A Systematic Review and Meta-Analysis of Moxibustion Treatment for Knee Osteoarthritis

  • The purpose of this study was to evaluate the evidence supporting the effectiveness of moxibustion treatment for osteoarthritis. There were 9 databases used to retrieve randomized controlled trials (RCTs) that used moxibustion as treatment for osteoarthritis. The quality of methodology for the RCTs was assessed using the Cochrane Risk of Bias tool [Review Manager (RevMan) Version 5.3 Windows, The Nordic Cochrane Centre, Copenhagen, Denmark]. The inclusion criteria for this review was met by 27 RCTs. All studies were conducted in China. A 4-week moxibustion treatment period was the most common. EX-LE4 and SP10 and GB34 acupoints were most frequently selected in the treatment of osteoarthritis. The most commonly used evaluation index was the visual analog scale (VAS). All studies, including a meta-analysis showed that moxibustion treatments were statistically significantly effective at treating knee osteoarthritis. However, well-designed randomized RCTs without a high risk of bias should be designed in the future.

  • Moxibustion plays an important role not only in the medical service, but also in the health care of Korea. So this study attempted to review of moxibustion and analyze of domestic study trend. And it also intends to examine the usefulness of moxibustion treatment in Korean nursing intervention. "Donguibogam" is famous for the most comprehensive medical book in ancient Korea, indicates that moxibustion makes up a large part of it and that its contents were composed of material, size, time, caring of one's health, indication, operational procedure, contraindication in the documented in the 25 reference books. Also, It confirms that it has been widely used since old times. Especially, it seems to be a great help to researchers because its content lay a theoretical foundation. From the literature, we identified that moxibustion was most widely using method for health promotion and an efficacious cure for a disease. I searched and investigated the journals and dissertations supplied by KERIS and RICH with the key word "moxibustion". Of 10 articles with moxibustion intervention, the trend of domestic studies shows, that of an area relating to nursing was 5. Therefore, moxibustion intervention steadily increases in frequency in nursing academic circles year by year. But there is no distinction between hand moxibustion and moxibustion. According to the above results, we need to develop moxibustion learning program and well designed experimental study with moxibustion for Korean Nursing intervention.

  • As compared with acupuncture, The moxibustion occurred differently in the background of formation and also disagreed in the process of development. From the Jin(晉) dynasty to the Song(宋) dynasty, The school that attached importance to moxibustion had occupied superority by far and had schemed the development of moxibustion. But after the Song(宋) dynasty, The school that attached importance to acupuncture rose. Therefore, The school that attached importance to moxibustion, even if the power decreased, still had kept in existence and had achieved the development of moxibustion. Especially, Among the chinese physicians that recognized "Moxibustion can cure all disease, so don't discuss the weakness the firmness the chills the fever and cauterize the skin" till the Song(宋) dynasty, Ge Hong(葛洪) put in order the basic theory for moxibustion in (A handbook of prescriptions for emergencies). Wang Tao(王燾) only respected the moxibustion and said "Moxibustion has a strange effect, then all acupuncture herb-med(medical decoction) herb-ex are unattainable to it" in volume 14 (Medical secrets of an official).   • Objectives : This research was projected to verify the validities of needling or moxibustion- acupoints in Illustrated Manual for the Practice of Acupuncture and Moxibustion with the Help of a Bronze Figure bearing Acupuncture Points (IMPAM) and The Classic of Supplementing Life with Acupuncture and moxibustion(CSLAM) Methods; We investigated acupoints which have prohibitions, notices or adverse events when performing needling or moxibustion in IMPAM and CSLAM, then verified it in various literatures and today's medical knowledge. Results; In IMPAM, 2 acupoints were prohibited both needling and moxibustion, 22 acupoints were prohibited from needling and 29 were prohibited from moxibustion. In CSLAM, 3 acupoints were prohibited from both needling and moxibustion, 24 acupoints were prohibited from needling and 31 were prohibited from moxibustion. Most of the prohibition, the notices and adverse events of the acupoints written in IMPAM and CSLAM had medical or philological bases, and the number of them increased compared to that in A-B Classic of Acupunture and Moxibustion(ABCAM). Conslusion : The prohibotions, the notices and the adverse events when performing acupunture or moxibustion in IMPAM and CSLAM reflected the medical experience and knowledge of the time. And the knowledge was increased compared to that in ABCAM.

  • Objectives: Moxibustion has been used for various post-stroke symptoms and has also been known to have effect on peripheral blood flow. This study investigated the effect of moxibustion on radial artery blood flow by Doppler ultrasound and on heart rate variability in post-stroke hemiplegia patients. Methods: Moxibustion was applied on the points of LI4, TE3, TE5 and LI11 on the affected side, and blood flow of the radial artery was measured using the Minimax-Doppler-K device. Blood flow velocity and pulsation index were analyzed before, during, and after moxibustion. Simultaneously LF, HF, and LF/HF as variables for HRV were measured by FM-150. Results: The mean value of blood flow velocity in all patients (n=23) showed significant increase between before and after moxibustion, but there was no significant difference in pulsation index and LF, HF, LF/HF ratio between before and after moxibustion. In Yin (n=9) and Yang (n=14) groups, both showed significant increase of blood flow velocity between before and after moxibustion, but there was no significant difference in pulsation index and LF, HF, LF/HF ratio between before and after moxibustion. In Deficiency (n=14) and Fullness (n=9) groups, only the Deficiency group showed significant increase of blood flow velocity between before and after moxibustion, while the Fullness group showed no significant difference in blood flow velocity between before and after moxibustion, and also there was no significant difference in pulsation index and LF, HF, LF/HF ratio between before and after moxibustion. Conclusions: This study suggests that moxibustion on LI4, TE3, TE5 and LI11 on the affected side of stroke patients increase the peripheral blood flow in the affected arm, which was most remarkable in those with Yin and Deficiency pattern.

  • It is well known that both of acupuncture and moxibustion are effective for treatment of human disease. For years, they have been utilizing a electro-acupuncture (EA) and moxibustion in attempt to manage gastric symptom. However, the data of combined EA and moxibustion on clinical effectiveness is conflicting and the action of combineAd acupoints is not well known. BL21 and ST36 which belong to bladder Meridian and stomach meridian are acupuncture points frequently used for healing gastrointestinal disease in oriental medicine. The present study was conducted to see the effects produced by combined electro-acupuncture(EA) and moxibustion using BL21 and ST36 on serum gastrin level in rats. In experiment, male Spraque-Dawley rats(body wt 140-160g) were selected. EA (2 Hz, intensity ; 10 times muscle twitch threshold) was applied for 30 minutes to ST36 under halothane anesthesia. Moxibustion($1.6{\pm}0.3 mg$, 5 times of a day) was applied during 3 days to BL21 under halothane anesthesia. Synergy effects of combined EA and moxibustion might be in according to stimulated method and used acupoints. The change of serum gastrin levels produced by combined EA and moxibustion is shown after vagotomy. These data suggested that many factors, such as the selection of acupoints, the stimulated EA methods, are capable of affecting combined EA and moxibustion effects and those effects may be not related with nervous system.

  • Objective ; This Study was carried out to review the history of acupuncture & moxibustion in Korea under the control of imperialized Japan and to compare the history among Korea, China and Japan. Results ; Under the control of imperialized Japan the science of acupuncture & moxibustion in Korea has experienced the dark ages because of the momentary disordered medical laws made by Japan. As we compare the history of acupuncture & moxibustion among Korea, China and Japan, Korea was simple but systemic, China was complicate but complex and Japan was nearly surgical. Conclusion ; The science of acupuncture & moxibustion of Korea was mostly advanced in the late Chosun and did not just imitate that of China and has advanced uniquely.

  • Objectives : This study was to compare the effect of moxibustion therapy on peripheral facial para- lysis according to location of acupoint. Methods : We investigated 41 cases of inpatients with Peripheral Facial Paralysis, and divided them into two groups, One group was treated by moxibustion on local point(ML), and the other was treated by moxibustion on distant point(MD). We evaluated the effect of moxibustion in each group by using Gross Grading System of House-Brackmann(HB score) and Yanagihara's Unweighed Grading System(Y score). Results : 1. In ML and MD, compared with baseline, at final, HB score was significantly decreased and Y score was significantly increased. 2. At final, ML showed significant decrease on HB score and significant increase on Y score compared with MD. Conclusions : These results suggested that ML should be more useful for improving symptoms related with peripheral facial paralysis than MD.

Effect of Moxibustion Therapy on the Degenerative Arthritis of Knee Joint with Osteochondroma (골연골종을 동반한 퇴행성 슬관절염에 구법(灸法)이 미치는 영향)

  • Objectives : This study was done for reporting effect of moxibustion therapy on the degenerative arthritis of knee joint with osteochondroma. Methods : Two patient with degenerative arthritis of knee joint with osteochondroma was treated by moxibustion therapy. We applied moxibustion therapy three times a day for 12 days(three days per week). To investigate effectiveness of treatment we used visual analogue scale, Korean Western Ontario and McMaster Universities. Results : 1. The moxibustion therapy deceased knee joint pain. 2. As a result of evaluation by visual analogue scale, Korean Western Ontario and McMaster Universities the score marked lower than before treatment and after treatment. Conclusion : Moxibustion therapy decreased knee joint pain that patient have degenerative arthritis of knee joint with osteochondroma.
