Vasovagal syncope with mild versus moderate autonomic dysfunction: a 13-year single-center experience

  • Background: An adequate large-scale pediatric cohort based on nationwide administrative data is lacking in Korea. Purpose: This study aimed to differentiate patients with VVS by autonomic dysfunction severity using the composite autonomic severity score (CASS) and compare the clinical manifestations and prognosis between patient subgroups. Methods: We retrospectively reviewed the medical records of 66 VVS patients divided into 3 groups by CASS. To compare the differences between these groups, we analyzed VVS type, triggers, prodromal symptoms, management of syncope, and prognosis between patients with mild versus moderate autonomic dysfunction. Results: Of our 66 patients with VVS, 41 had mild autonomic dysfunction (62.1%) and 25 had moderate autonomic dysfunction (37.9%). We found no significant intergroup differences in age, sex, inducible factor (P=0.172), prodromal symptoms, laboratory findings, head-up tilt test, type of syncope, or prognosis (P=0.154). Conclusion: We found no evidence that autonomic dysfunction degree is affected by VVS characteristics, test findings, parameters, or prognosis; therefore, no further evaluations are needed to classify autonomic dysfunction severity.

측두하악 장애 환자의 교합이개 시간에 관한 연구 (A STUDY ON DISCLUSION TIME OF PATIENT WITH TEMPOROMANDIBULAR DYSFUNCTION)

  • The purpose of this study was to investigate the disclusion time and occlusal pattern during lateral movement in normal group and temporomandibular dysfunction (TMD) group. Twenty dental college students of Chosun University without the abnormal occlusion and temporomandibular dysfunction were selected as a normal group, and twenty slight temporomandibular dysfunction (TMD) group and the ten moderate temporomandibular dysfunction (TMD) group classified according to Helkimo's dysfunction index were selected. Occlusal pattern was classified as canine guided occlusion, group functioned occlusion and the other group during lateral movement and disclusion time in lateral movement was measured using T-Scan system. The result were as follows: 1. The disclusion time according to each group were $1.24{\pm}0.58$ sec in normal $1.60{\pm}0.79$ sec in slight TMD group and $2.29{\pm}0.80$ sec in moderate TMD group. There was statistically significant between normal group and moderate TMD group(P<0.01), slight TMD group and moderate TMD group(P<0.05). 2. The distribution of occlusal pattern in normal group was 62.5% (25 side) in canine guided occlusion, 27.5% (15 side) in group functioned occlusion. 3. The distribution of occlusal pattern in slight TMD group was 45% (18side) in canine guided occlusion, 35% (14 side) in group functioned occlusion and 20% (8side) in others and that in moderate TMD group was 15% (3 side) in canine guided occlusion, 35% (7 side) in group funcconed occlusion and 50% (10 side) in other 4. The disclusion time in normal group was $1.05{\pm}0.59$ sec at canine guided occlusion and $1.53{\pm}0.72$ sec at group functioned occlusion. 5. The disclusion time in slight TMD group was $1.23{\pm}0.75$ sec in canine guided occlusion, $1.50{\pm}0.88$ sec in group functioned occlusion, and $2.61{\pm}0.57$ sec, in the other. There was staistically significant between canine guided occlusion and other(P<0.001)and group functioned occlusion and the other (P<0.05). 6. The disclusion time in moderate TMD group was $1.28{\pm}0.84$ sec in canine guided occlusion, $1.75{\pm}0.58$ sec in group functioned occlusion, and $2.98{\pm}0.08$ sec in the other(P<0.01).

Modelling Pasture-based Automatic Milking System Herds: The Impact of Large Herd on Milk Yield and Economics

  • The aim of this modelling study was to investigate the effect of large herd size (and land areas) on walking distances and milking interval (MI), and their impact on milk yield and economic penalties when 50% of the total diets were provided from home grown feed either as pasture or grazeable complementary forage rotation (CFR) in an automatic milking system (AMS). Twelve scenarios consisting of 3 AMS herds (400, 600, 800 cows), 2 levels of pasture utilisation (current AMS utilisation of 15.0 t dry matter [DM]/ha, termed as 'moderate'; optimum pasture utilisation of 19.7 t DM/ha, termed as 'high') and 2 rates of incorporation of grazeable complementary forage system (CFS: 0, 30%; CFS = 65% farm is CFR and 35% of farm is pasture) were investigated. Walking distances, energy loss due to walking, MI, reduction in milk yield and income loss were calculated for each treatment based on information available in the literature. With moderate pasture utilisation and 0% CFR, increasing the herd size from 400 to 800 cows resulted in an increase in total walking distances between the parlour and the paddock from 3.5 to 6.3 km. Consequently, MI increased from 15.2 to 16.4 h with increased herd size from 400 to 800 cows. High pasture utilisation (allowing for an increased stocking density) reduced the total walking distances up to 1 km, thus reduced the MI by up to 0.5 h compared to the moderate pasture, 800 cow herd combination. The high pasture utilisation combined with 30% of the farm in CFR in the farm reduced the total walking distances by up to 1.7 km and MI by up to 0.8 h compared to the moderate pasture and 800 cow herd combination. For moderate pasture utilisation, increasing the herd size from 400 to 800 cows resulted in more dramatic milk yield penalty as yield increasing from c.f. 2.6 and 5.1 kg/cow/d respectively, which incurred a loss of up to $AU 1.9/cow/d. Milk yield losses of 0.61 kg and 0.25 kg for every km increase in total walking distance (voluntary return trip from parlour to paddock) and every one hour increase in MI, respectively. The high pasture utilisation combined with 30% of the farm in CFR in the farm increased milk yield by up to 1.5 kg/cow/d, thereby reducing loss by up to $0.5/cow/d (c.f. the moderate pasture and 800 cow herd scenario). Thus, it was concluded that the successful integration of grazeable CFS with pasture has the potential to improve financial performance compared to the pasture only, large herd, AMS.

  • To maintain a predominantly pasture-based system, the large herd milked by automatic milking rotary would be required to walk significant distances. Walking distances of greater than 1-km are associated with an increased incidence of undesirably long milking intervals and reduced milk yield. Complementary forages can be incorporated into pasture-based systems to lift total home grown feed in a given area, thus potentially 'concentrating' feed closer to the dairy. The aim of this modelling study was to investigate the total land area required and associated walking distance for large automatic milking system (AMS) herds when incorporating complementary forage rotations (CFR) into the system. Thirty-six scenarios consisting of 3 AMS herds (400, 600, 800 cows), 2 levels of pasture utilisation (current AMS utilisation of 15.0 t dry matter [DM]/ha, termed as moderate; optimum pasture utilisation of 19.7 t DM/ha, termed as high) and 6 rates of replacement of each of these pastures by grazeable CFR (0%, 10%, 20%, 30%, 40%, 50%) were investigated. Results showed that AMS cows were required to walk greater than 1-km when the farm area was greater than 86 ha. Insufficient pasture could be produced within a 1 km distance (i.e. 86 ha land) with home-grown feed (HGF) providing 43%, 29%, and 22% of the metabolisable energy (ME) required by 400, 600, and 800 cows, respectively from pastures. Introduction of pasture (moderate): CFR in AMS at a ratio of 80:20 can feed a 400 cow AMS herd, and can supply 42% and 31% of the ME requirements for 600 and 800 cows, respectively with pasture (moderate): CFR at 50:50 levels. In contrast to moderate pasture, 400 cows can be managed on high pasture utilisation (provided 57% of the total ME requirements). However, similar to the scenarios conducted with moderate pasture, there was insufficient feed produced within 1-km distance of the dairy for 600 or 800 cows. An 800 cow herd required 140 and 130 ha on moderate and high pasture-based AMS system, respectively with the introduction of pasture: CFR at a ratio of 50:50. Given the impact of increasing land area past 86 ha on walking distance, cow numbers could be increased by purchasing feed from off the milking platform and/or using the land outside 1-km distance for conserved feed. However, this warrants further investigations into risk analyses of different management options including development of an innovative system to manage large herds in an AMS farming system.

Histochemical Study on Glycosaminoglycans of Esophageal Mucous Cells in Agramus agramus, Inimicus japonicus, Epinephelus chlorostigma and Helico-lenus dactylopterus

  • This experiment was performed to study the structure of esophageal mucousa and the histochemical properties of glycosaminoglycans of esophageal mucous cells in four teleostean species, i.e., Agramus agramus, Inimicus japonicus, Epinephelus chlorostigma and Helicolenus dactylopterus. To observe the structure of eosphageal mucosa, hematoxylin-eosin(H-E) staining was used. The glycosaminoglycans was stained with PAS(periodic acid schiff), alcain blue(AB) pH 2.5, AB pH 1.0, aldehyde fuchsin(AF) pH 1.7, AF pH 1.0, AB pH 2.5-PAS, AB pH 1.0-PAS and AF pH 1.7-alcian blue pH 2.5. As for the amount and histochemical composition of glycosaminglycans in Agramus agramus, most of mucous secreting columnar cell and mucous cells contain large and moderate amount of neutral glycosaminoglycans. A few of mucous cells having small amount of neutral glycosamino-glycans and minimal amount of sulfated glycosamino-glycans. In Inimicus japonicus, the esophageal mucous cells were composed of most of medium sized and large mucous cells with moderate amount of neutral glycosaminoglycanonly only, a few of medium sized and large mucous cells and most of small mucous cells with considerable amount of neutral glycosaminoglycans and minimal to small amount of nonsulfated glycosaminoglycans, and a feq of small mucous cells with small amount of neutral glycosaminoglycans and minimal amount of sulfated glycosaminoglycans. In Epinephelus chlorostigma, most of medium sized and large mucous cells were mixed small amount of neutral glycosaminoglycans with sulfated glycosaminoglycans, a few of which were contained with moderate or considerable amount of neutral glycosaminoglycans with sulfated glycosaminoglycans, while most of small mucous cells containing considerable amount of neutral glycosaminoglycans and small to moderate or considerable to minimal amount of nonsulfated glycosaminoglycans(sialomucin) a few of which containing a mixture of considerable amount of neutral glycosaminoglycans and considerable amount of nonsulfated glycosaminoglycans(sialomucin)only. In Helicolenus dactylopterus, most of medium sized and large mucous cells, mixing with moderate to considerable amount of neutral glycosaminoglycans and small to moderate amount of sulfated glycosaminoglycans, while most of small mucous cells with considerable amount of nonsulfated glycosaminoglycans (sialomucin), a few of which having a mixture of considerable amount of neutral glycosaminoglycans and considerable or small amount of nonsulfated glycosaminoglycans(sialomucin).

Safety and efficacy of target controlled infusion administration of propofol and remifentanil for moderate sedation in non-hospital dental practice

  • Background: Fearful and anxious patients who find dental treatment intolerable without sedative and analgesic support may benefit from moderate sedation. Target controlled infusion (TCI) pumps are superior to bolus injection in maintaining low plasma and effect-site concentration variability, resulting in stable, steady-state drug concentrations. We evaluated the safety and efficacy of moderate sedation with remifentanil and propofol using TCI pumps in non-hospital dental settings. Methods: A prospective chart review was conducted on 101 patients sedated with propofol and remifentanil using TCI pumps. The charts were completed at two oral surgeons and one general dentist's office over 6 months. Hypoxia, hypotension, bradycardia, and over-sedation were considered adverse events and were collected using Tracking and Reporting Outcomes of Procedural Sedation (TROOPS). Furthermore, patient recovery time, sedation length, drug dose, and patient satisfaction questionnaires were used to measure sedation effectiveness. Results: Of the 101 reviewed sedation charts, 54 were of men, and 47 were of women. The mean age of the patients was 40.5 ±18.7 years, and their mean BMI was 25.6 ± 4.4. The patients did not experience hypoxia, bradycardia, and hypotension during the 4694 min of sedation. The average minimum Mean Arterial Pressure (MAP) and heartbeats were 75.1 mmHg and 60.4 bpm, respectively. 98% of patients agreed that the sedation technique met their needs in reducing their anxiety, and 99% agreed that they were satisfied with the sedation 24 hours later. The average sedation time was 46.9 ± 55.6 min, and the average recovery time was 12.4 ± 4.4 min. Remifentanil and propofol had mean initial effect-site concentration doses of 0.96 µ/.ml and 1.0 ng/ml respectively. The overall total amount of drug administered was significantly higher in longer sedation procedures compared to shorter ones, while the infusion rate decreased as the procedural stimulus decreased. Conclusion: According to the results of this study, no patients experienced adverse events during sedation, and all patients were kept at a moderate sedation level for a wide range of sedation times and differing procedures. The results showed that TCI pumps are safe and effective for administering propofol and remifentanil for moderate sedation in dentistry.

청소년의 생활습관관리를 위한 신체활동과 건강행위와의 관련성 연구 (Relationship between Health Behaviors and Physical Activity for Adolescents' Life Care)

  • 본 연구는 제13차(2017년) 청소년건강행태온라인조사 통계자료를 이용하여 청소년의 건강행위와 신체 활동과의 관련성을 알아보는 이차분석연구이다. 연구 대상자는 중1~고3 청소년 62,276명이다. 신체활동은 중강도 신체활동 및 고강도 신체활동 항목을 활용하였고, 건강행위는 흡연, 음주, 식생활, 좌식행동, 수면 시간 변수를 활용하였다. 자료는 복합표본 분석방법을 사용하였다. 연구대상자의 건강행위 특성에 따른 신체활동 비교는 카이제곱 검정을 하였으며, 건강행위와 신체활동과의 관련성을 분석하기 위해 인구학적 특성을 보정한 후 다항 로지스틱 회귀분석을 실시하였다. 연구결과 현재 흡연, 현재 음주의 경우 중강도 및 고강도 신체활동이 증가하였다. 식생활의 경우 1일 1회 이상 과일섭취, 매일 3회 이상 채소섭취, 주 3회 이상 단맛음료섭취를 하는 경우 중강도 및 고강도 신체활동이 증가하였다. 주 5일 이상 아침식사는 높은 중강도 신체활동과 관련이 있었으나 고강도 신체활동과는 관련이 없었다. 2시간 이상 앉아서 보내는 시간이 증가할수록 중강도 및 고강도 신체활동이 감소하였다. 7시간 미만 수면을 취하는 경우 중강도 신체활동이 증가한 반면, 고강도 신체활동은 감소하였다. 본 연구결과 청소년의 건강행위와 신체활동은 서로 상호관련성이 있으므로, 청소년 신체활동 및 건강한 생활습관 증진을 위한 중재 개발 시 개별적인 접근보다는 다각적 측면에서의 통합적 접근이 필요하다고 여겨진다.

  • The sea water resistance of cement and concrete must be considered when it is used for construction on the seashore of in the ocean. The concrete specimens using seven type of cements such as ordinary Portland cement, moderate heat Portland cement, sulfate resistance Portland cement, type A. B. C Portland blastfurnace slag cement and Portland flyash cement were immersed for 10 years in seawater in Kunsan. This study proved that moderate heat Portland cement, sulfate resistance Portland cement, type A Portland blastfurnace slag cement had higher resistance for seawater.

  • Ligand field analyses for tris(biuret)chromium(III) chloride and hexaureachromium(III) bromide were performed and compared to understand the ligand field properties of both ligands. The optimized eσO and eπO values indicate that coordinated oxygen atom in biuret ligand is a moderate s- and strong p-donor, while that in urea ligand is a weak σ- and moderate π-donor. The electronic structures of those two complexes are quite different and they were well accounted by inclusion of an anisotropic p bonding.

  • Conducting solid samples are successfully analyzed with the spark ablation combined to the moderate power (500W) Helium Microwave Induced Plasma (He MIP). The relative standard deviations are in the range of 3-10% and the detection limits are around 50 ${\mu}$g $g^-1$. These values are higher than those of Ar MIP or Ar Inductively Coupled Plasma. Spark ablated particles are examined to investigate the analytical characteristics of the system.
