  • Chronic sclerosing sialadenitis, also known as Kuttner tumor, is a benign chronic inflammatory lesion of the salivary gland. Here, we describe a case of chronic sclerosing sialadenitis with mucinous ductal metaplasia in a parotid gland, which was confused with low-grade mucoepidermoid carcinoma on aspiration cytology.

  • Successful regeneration of a cartilage framework using perichondrium has been reported by several authors but there are some arguments surrounding mucosal regeneration. Some authors report that regeneration of mucosa is completed by ingrowth from neighboring tissue but others insist that it occurs via metaplasia from the squamous epithelium. This study was designed to investigate the differences, especially in mucosal regeneration between nonvascularized and vascularized flaps via immunohistochemical study. A morphologic study was carried out to elucidate the characteristics o( the regenerated mucosa which was sutured on the vascularized perichondrium and fabricated in a rabbit ear. A nonvascularized perichondrial-mucosal composite flap with the same dimension was transferred in the control group. BrdU was labelled on both normal mucosa and grafted mucosa in the experimental group without my statistically significant differences. In cytokeratin stain, it was regarded that mucosal coverage of the control group occurred by ingrowth from the neighboring mucosa. It can be conceived that metaplasia of the grafted mucosa occurs in a vascularized composite flap transferred group.

A Study of Environmental Hormone Characterisitics on Toxicity from Wooddust (목재분진의 독성에 의한 환경홀몬특성 연구)

  • This study was performed to evaluate tannin exposure by wooddusts for workers in furniture factories and to investigate the relationship between tannin exposure and sino-nasal cancer risk. In order to explore possible cytological changes leading to nasal cancer, we have examined 50 male furniture workers and 50 matched controls using brush cytology. The results we have obtained in this study were as follows: 1. The tannin contents of woods used in woodworking factories have been measured and varied from 0.43 to 8.72 mg tannic acid equivalent per gram wood, for reconstituted softwood and turpentine (Syncarpia glomuliferia) respectively. 2. Airborne tannins in wooddusts were also determined by area and personal exposure. The values of mean exposures for both methods are ranged from 3.1 to $5.0{\mu}g/m^3$ and from 4.6 to $14.5{\mu}g/m^3$ in furniture manufactures. 3. Over nasal cytology scores 2, the scores of study group were slightly more than control group and this kind of metaplasias seemed to be occurred over $2mg/m^3$ wooddust and $6{\mu}g/m^3$ tannin exposure. Keratinising squamous metaplasia was investigated at nasal cytology score 3 and $10{\mu}g/m^3$ tannin exposure. The nasal cytology score 4 was seemed to be atypical squamous metaplasia. 4. To find out contributing factors to nasal cytology change, odds ratio that is one of fundmental biostatistics was applied. Actually the relationship between wooddust, tannin concentration and metaplasia were not meaningful, but the relationship between working experence more than 15 years and metaplasia was calculated as 1.83. This reveals that significant clinical abnormalities could be influenced from the years of woodworking experiences. However further research is required to evaluate the significance of the data, for the purposes of sino-nasal risk assessment, standard setting to prevent nasal cancer occurrences and possibility of changing workplace.

Amyloidosis in a Whooper swan (Cygnus cygnus) (큰 고니에서 발생한 아밀로이드 침착증)

  • Two Whooper swan (Cygnus cygnus) died after suffering from pododermatitis, lethargy, and ataxia; necropsy was performed. Grossly, the liver was swollen and firm. The kidney and spleen were also enlarged and a pale tan color. On histopathologic examination with Congo red staining, amyloidosis was noted in liver, spleen, and kidney. In addition, marked osseous metaplasia was present in the liver. Based on these results, systemic amyloidosis involving liver, spleen, and kidney with osseous metaplasia in the liver was diagnosed. Study results indicate that an inflammatory reaction associated with pododermatitis had a role in the amyloidosis in this particular case.

A Systemic Review of Studies for Traditional Chinese Treatment of Intestinal Metaplasia - Research using CNKI (장상피화생(腸上皮化生) 치료에 대한 최신 임상 연구 동향 - CNKI 검색을 중심으로)

  • Objectives: The purpose of this research is to investigate recent clinical studies of Intestinal Metaplasia (IM) based on a search of the China National Knowledge Infrastructure (CNKI). Methods: We investigated recent clinical studies of Oriental medicine therapies in traditional Chinese medical journals for IM through a CNKI search. 20 clinical articles published from 2010 to 2016 were analyzed. This study examined the authors, publication years, types of studies, criteria for diagnosis and evaluation, periods, purposes of study and articles classified by methods of treatment. Results: Most of articles were classified as randomized controlled trial. IM was diagnosed using gastroscopy, pathological examination and clinical symptoms in a high proportion of articles. The criteria for evaluation most frequently used were the effective ratios of clinical symptoms, gastroscopy and pathological tests. The methods of treatment in all studies were Chinese herb medicine. Acupuncture, acupoint injection therapy, and embedding therapy were also used in several studies. Conclusion: In this study, the Chinese treatment for IM was markedly effective. To improve the treatment of IM in Korea, many clinical studies and case reports are required, based on Chinese medicine.

A Prospective Study on Duodenitis, Duodenal Ulcer, and Gastric Metaplasia in Children Infected by Helicobacter pylori (Helicobacter pylori에 감염된 소아에서 십이지장염, 십이지장 궤양 및 위 상피화생에 대한 전향적 연구)

  • Purpose: Helicobacter pylori (H. pylori) infection has been known to be vital in the pathogenesis of duodenal ulcer disease in children as well as in adults. But the relationship between H. pylori infection and the histopathologic findings of the duodenum has not been explained obviously in children yet. So the aim of this study is to determine whether duodenitis and/or gastric metaplasia in the duodenum increases the risk of duodenal ulcer disease in children infected by H. pylori. Methods: From October 2001 to April 2004 gastric and duodenal biopsies were performed in 177 children who visited Department of Pediatrics, Gil Hospital, Gachon Medical School. Biopsy sections were stained with hematoxylin and eosin and also with Giemsa for identification of H. pylori. The grades of duodenitis and gastric metaplasia were classified from 0 to 3 and from 0 to 4, respectively. Results: The incidence of H. pylori infection was 54% in total patients. Amongst 163 children with duodenitis there was a lack of correlation between H. pylori infection and the grade of duodenitis. Amongst 11 patients with duodenal ucler, only 4 children were infected by H. pylori. And amongst 5 patients with gastric metaplasia, H. pylori and duodenal ulcer were detected in 2 and 3 children, respectively. The occurrence of duodenal ulcer and gastric metaplasia were increased significantly in proportion to the grade of duodenitis (p<0.0001 and p=0.0365, respectively). Conclusion: As opposed to the results of previously reported articles, there were lacks of correlation between H. pylori infection and duodenitis, duodenal ulcer, and gastric metaplasia. So further study hould be done to clarify the effect of H. pylori on the duodenal histopathology in children infected by H. pylori.

Microsatellite Instability of Nuclear and Mitochondrial DNAs in Gastric Carcinogenesis

  • Genetic instability contributes to the development and progression of gastric cancer, one of the leading causes of cancer death worldwide. Microsatellite instability (MSI) has been hypothesized to be involved in carcinogenesis, althgough its mechanisms and exact roles in gastric cancer remain largely unknown. Our aim was to identify associated clinicopathological characteristics and prognostic value of MSI in gastric cancer and precancerous lesions including gastritis, metaplasia, dysplasia, and adenoma. Because mitochondrial DNA has a different genetic system from nuclear DNA, the results of both nuclear MSI and mitochondrial MSI in gastric cancer were reviewed. This review provides evidence that genetic instability of nuclear and mitochondrial DNAs contributes to early stages of gastric carcinogenesis and suggests possible roles in predicting prognosis.

Histopathology of canine basal cell tumor (개에서 발생한 기저세포종양에 관한 병리조직학적 연구)

  • Basal cell tumors from 53 dogs were examined histologically and classified as basal cell adenoma(n=44), granular basal cell adenoma(n=1), basal cell carcinoma(n=3), basosquamous cell adenoma(n=1), basosquamous carcinoma(n=4) on the basis of malignancy and squamous metaplasia. Female was twenty three and male was thirty. Affected dogs are usually 5.6 years and sex predilection have not been seen. None of tumor examined was metastasized into other organs. Distinct patterns identified in the basal cell adenoma are solid(n=15), cystic(n=13), adenoid(n=8), medusa(n=5) and ribbon(n=3). Solid basal cell adenoma is common type in benign basal cell adenoma. Only one neoplasm was granular basal cell adenoma which was characterized by eccentric nucleus and abundant granular cytoplasm. Basal cell carcinoma showed anaplastic appearance histologically and had atypical basaloid epithelial cells and multinucleated giant cells with moderate number of mitotic figures. Both basosquamous cell adenoma and carcinoma had squamous metaplasia.

Osseous metaplasia showing heterotopic ossification in the maxillary sinus

  • Radiopacity in the maxillary sinus can be observed in various conditions, such as in the presence of lesions in the maxillary sinus or as a sequela of maxillary sinus surgery. This report describes the case of a 57-year-old female patient who had no previous history of surgical treatment or traumatic injury of the nose or maxillary sinus. Both maxillary sinuses were indistinguishable on panoramic radiography and showed signs of radiopacity. Computed tomography images revealed that the maxillary sinuses were filled with bony tissue and exhibited signs of sinus mucosal thickening. Biopsy results showed fragments of trabecular bone with fibrous tissue.
