심실중격결손의 크기 측정에 있어서 술전 검사의 의의 (Significance of the Preoperative Examinations in Predicting the Defect Size of Ventricular Septal Defect)

  • We evaluated the correlationship between the predicted defect size at preoperative examination and the actual defect size at operation room, by examining 69 cases of ventricular septal defect operated at the deparment of Thoracic and Cardiovascular surgery, Kyungpook University Hospital from January 1988 to December 1990. We excluded cases associated with other cardiac anomalies. Of the 69 cases, 39 are male and 30 female, forming 1.3:1 sex ratio in males favor. Their age range from 6 months to 16 year, and 4.3 on the average Their body weights are from 6 to 45kg and 15 on the average. According to Soto`s classification, perimembranous type costitutes 42 cases[61%], doubly committed subarterial type 23 cases[33%], and muscular type 4 cases[6%]. The average diameter of defect size is 8.0$\pm$3.5mm measured in 2D-echocardiogram, 5.6$\pm$3.4mm in angiogram, and 7.4$\pm$4.4mm in operative field. There is statistically significant correlation between the size from 2D-echocardiogram and actual defect size[p=0.001], and no significant difference between the two. Especially in the cases without anurysmal formation, they are nearly the same. Cardiothoracic ratio, pulmonary to systemic flow ratio, pressure ratio and resistance ratio also have statistically significant correlation. Main pumonary artery to descending aorta diameter ratio is correlated with the actual defect size. There is statistically significant correlation between the size from angiogram and actual defect size with some difference.

It is Time to Have Rest: How do Break Types Affect Muscular Activity and Perceived Discomfort During Prolonged Sitting Work

  • Background: Prolonged sitting at work can lead to adverse health outcomes. The health risk of office workers is an increasing concern for the society and industry, with prolonged sitting work becoming more prevalent. Objective: This study aimed to explore the variation in muscle activities during prolonged sitting work and found out when and how to take a break to mitigate the risk of muscle symptoms. Methods: A preliminary survey was conducted to find out the prevalence of muscle discomfort in sedentary work. Firstly, a 2-h sedentary computer work was designed based on the preliminary study to investigate the variation in muscle activities. Twenty-four participants took part in the electromyography (EMG) measurement study. The EMG variations in the trapezius muscle and latissimus dorsi were investigated. Then the intervention time was determined based on the EMG measurement study. Secondly, 48 participants were divided into six groups to compare the effectiveness of every break type (passive break, active break of changing their posture, and stand and stretch their body with 5 or 10 mins). Finally, data consisting of EMG amplitudes and spectra and subjective assessment of discomfort were analyzed. Results: In the EMG experiment, results from the joint analysis of the spectral and amplitude method showed muscle fatigue after about 40 mins of sedentary work. In the intervention experiment, the results showed that standing and stretching for 5 mins was the most effective break type, and this type of break could keep the muscles' state at a recovery level for about 30-45 mins. Conclusions: This study offers the possibility of being applied to office workers and provides preliminary data support and theoretical exploration for a follow-up early muscle fatigue detection system.

흰쥐 골격근의 노화성 위축에 대한 운동훈련의 영향 III.노화 흰쥐에 적용한 단기간의 운동훈련의 영향 (Effect of Exercise Training on Aging Atrophyin Rat Skeletal Muscle III. Effect of Short Term Exercise Training for Senile Rat)

  • 본 실험은 골격의 위축이 일어난 노화 횐 쥐를 사용하여 단기간동안 트레드밀 훈련과 웨이트 트레이닝을 시켰을 때 골격근에 나타나는 변화들을 고찰하고자 고안되었다. 출생 후 24개월 된 수컷 횐 쥐를 사용하여 대조군과 트레드밀 훈련군, 웨이트 트레이닝군으로 구분하여 각 군별로 4주간 훈련시켜 각 군의 골격근은 조직 및 조직화학, 미세구조 및 세포 화학적 연구 및 입체해석학적 연구에 적용하였다. 각 훈련 기간동안 개체의 체중과 근육의 무게는 큰 변화가 없었고, 근섬유의 분포양상은 트레드밀 훈련군에서는 IIB형과 IIC형이 증가를 한 반면 IIA형은 감소를 하였고 I형의 근섬유 조성은 변화가 없었다 웨이트 트레이닝 후에는 IIA와 IIB형이 증가를 한 반면 IIC형은 감소하였다. 그러나 모든 근섬유 조성의 변화에는 유의성이 없었다. 노화 대조군의 조직구조 및 미세구조의 특징은 노화색소의 출현, 약한 수축대의 출현, 근섬유들의 분할현상 등이 관찰되었다 트레드밀 훈련 후에는 손가락형의 돌출부를 포함한 근초의 불규칙성이 나타났으며, 밝은 띠의 확장으로 인한 Z-선의 불규칙한 배열, 공포화, 장축과 평행한 근섬유의 분할현상이 나타났으며, 웨이트 트레이닝 후에는 많은 용해소체들과 함께 포식소자를 함유한 자가용해소체들이 나타났으며 심한 수축대가 나타났고 근원섬유들의 배열이 와해되는 현상도 나타났다. 조직화학 및 세포화학적인 측면에서 succinic dehydrogenase와 acid phosphatase는 활성도의 변화가 거의 없었으며, $Mg^{++}$-­ATPase의 활성은 훈련 후 약간 감소하는 경향이었다. 미세구조의 입체해석학적 분석에서는 측량한 구조물들의 체적밀도와 수밀도는 변화가 거의 없었고 사립체 내막의 표면밀도만 트레드밀 훈련군에서 증가되었다. 이상의 결과에서 볼 때 단기간의 지구력 훈련을 실시할 경우는 노화 골격근은 잘 적응하여 골격근의 위축 방지에 효율적이었으나 웨이트 트레이닝은 노화 골격근의 위축방지에 영향을 주지 못하였다.

알콜로 유도된 흰쥐의 근위축에서 갈근(葛根)의 보호 작용과 그 기전에 대한 고찰 (The Protective Effects of Pueraria Radix against Chronic Alcohol-induced Muscle Atrophy in Rats)

  • Objectives Ethanol is a potent inhibitor of muscle protein synthesis. Muscle mass is regulated by the balance between rates of protein synthesis and protein breakdown. Both acute and chronic alcohol consumption inhibits synthesis to a greater extent than degradation. Protein synthesis is more intensely decreased in type II fibers than in type I fibers. Apoptosis has been shown to occur frequently in a variety of tissues in response to chronic alcohol feeding. Increased muscle fiber apoptosis has been shown in alcoholics with myopathy. Pueraria radix has been used for many disorders such as fevers, gastrointestinal disorders, muscle aches, allergies, respiratory problems, skin problems, high blood pressure, migraine headaches, lowering cholesterol and treating chronic alcoholism. We therefore tested the hypothesis that oral treatment with Pueraria radix could reduce the ethanol-induced muscle atrophy. Methods Young male Sprague-Dawley rats were orally given 25% ethanol (5 ml/kg, body weight) daily with Ethanol for 4 weeks. Normal group was similarly administrated with saline. The Rats of Pueraria radix treated group (EtOH+PR) were orally administrated Pueraria radix water extract, and rats of EtOH group were given with the vehicle only. After 4 week, the morphology of gastrocnemius and plantaris muscles were assessed by hematoxylin and eosin staining. The immunoreactivities of pre-apoptotic BAX and anti-apoptotic Bcl-2 proteins were also measured. Results The muscles from rats of EtOH group represented a significant reduction in average cross section area compared to Normal group. EtOH+PR group had increased fiber compared to the EtOH group. Moreover, to investigate the ethanol-induced muscular apoptosis, the immunohistochemical analysis of Bax and Bcl-2 was carried out. The treatment with Pueraria radix (EtOH+PR) significantly decreased BAX expression and increased Bcl-2 expression 4 weeks after ethanol administration when compared with Normal group. Conclusions These results suggest that Pueraria radix water extract has protective effects on chronic alcohol induced myopathy.

중년여성의 건강증상호소와 피로조절행위 (A Study of Symptom of Health and Fatigue-regulation Behavior in Middle-aged Women)

  • The purpose of this study were to identify a relationship between symptom of health and fatigue-regulation behavior in middle-aged women. The subjects were 241 women living in Seoul and near the city, ranging in ages of 30-59(mean= 42.1 years) being interviewed during the month of Oct. to Dec., 2000. The following questionnaires were utilized in this study: a self reported symptom of health and a fatigue-regulation behavior scale originated from Kwon(1997). The analysis for the data was done by SAS program, t-test, ANOVA, and Pearson correlation. The results of this study were as follows 1. Total mean scores of health status were shown 29.1, physical symptom 17.9 and psychological symptom 11.2. According to symptom of body system were higher in muscular-skeletal sympom 5.5, neurologic symptom 3.5, and digestive symptom 2.7. 2. The mean scores of symptom of health were significantly different in duraion of marriage, status of menstruation, perceived fatigue, perceived cause of fatigue, perceived help of family, hours of sleep. Physical symptom was significantly different in years of educational, status of menstruation, perceived fatigue, perceived cause of fatigue, perceived help of family, hours of sleep. Psychological symptom was significantly different in parity, whether or not having past illness, perceived fatigue. 3. The neurologic symptom was significantly different in years of education, status of menstruation, perceived fatigue, time to rest The respiratory symptom was significantly different in years of education, status of menstruation and hours of physical exercise. The muscular-skeletal symptom was significantly different duration of marriage, status of menstruation, BMI. perceived fatigue, perceived cause of fatigue, perceived help of family. The cardiovascular symptom was significantly different in whether or not having religion, number of children, duration of marriage, number of pregnancy & abortion, parity, status of menstruation, BMI. The digestive symptom was significantly different in whether or not having religion and the number of children. The urologic symptom was significantly different in whether or not having job, type of family, status of menstruation, BMI, perceived fatigue, perceived cause of fatigue, perceived help of family. The score of fatigue was significantly different in number of pregnancy. 4. There were significantly positive relationships in the area of symptom on health, especially fatigue was significantly positive relationships in score of symptom of health, physical symptom and psychological symptom. 5. They choose fatigue-regulation behavior physical rest, method of diversion, management of stress, enough sleep, and psychological rest in order. In the near future, it is required that further studies investigate socio-environmental factors related to symptoms of health and develop programs motivating fatigue-regulation behaviors actively.

체력진단 평가를 통한 뇌성마비 아동과 정상아동의 운동능력 비교연구 (Comparison of Motor Fitness of Cerebral Palsy Chidren with normal throug Phyisical Fitness Diagnosis Evaluation)

  • The aim of this study is to compared the cerebral palsy children with normal children in the exercise and cardio vascular ability after this study as the fundamental data fer mating programs for the cerebral palsy children. The test of this study is about twenty girls, the control group of the normal children(n=10) and the experiment group of cerebral palsy children(n=10). They were studied in four aspects which were the anthropometry, the medical check in the rest, the physical fitness or exercise roads test and the change of the target heart rate during exercise. The result were as follows : 1. The characteristic of the physical type : The control group is higher than the experiment group in the standing height and the body weight but their's little difference between them (p<0.05). The control group is lower than the experiment group in the body fat. 2. The characteristic of the medical check in the rest : The control group is higher than the experiment group in the vital capacity and flood expiratoryvolume one second. The control group is higher that the experiment group in the blood pressure of systolic and Diastolic. There's little difference between them(p<0.01). The control is lower than the experiment group in the heart pulse rate. There's little difference between them(p<0.005). 3. The characteristic of basic physical strength evaluation : The experiment group is the lowest dynamic muscular endurance, balance, agility and endurance which need to move the body with weight. The control group is much higher than the experiment group in the flexibility and muscle strength(Back strength). There's no difference between them(p<0.05). 4. The characteristics of the exercise stress last : The control group is higher than the experiment group in the endurance, the maximum of oxygen intake, endurance level and the out take of calory. There's little difference between them(p<0.01). 5. The characteristic of the change of the target heart rate during exercise : The control group is lower than the experiment group exchange of target heart rate, There's no difference them.

고령자 근효율 향상을 위한 피트니스 압박웨어 시장조사 및 동작특성 조사 (Market Survey and Motion Characteristics Research on Fitness Compression Wear to Improve Muscle Efficiency for the Elderly)

  • The objective of this study is to investigate the market of fitness compression wear as well as to design an optimal fitness compression wear by analyzing the muscle and movement characteristics of the elderly women in Korea. In this regard, research for functional garments is needed to increase muscle activity of elderly people during physical exercise. Firstly, we investigated the brand, design, size, material, and pattern of fitness wear based on the market survey. Secondly, we identified preference, evaluation items, evaluation method, and pattern design method based on the literature review. Finally, in addition, the motion type, range, angle to improve the muscle strength of the elderly were investigated and the maximum muscle strengths of each motion were analyzed by using 2007 Size Korea data (n = 386). It is also designed for muscle fatigue through exercise and rapid fatigue recovery after exercise. The evaluation methods for fitness compression wear were classified as motor functionality, physiological comfort, pattern and material suitability evaluations. The muscle strength at leg (pushing force) and waist (lifting force) of the ages of 60 to 69 years old showed 239.3 N and 274.5 N, respectively, which were the lowest forces compared to younger age groups. By applying these results to the design process of fitness wear, it is anticipated that the fitness wear will have a proper fit to the body shape of elderly people in South Korea as well as it can increase muscle efficiency to promote physical capability and healthy life for senior people.

웨어러블 로봇의 기술 현황 조사 및 개발 방향 제안 연구 (Research on Technology Status and Development Direction of Wearable Robot)

  • Technology status was investigated by analyzing patents and development cases of wearable robots. Development direction of wearable robot for wearability was also suggested by understanding the problems of wearability from development cases through the FGI technique. The number of patents per technical field was the most in the field of strength support, but AI in the technology field was different in each country; Korea was found to be poor in the category of daily living assistance. The number of patents by technology category was the most in the category of muscular strength assistance. However, the values of AI in the technology category were different in each country; Korea was found to be poor in the category of daily living assistance. Development cases were focused on rehabilitation, so development is not fulfilled uniformly by use purpose. By wearing body parts, robots with single function type were mainly developed. Rigid material robots were mainly developed. It was confirmed that wearable robot technology is not developed evenly in the category of application because it is in the early stage of the technical proposal and centered on main performance improvement. We derived twelve wearable conditions for wearable robots: Shape and Size Appropriateness, Movement Appropriateness, Composition Appropriateness, Physiological Appropriateness, Performance Satisfaction, Ease of Operation, Safety, Durability, Ease of Dressing, Ease of Cleaning, Portability and Ease of Storage and Appearance Satisfaction. Finally, the development direction of a wearable robot for each wearable condition was suggested.

인휠형 스마트 휠체어를 위한 힘 보조 제어기 설계 (Design of the Power Assist Controller for the In-Wheel Type Smart Wheelchair)

  • 본 논문은 인휠형 휠체어에 있어 휠 림에 걸리는 외력을 차량의 전압과 회전 속도를 통해 예측하고 이를 통해 각 바퀴에 사용자의 힘에 따라 차량의 방향 및 속도를 제어할 수 있도록 고안된 제어기 설계에 관한 논문이다. 최근 노인 인구의 증가로 인해 노인 및 장애인을 위한 이동기기에 대한 관심이 증가되고 있다. 특히 많은 수의 고령자들이 휠체어를 이용하고 있다. 하지만 고령자의 경우 수동 휠체어 사용 시 국내 지형상의 문제로 인해 구동에 어려움을 겪는다. 또한 전동 휠체어의 경우 조이스틱으로 구동하므로 하체 근력 약화로 인해 휠체어를 이용하는 고령자의 경우, 상체 근력 또한 약화될 수 있다. 이를 극복하기 위해 림(rim)에 힘이 가해지는 힘의 크기를 파악하여 이에 상응하는 모터를 구동시키는 힘 보조형 인휠 전동기에 대한 연구가 진행되고 있다. 하지만 대부분의 힘 보조형 인휠 전동기의 경우 힘의 크기를 측정하기 위한 센서 모듈이 장착되어야 하며 이를 위해 힘의 크기를 측정하기 위한 별도의 림을 설계해야 하는 등 기구적 장치가 요구된다. 이에 본 논문에서는 이러한 힘의 크기를 측정하기 위한 림 설계 대신에 모터의 수학적 모델을 기초로 모터에 전달되는 전압과 바퀴의 현재 속도를 토대로 인휠형 모터에 걸리는 외력을 추정하고 이를 토대로 사용자의 이동 속도와 방향을 추정하여 모터를 이동시킬 수 있도록 하였다. 본 논문은 제안된 수학적 모델을 기초로 사용자의 구동 의지력을 정확하게 측정할 있었다. 또한 이를 기초로 한 제어기를 적용할 경우 인휠 휠체어를 사용자의 의지에 따라 이동할 수 있음을 시뮬레이션을 통해 검증하였다.

Anisakiasis에 대한 조사연구(調査硏究) 특히 유충(幼蟲)의 형태학적(形態學的) 구조(構造)에 대하여 (Studies on Anisakiasis Especially Morphological Studies on the Anisakinae Larva)

  • 1980년 9월(月)부터 1981년1월(月)까지 광주시도축장(光州市屠畜)에서 도살(屠殺)된 돈(豚)에서 검출(檢出)된 Anisakidae 유충(幼蟲)과 광주시장(光州市場)에서 구입(購入)한 고등어에서 채취(採取)한 유충(幼蟲)을 사용(使用)하여 그 충체(忠體)의 계측(計測)과 형태학적(形態學的) 구조(構造)를 관찰(觀察)하여 Anisakis I 형(型)으로 동정(同定)하게 된 결과(結果)는 다음과 같다. 1. 충체(蟲體)는 골벽내(骨壁內)에 침입(侵入)하였거나 골점막면(骨粘膜面)에 부착(附着)하고 있었다. 조직내(組織內) 충체(蟲體)는 변성(變性)의 정도(程度)에 따라 그 구조(構造)의 선명도(鮮明度)에 따라 차이(差異)가 있었으며 어느 것은 명확(明確)한 구조(構造)를 나타내었으나 어느 것은 식별(識別)할 수 없을 정도(程度)로 변화(變化)했다. 2. 돈(豚)과 고등어에서 채취(採取)한 충체(蟲體)의 계측평균직(計測平均直)는 각각(各各) 체장(體長)이 18.0mm, 18.7mm, 체부(體副)이 0.30mm, 0.41mm, 식도(食道)가 1.64mm, 1.18mm, 위(胃)가 0.56mm, 0.67mm 미(尾)가 0.13mm, 0.12mm이었다. 선인(先人)의 보고(報告)에서 체장(體長)에 있어서 사람의 예(例)의 24.4mm와 해어류(海魚類)의 예(例)의 28.4mm의 것보다는 적었으나 돈(豚)의 예(例)의 17.5mm하고는 비슷하였다. 3. 두부(頭部)에 천치(穿齒(boring tooth)와 미부(尾部)에 소극(小棘)(Mucron)이 있고 장위(長胃), 단미(短尾), 순원 (純圓)의 미단등(尾端等)의 특징(特徵)은 단위(短尾), 장미(長尾), 염미단(炎尾端), 미단(尾端)에 소극(小棘)이 없는 Anisakis II 형(型)과 구별(區別)되었다. 위측실(胃側室)(ventricular appendix)과 장맹낭(腸盲囊)(intestinal caecum) 등(等)이 없는 점(點)은 Terranova larvae, Contracaecum larvae, Raphidascaris larvae 그리고 Thynnascaris larvae 등(等)과 구별(區別)되었다. 4. 충체(蟲體)의 단면(斷面)은 각피(角皮). 근층(筋層), 배선(背線), 복선(腹線), Y자형(字形) 또는 쌍엽상(雙葉狀)의 측선(側線), 배세선(排泄線), 장(陽)의 단층도주장토피세절구등은 Anisakis I 형(型)의 특징등(特徵等)을 나타내었고 이것들은 변성(變性)의 진행정도(進行程度)에 따라 다양(多樣)한 변화(變化)를 나타냈다. 5.이상의 형태학적(形態學的) 및 현미경적(顯微鏡的)관찰(觀察)로 미루어 Anisakis I 형(型)의 특 유충(幼蟲)이 확실(確實)히 인정(認定)되었다. 6. 한국(韓國), 일본(日本), 대만(臺灣)에서 돈(豚)에의 Anisakis I 형(型)의 자연감염(自然感染)의 예(例)는 한국(韓國), 일본(日本)에서 각각(各各) 2예(二例)가 있을 뿐이고 사람의 증예(症例) 한국(韓國)에서 2예(例)가 있고 일본(日本)에서는 1,000여예(餘例)가 보고되었고, 대만(臺灣)에서는 인(人), 축(畜) 공(共)히도 보고(報告)된 바 없다.
