사립체근병증은 운동시 근력약화와 같은 경한 증상에서 부터 중추신경계 질환으로 조기 사망에 이르기까지 돌연변이의 정도에 따라서 다양한 증상를 나타내는 질환이다. 그러므로 질환이 의심되는 경우 반드시 조직검사를 고려하여야 한다. 사립체근병증이 있더라도 다른 증상이 없이 무호흡 증후군이나 저환기 증후군의 형태로 처음 진단되는 경우가 드물지만, 사립체근병증이 진단된 후에는 폐기능 검사를 통해 질환의 진행에 대한 예측이 필요하며, 호흡 부전이 발생할 가능성이 높은 경우 비침습적기계환기를 통해 환자를 치료 할 수 있다. 저자들은 초기 중추성 수면 무호흡 증후군으로 오인되었다가 저환기 증후군으로 진단하고, 원인질환으로 사립체근병증을 진단하였으며, 비침습적 기계환기를 통해 성공적으로 치료하여 추적관찰 중인 1례를 경험하였기에 보고하는 바이다.

신선 편의가공 채소류의 전처리기술로서 중온처리의 적용 가능성을 확인하고자 박피 양파의 열수처리에 따른 저장중 품질특성 변화를 살펴보았다. 건조외피 제거, 절단, 수세를 거친 양파 시료에 대해 $50-80^{\circ}C$의 열수로 1 분간 침지한 후 물기를 제거하고 $63\;{\mu}m$ 두께의 LDPE 필름에 밀봉 포장하여 $10^{\circ}C$에 저장하면서 이화학, 미생물, 관능적 특성 변화를 측정하였다. 생체 중량감소 및 절단 표면색 변화는 상대적으로 고온($70,\;80^{\circ}C$) 처리구에서만 유의적인 증가를 나타내었고, 미생물의 경누 열처리 직후 고온 처리구에서 90% 이상의 생균수 감균 효과를 확인할 수 있었으나, 저장 중기 이후에는 처리구별로 유의적인 생균수 차이를 구분할 수 없이 약 $10^6-10^7\;CFU/g$ 수준을 나타내었다. 관능 평가에서는 저장 28일까지 $60^{\circ}C$ 중온 처리구가 변색, 시듦, 부패 항목에서 상대적으로 가장 낮은 점수를 나타내었고 외관 품질도 비교적 우수하여 박피 양파의 저장중 품질유지에 가장 유리한 열수 처리온도임을 확인할 수 있었다.
Hibiscus hamabo, called "Hwang-geun", growing about 3m in height is a deciduous shrub or subtree of Malvaceae. Because the number of these species is very limited in the world, the Ministry of Environment has designated H. hamabo as a preserved plant. The Korea Forest Service also protects it strictly by law since H. hamabo is an out-of-the-way plant and possibly may be exterminated soon in Korea. Investigation for distribution and ecological characteristics of the habitat for H. hamabo was carried out on Soan Island. Two wild H. hamabo were found at the forest edge (equation omitted) along the sea coast located in the southern part of Soan Island and this was the first report in the Korean academic world. These two wild H. hamabos were growing in a naturally inhibited area. The diameters at the base were 12cm and 15cm. The Tree heights were 150cm and 210cm and the number of branches of each wild H. hamabo was 4 and 7. However, the present condition of these plants was not good. Environmental conditions of the naturally inhibited area of H. hamabo were very mild because it is located at the edge of the forest and is always sunny during the daytime since the slope of the inhibited area is facing South. The ground drained very well since the soil was made of gravels and sand. Because the percent of vegetation of the subtree layer where H. hamabo was growing was 40%, the cover degree and sociability of flex crenata trees and Eurya japonica were found to be high. In the naturally inhibited area of H. hamabo, a dominant value of Rubus parvifolius in the lower part of the herb layer was very high and many plants in Compositae, such as Artemisia princeps var. orientalis and Erigeron annuus, were also present. A dominant value of liana, such as Vitis thunbergii var. sinuata, Rosa multiflora, Clematis terniflora and Hedera rhombea, and Gramineae plants that rhizomes were well developed and aggressively propagated, such as Miscanthus sinensis var. purpurascens, Phragmites communis, Spodiopogon cotulifer and Oplismenus undulatifolius which were surveyed as high, too. These results imply that H. hamabo might be exterminated soon through a natural selection if the proper management of the naturally inhibited area of H. hamabo is not conducted continually.
Purpose: In the cases of a vascular compromised condition in an injured lower extremity, soft tissue coverage with free tissue transfer presents a challenging problem to the reconstructive surgeon. For this reason, cross - leg flaps are still used in unusual circumstances. Advances in surgical technique has made the cross - leg free flap possible although it may require long operation time along with significant donor site morbidity. Therefore, a pedicled cross - leg muscle flap may be an alternative treatment modality when local flap or free flap is not possible. Methods: Twelve patients(9 males and 3 females) underwent the operation between October of 2001 and December of 2008. The patients' age ranged from 6 to 82 years. The unusual defects included the regions such as the knee, popliteal fossa, distal third of the tibia, dorsal foot, and the heel. Indications for the cross - leg gastrocnemius flap are inadequate recipient vessels for free flap(in eight cases), extensive soft tissue injuries(in three cases) and free flap failure(in one case). The muscle flap was elevated from contralateral leg and transferred to the soft tissue defect on the lower leg while both legs were immobilized with two connected external fixator systems. Delay procedure was performed 2 weeks postoperatively, and detachment was done after the establishment of the adequate circulation. The average period from the initial flap surgery to detachment was 32 days (3 to 6 weeks). Mean follow - up period was 4 years. Results: Stable coverage was achieved in all twelve patients without any flap complications. Donor site had minimal scarring without any functional and cosmetic problems. No severe complications such as deep vein thrombosis or flap necrosis were noted although mild to moderate contracture of the knee and ankle joint developed due to external fixation requiring 3 to 4 weeks of physical treatment. All patients were able to walk without crutches 3 months postoperatively. Conclusion: Although pedicled cross - leg flaps may not substitute free flap surgery, it may be an alternative method of treatment when free flap is not feasible. Using this modification of the gastrocnemius flap we managed to close successfully soft tissue defects in twelve patients without using free tissue transfers.
분자구조가 서로 다른 7종의 폴리올에스터 오일에 대한 가수분해속도가 측정되었다. 사용된 폴리올에스터는 2가 및 4가의 다가알콜과 서로 다른 탄소수의 직쇄 흑은 분기지방산으로 합성된 다가에스터 화합물이며, 이들의 가수분해반응은 p-톨루엔설폰산을 촉매로 사용하는 온건한 산성조건하에서 수행되었다 폴리올에스터 오일의 가수분해과정에서 생성되 는 부분에스터와 다가알콜 및 지방산 등의 구조가 확인되었고 반응시간의 경과에 따른 가수분해 생성물들의 농도가 측정되었다. 각 반응단계의 속도상수는 속도식과 실험적으로 얻은 각 화합물들의 농도로부터 최소제곱법에 의해 구하였고, 얻어진 가수분해 속도상수가 서로 비교되었다. 폴리올에스터 오일의 가수분해 속도는 지방산의 분자구조에 가장 크게 영향을 받는다. 즉, 직쇄지방산에 의한 폴리올에스터 오일은 분기지방산에 의한 폴리올에스터 오일보다 가수분해속도가 매우 마르며, 분기지방산에 의한 폴리을에스터 오일 중에서는 가지사슬의 모양과 위치가 가수분해속도에 영향을 미치는 것으로 확인되었다. 본연구에서 사용된 7종의 폴리올에스터 오일에 대한 가수분해속도는 물분자가 폴리올에스터 오일의 카르보닐 탄소를 공격할 때 인접한 가지사슬에 의한 입체장애효과를 비교함으로써 효과적으로 설명할 수 있다.
응집이 없는 단분산의 $PMSQ/TiO_2$ 복합 미립자를 얻기 위하여 300 nm 크기의 $TiO_2$ seed가 분산되어 있는 메탄올 수용액과 MTMS (Methyltrimethoxysilane)를 메탄올에 녹인 용액을 혼합하여 $TiO_2$ seed 표면에서 MTMS가 가수분해 및 축합 반응이 일어나도록 유도하여 복합 미분말을 제조하였다. 촉매로 암모니아를 사용하였고, 반응온도는 실온이었으며, 모든 반응은 질소분위기에서 행하였다. 교반속도, 반응온도, $[H_2O]/[MTMS]$, $[MTMS]/[TiO_2]$ 등을 변화하여 입자의 크기 및 형태에 영향을 주는 인자들을 조사한 결과, [MTMS]=0.2 M, $[NH_4OH]=0.6M$, $[H_2O]/[MTMS]=100$, $[MTMS]/[TiO_2]=10-50$이고 실온에서 서서히 교반한 경우 단분산된 약 $1-2{\mu}m$의 크기를 갖는 복합입자를 얻을 수 있었다. 얻어진 입자에 대한 소수성을 물에 대한 접촉각 측정을 통해 조사한 결과 거의 180도에 가까운 접촉각을 보임으로써 복합입자의 소수성이 매우 뛰어남을 확인할 수 있었다. 자외선 차폐효과도 UV 투과도 측정을 통해 조사되었다.
Background: Hypercalcemia is an important metabolic emergency condition in cancer patients. Bisphosphonate is the treatment of choice for hypercalcemia, whereas calcitonin and hydration with furosemide are recommended for acute supportive therapy. However, data regarding real-world treatment patterns and outcomes of pharmacological treatments are limited. Therefore, we aimed to investigate the treatment patterns and clinical outcomes of hypercalcemia treatment in solid tumor patients. Methods: Electronic medical records of 123 adults with solid cancers and albumin-corrected calcium levels >10.5 mg/dL or ionized calcium levels >1.35 mmol/L were reviewed. We retrospectively analyzed the pharmacological treatment and recovery rate according to the severity of hypercalcemia. Results: A total of 177 cases were identified, of which 49 were not treated and 30 were treated with hydration only. In moderate-to-severe cases, 86.5% received pharmacological treatment. Thirty-four cases (19.2%) were treated with bisphosphonate alone and 58 cases (32.8%) were treated with bisphosphonate and calcitonin. In mild hypercalcemia cases, the recovery rate was higher for those receiving hydration only or pharmacological treatment (79.7%) than for those receiving no treatment (61.4%, p = 0.041). Most moderate-to-severe cases were treated with medication and of those treated, 56.3% recovered. The recovery rate was lower in those treated with bisphosphonate alone (38.2%) than in those who underwent calcitonin combination treatment (73.7%, p = 0.001). Conclusions: Bisphosphonate combined with calcitonin was found to be more effective than bisphosphonate alone for the treatment of moderate-to-severe hypercalcemia. Considering the current shortage of calcitonin, further efforts are required to ensure its stable supply.
본 연구에서는 3차원 기공구조를 지닌 금속 및 고분자 소재를 이용한 수직 마찰모드의 정전기반 나노발전기(triboelectric nanogenerator, TENG) 제조기술을 소개하고 이에 관한 응용 연구를 수행하였다. 다양한 장점을 지닌 3차원 기공구조를 활용하여 설계된 간단하며 효율적인 나노발전기로, 반복적인 접촉/분리를 통해, 120 V에 이르는 순간 전압특성과 최대 출력 $0.74mW/m^2$을 획득하였다. 실제적인 응용 연구로 48개의 발광소자 구동 실험을 실시하였으며, 저전력 소비 전자소자 장치로의 응용 확장성을 확인하기 위해 회로 구성을 통한 커패시터 축적기능을 확인하였다. 본 연구에서 소개하는 정전기반 에너지 하베스팅 기술은 매우 경제적으로 제조할 수 있는 실용적인 접근방식으로, 반복적으로 가해지는 마찰에 의한 정전력을 효율적으로 획득하여 가까운 미래에 자가발전(self-powered)형 소형 전기소자 구동, 휴대형 전자기기 및 대규모의 전자 발전 장치에 적용 가능할 것으로 기대된다.
