뇌기능 장애 환자에서 자세균형 제어능력의 저하는 보행 및 일상생활동작 수행 등에 어려움을 초래하며 이에 대한 정확한 평가 및 치료를 위하여 일상의 환경변화와 유사한 상황을 제공하고 이에 따른 자세균형 조절 능력을 파악하는 것이 중요하다. 본 연구는 뇌기능 장애 환자에서 가상적 움직이는 환경에 따른 자세균형 조절 기능을 정확히 평가하고 환경의 움직임이 자세균형 조절에 미치는 영향을 분석하였다. 15명의 뇌기능 장애 환자들과 정상인 15명을 대상으로 실생활과 유사한 환경의 조성을 위하여 HMD를 이용한 가상 환경 움직임(Virtual Moving Surround)을 네 가지 다른 패턴으로 제공하였다. 자세동요의 정도는 힘판을 이용하여 신체압력중심의 변화를 전체이동거리, 동요주파수, 최대 빈도 COP 위치로 측정하였으며 가상 환경의 차이에 따른 변화를 비교 분석하였다. 연구결과 검사 재검사 신뢰도평가에서 일관된 분석결과를 나타냈고 뇌 기능장애 환자와 정상인의 분석에서는 두 그룹간의 차이를 확인할 수 있었다. 특히 전후로 빠르게 변하는 가상 환경에서 가장 큰 자세동요를 나타내었고 통계적으로 유의한 차이가 있었다. 본 연구를 통해 뇌기능 장애 환자에서 가상 환경 변화가 자세균형 조절에 미치는 영향을 확인할 수 있었으며 이러한 환자들을 위한 치료와 평가 환경 조성 등에도 유용한 자료로 쓰일 수 있을 것으로 사료된다
의료자원량의 지역 간 격차 해소는 오랫동안 정책과제로 지적되어 왔으며, 이에 따라 공공의료 확충계획과 병상수급관리 정책 등 지역 간 자원균점을 목표로 하는 정책방향이 수립되어 왔다. 그러나 정책방향으로서의 형평의 성격이나 측정기준이 무엇인지에 대한 구체적인 방향이 정립되어 있지는 않았기에, 정책방향에 대한 폭넓은 동의가 이루어지지 못해왔다. 본 연구는 의료기관들의 불균등분포가 초래하는 문제는 결국 진료를 위해 환자들이 이동해야 하는 불편함이라는 것에 착안하여, 우선 환자이동상황을 관찰한 후 이를 통해 병상수급관리에 대한 시사점을 도출하는 것을 목표로 한다. 또한 불편함을 초래하는 환자의 이동은 단일하고 동질적인 현상이 아니라 다양한 성격과 수준을 가진 현상이라는 점을 고려하였다. 먼저 이동기준에 관해서는 거주 행정구역을 벗어나거나, 인접지역을 포함한 지역을 벗어나거나, 생활권기준의 지역을 벗어나는 경우 등 세가지의 이동기준을 이용했고, 다양한 이동 수준을 반영한 다항로짓모형을 분석하였다. 결과적으로, 생활권을 기준으로 이동현황을 측정하는 경우에는 거주행정구역을 기준으로 한 경우보다 지역 간 불균등 정도가 크게 감소하였고, 이동기준과 이동수준에 따라 이동형태나 이동요인에도 차이가 나타났기에, 지역의 자원보유현황을 평가함에 있어 이동기준 설정과 이동의 원인과 폭을 파악하는 과정이 중요시되어야 한다는 점을 확인하였다.
Purpose: This study was done to compare sleep patterns, factors of sleep disturbance and sleep enhancement behaviors between sleep disturbance and non-sleep disturbance elderly patients. Method: The participants were 117 patients over 65 years old who were hospitalized at 5 general hospitals in Seoul. There were 83 patients in the good sleeper group and 34 in the poor sleeper group. The survey questionnaires included questions on general characteristics, sleep patterns, sleep disturbing factors, and sleep enhancement behaviors of the patients. Results: For sleep patterns and environmental factors of sleep disturbance, the poor sleepers was significantly higher than that of the good sleepers. Significant differences between good sleepers and poor sleepers were also found on the following variables: Environmental factors of sleep disturbance -'Sound of other patient's or care-giver's', 'Discomfort of bed, linen, pillow and patient's gown', 'Light in the room', 'Emergency situations', 'Temperature of patients rooms too hot or too cold'. The significant physical factor of sleep disturbance was 'Hard to breath even without moving' and for sleep enhancement behaviors: 'Use of ear plugs or eye covers' Conclusion: Consequently it is necessary to develop positive and differentiated programs for sleep enhancement for the inactive and poor sleeper among the elderly patients.
Objective: The aim of this study was to provide kinematic data on the characteristics of spinal disease patients by comparing and analyzing kinematic variables related to foot balance and gait pattern of spinal disease. Method: The subjects of the study included 40 adult men and 60 adult women who visited the hospital in Busan. Patients who were diagnosed with spinal disease by a physician through X-ray examination were selected as subjects for the diagnosis of vertebral disc herniation, spinal stenosis, spinal disease diagnosed with spinal disease and the general public. Left and right foot pressure and contact area were checked by Gaitview pro meter. X-ray photographs were taken with a Zen-2090 mobile fluoroscopy under physicians' direct participation. One-way ANOVA was performed to compare the differences between the kinematic variables and post-hoc was performed by the Duncan method. Results: The difference in contact area between the left foot and the right foot was $115.30{\pm}14.15cm^2$ in the left side and $124.25{\pm}13.65cm^2$ in the left side in the spinal disease patients. The difference in pressure between the left and right side of the spinal disease patients was wider than that of the general people. Especially, the right side of the spinal disease patients showed a larger area of left foot contact than the general population. Conclusion: Spinal disease patients have wider contact area of the left foot than those of the general population. In the case of right spinal disease, the left foot support area is widened due to pain. In the gait, women showed slightly more posterior body center than men, and the upper body muscle imbalance and immobilization due to the spinal disease caused imbalance of the muscles moving to the lower limb, It was analyzed to inhibit movement.
The present case study has a object to investigate the changes in locomotion patterns of infant with athetoid cerebral palsy would be occured by the program when it is applied with upper extremity weight bearing. The subject has been limited to one infant over one year of age, selected from the patients in the physical therapy clinic, Rehabilitation Center, Taegu University. Subject is normal in the visual and auditory sense, but he is unable to walk on his own Subject weighted 2.9kg at birth and underwent severe postnatal kernicterus, always on the baby-walker at homo. He disliked supine position characteristic in moving in athetoid type before he was under the program. The program was applied 7 months. Each session of the program is composed of 7 stages : (1) prebriefing between the therapist and the parents (2) pretherapy amusement time of the infant (3) warming-up (4) upper extremity weight bearing (5) cooling-down (6) post-therapy amusement time (7) postbriefing. The locomotion of the subject is proved to be influenced by the program. He showed a leftward circular movement as a result of the exercise, reducing the involuntary movement of his head when he was positioned for crawling. Later he proceeded to develop into creeping, crawling, kneeling and finally cruising. In conclusion, it appeared evident that the locomotive abilities of the subject is improved by the program explored in this study. The higher locomotive patterns could be achieved such as crawling, sitting, kneeling and cruising wich enable the upper extremities weight bearing.

This study was conducted to provide basic policy of home care service centered on early discharging patients from general hospitals. This subjects for this study were 291 discharging patients from university hospital in Chon-ju area. The data were collected during the period from July 1, 1993 through July 16, 1993. The measurement tools were developed based on 9 categorized human responses patterns suggested by NANDA and modified by the research team. The collected data were processed with SPSS/PC + frequency, percentage and mean were used for analysis. From the study, the following summerized conclusions have been drown. A. On the basis of this findings, further studies are required to compare home care needs between patients, their family and community health people groups. And also required to develop to information strategies for home care nursing service.
사이버나이프 로봇 방사선 수술 시스템은 환자를 치료하는 동안 환자의 내부 기준 마커를 이용하여 종양의 위치를 실시간으로 추적 할 수 있는 시스템이다. 최근 폐종양 치료의 경우 기준 마커의 삽입 없이 폐종양의 밀도 차이를 이용하여 실시간 종양 추적을 할 수 있는 폐종양추적시스템이 개발되어 치료에 적용되고 있다. 최근 개발되어 국내 최초 도입된 폐종양 추적치료시스템의 위치 추적 정확도의 분석은 동적흉부 팬톰과 GafChromic film을 이용하였다. 폐종양추적시스템을 이용하여 종양의 위치 추적 정확도는 평균 오차 $0.85{\pm}0.22$ mm로 분석되었다. 기준마커 삽입 없이 폐종양추적시스템을 이용하여 폐종양 치료에 매우 유용한 것으로 판단되었다.
정위신체방사선치료(SBRT)에서 환자의 호흡에 대한 정확한 치료위치의 확보는 필수적으로 고려되어야 하며 그 정확성에 관련하여 많은 연구들이 진행되어왔다. 본 연구에서는 실제 호흡에 의한 움직임과 실제 환자 폐의 형태를 고려한 팬텀실험으로 실제 치료에서 일어나는 임상적 상황을 모사함으로 호흡 동조 부피적조절회전 방사선치료(Volumeric Modulated Arc Therapy, VMAT) 기법을 이용한 폐부 SBRT의 정확성을 분석하는 방법을 제시하고자 하였다. SBRT을 받은 폐암 환자의 CT 영상을 기반으로 3D 프린터를 이용하여 치료부위와 유사하게 폐 팬텀을 제작하였고 환자 호흡과 동일하게 움직임을 재현할 수 있도록 $QUASAR^{TM}$ 호흡 동조 구동 팬텀(Modus Medical Devices, London, Canada)에 장착하여 호흡동조 VMAT에서의 2차원 선량 분포를 평가할 수 있는 시스템을 구축하였다. 폐 팬텀은 종양부위를 중심으로 2등분하여 EBT3 필름을 삽입하고 선량분포를 측정할 수 있도록 제작되었다. 비균질 조건에서의 선량계산의 정확성을 확인하기 위하여 균질한 플라스틱 팬텀과 제작된 비균질 폐 팬텀에서 Analytical Anisotropic Algorithm (AAA)와 AcurosXB (AXB) 두가지 알고리즘으로 선량계산을 하여 비교, 분석하였다. 움직임에 대한 치료의 정확성을 평가하기 위하여 호흡동조와 비 호흡동조의 경우, 그리고 움직임이 없는 조건에서 선량분포를 취득하여 치료계획 선량에 대한 감마지표를 분석하였다. 치료부위 GTV에서의 CT number는 실제 환자의 경우 78 HU를 나타내었고 모사된 폐 팬텀의 경우 92 HU를 나타내었다. 팬텀 내 폐 조직부분은 3D프린터로 적층하는 과정에서 격자구조의 형태를 이용하여 구현하였다. 측정된 필름선량은 AAA 알고리즘을 이용한 치료계획 선량에 대하여 움직이는 팬텀에서 호흡동조의 유무에 따라 3%/3 mm 감마지표 조건하에서 각각 88%와 78%의 감마합격률을 나타내었으며, 움직임이 없는 경우 95% 이상의 감마합격률을 보였다. AXB 알고리즘을 적용하였을 경우에는 모든 경우에서 98% 이상의 합격률을 나타내었다. 균질한 플라스틱 팬텀에 대하여 측정하였을 때 두가지 선량계산 알고리즘을 포함한 모든 조건에서 99% 이상의 감마합격률을 나타내었다. 선택된 환자의 호흡 진폭이 비교적 작고 inhale보다는 exhale에 더 오래 머무르는 호흡패턴 때문에 3%/3 mm 감마 기준에서는 호흡에 따른 차이가 거의 나타나지 않은 것으로 이해되었다. 선량계산의 정확성에서는 AAA 알고리즘을 적용하였을 때보다 AXB 알고리즘을 적용하였을 때가 균질과 비균질 환경에서의 선량 분포에 따른 감마 합격률의 차이가 적게 나타남을 확인 할 수 있었다. 본 논문에서는 환자와 유사하게 제작된 폐 팬텀에 실제 환자 호흡 패턴을 연동함으로 새로운 4D 치료선량 분포 검증 방법을 제시하였고 보다 사실적인 선량분포를 반영한 개별 환자 치료의 정확성 검증이 가능할 것으로 평가되었다.
Introduction : Orthostatic tremor develops in the legs while standing up with no weakness, pain or imbalance in the leg and the tremor is characteristically not observed when walking. However there have been some confusions about orthostatic tremor in several aspects. For the past ten years, we have observed 4 patients with orthostatic tremor. In each case tests were performed to investigate the following three important areas of inquiry about orthostatic tremor. Firstly, whether this disorder is an independent diagnostic entity or a variant of essential tremor. Secondly, whether the progress of this disorder is specifically related with standing posture. Lastly, the nature of the pathophysiologic mechanism behind the appearance of the tremor when standing after the lapse of a certain latent period and its disappearance upon the commencement of walking. Methods : Our 4 cases of orthostatic tremor were studied clinically, electrophysiologically, and pharmacologically. Electrophysiological tests included tremor spectrum test and electromyography. Results : We observed the presence of this tremor in several other tonic postures, as well as its absence, in a vertically lifted position from all our cases. Our cases registered a variable tremor frequency between 5 and 12 Hz according to the tremor spectrum test and EMG. Furthermore all our 4 cases demonstrated patterns of both synchronous EMG activity and alternating EMG activity at various times in homologous muscles of both legs. Orthostatic tremor was improved significantly with propranolol as well as clonazepam. Conclusions : From the results of our study we drew the following conclusions. It is probable that orthostatic tremor is simply a variant of essential tremor rather than being an independent diagnostic entity and that in most cases its development is specifically related with muscle contraction rather than merely with the act of standing. Furthermore we discovered a clue in the previously described neural control mechanism that the nuclear bag fibers in the muscle spindle have lag time of several seconds in their response to muscle strength and that their baseline does not reset fully in rapidly moving muscle. This neural control mechanism could offer sufficient explanation for the phenomena of tremor appearance when standing and disappearance when walking in orthostatic tremor.
