99mTc-DTPA 신장 동적 검사(Renal Dynamic Scan) 시 동위원소 용량 변화와 Matrix Size 변경에 따른 사구체 여과율(Glomerular Filtration Rate, GFR) 수치 변화 비교 (Comparison of Activity Capacity Change and GFR Value Change According to Matrix Size during 99mTc-DTPA Renal Dynamic Scan)

  • 사구체 여과율은 신 기능의 평가와 신 질환의 경과 관찰에 중요한 지표다. 현재 임상에서 사구체 여과율을 측정하는 방법은 크게 혈청 크레아티닌 수치와 99mTc-DTPA을 이용해서 계산하는 검사가 유용하게 이용되고 있다. Gates 공식이 발표된 이후, 99mTc-DTPA Renal Dynamic Scan 검사시 이를 적용해 GFR을 감마카메라 장비를 이용하여 측정하고 있다. 본 논문의 목적은 Gates 공식을 적용해 사구체 여과율을 측정할 때 동위원소의 용량 변화, Matrix Size 변경에 따른 사구체 여과율을 분석해 보고자 한다. 본원에서 99mTc-DTPA Renal Dynamic Scan 검사를 받은 성인 환자 5명 (환자나이 = 62 ± 5, 남자 3명, 여자 2명)의 결과를 분석하였다. 99mTc-DTPA 15 mCi를 환자의 정맥에 순간 주사 후 21분간 동적 영상을 획득하였다. Activity, Matrix Size 변화에 따라 사구체 여과율을 평가하기 위해 2-3분 구간의 양쪽 신장과 조직에 관심 영역을 설정 후 총 계수를 측정하였다. Detector와 Table 간의 거리는 30cm로 유지하였고, 동위원소의 용량 변화 평가를 위해 Pre Syringe(PR)의 용량을 15, 20, 25, 30 mCi로 설정하였고, Post Syringe(PO)의 용량을 1, 5, 10, 15 mCi로 각각 설정하였다. 그리고 각각의 Matrix size를 32 × 32, 64 × 64, 128 × 128, 256 × 256, 512 × 512, 1024 × 1024로 변경하여 값을 비교 평가하였다. Matrix Size에서 동위원소의 용량이 증가할수록 사구체 여과율의 차이는 최대 52.95%에서 최소 16.67%로 점차 감소하였다. Matrix size 변화에 따른 사구체 여과율 값은 128에서 256, 256에서 512, 512에서 1024로 변경 시 2.4%, 0.2%, 0.2%로 유사하게 나타났으나, 32에서 64, 64에서 128로 변경 시 54.3 %, 39.43%로 GFR 값의 차이가 발생하였다. 마지막으로, 현재 사용 중인 256 Matrix, Pre syringe 15 mCi의 GFR 값을 기준으로 32 Matrix, Pre Syringe 15 mCi, Post Syringe 1 mCi 조건에서 82%로 가장 큰 차이가 발생하였고, 64 Matrix, Pre Syringe 30 mCi, Post Syringe 15 mCi의 조건에서 0.2%로 유사한 결과 값을 나타내었다. 본 논문을 통해 99mTc-DTPA Renal Dynamic Scan에서 Gates 공식을 이용해 사구체 여과율을 측정할 때, 동위원소의 용량과 Matrix Size 변화에 따라 사구체 여과율 측정에 영향이 있음을 확인하였다. 그러므로 각 병원에서 이를 적용해 GFR을 측정할 때 적절한 Parameter를 적용해서 검사의 유용성을 높아야 한다고 생각한다.

온실의 잉여 태양에너지 회수용 FCU 소요대수 검토 (Estimation of the Required Number of Fan Coil Unit for Surplus Solar Energy Recovery of Greenhouse)

  • 본 연구에서는 이미 보고된 잉여 태양에너지 관련 연구결과와 현재 현장에 설치되어 있는 냉난방용 FCU 현황을 개략적으로 검토한 후, 잉여 태양에너지 회수에 필요한 FCU의 소요대수 결정 방법을 개략적으로 제시하여 앞으로 이 분야의 연구자 및 기술자들에게 기조자료를 제시할 목적으로 연구를 수행하였다. 실험기간 동안 최대, 평균 및 최저 외기온은 각각 $28.2^{\circ}C$, $4.4^{\circ}C$$-11.5^{\circ}C$정도였다. 온실 밖의 수평면 일사량은 $0.8{\sim}20.5MJ{\cdot}m^{-2}$로 정도의 범위였으며, 평균 및 총 일사량은 $10.8MJ{\cdot}m^{-2}$$1,187.5MJ{\cdot}m^{-2}$으로 나타났다. 그리고 주간동안 온실 내의 평균기온과 상대습도는 각각 18.8~45.5 및 53.5~77.5%정도였다. 실험기간 동안 온실로부터 회수한 총 잉여 태양에너지는 6,613.4MJ정도로서 총 난방에너지인 98,600.2MJ 약 6.7%정도를 보충할 수 있을 것으로 나타났다. 또한 사양이 유사한 FCU를 사용하지만, 난방을 위하여 설치되는 FCU의 대수는 제각각 다른 것을 알 수 있었고, 좀 더 효율적이고 경제적인 관점에서 설치높이, 방향 및 설치간격, 적정 대수에 대한 연구가 이루어져야 할 것으로 판단된다. 잉여 태양에너지 회수용 FCU의 적정 소요 대수는 FCU를 통과하는 공기의 질량 및 순환유량을 기준으로 각각 8.4~10.9대 및 6.1~8.0대 정도이었다. 여기에 계산방법이나 FCU의 효율 및 사용 환경 등 위험률을 고려하면, 결국 9대 전후(약 $24m^3$당 1대 정도)를 설치하면 될 것으로 판단되었다.

직장암에서 수술후 방사선치료의 역활 (The Role of Radiation Therapy on Local Recurrence of Rectal Cancer)

  • 1982년 1월 부터 1990년 12월까지 전남대학교 병원에서 치료를 받은 95명의 직장암 환자를 대상으로 국소종양제어에 대한 수술후 방사선 치료의 역할을 알아보기 위하여 후향적 분석을 시행하였다. 95명의 환자중 72명은 수술만 받았으며 나머지 23명은 수술후 방사선 치료를 함께 받았다. 환자의 성별 분포는 남자가 45명 여자가 50명으로 비슷하였으며 연령의 중앙치는 53세 였다. 최소 추적기간은 19개월(범위 : $19\~125$)이었으며 중앙치는 47개월이었다. 종양의 국소재발율과 생존율은 Kaplan-Meier 법으로 계산하였으며 두 군간의 비교는 Log-rank test에 의하였다. 전체 95명의 환자중 27명 ($28.4\%$)에서 국소 종양의 재발이 관찰되었으며 13명($17.3\%$)에서는 원격 전이가 동시에 관찰되었다. 국소 종양 재발이 관찰된 27명중 24명 ($89\%$)이 수술 후 24개월이내에 재발되었으며 부위는 원발 병소 주위의 골반강내 조직에 가장 흔하였다. 수술만 받은 72명의 환자중 24명에서 국소 재발이 관찰되었다. 병기 A와 B1의 17명중 6명에서, B2와 B3 33 명중 7명 ($29.9\%$)에서, C2와 C3 19명중 11 명 ($54.7\%$)에서 각각 재발되었으며 병기에 따른 재발율의 차이는 통계학적 의의가 있었다(p<0.05). 수술후 방사선 치료를 받은 23명의 환자중 병기 B2와 B3 10명중 1명 ($10\%$)에서, 병기 C2와 C3 10명중 2명($22.2\%$)에서 재발이 관찰되었으며 두 군간의 차이는 통계학적 의의가 없었다(p<0.05). 한편 병기 B2와 B3에서 수술 후 방사선 치료를 시행한지 않은 군 보다 시행한 군에서 국소 종양 재발율이 낮았으나 통계학적 의의를 갖지 못하였으며 ($29.9\%$ vs $10.0\%$, p<0.05), 병기 C2와 C3의 경우는 수술 후 방사선 치료를 시행한 군에서 종양의 재발이 낮았으며 통계학적 의의가 있었다($34.7\%$ vs $22.2\%$, p<0.05).

참외 시설 재배 시 고온에서의 환기 처리에 의한 상대습도 상승과 흰가루병, 담배가루이, 응애 방제 및 개화 억제 (Ventilation at Supra-Optimal Temperature Leading High Relative Humidity Controls Powdery Mildew, Silverleaf Whitefly, Mite and Inhibits the Flowering of Korean Melon in a Greenhouse Cultivation)

  • 본 연구는 참외 재배 지에서 흰가루병, 담배가루이 및 두점박이응애가 동시에 발생하였을 때 45, 40, 35℃(대조구)의 온도에서 측창으로 환기 처리 시, 온실 내 온·습도의 변화, 병충해 발생과 잎말림, 그리고 개화조절에 미치는 효과를 검토하였다. 3월 3일 '히든파워' 대목에 접붙여진 '알찬꿀' 참외를 40cm 간격으로 격리상에 심었고, 위에 언급한 병해충이 모든 처리구에서 발생한 6월 18일부터 7월 13일까지 처리하였다. 온실의 온도는 맑은 날에는 설정 온도 지점까지 증가되었고, 45℃ 환기 처리에서 고온 고습이 약 9시간 동안 유지되었다. 주간 최고 기온과 최저 상대습도 차이는 45℃ 환기 처리에서 가장 높았다. 환기 처리 11일 후에는 흰가루병과 두점박이응애 피해가 45℃ 환기 처리에서 거의 회복되었지만 40℃와 35℃에서는 그렇지 않았다. 처리 14일 후, 담배가루이와 두점박이 응애 밀도는 45℃에서 유의하게 감소하였으나 흰가루병 증상은 유의하게 감소하지는 않았다. 잎말림은 고온에서 유발되었으나 45℃에서도 심하지 않았다. 처리 26일 후, 새로 나온 줄기의 15 마디의 개화수를 조사한 결과, 45℃에서 암꽃이 전혀 나오지 않았고 수꽃은 1.2개로 나타났다. 이상의 결과는, 고온기에 45℃의 고온에서 2-3주간 환기 처리는 온실 내부의 고온 고습을 유도하여 흰가루병, 담배가루이, 두점박이응애를 통제하고, 개화를 억제하여 참외의 영양 생장을 회복할 수 있는 방법으로 사료되었다.

HOT차로 운영에 대한 효과분석 및 국내활용방안 (Effectiveness Analysis of HOT Lane and Application Scheme for Korean Environment)

  • 교통시설공급정책의 한계를 극복하고자 다양한 교통수요관리 정책이 연구 시행되고 있으며, 본 연구는 이러한 연장선상 에서 현재 국내에 연구된 적이 없는 HOT(High Occupancy Toll) Lane에 대해 국내 도입여부 및 효과분석을 실시하였다. 본 연구에서는 HOT Lane의 국내 도입을 위해 현재 버스전용차로제가 시행되고 있는 경부고속도로 일부구간을 대상으로 IC에서 HOT Lane으로 접근까지의 최소차로변경구간과 HOT Lane으로 진입하려는 차량, 일반차로로 진출하려는 차량들의 엇갈림구간에 대한 교통처리방안을 제시함은 물론, HOT Lane의 효과분석을 수행하였다. 효과는 평소 교통수요가 많아 상습 적인 정체가 발생되는 경부고속도로 기흥IC~수원IC 구간(5.2km)과 서울외곽고속도로 학의JC~판교JC(8.3km) 구간에 대해 1 개 차로가 HOT Lane으로 운영되는 것을 가정하에 차선변경율(5~30%)을 고려하여 시뮬레이션 프로그램(VISSIM)을 사용하여 각 시나리오별로 분석한 결과, 일반차로의 통행속도가 시행 전에 비해 1.57~2.62km/h 향상되는 것으로 나타났다. 통행속도의 향상효과와 더불어 다양한 교통수요정책 연구라는 취지에서 국내에서의 연구가 전무한 환경에서 HOT Lane운영이라는 하나의 교통수요관리 정책분석은 향후 유사 연구의 기초가 될 수 있는 자료로서의 의미가 있다고 할 수 있을 것이다. 향후 버스의 수단분담률이 증가할 것으로 예상되나 버스간의 통행사이에서 여유gap 등이 생기는 버스전용차로에 대한 효율적인 운영방안 중 하나의 대안으로 HOT Lane의 연구가 향후 다른 고속도로 구간에서도 유용한 버스차로 관리정책대안으로서 구체적으로 검토되어야 할 필요성이 있는 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
