Maternal risk factors associated with the low birth weight (저출생체중아의 출생과 산모의 제요인과의 연관성)

  • This study was performed to identify the maternal risk factors for the low birth weight. During the period from February to June in 1991, the medical record review and questionnaire interview were conducted upon the 465 pregnant women who were admitted to and delivered a baby in 3 general hospitals and an obstetric hospital in Kwangju area. The health and other related information from women who bore the low birth weight infant was compared with those from women who bore the normal birth weight infant. The results obtained were as follows : 1. Maternal age, low body weight at term, illegitimate birth, and maintaining work activity during pregnancy were positively associated with low birth weight. 2. The positive association was revealed between low birth weight and the previous abortion, short gestational weeks, anemia, low maternal weight gain during pregnancy, the obstetric present illness and hypertension. 3. Some maternal working conditions were associated with low birth weight although statistically not significant. 4. In multivariate logistic regression analysis, gestational weeks and maternal weight gain during pregnancy were related with low birth weight.

Factors Influencing Adherence to Self Care in Patients with Chronic Heart Failure (만성 심부전 환자의 자가간호 이행 관련요인)

  • Purpose: The objective of this study was to evaluate adherence to self-care and identify associated factors in outpatients with Heart Failure (HF). Methods: Using a cross-sectional design, a convenience sample of 249 outpatient clinic patients were recruited at S university hospital. Between October 2009 and December 2009, data were collected through questionnaires and medical record review. Results: The total mean score of adherence to self-care was $18.07{\pm}3.56$ out of a possible 45 points. Among self-care dimensions, adherence to medication and low salt diet was high, while lower adherence was reported in contact with health professionals if symptoms such as weight gain, edema and fatigue were presented. Multivariate analysis adjusted for other socio-demographic and clinical factors showed that disease knowledge related to heart failure (p<.001) and left ventricular ejection fraction (p=.027) were independent predictors of adherence to self-care. These factors explained 23% of total variance in the adherence to self-care. Conclusion: Heart failure patients with higher disease knowledge and those who have good systolic function may be more likely to engage in adherence to self-care than those with lack of disease knowledge and low contractility. Further research is needed to confirm these results and identify other predictors of adherence to self-care.

Risk Factors for Acquisition of Methicillin-Resistant Staphylococcus aureus in a Neurosurgical Intensive Care Unit(NSICU): Case-Control Study (신경외과 중환자실에서의 MRSA 획득 위험요인 분석)

  • Purpose: Methicillin-resistant Staphylococcus aureus (MRSA) is a major nosocomial pathogen in the intensive care units (ICUS). The purpose of this case-control study is to identify risk factors for acquisition of MRSA during ICU stays in patients with and without MRSA. Method: The study was conducted in a 16 beds-neurosurgical intensive care unit of a 2200-bed tertiary care university hospital in Seoul, Korea. Medical record and Critical Classification Scoring System were reviewed retrospectively in patients who were admitted more than 3 days from August 1, 2003 to May 30, 2004. Cases and controls were matched for age and gender. The obtained specimens were nasal swab and sputum. Result: There were 950 patients' admissions during the period. Among them, MRSA was isolated from twenty-three patients who were considered as hospital acquired. Artificial airway (p=.045), frequency of suction (p=.002), nasogastric tube (p=.004), wound drain (p=.045), and vancomycin (p=.019) were risk factors for MRSA acquisition in univariate analysis. Frequency of suction (p=.012, OR 3.5) was revealed as the only risk factor in multivariate conditional logistic regression. Conclusion: Our findings give support to recent studies that suggest that frequent physical contact maγ increase the nosocomial acquisition of MRSA in a neurosurgical ICU.

Influencing Factors on Sexual Knowledge among Elementary, Middle, and High School Students (초, 중, 고등학생의 성지식에 영향을 미치는 요인)

  • Purpose: This study investigated sexual knowledge, its related factors, and the needs of sex education among elementary, middle, and high school students in order to develop a sex education program. Method: The participants comprised 721 students in grades 5 through 12, from Seoul and the Kyeonggi-do area, in South Korea. Data was collected from the end of August through the end of September 2014, using an anonymous self-administered questionnaire. SPSS Statistics 21.0 was used for the descriptive analysis, chi-square, t-test, ANOVA, and multiple regressions. Results: Higher-grade students had a higher level of sexual knowledge than lower-grade students, but the overall level of sexual knowledge was low. Multivariate analysis showed that gender, grade, school record, and experiences of sex education from family and school were significant predictors of sexual knowledge among adolescents. Coping skills related to sexual crisis situations was the most wanted content of sex education. Conclusion: The results of this study suggested that it is important to develop a sex education program that considers the needs of sex education among adolescents. Also, it is important to periodically perform a survey to identify the need of sex education among elementary, middle, and high school students.

Factors Associated with Discontinuation of Postoperative Intravenous Patient Controlled Analgesia (수술 후 자가통증조절요법 중단 관련요인)

  • Purpose: The purpose of this study was to identify the factors associated with the discontinuation of patient controlled analgesia (PCA) after surgery. Methods: The data of 1,092 adult patients that were over 20 years of age and underwent PCA after surgery in the Gachon University Hospital from May 1 to June 30, 2017, were collected through the patients' Electronic Medical Record (EMR). The collected data was analyzed via the use of the Chi-test, t-test and multivariate logistic regression analysis using SPSS 18.0 program. Results: The postoperative PCA discontinuation rate was 26.1%. It was associated with various symptoms, such as those of nausea, dizziness, and headache. The PCA discontinuation was also related with female (odds ratio, OR= 1.75; confidence interval, CI= 1.09-2.82), nausea (OR= 105.27; CI= 61.03-181.58), total intravenous anesthesia (TIVA) of the thyroidectomy (OR= 10.43; CI= 5.01-21.70). Conclusion: It is necessary to provide additional medication and nursing interventions to reduce nausea, which is the symptom associated with PCA discontinuation, especially in the operation of female subjects and thyroidectomy under TIVA. That is, those who are at a high risk for PCA discontinuation should be able to administer additional antiemetics or reduce non medication nursing interventions.

The influences of mental health problem on suicide-related behaviors among adolescents: Based on Korean Youth Health Behavior Survey (청소년의 정신건강문제가 자살 관련 행위에 미치는 영향: 청소년 건강행태조사 자료를 이용하여)

  • Purpose: This study explored the influencing factors on suicide-related behaviors (ideation, plans, and attempts) focusing on mental health problems (anxiety, depression, and loneliness) among Korean adolescents. Methods: A secondary analysis was conducted with data from the 16th Korean Youth Health Behavior Survey collected from in 2020 by the Korea Centers for Disease Control and Prevention. Chi-square tests and multivariate logistic regression analyses were performed. Results: After the adjustment of demographic characteristics and health risk behaviors, the influences of mental health problems on suicidal ideation, plans, and attempts showed the anxiety odds ratio (OR) for severe anxiety vs. minimal (OR 4.65, 4.67, and 3.75), depression (OR 4.27, 3.69, and 4.49), loneliness (OR 2.18, 1.96, and 1.96). Health risk behaviors (violence experience, drug use, stress, smoking, and drinking alcohol) and demographic variables (gender, school record, and socioeconomic status) were also significantly associated with suicide-related behaviors. Conclusion: Anxiety, depression, and loneliness were strong predictors of suicide-related behaviors. Early detection of suicide risks through screening for comprehensive mental health problems was recommended. Suicide prevention that considers the risk factors, including mental health problems and other risk factors, needs to be developed and implemented to reduce suicide risks among adolescents.

  • The world's most extensive and active deltas, Louisiana's wetlands, are deteriorating rapidly due to multiple stressors such as the discharge of the Mississippi River, sea-level rise, and coastal retreat, the substantial but spatially and temporally variable impacts. However, the ecological and anthropogenic histories, the mode of environmental changes on a multi-millennial timescale have not been thoroughly documented. This study, a palynology-based multiproxy analysis, investigates hydrological, geological, geochemical, and anthropogenic impacts on southern Louisiana wetlands and a variety of external forcing agents influencing ecological succession. Sediment cores extracted from a small pond on a mangrove-dominate island near Port Fourchon, Louisiana, USA yielded a 4,000-year record. The site has been transformed from freshwater to saline water environments, to a mangrove dominant island over the late Holocene. The multivariate principal component analysis identified the relative strength of external drivers responsible for each ecological shift. The Mississippi River delta cycle (lobe switching) was the dominant driver of ecosystem changes during the late Holocene, while relative sea-level rise, tropical cyclones, climate, and anthropogenic effects have been the main drivers late in the site's history.

Socioeconomic and Sociodemographic Factors related to Allergic Diseases in Korean adolescents based on the 14th Korean Youth Risk Behavior Survey (청소년의 알레르기성 질환과 관련된 사회경제적 및 사회인구학적 요인: 제14차 한국청소년건강행태조사를 기반으로)

  • Various international reports have shown strong proof that socioeconomic and sociodemographic variables are correlated with allergic diseases, yet little is known about how these variables affect Korean adolescents. This study was conducted to identify the socioeconomic and sociodemographic risk factors for allergic diseases of Korean adolescents, and to provide information for preventing and managing such conditions. Data from the 2018 Korean Youth Risk Behavior Survey (KYRBS) of 60,040 adolescents was used for this study. An anonymously administered online survey was conducted to collect information on the dependent variables. The independent variables were asthma, allergic rhinitis and atopic dermatitis. Multivariate logistic regression analysis was performed to analyze the correlations between the dependent variables and the independent variables. Low economic status, living with mother, high education level of parents, high school record, obesity, drinking and smoking were the risk factors for asthma. Low economic status, the place of residence, living with mother, high education level of the parents, gender, high school record, obesity, drinking and smoking were the risk factors for allergic rhinitis. Low economic status, living with mother, high education level of parents, genderhigh school record, obesity and drinking were the risk factors for atopic dermatitis. We found that low socioeconomic status and unhealthy behavior were the risk factor for allergic diseases of Korean adolescents. Balanced economic growth in the community and controlling unhealthy behavior can help reduce the prevalence of allergic diseases.

Convergence Educational Exploration of Anxiety about PE Class in Middle School Coed Class and Single-sex Class (중학교 혼성·동성학급의 체육수업 걱정거리에 대한 융복합교육 방안 탐색)

  • This study looks into the level of anxiety that middle school students have about PE class, dividing the classes into coed class and single-sex class and aims to conduct an in-depth analysis of differences in sub-factors of the anxiety. Subjects of the study were 554 students in randomly selected six middle schools located in Gyeonggi-do. To figure out the relationships among the sub-factors of the anxiety according to class, grade, academic record and the level of physical strength, a Multivariate Analysis of Variance model was set up and analysis was conducted. The results obtained through the research are as follows. First, it was predicted that the level of anxiety about PE class in middle school would have a great impact on the factor, 'punishment' by class (coed and single-sex). Second, it was predicted that the level of anxiety about PE class in middle school would have a great impact on the factors, 'anxiety about the event of sports' and 'conflict during class' by grade. Third, it was found that there was no difference in the level of anxiety about PE class that middle school students had according to their academic record in PE. Fourth, it was predicted that the level of anxiety about PE class in middle school would have a great impact on the factor of 'anxiety about the event of sports' by the level of physical strength.

Usability of a smartphone food picture app for assisting 24-hour dietary recall: a pilot study

  • BACKGROUND/OBJECTIVES: The Recaller app was developed to help individuals record their food intakes. This pilot study evaluated the usability of this new food picture application (app), which operates on a smartphone with an embedded camera and Internet capability. SUBJECTS/METHODS: Adults aged 19 to 28 years (23 males and 22 females) were assigned to use the Recaller app on six designated, nonconsecutive days in order to capture an image of each meal and snack before and after eating. The images were automatically time-stamped and uploaded by the app to the Recaller website. A trained nutritionist administered a 24-hour dietary recall interview 1 day after food images were taken. Participants' opinions of the Recaller app and its usability were determined by a follow-up survey. As an evaluation indicator of usability, the number of images taken was analyzed and multivariate Poisson regression used to model the factors determining the number of images sent. RESULTS: A total of 3,315 food images were uploaded throughout the study period. The median number of images taken per day was nine for males and 13 for females. The survey showed that the Recaller app was easy to use, and 50% of the participants would consider using the app daily. Predictors of a higher number of images were as follows: greater interval (hours) between the first and last food images sent, weekend, and female. CONCLUSIONS: The results of this pilot study provide valuable information for understanding the usability of the Recaller smartphone food picture app as well as other similarly designed apps. This study provides a model for assisting nutrition educators in their collection of food intake information by using tools available on smartphones. This innovative approach has the potential to improve recall of foods eaten and monitoring of dietary intake in nutritional studies.
