BACKGROUND/OBJECTIVES: This study evaluated whether a mobile health (mHealth) application can instigate healthy behavioral changes and improvements in metabolic disorders in individuals with metabolic abnormalities. SUBJECTS/METHODS: Participants were divided into an mHealth intervention group (IG), which used a mobile app for 24 weeks, and a conventional IG. All mobile apps featured activity monitors, with blood pressure and glucose monitors, and body-composition measuring devices. The two groups were compared after 24 weeks in terms of health-behavior practice rate and changes in the proportion of people with health risks, and health behaviors performed by the IG that contributed to reductions in more than one health risk factor were analyzed using multiple logistic regression. RESULTS: Preference for low-sodium diet, reading nutritional facts, having breakfast, and performing moderate physical activity significantly increased in the mHealth IG. Furthermore, the mHealth IG showed a significant increase of eight items in the mini-dietary assessment; particularly, the items "I eat at least two types of vegetables of various colors at every meal" and "I consume dairies, such as milk, yogurt, and cheese, every day." The proportion of people with health risks, with the exception of fasting glucose, significantly decreased in the mHealth IG, while only the proportion of people with at-risk triglycerides and waist circumference of females significantly decreased in the control group. Finally, compared to those who did not show improvements of health risks, those who showed improvements of health risks in the mHealth IG had an odds ratio of 1.61 for moderate to vigorous physical activity, 1.65 for "I do not add more salt or soy sauce in my food," and 1.77 for "I remove fat in my meat before eating." CONCLUSIONS: The findings suggest that the additional use of a community-based mHealth service through a mobile application is effective for improving health behaviors and lowering metabolic risks in Koreans.
Cytochrome P450 (P450) enzymes are the major catalysts involved in the biotransformation of various drugs, pollutants, carcinogens, and many endogenous compounds. Most of chemical carcinogens are not active by themselves but they require metabolic activation. P450 isozymes playa pivotal role in the metabolic activation. The activation of arylamines and heterocyclic arylamines (HAAs) involves critical N-hydroxylation, usually by P450. CYP1A2 plays an important role in these reactions. Broad exposure to many of these compounds might cause carcinogenicity in animals and humans. On the other hand, P450s can be also involved in the bioactivation of other chemicals including alcohols, aflatoxin B1, acetaminophen, and trichloroethylene, both in humans and in experimental animals. Understanding the P450 metabolic activation of many chemicals is necessary to develop rational strategies for prevention of their toxicities in human health. An important part is the issues of extrapolation between species in predicting risks and variation of P450 enzyme activities in humans.
The principal objective of this study was to evaluate the association between plasma antioxidant levels and metabolic syndrome in male workers, and to provide basic information regarding the control and prevention of metabolic syndrome. We analyzed 163 male workers who had participated in annual medical examinations from January to December 2007. The subjects were classified into normal and metabolic syndrome groups according to the NCEP-ATP III criteria and the Asia-Pacific criteria for waist circumference. Anthropometric parameters, lifestyles, blood lipid profiles, and antioxidant levels were evaluated. As compared to the normal group, the metabolic syndrome group evidenced significantly higher plasma levels of $\alpha$- tocopherol(p<0.05) and retinol(p<0.05), but significantly lower plasma levels of lycopene(p<0.05) and $\beta$-carotene(p<0.05). This tendency was found to be predominantly attributable to increases in the number of metabolic syndrome components. In our simple regression analysis, higher plasma levels of $\alpha$-tocopherol($\beta$=0.001, p<0.01) and retinol($\beta$=0.021, p<0.001) were associated with significantly higher risks of metabolic syndrome, but lycopene($\beta$=-1.499, p<0.01) and $\beta$-carotene($\beta$=-0.048, p<0.01) were associated with significantly lower risks of metabolic syndrome. Retinol($\beta$=0.013, p<0.05) and $\beta$-carotene($beta$=-0.044, p<0.01) were associated significantly with metabolic syndrome, when adjusted for age and BMI. These data indicate that the plasma levels of $\alpha$-tocopherol, retinol, lycopene, and $\beta$-carotene are associated with metabolic syndrome. Specifically, low lycopene and $\beta$-carotene levels in the plasma appear to increase the risk of metabolic syndrome. Therefore, proper nutritional education programs for male workers are required to increase dietary intakes of antioxidant vitamins. Further studies will be necessary to determine whether antioxidant levels can be utilized as a predictive or a preventive factor.
The risks of the metabolic syndrome (MS) is known to be related to the dietary behavior. The objective of this study is to evaluate the association between the relative risks of MS and the dietary habit and to provide the ideal dietary habits for prevention of chronic disease of the middle-aged. Healthy subjects aged 40-64 years (male n=122, female n=173) were recruited throughout Seoul area. MS was defined according to NCEP-ATP III criteria except central obesity, and Asia-Pacific Area criteria for central obesity (2000) was adapted. Subjects were stratified into 3 groups according to the number of total risk factors:'MS group' was defined as the subjects who have three or more risk factors, 'Risk group' was defined as ones to have one or two risk factors, and 'Healthy group' defined as ones with no risk factor. In this study, 'Health group' comprised of $40.7\%$, the percentage of 'Risk group' was $49.0\%$, and the 'MS group' was $9.5\%$ of the total subjects. The MS incidence was associated with low education (p<0.001), low economic status (p<0.05), and low self-assessed health recognition (p<0.05). The risk of MS increased with adverse life styles such as cigarette smoking (p<0.05), irregular meal time (p<0.05), skipping lunch (p<0.05), low interests in balanced diet (p<0.05), and higher salt intake (p<0.01). Healthy group self-evaluated nutritional knowledge more highly (p<0.05) and scored higher nutritional knowledge (p<0.001). However, there was no difference in overall nutritional behavior among the three groups, which implies that nutritional education method should be developed for the subjects to practice their teaming efficiently.
Neonates, especially extremely low birth weight infants, are among the groups of patients undergoing transfusion frequently. Since they are exposed to higher specific transfusion risks compared to the patients of other age groups, there are many special aspects that must be considered for transfusion therapy in neonates. The transfusion risks in neonates include adverse outcomes specific for preterm infants as well as increased metabolic, immunologic, and infectious complications. To reduce the risks of transfusion-transmitted cytomegalovirus infection and transfusion-associated graft-versus-host disease, leukoreduced and irradiated cellular blood products should be used for all neonates. This review summarizes the risks of neonatal transfusion therapy, specific methods to reduce risk, and current trends and practices of red blood cell and platelet transfusions in neonates, to facilitate decision-making for neonatal transfusion.

Metabolic syndrome has become quite prevalent within our society. Over the past two decades, the prevalence of metabolic syndrome has sharply increased worldwide and it has become a major public health problem in several countries. It is associated with the global epidemic of obesity and diabetes mellitus and imposes numerous cardiovascular risks. Prostate cancer is the second most common cancer among men, surpassed only by non-melanoma skin cancer. A considerable body of evidence exists suggesting that some components of the metabolic syndrome have been associated with the risk of prostate cancer. These components include obesity, an abdominal fat distribution, and hyperinsulinemia. Androgen deprivation therapy (ADT) is the most widely used therapeutic modality in prostate cancer. It changed the body composition and lipid profile of men with prostate cancer. Androgen deficiency is associated with increased levels of total cholesterol, low-density lipoprotein (LDL)-cholesterol, increased production of proinflammatory factors, and increased thickness of the arterial wall and contributes to endothelial dysfunction. The aim of this review is to evaluate the association between metabolic syndrome and prostate cancer and to discuss the implications of androgen deficiency in men with cardiovascular risk factors. A comprehensive literature search was carried out with the use of PubMed from 1980 through 2011, and relevant articles pertinent to metabolic syndrome and prostate cancer are evaluated and discussed.

There is a lack of data on metabolic risk factors during pre-puberty, which is important for identifying the subgroups of youth, at whom early interventions should be targeted. In this study, we evaluated the prevalence of metabolic risk factors and its subsequent relations with dietary patterns in Korean pre-pubertal children through a cross-sectional sample (n = 1,008; boys = 513) of pre-pubertal children (aged 8-9 years) from a sub-study of the Korea Metabolic Syndrome Research Initiatives (KMSRI) in Seoul, Korea. Measures of anthropometry and blood pressure as well as fasting blood samples were used in the analysis. A three-day food records were collected. The metabolic syndrome was defined according to the age-adjusted National Cholesterol Education Program Adult Treatment Panel III guidelines. An added metabolic risk score was calculated for each subject by summing the quintile values of the five individual risk factors. Among the 5 risk components of metabolic syndrome, high waist circumference (WC) was the major factor (P < 0.001). A significant increasing trend of the added metabolic syndrome risk score was observed with the increase of WC (P (trend) < 0.001) among both genders. The cutoff point for high WC for pre-pubertal children was 61.3 cm for boys and 59.9 cm for girls. The prevalence of high triglyceride (TG) values was significantly higher in girls than it was in boys (P < 0.01). Girls in the highest quintile of balanced dietary pattern scores had lower TG values (P (trend) = 0.032) than did those in the lowest quintile. Moreover, girls in the highest quintile of western dietary pattern scores showed increasing trend for the added metabolic risk score (P (trend) = 0.026) compared with those in the lowest quintile. Adverse associations exist between western dietary patterns and the accumulation of metabolic risks among girls, not in boys, even during pre-puberty.
Purpose: This study was conducted to determine the impact of premature menopause (PM) on metabolic syndrome and its components to determine its contribution to the health of Korean women undergoing natural menopause. Methods: This study used data from the 4th to 8th National Health and Nutrition Examination Survey (2007~2021). A total of 11,989 women aged 30 to 65 who responded to questions about their current menstrual status, natural menopause status and its age. Complex sample multivariate Cox proportional hazards regression analysis was used. Results: Compared with women with late menopause, women with PM had a 1.34 times higher risk of developing metabolic syndrome. Excluding the risk of high blood pressure, women with PM had a significantly higher risks of increased waist circumference, decreased high-density lipoprotein, increased risk of high triglycerides, and increased fasting blood glucose. Conclusions: The results of this study provided data that can be applied to policies or interventions to prevent or manage metabolic syndrome in women with PM in Korea.
Research on walking assistance exoskeletons that provide optimized torque to individuals has been conducted steadily, and these studies aim to help users feel stable when walking and get help that suits their intentions. Because exoskeleton auxiliary efficiency evaluation is based on metabolic cost savings, experiments on real people are needed to evaluate continuously evolving control algorithms. However, experiments with real people always require risks and high costs. Therefore, in this study, we intend to actively utilize human musculoskeletal simulation. First, to improve the accuracy of musculoskeletal models, we propose a body segment mass distribution algorithm using body composition analysis data that reflects body characteristics. Secondly, the efficiency of most exoskeleton torque control algorithms is evaluated as the reduction rate of Metabolic Cost. In this study, we assume that the torque minimizing the Metabolic Cost is the optimal torque and propose a method for obtaining the torque.
