유비쿼터스 지능공간에서 멀티모달센서를 이용한 향상된 u-헬스케어 서비스 구현에 대한 연구 (Advanced u-Healthcare Service using A Multimodal Sensor in Ubiquitous Smart Space)

  • 고령화 사회로의 진입과 삶의 질 향상에 따라 의료산업의 패러다임이 u-헬스케어(Healthcare)로 빠르게 변화하고 있다. u-헬스케어로의 변화는, 질병의 진단과 치료와 같은 의료서비스의 사후(事後)처리적인 관점에서 예방과 관리라는 사전(事前)예방적인 관점으로의 변화라는 점에서 의미가 있다. 그러나 u-헬스케어에 대한 관심은 새로운 서비스 도출과 측정기기(센서류)의 개발, 의료정보 표준화와 통합에만 그 관심이 집중되고 있다. 따라서 본고에서는 유비쿼터스 지능공간을 구성하고 사용자에게 u-헬스케어 기술을 제공함에 있어 "사용자 중심의 u-헬스케어 기술"이라는 개념을 적용함으로써, u-헬스케어 기술을 유비쿼터스 구성의 기본철학인 사라지는 컴퓨팅(Disappear Computing), 보이지 않는 컴퓨팅(Invisible Computing), 조용한 컴퓨팅(Calm Computing)과 사용자 중심(User Centered)의 기술 개발을 통해 실현 가능하도록 하는 방법에 대하여 소개하고자 한다.

단기 집단 복합중재가 정상 노인의 인지기능 및 우울에 미치는 영향 (Effects of a Short-term Multimodal Group Intervention Program on Cognitive Function and Depression of the Elderly)

  • 목적: 신체활동과 인지운동, 사회적 상호작용을 혼합한 단기 집단 복합중재가 75세 이상의 고령 정상 노인의 인지기능과 우울 수준에 미치는 영향을 알아보고자 하였다. 연구방법: 본 연구 설계는 단일 집단 사전-사후 설계(one group pretest-posttest design)로 중재는 회당 70분, 주 1회, 총 4회기 동안 실시하였다. 중재 전과 후의 인지기능과 우울 수준, 신체기능의 변화를 비교하기 위해서 치매 선별용 간이 정신상태검사(Mini-Mental State Examination-Dementia Screening; MMSE-DS)와 단축형 노인우울척도(Short Form for Geriatric Depression Scale; SGDS), 버그균형척도(Berg Balance Scale; BBS)를 사용하였다. 결과: 75세 이상의 고령 정상노인에게 집단 복합중재를 적용한 후 인지기능은 통계적으로 유의미한 향상이 있었고(p<.01), 우울 수준은 통계적으로 유의미한 감소가 있었다(p<.05). 균형수준도 중재 전$46.83{\pm}9.11$점에서 중재 후 $48.08{\pm}7.00$점으로 평가점수의 증가는 있었으나, 통계적으로 유의미하지는 않았다(p>.05). 결론: 신체활동과 인지운동, 사회적 상호작용을 혼합한 단기 집단 복합중재는 75세 이상의 고령 정상노인의 인지기능의 저하를 늦추고 우울수준을 감소시키는데 유의한 효과가 있었다. 본 연구는 정상노인들의 치매, 우울 예방을 위한 중재에 있어서 보다 체계적인 중재를 제공하기 위한 근거를 제시한다는 점에 의의가 있다. 향후 연구에서는 작업치료사에 의한 전문적인 치료에 대한 효과연구와 실험군-대조군 연구를 통해 효과입증이 이루어져야 할 것이다.

Treatment outcomes of radiotherapy for anaplastic thyroid cancer

  • Purpose: Anaplastic thyroid cancer (ATC) is a rare tumor with a lethal clinical course despite aggressive multimodal therapy. Intensity-modulated radiotherapy (IMRT) may achieve a good therapeutic outcome in ATC patients, and the role of IMRT should be assessed. We retrospectively reviewed outcomes for ATC treated with three-dimensional conformal radiotherapy (3D-CRT) or IMRT to determine the optimal treatment option and explore the role of radiotherapy (RT). Materials and Methods: Between December 2000 and December 2015, 41 patients with pathologically proven ATC received RT with a sufficient dose of ${\geq}40Gy$. Among them, 21 patients (51%) underwent surgery before RT. Twenty-eight patients received IMRT, and 13 received 3D-CRT. Overall survival (OS) and progression-free survival (PFS), patterns of failure, and toxicity were examined. Results: The median follow-up time for survivors was 38.0 months. The median and 1-year OS and PFS rates were 7.2 months and 29%, 4.5 months and 15%, respectively. Surgery significantly improved the prognosis (median OS: 10.7 vs. 3.9 months, p = 0.001; median PFS: 5.9 vs. 2.5 months, p = 0.007). IMRT showed significantly better PFS and OS than 3D-CRT, even in multivariate analysis (OS: hazard ratio [HR] = 0.30, p = 0.005; PFS: HR = 0.33, p = 0.005). Significantly higher radiation dose could be delivered with IMRT than 3D-CRT ($EQD2_{10}$ 66 vs. 60 Gy, p = 0.005). Only 2 patients had grade III dermatitis after IMRT. No other severe toxicity ${\geq}grade$ III occurred. Conclusion: Patients with ATC showed better prognosis through multimodal treatment. Furthermore, IMRT could achieve favorable survival rates by safely delivering higher dose than 3D-CRT.

The Current Evidence on Neoadjuvant Therapy for Locally Advanced Esophageal Squamous Cell Carcinoma

  • Surgical resection is the mainstay of treatment for locally advanced esophageal cancer. Neoadjuvant therapy is recommended to improve survival, based on the results of several randomized trials and meta-analyses. However, controversy remains regarding how to combine surgery, radiotherapy, and chemotherapy. Moreover, in East Asia, the predominant histological type is esophageal squamous cell carcinoma, which has a different epidemiology and tumor biology from esophageal or gastroesophageal junctional adenocarcinoma. As such, the management of esophageal cancer in East Asia seems to be different from that in Western countries. Thus, this article reviews the current evidence on neoadjuvant therapy and considers the optimal combinations and ongoing strategies of multimodal therapy for esophageal squamous cell carcinoma.

Multimodal Treatment of Poorly Differentiated Tracheobronchial Carcinoma in a Persian Cat

  • A 14-year-old castrated male Persian cat presented with a 2-week history of respiratory difficulty. On physical examination, the patient showed intermittent open-mouth breathing and thoracic auscultation revealed wheezing. Thoracic radiographs revealed a narrowed upper airway and pulmonary infiltration. Computed tomography detected a mass occluding the lumen of the trachea at the level of the entrance to the thorax, a mass causing right main bronchus stenosis, and a nodule on the right caudal lung lobe. Bronchoalveolar lavage cytology tentatively diagnosed a carcinoma. Tracheal mass resection was performed through tracheostomy. Histopathology confirmed the presence of tracheobronchial carcinoma. The survival time after diagnosis was 10 months, during which time the cat underwent tracheostomy, debulking by endotracheal tube, and tracheal stent placement procedures in combination with toceranib phosphate adjuvant chemotherapy.

Successful long-term management of late-stage precursor-targeted immune-mediated anemia with prednisolone and mycophenolate mofetil in a dog

  • A 13-year-old neutered male Maltese dog presented to our hospital with lethargy and anorexia. Laboratory abnormalities included severe non-regenerative anemia (hematocrit, 12.9%; reticulocyte count 12.8 K/µL). The cytology of bone marrow revealed erythroid hypercellularity with mild myelofibrosis. Therefore, late-stage precursor-targeted immune-mediated anemia (PIMA) was diagnosed. Multimodal treatment including 2 immunosuppressant drugs (prednisolone and mycophenolate mofetil), antithrombic drug (clopidogrel), and blood transfusion was performed. The dog showed complete remission from PIMA, and the total duration of follow-up was 622 days. This is the first case report of canine PIMA managed successfully with prednisolone and mycophenolate mofetil in Korea.

Computed Tomography and Magnetic Resonance Imaging Findings of Bicuspid Aortic Valve and Related Abnormalities of the Heart and Thoracic Aorta

  • The bicuspid aortic valve (BAV) is the most common congenital cardiovascular malformation. Patients with BAV are at higher risk of other congenital cardiovascular malformations and valvular dysfunction, including aortic stenosis/regurgitation and infective endocarditis. BAV may also be related to aortic wall abnormalities such as aortic dilatation, aneurysm, and dissection. The morphology of the BAV varies with the presence and position of the raphe and is associated with the type of valvular dysfunction and aortopathy. Therefore, accurate diagnosis and effective treatment at an early stage are essential to prevent complications in patients with BAV. This pictorial essay highlights the characteristics of BAV and its related congenital cardiovascular malformations, valvular dysfunction, aortopathy, and other rare cardiac complications using multimodal imaging.

  • 결핵은 새로운 항결핵제의 개발, 생활 여건의 향상, 예방 대책의 발달로 유병률이 적어진 질환으로 알려지고 있으나, 최근 예방 접종률의 저하와 항결핵제에 내성을 보이는 균주가 출연하는 등 다시 증가하는 추세이며 또한, 견관절 질환에 대한 관심이 높아지며 일차 치료로써 다양한 견관절내 주사 치료가 시행되면서 화농성 견관절염 역시 증가하는 추세이다. 이에 저자들은 견관절의 결핵과 화농성 관절염이 동반 감염된 증례를 경험하여 만족할 만한 결과를 얻었기에, 관련 문헌과 함께 보고하는 바이다.

Analysis of Semantic Relations Between Multimodal Medical Images Based on Coronary Anatomy for Acute Myocardial Infarction

  • Acute myocardial infarction (AMI) is one of the three emergency diseases that require urgent diagnosis and treatment in the golden hour. It is important to identify the status of the coronary artery in AMI due to the nature of disease. Therefore, multi-modal medical images, which can effectively show the status of the coronary artery, have been widely used to diagnose AMI. However, the legacy system has provided multi-modal medical images with flat and unstructured data. It has a lack of semantic information between multi-modal images, which are distributed and stored individually. If we can see the status of the coronary artery all at once by integrating the core information extracted from multi-modal medical images, the time for diagnosis and treatment will be reduced. In this paper, we analyze semantic relations between multi-modal medical images based on coronary anatomy for AMI. First, we selected a coronary arteriogram, coronary angiography, and echocardiography as the representative medical images for AMI and extracted semantic features from them, respectively. We then analyzed the semantic relations between them and defined the convergence data model for AMI. As a result, we show that the data model can present core information from multi-modal medical images and enable to diagnose through the united view of AMI intuitively.
