보고되고 있음이 나타났다. 이러한 결과는 음악치료 중재문헌 내에서 객관적, 체계적 평가 시도가 증가함에도 불구하고, 동일 기능 수준에 접근하는 개별 연구 간 평가 방안 및 전략에 있어 일관성이 떨어지고, 이로 인한 임상근거 도출에 제한이 있음을 시사한다. 결론적으로 자폐범주성장애의 고유한 특성을 고려하여, 사회적 의사소통 평가 시 전반적 기능 수준 변화뿐만 아니라 특정한 행동 습득, 사회 인지 수준 등을 고려할 필요가 있음을 제언하였다.
뇌성마비는 중추신경계의 손상으로 인하여 운동장애 뿐 아니라 지능장애, 언어장애, 경련, 감각장애, 지각장애, 그리고 정서장애와 같은 중복적인 장애를 수반하는 장애이다. 최근 의학의 발달로 인하여 미숙아들의 생존율이 높아지면서 경직형 뇌성마비의 출현율도 높아지고 있음이 보고되고 있다. 뇌성마비 유아들에게는 생후 1년 사이의 치료적 중재가 특별히 중요하며, 7세 이후의 치료효과는 거의 나타나지 않는다. 그러므로 뇌성마비를 대상으로 하는 조기 중재의 필요성이 증가하고 있다. 따라서 본 연구에서는 신체 운동 영역에 장애를 가진 사람들의 신체재활을 목적으로 접근하는 음악치료 영역인 신경학적 음악치료(NMT)의 기법들을 활용하여 경직형 뇌성마비 유아들의 운동발달을 위한 음악치료활동 프로그램을 구성하여 제시하는 것을 목적으로 한다. 이를 위하여 본 연구에서는 일반 유아들이 0~2세에 가장 기본적인 운동 기능들을 습득하는 단계인 초보운동단계의 주요 발달과제들을 바탕으로 하고, 여기에 경직형 뇌성마비 유아의 발달적 특성을 적용하여 프로그램을 구성하였다. 본 음악치료활동 프로그램은 크게 안정성, 이동성, 그리고 조작기능의 세 가지 하위 범주로 나누어 구성되었다. 이렇게 구성된 음악치료활동 프로그램은 임상 현장에서 경직형 뇌성마비 유아를 대상으로 치료를 진행하고 있는 음악치료사들을 통하여 시행된 후 활동에 대한 타당성과 적용의 효율성을 평가받았다. 경직형 뇌성마비 유아의 운동 발달을 위한 음악치료활동 프로그램의 개발은 치료활동의 개발, 전문가의 검증, 치료활동의 수정 및 보완 세 단계로 진행되었다. 치료활동의 개발을 위하여 문헌조사와 인터뷰를 시행하였고, 전문가의 검증은 실제 임상 현장에서 경직형 뇌성마비 유아 혹은 아동을 대상으로 음악치료를 시행하고 있는 음악치료사 6명을 통하여 이루어졌다. 전문가들을 통한 음악치료활동 프로그램 검증에서는 활동의 내용상 타당도 및 실제 현장에서의 활용도를 살펴보았고, 이를 위하여 설문지를 사용하였다. 전문가들의 평가를 바탕으로 치료활동의 수정 및 보완작업이 진행되었으며 수정 및 보완의 결과에 대해서는 검증집단을 통하여 재확인과정을 거쳤다. 음악치료활동 프로그램의 평가 결과는 각 평가 항목별로 평가 값의 평균과 표준편차로 제시하였고, 활동별로 전문가들의 조언과 함께 수정 보완한 내용을 함께 제시하였다. 전문가들의 조언을 바탕으로 수정 및 보완된 음악치료활동 프로그램은 부록으로 첨부하였다. 본 연구의 결과를 요약하면 다음과 같다. 초보운동단계에서의 발달과제인 안정성, 이동성, 조작기능을 바탕으로 경직형 뇌성마비 유아의 운동 발달 특성을 적용한 음악치료활동 프로그램은 총 38개의 활동으로 구성되었다. 안정성 발달을 위한 음악치료활동이 14개, 이동성 발달을 위한 음악치료활동이 10개, 그리고 조작기능 발달을 위한 음악치료활동이 14개이다. 전체 음악치료활동 프로그램은 운동발달단계 적합성, 목표-내용 일치도, 적용 대상 적합성, 음악의 적절성, 악기사용의 적절성, 그리고 실제 활용의 유용성이라는 여섯 가지 평가에서 긍정적인 평가를 받았다. 이것은 본 연구의 결과가 경직형 뇌성마비 유아의 조기 중재 필요성이 증가하고 있으나 발달 단계별 음악치료활동 프로그램이 부족한 국내의 현실적 요구에 맞게 유용하게 활용될 수 있음을 시사한다.
