뇌졸중의 운동신경기능 회복 (Motor Recovery in Stroke Patients)

  • Stroke is a leading cause of chronic physical disability. The recent randomized controlled trials have that motor function of chronic stroke survivors could be improved through physical or pharmacologic intervention in the stroke rehabilitation setting. In addition, several functional neuroimaging techniques have recently developed, it is available to study the functional topography of sensorimotor area of the brain. However, the mechanisms involved in motor recovery after stroke, are still poorly understood. Four motor recovery mechanisms have been suggested, such as reorganization into areas adjacent to the injured primary motor cortex (M1), unmasking of the motor pathway from the unaffected motor cortex to the affected hand, attribution of secondary motor areas, and recovery of the damaged contralateral corticospinal tract. Understanding the motor recovery mechanisms would provide neurorehabilitation specialists with more information to allow for precise prognosis and therapeutic strategies based on the scientific evidence; this may help promote recovery of motor function. This review introduces several methodologies for neuroimaging techniques and discusses theoretical issues that impact interpretation of functional imaging studies of motor recovery after stroke. Perspectives, for future research are presented.

감각통합치료가 뇌성마비 아동의 대근육 운동발달 및 호흡기능에 미치는 영향 (Effect of Sensory Integration Therapy on Gross Motor Development and respiratory Function of Cerebral Palsy Children)

  • The purpose of this study was to examine the effects of sensory integration therapy (SIT) on gross motor development and respiratory function of cerebral palsy children. The design of this study was one-group pre-and post-test design. Subjects of the study were arbitrarily chosen based on predetermined selection criteria among the cerebral palsy children who were treated as out-patients at one rehabilitation hospital in Kyunggi-do. The study was conducted between early April and late July in 2000. Twelve children were in the experimental group. A five-step SIT program was devised from a combination of SIT programs suggested by Ayres(1985) and Fink(1989), and an author-designed SIT program for cerebral palsy children. The experimental group was subjected to 20 to 30 minutes of SIT per session, two sessions a week for ten-week period. Collected data were statistically analyzed by SPSS PC for Wilcoxon signed rank test, and paired t-test. The results were as follows: 1. In gross motor development, post-experimental gross motor scores were higher compared to pre-experimental scores with statistical significance. 2. In respiratory function, post-experimental forced capacity vital scores were higher compared to pre-experimental scores with statistical significance. In conclusion, SIT was found to be effective in gross motor development and respiratory function. But, for the more effectiveness of SIT on gross motor development and respiratory function, further studies employing longer-time experiments are recommended.

손 재활 로봇의 적용이 만성 뇌졸중 환자의 손 기능 향상에 미치는 영향 (Effect of Robot-Assisted Hand Rehabilitation on Hand Function in Chronic Stroke Patients)

  • The purpose of this study was to investigate effect of robot-assisted hand rehabilitation(Amadeo(R)) on hand motor function in chronic stroke patients. This study used a single-subject experimental design with multiple baselines across individuals. Three chronic stroke survivors with mild to sever motor impairment took part in study. Each participants had 2 weeks interval of starting intervention. Participants received robot-assisted therapy(45min/session. 3session/wk for 6wks). Finger active range of motion(AROM) was assessed by Range of Assessment program in Amadeo(R), and test-retest reliability was verified using Pearson correlation analysis. To investigate effect of Amadeo(R), finger AROM was measured immediately after each sessions and Fugl-Meyer Assessment of Upper extremity, Motor Activity Log, Nine hole peg board test and Jebsen-Taylor hand motor function test were assessed at pre-post intervention. Results were analyzed by visual analysis and comparison of pre-post tests. The test-retest reliability of Range of Assessment was good(r=.99). After robot-assisted therapy, finger AROM of participant 1, 2, and 3 was respectively improved by 18%, 3.6%, and 6% each. Hand motor function of participant 1, 3 was improved on all four tests, but not effect in participant 2. Robot-assisted hand rehabilitation could improve finger AROM and effect on hand motor function in chronic stroke patients.

가상현실 프로그램이 경직성 뇌성마비 아동의 대동작 기능 및 균형에 미치는 영향 (Effects of Virtual Reality Based Exercise Program on Gross Motor Function and Balance of Children with Spastic Cerebral Palsy)

  • Purpose : The purpose of this study is to evaluate the effects of virtual reality based exercise program on gross motor function and balance of children with spastic cerebral palsy. Method : Subjects of this study, among the children who received the diagnosis spastic cerebral palsy, for children total of 8 people have agreed to research. Experimental group 4 people, control group 4 people, was a total of 8 people. Group-specific arbitration method, was applied to Nintendo Wii Fit game (experimental group) and exercise program (control group). Each training courses 30 minutes for 4 weeks, examined the changes in Gross Motor Function Measure(GMFM) and Pediatrics Balance Scale(PBS) ability to examine a total of 4-week course effectively. The intervention were compared by measuring before and after. Result : There were significant improvements in the subscales of the gross motor function and balance test of those who practiced with the Nintendo Wii Fit game, while the control group showed no significant changes. Conclusion : Therefore, the virtual based on exercise is effective in improvement of to improve the gross motor function and balance in children with spastic cerebral palsy.

Effects of Goal-Oriented Functional Tasks on Gross Motor Function and Activities of Daily Living in Children with Cerebral Palsy -A Single Case Study-

  • Purpose: The study sought to determine whether goal achievement, performance, satisfaction, activities of daily living, and gross motor function in children with spastic cerebral palsy are positive affected by goal-oriented functional task training. Methods: In a single case study, two 7-year-old girls diagnosed with spastic cerebral palsy were given functional task training based on individualized goals for 24 60-minute sessions over a period of 6 months. The individualized goals reflected the needs of both children and parents. The Canadian occupational performance measure (COPM) and the goal attainment scale (GAS) were used to measure goal performance and satisfaction before and after intervention. Independence of activities of daily living before and after intervention were assessed using the functional independence measure for children (Wee-FIM). And change of gross motor function were assessed using gross motor function measure (GMFM). Results: Clinically significant changes were observed in COPM and GAS, as well as positive changes in independence of gross motor function and activities of daily living. Conclusion: The findings indicate that goal-oriented functional task training addresses the needs of children with spastic cerebral palsy, which increases treatment satisfaction and has a positive effect on independent activities and participation in daily life.

정신훈련이 편마비환자의 상지기능 향상에 미치는 영향 (Effect of the Mental Practice on the Upper Limb Motor Function Improvement of Hemiplegic Patients)

  • This study was performed to examine the influences of the mental practice to the hemiplegic upper limb motor function improvement. 20 minute neurologic treatment based on the neurophysiological theory, 10 minute activities of daily living training, and 10 minute mental practice 5 times a week were given in turn to the experimental group(N=11). On the other hand 20 minute neurologic treatment, and 10 minute activities of daily living training 5 times a week were given in turn to the control group(N=11). Both Fugl-Meyer Assessment Scale and Manual Function Test were used to evaluate upper limb motor recovery, upper limb motor function and movement ability. And the Motor Activity Log; Amount of Use and Motor Activity Log; Quality of Movement before training, 2 weeks after training, and 4 weeks after training were measured to assess the upper limb motor quantitatively and qualitatively each. The results are as follows. 1) Considering the interactions of the rate of change on the upper limb motor recovery, motor function, movement ability improvement, and qualitative motor improvement in ADL of experimental group and control group, the change rates of experimental group were found to be greater than those of the control group. 2) In experimental group, the higher the achievements were, the better upper motor recovery was.

운동기술훈련이 태아알코올증후군 흰쥐 모델의 소뇌 발달과 운동기능에 미치는 영향 (The Effect of Motor Skill Training on Motor Function and Cerebellar Development After Alcohol Exposure in Neonatal Rats)

  • The purpose of this study was to test that motor skill training enhance motor function and cerebellar development. Using an animal model of fetal alcohol syndrome-which equates peak blood alcohol concentrations across developmental period-critifical periods for the effect of alcohol on body and cerebellar weigh was examined. The effect of motor skill training on motor function and cerebellar development of rat exposed alcohol on postnatal days 4 through 10 were studied. Newborn rats were assigned to one of two groups: (1) Control group (CG), via artificial rearing to milk formula and (2) experimental groups (EG), via 4.5g/kg/day of ethanol in a milk solution. After completion of the treatments, the pups were fostered back to lactating dams, and wearing they were raised in standard caged until they were postnatal 48 days. Rats from experimental group of postnatal treatment then spent 10 days in one of two groups: Experimental group II (EGII) was had got motor skill training (training traverse a set of 6 elevated obstacles) for 4 weeks. Experimental group I (EGI) was not trained. Before sacrificing, the rat got examined two behavioral test, body weigh and cerebellar weigh, then coronal sections were processed. The section was investigated the Purkije cell in the cerebellum using light microscope. The results of this study were as follows. 1. In body weight test, the outcome of alcohol groups were significantly lower than the normal group. 2. In cerebellar weight test, the outcome of EGI were significantly lower than CG and EGII. 3. In motor behavioral test, the outcome of EGI was significantly lower than NG and EGII. 4. In Purkinje cells counting test, the outcome of EGI was significantly lower than the NG and EGII. These result suggest that improved motor function induced by motor skill training after postnatal exposure is associated with dynamically altered expression of Purkinje cells and that is related with cerebellar function. Also, these data can potentially serve as a model for therapeutic intervention.

뇌성마비아 중재에 있어서 신경생리학적 접근기법의 분석 (Analysis of Neurophysiological Approaches to the Intervention of Cerebral palsid child)

  • The main purpose of this paper is to analyze the modes of therapeutic intervention. The emphasis is on the neurophysiological perpective arising out of neurological principles and developmental concepts. The obtained results are as follows. 1. The important hypostheses predicted that the group intervened by neurodevelopmental approach would improve motor function better than the group done by traditional approach and it was proved that neurodevelopmental approach was more effective in gross motor region(P<.01) 2. In the comparison of type of involovement, neurodevelopmental intervention group in spastic type showed improvements in the region of gross motor.(P<.001) 3. In the comparision of degree of disorder, neurodevelopmental intervention group showed improvement of motor function in all the gross motor region in the mild, moderate and severe case.(P<.001) 4. In the comparison of ages of intervention beginning, the group of child between 25-36, 49-60 and 61-72 months(P<.001) intervened by neurodevelopmental approach showed improvements of motor function. 5. In the comparison of intervention duration, neurodevelopmental intervention group showed improvements of motor function in gross motor region according to intervention durstion(P<.001)

보이타 치료가 경직형 양하지 뇌성마비 아동의 대동작 운동기능 및 선택적인 수의적 운동조절에 미치는 영향 (The Effect of Vojta therapy on Gross Motor Function Measure and Selective Voluntary Motor Control in Children with Spastic diplegia)

  • Purpose : The purpose of this study was to investigate the effect of Vojta therapy on Gross Motor Function Measurement(GMFM) and Selective Voluntary Motor Control (SVMC) in children with Spastic diplegia. Methods : During this experiment, the subject of four children diagnosed with spastic diplegia was tested using ABA design for Single-subject Experimental Research Design. The procedure consisted of baseline, intervention and follow-up phase which was held thirty minutes each for three times a week for a total of 24 times. Gross motor function was measured using GMFM and selective voluntary motor control was measured using SCALE. Results : According to this study, the gross motor function and selective voluntary motor control of all subjects were improved from their intervention phase to their baseline phase. During the follow phase which the intervention was removed, the ability that was enhanced during the prior phases was still either maintained or only reduced slightly. Conclusion : The Vojta therapy used on children diagnosed with spastic diplegia was effective on both GMFM and SVMC. In other words, the therapy was effective on coordination. However, this study is difficult to be generalized due to the insufficient number of subject. In further studies, it will be necessary to increase the number of trials with a control group in order to generalize the effectiveness of Vojta therapy.

Effects of task-oriented training for Gross Motor Function Measure, balance and gait function in persons with cerebral palsy

  • Objective: This study was to investigate the effects of Task-oriented training for Gross Motor Function Measure (GMFM), gait and balance function in cerebral palsy. Design: Randomized controlled trials. Methods: Twenty four subjects were recruited by means of a convenience sampling from Kangseo-Gu G rehabilitation center. Subjects were 24 inpatients and were randomly divided into a task-oriented training group and a conventional group. Twelve patients were experimental group who executed the task-oriented training (5 times/wk) for 4 weeks. The task-oriented program mainly focused on the capabilities of independent walking, with the angle of inclination set at 0 degrees and walking at a self-selected comfortable speed. In addition, balance training included the one-legged standing with weight-shifting and task-oriented training. Twelve patients were control group who executed only general conventional therapy (5 times/wk) for 4 weeks. All subjects were evaluated about the motor function, gait and balance function. Subjects have conducted the measured variables, GMFM, GAITRite, PDM Multifunction Force Measuring Plate after treatment. Results: There was statistically significant increase of Gross Motor Function Measure scores of the experimental group and control group after 4 weeks (p<.05). There was statistically significant increase of gait and balance function of the experimental group after 4 weeks of task-oriented training (p<.05). The experimental group showed a significantly improvement in GMFM, gait, and balance compared to the control group (p<0.05). Conclusions: This study proved that task-oriented training after stroke can improve Gross Motor Function Measure, gait and balance. Thus this study can suggest that task-oriented training for gross motor function, gait and balance be effective on the cerebral palsy.
