Restoration of Sagittal Balance in Spinal Deformity Surgery

  • The prevalence of patients with adult spinal deformity (ASD) has been reported as high as 68%. ASD often leads to significant pain and disability. Recent emphasis has been placed on sagittal plane balance and restoring normal sagittal alignment with regards to the three dimensional deformity of ASD. Optimal sagittal alignment has been known to increase spinal biomechanical efficiency, reduce energy expenditure by maintaining a stable posture with improved load absorption, influence better bony union, and help to decelerate adjacent segment deterioration. Increasingly positive sagittal imbalance has been shown to correlate with poor functional outcome and poor self-image along with poor psychological function. Compensatory mechanisms attempt to maintain sagittal balance through pelvic rotation, alterations in lumbar lordosis as well as knee and ankle flexion at the cost of increased energy expenditure. Restoring normal spinopelvic alignment is paramount to the treatment of complex spinal deformity with sagittal imbalance. Posterior osteotomies including posterior column osteotomies, pedicle subtraction osteotomies, and posterior vertebral column resection, as well anterior column support are well known to improve sagittal alignment. Understanding of whole spinal alignment and dynamics of spinopelvic alignment is essential to restore sagittal balance while minimizing the risk of developing sagittal decompensation after surgical intervention.

An Approach to Global Path Replanning Method Considering 4D Environmental Information (4D 환경정보를 반영한 광역 경로수정계획 기법에 관한 연구)

  • In this paper, a global path replanning method is proposed in order to plan a global path minimizing the risk of the unmanned vehicle on the battlefield. We first introduce 4D environmental information consisting of mobility, visibility, kill, and hit attributes, and a unified threat map and a mobility map are defined by the four attributes. Using the mobility map, the unmanned vehicle can find the shortest path on the traversable area. And then taking into account the deterrent according to the type of the unmanned vehicle on the integrated threat map, the vehicle can generate a route to suppress or avoid the threat of enemy as well. Moreover, we present a waypoints bypassing method to exclude unnecessary waypoints rather than the mission point when planning paths for the multiple waypoints.

Changes in Oxalate and Phytate Concentrations During Soymilk Processing from the Seeds of Korean Soybean Cultivars

  • A high dietary oxalate intake may lead to calcium oxalate (CaOx) kidney stones in the gastrointestinal tract. Most soy foods contain high concentrations of oxalate and/or phytate. This study analyzed the changes in oxalate (Ox), phytate ($InsP_6$), and calcium (Ca) during soymilk processing from the seeds of Korean recommended soybean cultivars (cvs). The contents of Ox, $InsP_6$, and Ca in 21 cvs ranged from 14, 108, and 148 to 231, 279, and 246 mg/100 g of dry seed, respectively. Seven cultivars were selected from the 21 cvs by the distributions of Ox, $InsP_6$, and Ca. Then, each contents of soymilk prepared from the 7 cvs were determined. All contents were lower in soymilk than in seeds, but the Ox to $InsP_6$ ratios changed from varying ratios (0.1-0.8) to normal ratios (0.8-1.0) in all cvs except 'Paldalkong'. Consequently, during soymilk processing, the Ox content was decreased and the $InsP_6$ content was remained higher than the Ox content although the Ox was likely to be less reductive than the $InsP_6$. These results may provide better information for minimizing the risk of formation of Ox kidney stones due to consumption of soy products.

  • Banks are preparing the system for the new concept of the 2020's. The framework of the core-banking solution is that various demands must be accommodated. In addition, the development process of new system must solve problems, such as obtaining the economic feasibility and expandability as well as minimizing the risk elements. This study verified the superiority of converging framework by analyzing the framework of new converging core-banking solution that can accommodate the current core-banking solution and future requirements. In addition, the study proposed application methods and the pros & cons of a converging framework. The migration effects and successful elements for changing to a new system are proposed.

  • Aortic arch aneurysm generally requires an urgent treatment due to the risk of catastrophic aortic rupture. However conventional surgery for aortic arch repair still carries significant risks of postoperative morbidity and mortality, especially in patients with old age. In an effort to correct the aortic pathology while minimizing the risks of complication, we performed a hybrid technique which comprises the off pump arch aortic stent grafting in an 86-old male patient with an aortic arch aneurysm.

  • Severe maxilla1y deficiency can be caused by cleft lip and palate(CLP), other craniofacial deformities, atrophy in the edentulous maxilla, and trauma. Patients with maxillary deficiency present a difficult treatment challenge. Traditionally, this skeletal deformity has been treated by Le Fort osteotomy, skeletal repositioning, and fixation with mini-plates and screws. The drawbacks of this method include a limited amount of anterior maxillary advancement often requiring simultaneous mandibular setback, the inability to create new bone, and minimal soft tissue adaptation to the new position, all of which increase the potential of relapse in case of large advancement. The alternative method of maxillary distraction osteogenesis offers promising results for successfully treatment of these patients while potentially minimizing the risk of relapse.

  • The control network has been operated in a closed. But it changes to open to external for business convenience and cooperation with several organizations. As the way of connecting with user extends, the risk of control network gets high. Thus, in this paper, proposed the technique of an anomalous event detection using white-list for control network security and minimizing the cyber threats. The proposed method can be collected and cataloged of only normal data from traffic of internal network, control network and field devices. Through way to check the this situation, we can separate normal and abnormal behavior.

  • With regard to the personnel safety and other safety when the gas suppressants are discharged into the area where occupants exist, the short term and long term effects to the health of people are discussed mainly with the Carbon dioxide agent and Halon Replacement agents system. To gain the benefits of CO2 extinguishing systems while minimizing risk to people serious attention must be given to personnel safety in the design, installation, and maintenance of CO2 systems. Training of personnel is essential. A major factor in the use of a clean agent fire suppressant in a normally occupied area is toxicity. While all halocarbon agents are tested for long-term health hazards, the primary endpoint is acute or short-term exposure, The primary acute toxicity effects of the halocarbon agents described here are anesthesia and cardiac sensitization. For inert gases, the primary physiological concern is reduced oxygen concentration.

  • Nanomaterials, which have the issues related to toxicity, environmental exposures, and human health, have been focused on significance of their safety management. Regarding their higher applicability for multiple sectors from chemicals to consumer products, the most important thing is to consider international policy cases categorized by regulatory intensities, applicable sectors, and substance types for establishing of convincing safety management system on nanomaterials. For minimizing the nanomaterials' risk potential, developing the system that underlines a precautionary principle is also needed. Regulatory system on nanomaterials should be applicable to the status quo and be a proactive approach to rapidly changing international trends.

  • Electric Vehicles(EVs) and Plug-in Hybrid Electric Vehicles(PHEVs) which have the grid connection capability, represent an important power system issue of charging demands. Analyzing impacts EVs charging demands of the power system such as increased peak demands, developed by means of modeling a stochastic distribution of charging and a demand dispatch calculation. Optimization processes proposed to determine optimal demand distribution portions so that charging costs and demand can possibly be managed. In order to solve the problems due to increasing charging demand at the peak time, alternative electricity rate such as Time-of-Use(TOU) rate has been in effect since last year. The TOU rate would in practice change the tendencies of charging time at the peak time. Nevertheless, since it focus only minimizing costs of charging from owners of the EVs, loads would be concentrated at times which have a lowest charging rate and would form a new peak load. The purpose of this paper is that to suggest a scenario of load leveling for a power system operator side. In case study results, the vehicles as regular load with time constraints, battery charging patterns and changed daily demand in the charging areas are investigated and optimization results are analyzed regarding cost and operation aspects by determining optimal demand distribution portions.
