65세 이상 고령자에서 대사증후군 예측을 위한 지질비율 지표의 유용성 비교 (Comparison of the Usefulness of Lipid Ratio Indicators for Prediction of Metabolic Syndrome in the Elderly Aged 65 Years or Older)

  • 본 연구에서는 65세 이상 고령자를 대상으로 대사증후군 진단을 위한 지질비율 지표의 유용성을 비교하고자 하였다. 2018년 1월부터 2020년 12월까지 서울지역 종합병원에서 건강검진을 받은 65세 이상 1,464명을 대상으로 하였다. 혈액검사를 통해 혈장 동맥경화 지수를 포함한 지질비율 지표를 측정하였다. 지질비율 지표의 사분위수에 따른 대사증후군 유병률은 로지스틱 회귀분석으로 확인하였다. 또한 수신자 조작 특성(receiver operating characteristic, ROC) 곡선으로 지질비율 지표의 대사증후군 예측능력과 절단값을 추정하였다. 동맥경화 지수와 허리둘레의 상관성이 남녀 모두에서 가장 높았으며(r=0.278, p<0.001 vs r=0.252, p<0.001), 지질비율 지표는 1사분위수와 비교하여 4사분위수에서 대사증후군 발병률이 높았다. 혈장 동맥경화 지수는 다른 지질비율 지표보다 ROC 곡선 아래의 면적(area under the ROC curve; AUC)값이 남녀 각각 0.826(95% CI=0.799-0.850), 0.852(95% CI=0.820-0.881)로 가장 높게 나타났으며, 최적 절단값은 남녀 모두 0.44였다(p<0.001). 따라서 지질비율 지표 중 혈장 동맥경화 지수는 65세 이상 고령자의 대사증후군 진단에 가장 유용한 지표로 나타났다.

  • 한국 노인을 대상으로 대사증후군 예측인자로서 혈중 Non HDL-콜레스테롤과 중성지방/HDL-콜레스테롤 비율의 임상적용 가능성을 평가하고자 하였다. 2015년 1월부터 2017년 12월까지 경기지역 종합병원의 건강검진센터를 내원하여 건강검진을 실시한 65세 이상 노인을 1,543명을 대상으로 하였다. 대사증후군은 American Heart Association/National Heart, Lung, and Blood Institute (AHA/NHLBI)기준에 따라 진단하였다. 복부비만은 World Health Organization (WHO) West Pacific Region에서 제시하는 아시아-태평양인의 기준을 적용하였다. Non HDL-콜레스테롤은 총콜레스테롤과 HDL-콜레스테롤의 차이로 계산하였다. 대사증후군 예측력은 Non HDL-콜레스테롤보다 중성지방/HDL-콜레스테롤 비율이 높게 나타났다. 관련요인을 보정한 후 중성지방/HDL-콜레스테롤 비율은 1사분위수보다 4사분위수의 대사증후군 발병 위험이 높았다. 또한 대사증후군 발병을 예측하는 중성지방/HDL-콜레스테롤 비율의 적정 기준값은 2.8이었다. 중성지방/HDL-콜레스테롤 비율은 대사증후군 위험을 평가하는 간단하고 실용적인 지표가 될 수 있겠다.

Gender difference in the association of metabolic syndrome with hs-CRP Concentration of Blood

  • The high sensitivity C-reactive protein (hs-CRP) as one of the typical acute phase reactants is used for predictive factor of the cardiovascular disease and diabetes mellitus. In addition, there are claims that must be included as factors of metabolic syndrome. This research examined the relationship between the concentration of hs-CRP in blood and risk factors of the metabolic syndrome by gender, and the rates of metabolic syndrome depending on the hs-CRP level based on the general public who took the comprehensive medical check-up at Chonbuk National University Hospital in the Jeonbuk province. The subjects aged 17-87 years were participated, and 2,000 people were included as the final subjects except the persons with more than 10 mg/L of the hs-CRP of blood level. The hs-CRP concentrations increased according to the number of risk factors of metabolic syndrome in both men and women. In regards to the risk ratio of metabolic syndrome based on hs-CRP level in blood according to gender, the risk ratio increased by 3.07 times in male and 4.55 times in female intermediate risk group and 3.60 times in male and 6.15 times in female high risk group compared to hs-CRP low risk group. As a result, there was a proportional relation between hs-CRP level and the occurrence of metabolic syndrome, and it occurs more frequently among women than men.

2005년, 2007년 국민건강영양조사를 이용한 중년성인의 대사증후군 유무에 따른 영양섭취 평가-(1) (Assessment of Nutrient Intake for Middle Aged with and without Metabolic Syndrome Using 2005 and 2007 Korean National Health and Nutrition Survey)

  • The prevalence rate of metabolic syndrome has increased rapidly among the middle-aged and seems to be affected by socioeconomic factors, lifestyles and dietary habits. This research tries to find out the difference in dietary intake between middle-aged with and without metabolic syndrome. Using Korean National Health and Nutrition Survey (KNHANES) in 2005 and 2007, this study investigated 2,382 people (normal: 1,575, disease: 807) in 2005 and 1152 people (normal: 747, disease: 405) in 2007 (between the age of 40 and 64). Analysis was performed to discover the difference in nutrient intakes between people with and without metabolic syndrome. Also differences among people with various socioeconomic factors (such as age, education level, and income level), which can affect nutrient intake, were analyzed. In the nutrient intake people with metabolic syndrome has lower intake in most of nutrients than those of normal group, except carbohydrate. In 2007, normal group had higher intake in most nutrients, except for carbohydrate, sodium, potassium and vitamin A. Carbohydrate: protein: fat (C: P: F) ratio for metabolic syndrome group showed higher rate for carbohydrate, than normal group, in 2005, and 2007. Overall, the higher age and income level, the more carbohydrate intake rate is increase in metabolic syndrome group. The Quality of each nutrient intake was assessed using NAR (Nutrient adequacy ratio) and MAR (Mean adequacy ratio). According to the MAR, there was significant difference in 2005, 0.83 for normal group and 0.81 for metabolic syndrome group but there wasn't any in 2007 (0.81 for normal group, 0.82 for metabolic syndrome group). By NAR, in 2005, all nutrient except phosphorus, iron, vitamin A for Normal group higher then those of metabolic syndrome group (p < 0.05). In 2007 intake of metabolic syndrome group were higher then those normal group in most of nutrient by NAR. For age, education and income, MAR for normal group is higher then that of metabolic syndrome. In conclusion, Quality of nutrient intake in normal group is better then in metabolic syndrome group. Therefore, it is necessary to monitor dietary of intake people with metabolic syndrome, and necessary measures should be taken.

건강한 성인 남성의 대사증후군 위험 예측인자로서 혈청 요산/크레아티닌 비율 (Serum Uric Acid to Creatinine Ratio as a Predictor of Metabolic Syndrome in Healthy Adults Men)

  • 이 연구는 대사증후군 위험 예측인자로서 요산과 비교하여 혈청 요산/크레아티닌 비율의 유용성을 확인하고자 하였다. 2016년 11월부터 2018년 10월까지 수도권 소재의 일개 종합병원에서 종합건강검진을 받은 20세 이상 80세 미만의 성인 남성 14,190명을 대상으로 하였다. 대사증후군 진단기준은 American Heart Association/National Heart Lung and Blood Institute (AHA/NHLBI)의 기준에 따라 5가지 대사증후군 구성요소 중 3가지 이상 해당되는 경우 대사증후군을 평가하였으며, 이 중 복부비만은 WHO 서태평양지역의 기준에 따랐다. 이 연구결과 혈청 요산/크레아티닌 비율은 1사분위수보다 4사분위수의 대사증후군 및 대사증후군 구성요소의 높은 발생빈도를 보였다. 그러나 ROC 분석 결과 혈청 요산/크레아티닌 비율은 대사증후군 위험 예측에 있어 혈청 요산치보다 더 유용한 지표는 아니었으며(AUC, 0.554 vs 0.566), 요산치보다 낮은 민감도와 높은 특이도를 보였다. 결론적으로 혈청 요산/크레아티닌 비율의 유용성을 확인한 결과 대사증후군 위험을 예측하는 독립적인 지표로 사용하는데 제한이 있으며, 보조적인 표지자로 사용하는 것이 바람직하겠다.

일개 대학병원에 내원한 무증상성 열공성 뇌경색 환자와 정상인에서 비만 지수와 대사증후군 유병률 비교 (Comparison the Prevalence of Metabolic Syndrome and Obesity Index between Patients with Silent Lacunar Infarction and Normal Controls in a University Hospital)

  • This clinical study was done to examine comparison the prevalence of metabolic syndrome and obesity index between patients with silent lacunar infarction and normal controls. We compared the components of metabolic syndrome and obesity index between patients silent lacunar infarction(n=62) and normal controls(n=72). The analysis of the data was done by Frequency Analysis, two sample t-test, Chi-square Test. Hypertension and diabetes mellitus was significantly higher in patients of silent lacunar infarctions. Body mass index (BMI), waist-hip ratio (WHR), body fat percentage, obesity ratio, metabolic syndrome were higher significantly in patients of silent lacunar infarctions than normal controls. Although statistically insignificant, waist circumference showed relatively higher in patients of silent lacunar infarction. This result emphasizes the importance of managing that hypertension, diabetes, body mass index (BMI), waist-hip ratio (WHR), body fat percentage, obesity ratio, metabolic syndrome in silent lacunar infarction.

한국인 대상 대사증후군 진단 예측인자로서 허리-신장비 (The Study on Availability as a Predictor of Waist-to-Height Ratio for Metabolic Syndrome Diagnosis in Koreans)

  • Objectives: Recently, many studies have demonstrated that the waist-to-height ratio (WHtR) is more correlated with metabolic syndrome than the waist-hip ratio (WHR), body mass index (BMI). The aim of this study was to investigate and compare WHtR as a predictor of metabolic syndrome with other anthropometric indices. Methods: Approximately 3,800 data of subjects were used for the analysis, and data from Korean Medicine Data Centar). Receiver operating characteristic curve was performed and area under the curve (AUC) was calculated to find the usefulness of WHtR, WHR, and BMI. Results: WHtR was the most predictive factor with the highest diagnostic value for metabolic syndrome than WHR, BMI. AUC of WHtR was significantly higher in total subjects, men and women. Conclusions: WHtR may be the simple and effective anthropometric index for metabolic syndrome.

흡연 및 비타민 D 수준과 한국 성인 남녀 대사증후군의 연관성 (Association between Smoking Status, Vitamin D Levels and the Korean Adult Male and Female Metabolic Syndrome)

  • Purpose : Several studies suggest that smoking and vitamin D level is a risk factor of metabolic syndrome. The objective of this study is to evaluate the association between smoking status, vitamin D levels and the Korean adult male and female metabolic syndrome. Methods : We assessed 3796 participants aged 19 years and older from the Korean National Health and Nutritional Examination Survey 2013, 2014. Smoking statuses were collected from self-reported questionnaires. Subjects were divided into three categories: non-smokers, former smokers, and current smokers. Vitamin D was tested by Radioimmunoassay method and the value of serum 25-hydroxyvitamin D, which is an index of vitamin D status in the body, was used. The diagnosis of metabolic syndrome was made using criteria modified NCEP-ATP III. Logistic regression analysis was used to calculate odds ratios between smoking status, vitamin D levels, and metabolic syndrome. Results : The overall prevalence of metabolic syndrome was 23.0 % in men and 15.4 % in women. After adjusting for smoking, the odds ratio for men's metabolic syndrome in current smokers was 1.77 (95 % CI, 1.30~2.41), while for former smokers OR was 1.63 (95 % CI, 1.15~2.31) compared with nonsmokers. After adjustment vitamin D, the odds ratio for women's metabolic syndrome in vitamin D deficiency was 1.44 (95 % CI, 1.11~1.87) compared with normal. Conclusion : Smoking status was associated with an increased risk of metabolic syndrome in Korean adult males and decreased vitamin D level was associated with an increased risk of metabolic syndrome in Korean adult females.

펜타입 압저항 센서를 활용한 연령별 맥파 특성 및 맥파의 대사증후군에의 적용 가능성 평가 (Characteristics of Pulse Waves in Various Age Categories and Applicability of Pulse Wave to Metabolic Syndrome Using Pen-type Piezoresistive Sensor)

  • Objectives : The purpose of this study was to confirm that the pulse analyzer is useful for analyzing characteristics of variables of pulse waves in age categories, evaluating pulse waves of the metabolic syndrome group, compared with those of the non-metabolic syndrome group in Korean adults. Methods : The pulse wave variables were measured in Guan of all 1,056 subjects by the pulse analyzer, using a pen-type piezoresistive sensor. The physical measurement, blood test and survey were also performed by each subject. Results : In the age categories, height of pre-incisura (h2), height of tidal wave (h3), area of percussion wave (Aw), and width of percussion wave (w) increased in accordance with increase in age. While ratio of systolic period area (As) went up according to the increase of age, ratio of diastolic period area (Ad) went down. Radial augmentation index (R-AI), h2/h1, h3/h1, w/t and angle of percussion wave went up by aging, generally. Aw rate (Aw/At) also increased. Among the metabolic syndrome group, in the ages of 19 and 44, ratio of systolic period area (As) was higher and ratio of diastolic period area (Ad) was lower than in the non-metabolic group. w/t, Aw/At, and angle of percussion wave were higher than in the non-metabolic syndrome group. Among the metabolic syndrome group over the age of 60, height of pre-incisura (h2), height of tidal wave (h3), total area (At), area of percussion wave (Aw), radial augmentation index (R-AI), h2/h1 and h3/h1 were higher than in the non-metabolic syndrome group. Conclusions : The pulse analyzer is useful to analyze arterial stiffness in the age categories and in the metabolic syndrome group by some measures.

한국 남성의 금연과 대사증후군과의 관련성: 2013-2014년 국민건강영양조사 활용 (Association between smoking cessation and metabolic syndrome in Korean Men: Korean National Health and Nutrition Examination Survey 2013-2014)

  • Objectives: This study aimed to investigate the association between smoking cessation and metabolic syndrome components in Korean male and to clarify how long metabolic syndrome and its components remains after smoking cessation. Methods: A total of 4,408 participants from the Korean National Health and Nutrition Examination Surveys were included in this cross-sectional study. Smoking status was categorized into three groups, current-smoker, ex-smoker and non-smoker. Also smoking cessation period was categorized into three groups, less than 5 years, 5 years to 15 years and more than 15 years. Results: Smoking cessation were associated with prevalence of metabolic syndrome and its components. The multi-variate adjusted odds ratio for the prevalence of metabolic syndrome was 1.68(1.22-2.31) for smoker, 1.04(0.74-1.46) for ex-smoker, as compare with non-smoker; adjusted odds ratio for the prevalence of hypertriglyceride 2.17(1.77-2.66) for smoker, 1.27(1.02-1.59) for ex-smoker and high waist circumference 1.36(0.98-1.88) for smoker, 1.07(0.78-1.48) for ex-smoker, as compare with non-smoker. Conclusions: Smoking cessation in Korean male reduced odds ratio for the prevalence of metabolic syndrome and its 4 components except for hypertension.
