A STUDY ON AN INTERPRETATION OF THE ORTHOPANTOMOGRAPH IN THE MIXED DENTITION (Orthopantomograph에 의(依)한 혼합치열상(混合齒列像)의 해석(解析)에 관(關)한 연구(硏究))

  • Plaster models were constructed and orthopantomographs were taken for 86 male and 70 female primary school pupils, whose upper and lower permanent 4 incisors and 1st molars were completely erupted without crowding; whose deciduous canines and molars were found almost uniformly even without any visible tooth fractures, dental caries or restorations on proximal surfaces of the teeth. Certain reference points on the orthophantomograph were set up and measured and the values were compared with actual or predicted values from the models. The following results were obtained: 1. In regards to available space, the values from the orthopantomograph were greater than the values from the models by a mean of 3.24% on the upper and 10.06% on the lower for males; 3.05% on the upper and 10.01% on the lower for females. 2. In regards to total mesiodistal widths of permanent canine, 1st and 2nd premolars, the values from the orthopantomograph were greater than the presumed values based on the size of lower permanent 4 incisors from the models by a mean of 18.50% on the upper and 24.09% on the lower for males; 14.54 on the upper and 20.51% on the lower for females. 3. Comparing the magnified values of total mesiodistal widths of permanent canine, 1st and 2nd premolars with those of available space, the regression constants of regression equation (Y = a + bX) between them were a=3.2336, b=0.6533 on the upper and a=5.0138, b=0.3290 on the lower for males; a=2.5994, b=0.6521 on the upper and a=3.0113, b=0.6586 on the lower for females. 4. The correlation coefficients between the magnified values of available space and permanent canine, 1st and 2nd premolars were moderately positive as 0.6474 in the upper and 0.505 on the lower for males; 0.6493 on the upper and 0.6183 on the lower for females. 5. In regards to magnified values of the available space from the orthopantomographs there were no significant difference between sexes, (P>0.05) but of the total mesiodistal widths of permanent canine, 1st and 2nd premolars, a significant difference between sexes was found.(p<0.01).

A THEMATIC SURVEY ON THE REPORTS PUBLISHED IN THE JOURNAL OF THE KOREAN ACADEMY OF PEDIATRIC DENTISTRY (역대 대한소아치과학회지 게재논문의 분야별 분포에 대한 조사)

  • Since founded in 1959, it's well known that the KAPD has pioneered in the researches and clinical aspects of pediatric dentistry in Korea. It's official journal, the Journal of the KAPD, was first published in 1974 and has pressed total 956 articles up to now(March, 2001). In this study, all the articles pressed in this journal have been surveyed, focussing in their main theme, their chronological and thematic distribution. The thematic classification was made with the reference of the previous studies and renowned textbooks in pediatric dentistry. And we obtained the results as follows: 1. The researches on dental materials and dental equipments have shown continuous increase throughout the period. 2. The researches on dental caries, caries prevention and systemic disorders have occupied relatively high proportion consistently. 3. The researches on malocclusions and cysts/minor surgery have shown increasing tendency in the second period, but are decreasing in the third period. 4. The researches on craniofacial growth/development, tooth development/eruption, developmental disorders of teeth, management of eruption space have shown decreasing tendency. 5. The researches on behavioral research, oral habits, occlusion of primary-mixed dentition have shown very low proportion, reaching no more than 1% throughout the period.

MANAGEMENT OF MULTIPLE INFRAOCCLUDED PRIMARY MOLARS WITHOUT PERMANENT SUCCESSORS: A CASE REPORT (계승치 결손을 동반한 다발성 저위교합 유착 유구치의 관리)

  • The infraocclusion usually occurs in the mixed-dentition stage, and it is commonly accepted that the cause of the infraocclusion is ankylosis. The treatment options for patient with infraocclusion of primary molars are observation, restoration or surgical removal of the affected teeth. If the successors are present, most of the infraoccluded and ankylosed primary molars may occur normally. However, when the permanent successors are absent and the progression of infraocclusion is found, affected teeth may need to be extracted. In the case of infraocclusion which can cause vertical alveolar defect due to ankylosis, extraction before growth spurt should be performed for the future prosthetic treatment. A six-year-old female had the ankylosis and infraocclusion of multiple primary molars and congenital missing of premolars. The affected primary molars were extracted before growth spurt to avoid a significant vertical ridge defect and to promote the vertical development of alveolar bone, and the result was observed for many years. The purpose of this report is to report the management of multiple infraoccluded primary molars without permanent successors in a young patient.

A STUDY ON THE DENTAL MATURATION IN CHILDREN WITH SKELETAL ANTERIOR CROSSBITE (골격성 전치부 반대교합 아동의 치아성숙도에 관한 연구)

  • It was easy to find that children of a skeletal anterior crossbite in the early mixed dentition period showed a stark difference in the dental maturity between their maxillary and mandibular teeth, if they have stronger physical characteristics. If the difference of dental age between maxillary and mandibular teeth which can be identified via panoramic radiographs may serve as an early sign of class III malocclusion, this is considered valuable as a tool of early detection diagnosis. We obtained lateral cephalometric radiographs, panoramic radiographs, working model and clinical images of patients of Hellman dental age IIA and IIC who visited the department of pediatric dentistry, Pusan National University Dental Hospital and examined them to select 50 patents for normal occlusion group and skeletal anterior crossbite group, respectively. Their panoramic radiographs were used for the Demirjian's method to figure out dental ages of maxillary and mandibular teeth of each group and the eruption rate of the first molars. Their differences are as follows: 1. In both groups, the dental ages from Demirjian's method were advanced than the chronological ages. No sexual dimorphism was detected for the chronological or dental age in either group (p>0.05). 2. The difference of dental age of maxillary and mandibular teeth between the normal occlusion group and crossbite group was 0.22 and 0.69 years, respectively, with a higher difference in crossbite group(p<0.05). 3. Compared to the normal occlusion group, the crossbite group showed a higher difference in the eruption rate between maxillary and mandibular first molar(p<0.05).

INTENTIONAL REPLANTATION OF THE CROWN-ROOT FRACTURED TOOTH: A CASE REPORT (치관-치근 파절된 치아의 의도적 재식술 치험례)

  • As the vertical fracture occurs at the various locations following the long axis of a root, treatment method of crown-root fractured anterior teeth is decided according to the depth. If the fracture line is close to the crown, gingivectomy, orthodontic - forced eruption or surgical extrusion of apical fragment could be done. If the line is over 1/3 length of the root, the prognosis is poor and extraction is usually undertaken. However, extraction of maxillary incisor at growing children causes many complications such as esthetic, phonetic problem and alveolar bone resorption. Therefore, preservation of tooth is the highest priority. Recently, intentional replantation with composite resin could be considered as alternative treatment of crown-root fractured anterior tooth. This report presents a patient in mixed dentition with deep vertical crown-root fracture of the maxillary permanent central incisors by trauma. Intentional replantation of the fractured teeth was performed using composite resin. After 2 years, specific clinical symptom has not been found and the patient was satisfied of esthetic result. This method suggests the new technique to preserve a tooth as an alternative to extraction, although it is technically sensitive and the reports of long-term prognosis is insufficient.

CONSERVATIVE TREATMENT OF A UPPER CENTRAL INCISOR WITH POOR PROGNOSIS (예후가 불량한 상악 중절치의 유지)

  • The incisors function as instruments for biting and cutting food during mastication. They also support the lips and face and maintain vertical dimension. In addition, they contribute to overall normal arch appearance. They play important role during the articulation of speech and assist in guiding jaw closure. Extraction and space maintenance are the most common treatment for a tooth with poor prognosis. However, in the mixed dentition, extraction of the upper permanent incisors results in many complications, such as resorption of alveolar bone, poor esthetics, pronunciation, and mastication. Considering these various roles of incisors in oral cavity, approach for traumatized incisors, even the ones with poor prognosis, should be considered first prior to simple extraction. The dentist must take into account the age of the patient, growth potential, occlusion, oral hygiene status, economic status and motivation towards dental health in addition to patient compliance. In this case, although the prognosis was predicted to be unfavorable due to short root and mobility, we could save the central incisor using conservative treatment, reposition by orthodontic appliance instead of extraction.

STUDY OF INCIPIENT ENAMEL CARIES USING A DIGITAL ILLUMINATION FIBER-OPTIC TRANSILLUMINATION AND CONFOCAL LASER SCANNING MICROSCOPE (Digital Imaging Fiber-Optic Trans-Illuminational과 Confocal Laser Scanning Microscope를 이용한 초기 법랑질 우식증 연구)

  • The purpose of this study were to evaluate the efficacy of the newly developed Digital Imaging Fiber-Optic Trans-illumination (DIFOTI) system in detecting carious lesions in vivo as gold standard with confocal laser scanning microscopy and compared the efficacy of traditional radiography and DIFOTI system in vito as gold standard with confocal laser scanning microscopy, too. For the in vivo study, the subject pool consisted of 23 grammar school age patients just prior to entering the mixed dentition phase Each patient was given a DIFOTI examination of the anterior and posterior teeth. During $6{\sim}8$ months, the naturally expire primary teeth were collected and the efficacy of DIFOTI system was compared with confocal laser scanning microscopy. For in vitro study, 40 primary teeth were collected and decalcified by Carbopol decalcification solution for 1, 2, 4 and 8 days. Every experiment period, all teeth were DIFOTI examined and sectioned to take an image of confocal laser scanning microscopy Sensitivity and specificity were calculated from the result of DIFOTI examine and confocal laser scanning microscopy analysis. The results are as follows : 1. From the in vivo study, the sensitivity of DIFOTI examine was 0. 61 and specificity was 0.63. 2. From the in vivo study, the sensitivity of DIFOTI examine was 0.71 and specificity was 0.75.

STUDY ON ADAPTABILITY OF RUBBER DAM CLAMPS ON PRIMARY SECOND MOLARS IN KOREAN CHILDREN (한국인 소아에서 제2유구치에 대한 러버댐 클램프의 적합도에 관한 연구)

  • The purpose of this study was to investigate the mesiodistal widths at the cervical level of primary second molars in Korean children, and to compare them with commercial rubber dam clamps commonly used in pediatric dentistry. Dental casts of 115 primary and mixed dentition children were studied. Cervical mesiodistal width (C-MD) was measured at the clinical cervical level of each primary second molar from buccal and lingual sides using a digital caliper (Absolute, Mitutoyo, Kawasaki, Japan). The data were compared with mesiodistal widths of rubber dam clamps #203/204, #10/11, and #205 (Dentech, Japan). C-MDs of primary mandibular second molars were larger than those of primary maxillary second molars, and C-MDs at buccal sides were larger than those of lingual sides. All C-MDs showed statistically significant discrepancies to corresponding widths on clamps (p < 0.05). However the amount of discrepancy was mild in maxillary teeth, while up to 1 mm of discrepancy was shown in mandibular teeth. In conclusion, C-MDs measured in this study imply a relatively fair fit of #10/#11 or #203/#204 clamps on primary maxillary second molars, while suggesting our need for a better clamp with proper size for primary mandibular second molars.

Changes of symphysis morphology after chincup treatment (이모장치 착용 후 하악 이부의 헝태변화)

  • Although it is well known that the chincup, used to correct a skeletal class III malocclusion in growing children, reduce the mandibular prognathism by arresting the growth of the mandibular length and rotating the mandible posteroinferiorly, the majority of the studies about chincup is focused on condylar head that plays an Important role in mandibular growth. The aim of this study was to evaluate the morphologic change of the mandibular symphysis where extraoral force is applied directly during chincup treatment. The data lot this study were obtained from lateral cephalometric radiographs of 62 growing children(chincup group:32, control group:30) with mixed dentition who had been accepted lot the orthodontic treatment at Chonbuk National University Dental Hospital. The results were as follows : 1. Symphysis height was increased both in chincup therapy group and control group during treatment. Symphysis depth was decreased or maintained the initial values in chin cup therapy group, whereas increased in control group. Posterior symphysis depth was decreased both in chin cup therapy group and control group, but anterior svmphysis detph was increased in control group, whereas decreased in chincup therapy group. 2. Chin depth and chin curvature were increased in control group, whereas maintained or decreased in chincup therapy group during treatment. Chin angle, menton ang1e and symphysis angle were decreased in control group, whereas increased in chincup therapy group. It suggested that bone deposition in pogonion area that occur normally with mandibular growth was supressed by direct contact of chincup. 3. When growing children wear chincup, symphysis morphology was maintained due to inhibition of forward growth at mandibular symphysis. It may be due to the suppression of bone deposition in anterior part of symphysis.

THE EARLY EFFECTS OF SODIUM FLUORIDE ON THE RAPID PALATAL EXPANSION IN GROWING DOGS (유성견의 정중구개봉합 급속확대시 투여된 불화나트륨의 초기 효과에 관한 연구)

  • The purpose of this study was to observe the effects of sodium fluoride on the bony repair and regeneration processes after the rapid palatal expansion in the growing dogs. Eighteen dogs were divided into experimental and control groups. They were in the late mixed dentition. The rapid Palatal expansion was undertaken in all the animals($180^{\circ}$ turn/day) for ten days. The animals were sacrificed on 0, 15 and 45 days after the finish of expansion. One mg NaF/kg of body weight/day were given orally to the experimental group. Blood samples were drawn before and after expansion and the se겨m calcium, phosphate and alkaline phosphatase level were measured. The undecalcified bone section of midpalatal suture area was made, and observed under the light microscopy The results were as follows ; 1. The day after expansion, the infiltration of inflammatory cells were prominent and the new bone formation started at the edges of the two palatal plates bodering the midpalatal suture in both groups. Especially, the newly formed osteoid were very extensive and the osteoblasts lining the osteoid were very active in the experimental group. 2. At fifteen days after expansion, the active osteoblasts lining the osteold at the surface of trabecular bony spicules and active new bone formation were observed in the both groups. However, the cellular activity and new bone formation were more prominent In the experimental group. 3. At forty five days after expansion, the continuous osteoid and new bone formation and active osteoblasts were observed in the experimental group. But these phenomena were not observed in the control group. In the control group, the numerous osteoclasts were adjacent midpalatal suture and the bony remodeling process was begun. The serum alkaline phosphatase level was maintained highly in the experimental group, but decreased in the control. According to the above results, the author reached the conclusion that sodium fluoride has the stimulation effects on the osteoid production of the osteoblasts during the healing process after the rapid Palatal expansion more continuously.
