The Effect of Maternal Pre-pregnancy Body Mass Index on Very Low Birth Weight Infants

  • Purpose: The pre-pregnancy body mass index (BMI) is associated with adverse neonatal outcomes. However, studies on very low birth weight (VLBW) infants are rare. This study aimed to investigate the effect of maternal pre-pregnancy BMI on VLBW infants. Methods: This retrospective study evaluated singleton VLBW infants born at the CHA Gangnam Medical Center from 2006 to 2016. The neonates were classified into three groups according to the maternal pre-pregnancy BMI: underweight (<$18.5kg/m^2$), normal weight (${\geq}18.5$ to <$23kg/m^2$), and overweight or obese (${\geq}23kg/m^2$). Clinical characteristics and morbidities of mothers and infants were analyzed. Results: A total of 181 infants belonging to underweight (16.6%), normal weight (58.6%), and overweight or obese (24.8%) groups were enrolled. The pre-pregnancy BMI had a significant negative correlation with gestational age (r=-0.198, P=0.001) and a significant positive correlation with the z-score of the birth weight (r=0.078, P=0.001) and body length (r=0.067, P=0.008). The number of extremely preterm infants was significantly higher in the overweight or obese group. The proportion of risk of small for gestational age infants was higher in the underweight group (adjusted odds ratio [OR], 2.958; 95% confidence interval [CI], 1.113 to 7.864), whereas that of infants with severe retinopathy of prematurity was higher in the overweight or obese group (adjusted OR, 9.546; 95% CI, 1.230 to 74.109). Conclusion: In our population of VLBW infants, the pre-pregnancy BMI was associated with gestational age, intrauterine growth, and adverse neonatal outcomes. Therefore, proper weight control before pregnancy is important.

Studies on the Safety of Artemisiae Capillaris Herba - With the Perinatal and Lactational Reproductive Toxicity - (인진의 안정성에 대한 연구 - 주산$\cdot$수유기 생식 독성을 중심으로 -)

  • Objectives: To study the effect of Artemisiae Capillaris Herba extracts, that have been used as oriental medicine to treat liver disease, on the perinatal and lactational n;)productive toxicity of SD rats when administered by oral lavage. Methods: Female SD rats were dosed from 6 days of gestation to 3 weeks postpartum. This was conducted in accordance with the recommendations of the KFDA Guidelines for Detection of Toxicity to Reproduction for Medicinal Products. Results: No Artemisiae Capillaris Herba extracts treatment-related changes in clinical signs, mortalities, implantation number, dead fetus number, loss rate of fetus, number of live young, survival rate of fetus, sex ratio of live young, external anomalies, pregnancy periods, viability index, lactational index, survival rate of litters at 4 days after birth or delivery index were demonstrated in any dosed levels in this study. However, the body weight and gains, food consumption and absolute organ weights of brain, adrenal glands, liver, spleen, kidney, ovaries and heart were significantly increased in 2000 or 1000mg/kg-dosing groups and the relative organ Weights of adrenal glands were significantly increased in 2,000mg/kg-dosing groups. Therefore, it was concluded that this increase was natural according to growth. Also, no changes of gross findings, clinical signs, mortalities, body weight and gains, physical development results, necropsy findings, organ weight, faculty test, open filed test and water-filled simple T-maze test, copulation, fertility, pregnancy indices, body weight and gains during gestation periods, necropsy findings, corpora lutea number, implantation number, implantation rate, dead fetus number, post-implantation loss rate, live young, post-implantation survival rate, sex ratio of live young, external anomalies and individual body weights of live young were demonstrated in any dosed levels in this study. Conclusions: It is considered that the NOAEL (No-Observed-Adverse-Effect Level) for perinatal and lactational reproductive toxicity of Artemisiae Capillaris Herba extracts was up to 2000mg/kg/day because no changes of other perinatal and lactational reproductive indices were demonstrated.

Analysis of Facial Asymmetry with Three-Dimensional Morphometry (3차원 영상에 의한 안면 비대칭의 분석)

  • This study is to show the qualitative analytic methods of facial asymmetry with three-dimensional morphometry and find out asymmetry change resulted from enlarging three local regions. Steel balls (1.2mm in diameter) were attached in twenty seven landmarks of a symmetrical artificial human skull. This artificial human skull was used as experimental materials. Twelve different asymmetrical artificial human skulls were formed by gradually enlarging the mandibular body length, gonial angle, and ramus height of the left hemiface. From the three-dimensional morphometry of each skull type, nine local area measurements and three total sum area measurements(representing the mandibular area, maxillary area, and lower facial area) were acquired and made into the surface area asymmetry degree. Menton deviation itself was used as the surface area asymmetry degree while right-left percentages were used in the other measurements. These surface area asymmetry degrees were compared with each other to find out asymmetry change according to the degree of actual facial asymmetry. Through the statistical analysis, following results were obtained. The results were as follows: 1. Left maxillary area of artificial human skull was 7.13$\pm$0.26% larger while mandibular area was 4.14$\pm$0.12% smaller than each those of right hemiface. After all, left lower facial area was 1.44$\pm$0.07% larger than those of right hemiface.(n=7). 2. Among the reduce rates of surface area asymmetry degree resulted from enlarging three local regions, ramus height was similar to mandibular body length while it was bigger than those of gonial angle. 3. Among the increase rates of menton deviation resulted from enlarging the local regions, ramus height was the biggest, mandibular body length was the second and gonial angle was the smallest. These results suggest that three-dimensional morphometry can be used to qualitatively analyse facial asymmetry and the asymmetry degree is more influenced by enlarging the ramus height, mandibular body length than those of gonial angle.

The Study on the Factors which are Related to Body Mass Index in Male (남성 체질량지수와 관련된 요인에 대한 임상적 연구)

  • Objectives : This study was conducted to know how different Body mass index(BMI) were, according to age group, drinking, amount of smoking, fast blood sugar. Methods: We measured the Body mass index(BMI) in 5573 male, using Inbody 2.0(Biospace Co. Ltd, Korea), and then we analysed the 4 factors - age group, amount of drinking, smoking, fast blood sugar - which are related to BMI of male. Results: 1. BMI according to age group increased from -30 years group at the lowest to 41-50 years group at the highest and decreased after 51-60 years. 2. BMI according to amount of drinking increased from Nondrinker group(men who don't drink) years at the lowest to Drinker IT group(men who drink more than 4 bottles of Soju(360ml, 21%, distilled liquor) for a week) at the highest. 3. BMI according to amount of smoking increased from Smoker I group(men who smoke 1-10 pieces for a day) at the lowest to Smoker N group(men who smoke more than 31 pieces for a day) at the highest except Nonsmoker group(men who don't smoke). 4. BMI of group that not exceed 110mg/d${\ell}$ in fast blood sugar was lower in the group that exceed 110 mg/d${\ell}$ in fast blood sugar. Conclusions: We analysed the 4 factors - age group, amount of drinking, smoking, fast blood sugar - which are related to BMI of male.

A Clinical Study on the Effects of Intravenous Wild ginseng Herbal Acupuncture on the Human Body (정맥 주입용 산삼약침이 인체에 미치는 영향에 관한 임상적 연구)

  • Background : This study was conducted to evaluate the effects of wild ginseng herbal acupuncture developed for the intravenous use. Healthy male and female volunteers(n=57) went through Randomized Control Trials(RCT). Methods : For those who are under a medication due to common cold or other illnesses were excluded in the primary stage and the subjects with possible abnormalities in the pre-screening process were also excluded in the secondary stage. Then the examination groups were determined by random sampling. Experiment groups were divided into Normal saline injection group(control group), cultivated wild ginseng herbal acupuncture group(experiment group 1) and natural wild ginseng herbal acupuncture group(experiment group 2) Blood tension, body temperature, pulse, and other criteria were measured and analyzed. Results : 1. Intravenous injection of cultivated wild ginseng herbal acupuncture and natural wild ginseng herbal acupuncture didn't cause significant changes in the blood tension, pulse, body temperature, and etc. 2. No significant differences were witnessed in CBC, ESR, biochemistry of blood test and UA between the experiment groups. 3. No significant changes were noted in the thermography before and after the test in the experiment groups. 4. Some of the common physical changes occurring during and after the administration were fatigue, chest distension, and headache in all of the experiment groups. 5. Comparing general condition after one week from the termination of administration, the control group showed worst condition while as the natural wild ginseng herbal acupuncture group displayed best condition. Conclusion : From the above results, we can carefully deduce that the intravenous injection of the wild ginseng herbal acupuncture didn't show significant differences compared to injection of the normal saline. We can infer it is safe on the human body and further studies and reports must be followed.

Process Evaluation of a Mobile Weight Loss Intervention for Truck Drivers

  • Background: In a cluster-randomized trial, the Safety and Health Involvement For Truck drivers intervention produced statistically significant and medically meaningful weight loss at 6 months (-3.31 kg between-group difference). The current manuscript evaluates the relative impact of intervention components on study outcomes among participants in the intervention condition who reported for a post-intervention health assessment (n = 134) to encourage the adoption of effective tactics and inform future replications, tailoring, and enhancements. Methods: The Safety and Health Involvement For Truck drivers intervention was implemented in a Web-based computer and smartphone-accessible format and included a group weight loss competition and body weight and behavioral self-monitoring with feedback, computer-based training, and motivational interviewing. Indices were calculated to reflect engagement patterns for these components, and generalized linear models quantified predictive relationships between participation in intervention components and outcomes. Results: Participants who completed the full program-defined dose of the intervention had significantly greater weight loss than those who did not. Behavioral self-monitoring, computer-based training, and health coaching were significant predictors of dietary changes, whereas behavioral and body weight self-monitoring was the only significant predictor of changes in physical activity. Behavioral and body weight self-monitoring was the strongest predictor of weight loss. Conclusion: Web-based self-monitoring of body weight and health behaviors was a particularly impactful tactic in our mobile health intervention. Findings advance the science of behavior change in mobile health intervention delivery and inform the development of health programs for dispersed populations.

Comparative Evaluation of Korean Medicine well-matched with Chicken through an inhibition of Pancreatic Lipase (닭고기와 잘 어울리는 췌장라이페이즈 억제능을 가진 한약재의 비교평가)

  • Objectives : In this study, the inhibitory activities of Korean Medicine against pancreatic lipase in vitro and biochemical analyses in vivo were measured to determine its possibility as a well-matched sauce material with chicken. Methods : The inhibitory activity on pancreatic lipase enzyme of 11 samples were evaluated in vitro and then 5 samples were selected. The activity of pancreatic lipase was investigated using orlistat as a positive control. Animals were divided into eight groups (n=7). The experimental groups except for normal group were fed 60% high-fat diet for 7 days. 5 samples were orally administered at a dose of 200 mg/kg body weight and orlistat were orally administrated at a dose of 60 mg/kg body weight for 7 days. Biochemical anaylses of 5 samples were executed based on lipid parameters analysis. Results : Korean Medicines with an $IC_{50}$ of below 1 mg/kg were Scutellariae Radix, Gardeniae Fructus, Theae Folium Coptidis Radix, and Mori Cortex Radicis. Body weight change of Mori Cortex Radicis reduced significantly, however fecal triglyceride couldn't regulate effectively. The most excellent inhibitory effect of pancreatic lipase showed in Scutellariae Radix treatment and also regulated significantly serum triglyceride and total cholesterol. Moreover, the supplementation of Coptidis Radix excreted meaningfully triglyceride to fece. Conclusions : In conclusion, Coptidis Radix may exert anti-obesity effect by directly inhibiting pancreatic lipase, which would prevent the absorption of lipid from the small intestine. Besides, Mori Cortex Radicis may led to the decrease of the body weight via the different pathway.

Association between dietary intake, body measurements, and urinary bone resorption markers in young adults with osteopenia and osteoporosis: a cross-sectional study

  • Objectives: Bone health in early adulthood, as individuals approach peak bone mass, plays a critical role in preventing osteoporosis later in life. This study aimed to investigate the associations between lifestyle and dietary factors, anthropometric measurements, and urinary bone resorption markers in young adults. Methods: A cross-sectional study was conducted with 100 healthy Korean adults (50 men and 50 women) in their 20s and early 30s. Bone mineral density (BMD), anthropometric measurements, dietary intake (24-hour recall), and urinary bone resorption indicators (deoxypyridinoline and N-terminal telopeptide of type I collagen) were analyzed. Variables were compared between the osteopenia and osteoporosis groups (OSTEO group: 30% men and 60% women) and the healthy control group. Results: Men in the OSTEO group were significantly taller than those in the control group (P < 0.05). Women in the OSTEO group had significantly lower body weight and body composition (muscle and body fat) than those in the normal group (P < 0.01). Men in the OSTEO group had a significantly higher intake of animal calcium (Ca) than those in the normal group (P < 0.05). Women in the OSTEO group had significantly higher dietary fiber, vitamin A, Ca, plant Ca, and potassium intake than did those in the normal group (P < 0.05). There were no significant differences in caffeinated beverage consumption, eating habits, or urinary bone resorption indicators between the OSTEO and control groups of either sex. Conclusions: In our study of young South Korean adults, we observed low bone density levels, with particularly low BMD in taller men and underweight women. We found a higher nutrient intake in the OSTEO group, indicating the possibility of reverse causality, a phenomenon often found in cross-sectional studies. Therefore, there is a need to further elucidate dietary factors related to osteoporosis in young adults through prospective cohort studies involving a larger population.

Newly developed care food enhances grip strength in older adults with dysphagia: a preliminary study

  • BACKGROUND/OBJECTIVES: Maintaining total muscle mass in the older adults with swallowing difficulty (dysphagia) is important for preserving swallowing function. Increasing protein intake can help sustain lean body mass in the older adults. The aim of this study was to evaluate the effect of various high-protein texture-modified foods (HPTMFs) on muscle mass and perform dietary assessment in ≥ 65-yrs-old patients with dysphagia. SUBJECTS/METHODS: Participants (n = 10) received the newly developed HPTMFs (average 595.23 ± 66.75 kcal/day of energy, 54.22 ± 6.32 g/day of protein) for 10 days. Relative handgrip strength (RHS), mid-upper arm circumference (MUAC), body composition, mini nutritional assessment (MNA), mini dietary assessment (MDA), and Euro Quality-of-Life questionnaire 5-dimensional classification (EQ-5D) were assessed. RESULTS: After 10 days, an increase in MUAC (26.36 ± 2.35 cm to 28.50 ± 3.17 cm, P = 0.013) and RHS (0.38 ± 0.24 kg/kg body weight to 0.42 ± 0.22 kg/kg body weight, P = 0.046) was observed. Although MNA, MDA, EQ-5D, subjective health status, muscle mass, and calf circumference showed a tendency to increase after intervention, no significant differences were found. CONCLUSIONS: These results suggest that the HPTMFs can be used for improving the nutritional and health status in patients with dysphagia.

Indoor feeding combined with restricted grazing time improves body health, slaughter performance, and meat quality in Huang-huai sheep

  • Objective: The aim of this study was to evaluate the effects of three feeding systems, i.e., indoor feeding (CON), indoor feeding with 4-h daily access to grazing artificial pasture (ITGP), and indoor feeding with 8-h daily access to grazing artificial pasture (IEGP), on the plasma antioxidant and immunological capacity, slaughter characteristics, meat quality and economic efficiency of Huang-huai lambs. Methods: Thirty-three healthy Huang-huai rams with similar body weight (approximately 5 mo of age, 28.96±1.01 kg) were assigned equally to three experimental groups. When finished fattening, six lambs from each group were collect blood samples for plasma analyses and then slaughtered to determine slaughter characteristics and obtain biceps brachii muscle for further analysis of meat quality and fatty acid profile. Results: Compared to CON group, animals submitted to ITGP and IEGP groups resulted in greater contents of serum glutathione peroxidase, immunoglobulins (IgA, IgG, and IgM), polyunsaturated fatty acids (PUFA), n-6 PUFA, and PUFA/saturated fatty acid (FA) ratio and lower palmitic /oleic acid ratio (p<0.05). Moreover, animals in ITGP group exhibited a higher (p<0.05) loin eye area, content of meat crude protein (CP), and eicosetrienoic acid compared to CON group, while slaughter performance was superior (p<0.05) to that of the IEGP group. The economic efficiency of ITGP group was 70.12% higher than that of CON group, while the IEGP group exhibited a decrease of 92.54% in economic efficiency compared to the CON group. Conclusion: Restricted grazing time combined with indoor feeding was more effective in conferring superior body health, carcass traits and economic efficiency in Huang-huai lambs, as well as higher CP content and healthier FA composition in the resulting meat.
