Hormone Therapy Review for Perimenopausal Symptoms: Focused on Perimenopausal Women without Other Risk Factors (폐경기 증상을 완화시키기 위한 개별화 호르몬 요법에 대한 고찰: 다른 위험요소가 없는 폐경기 여성을 중심으로)

  • As the population ages, the life of women after menopause becomes much longer than the past, and the quality of life of old age becomes increasingly principal issue. There is a period that women experience the physical symptoms of menopause, although there are differences in degree, and the management of this period is a suitable time for women to improve their quality of life. According to the menopausal hormone therapy (MHT) and Timing Hypothesis, which has been proven in the Kronos Early Estrogen Prevention Study (KEEPS) and the Early vs Late Intervention Trial with Estradiol (ELITE) study, a relatively young woman before and after menopause can benefit from long-term beneficial effects such as prevention of osteoporosis and cardiovascular disease by early initiation of hormone therapy to alleviate menopausal symptoms. MHT should be considered for all women in healthy (without other important diseases) menopausal years, expecting to improve their quality of life through symptom relief in menopausal women and, in the long term, to prevent cardiovascular disease and osteoporosis. When applying hormone therapy to individuals, it is necessary to establish various treatment strategies according to the menopausal symptoms of individual patients (individualization of treatment) and judge the suitability of clinical application.

A Case Report of Hwa-Byung with Menopausal Symptoms (갱년기 장애를 동반한 화병 환자 치험 1례)

  • Objectives : This study was performed to report the effect of Korean traditional treatments for Hwa-Byung with menopausal symptoms. Methods : In this case, a 48 years old female patient has been suffered by Hwa-Byung with menopausal symptoms for 3 years old. We treated the patient with Korean traditional treatments (acupuncture, herb-medication and aroma theraphy), mindfulness and Korean traditional psychotherapy. The effects of treatment were measured by Beck Depression Inventory(BDI), Zung Depression Rating Scale(SDS), Kupperman Index(KI), Menopause Rating Scale(MRS). Results : After treatment, the chief complaints and accompanying symptoms were subsided and improved. And BDI, SDS, KI and MRS score were decreased. Conclusions : According to this study, the Korean traditional treatment such as acupuncture, herb-medication is effective for the cure of Hwa-Byung with menopausal symptoms.

Factors Influencing Sexual Function in Postmenopausal Married Women (폐경후기 기혼여성의 성기능 영향요인)

  • Purpose: This study identified the level of marital intimacy, menopausal symptom and sexual function, and examined factors affecting sexual function in postmenopausal married women. Methods: With cross-sectional survey, a sample of 245 subjects aged 45 to 60 years was recruited from January to March, 2014. Data were collected using self-reported questionnaires which included marital intimacy questionnaire (MIQ), menopause rating scale (MRS), and female sexual function index (FSFI). Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and stepwise multiple regression. Results: Marital intimacy, menopausal symptom and sexual function scores of subjects were $3.46{\pm}1.14$, $13.71{\pm}6.61$, and $12.55{\pm}8.33$, respectively. Marital intimacy and menopausal symptom were related to sexual function. Variables including marital intimacy, psychosocial and urogenital menopausal symptom, period since menopause and body mass index therapy explained 51.5% in sexual function of participants. Conclusion: When nurses plan sexual counseling and education for sexual function of postmenopausal married women, it is necessary to consider marital intimacy, menopausal symptoms, and menopause-related factors.

A Case Report of Menopausal Syndrome treated by Herbal medicine based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 준거하여 한약 투여 후 호전된 갱년기 증후군 1례 임상 보고)

  • Objective : This case report aims to demonstrate the effect of Herbal medicine on menopausal syndrome. Methods : A 54 years woman suffered from menopausal syndrome with hot flush, insomnia and dyspepsia. According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Taeyang-byung and treated by Sosiho-tang decoction at first. But the next time the patient visited, she was treated by Oryeong-san decoction. The result of administrations were evaluated by Kupperman's index(KI), Menopause rating scale(MRS). Results : After administration of Sosiho-tang decoction for 35 days, KI was decreased from 50 to 19, MRS was decreased from 9 to 4. But her chronic dyspepsia wasn't getting better. So the KMD prescribed Oryeong-san decoction. After administration of Oryeong-san decoction for 14 days, KI was decreased from 19 to 0, MRS was decreased from 4 to 1. And her chronic dyspepsia was functionally improved. Conclusions : The patient completely recovered from menopausal syndrome and dyspepsia as treated by Sosiho-tang decoction and Oryeong-san decoction according to Diagnostic system based on Shanghanlun provisions.

Risk Factors for Endometrial Hyperplasia Concomitant Endometrial Polyps in Pre- and Post-menopausal Women

  • Purpose: To evaluate the risk factors for endometrial hyperplasia concomitant endometrial polyps in pre- and post-menopausal women. Materials and Methods: A total of 203 patients undergoing endometrial sampling before hysterectomy were evaluated in this retrospective study. Data recorded were age, gravidity, parity, body mass index (BMI: weight(kg)/$height(m)^2$), endometrial thickness (ET), menopausal status, presence of adenomyosis and diabetes mellitus. Results: Endometrial hyperplasia and polyps were detected in 13 patients. There were statistically significant differences in terms of age, menopausal status, morbid obesity and diabetes mellitus (p<0.005). Logistic regression demonstrated that menopausal status and presence of diabetes mellitus were independent risk factors. Conclusions: According to the current study; menopause and diabetes mellitus are strong risk factors for the presence of concomitant endometrial polyps and endometrial hyperplasia.

Two Case Reports of the Menopausal Disorder with Hot Flashes Treated with Hoechunboeum-tang (회춘보음탕(回春補陰湯) 투여로 호전된 열성홍조를 동반한 폐경기 장애 치험 2례)

  • Objectives: Hormone replacement therapy may relieve many symptoms of menopause; however, hormone replacement therapy may increase the risk of breast cancer, stroke, and dementia. This case study describes the effect of Hoechunboeum-tang on the menopausal disorder of hot flashes, which has been well documented as a cause of distress to patients undergoing menopause. Methods and Results: Two female patients suffering from severe hot flashes related to menopausal disorders were administered the herbal medicine, Hoechunboeum-tang. Measurements included hot flash frequency over a 24 hour period, hot flash severity (visual analogue scale), and hot flash-related daily interference and were recorded during the last week before herbal medicine and again after 7 and 15 days of herbal treatment. The patients were asked about their hot flash symptoms 6 months later and both reported that the symptoms had been successfully reduced after herbal treatment and were still reduced after 6 months. Conclusions: Hoechunboeum-tang could have a beneficial effect on menopausal disorder with hot flashes.

Influences of Physical Activities on Mental Health of Middle and Old Age Menopause Women : Using the 2010 Korea National Health and Nutritional Examination Survey Data (중년기와 노년기 폐경 여성의 신체활동이 정신건강에 미치는 영향 - 2010년 국민건강영양조사 자료 이용 -)

  • Purpose: The aim of this study is to examine the effects of physical activities on mental health of menopausal women. Method: The data was collected from 5th National Health and Nutrition Examination Survey. 810 samples of 40 to 64 year age group and 660 samples of 65 and over age group were used. To achieve the results estimated without bias stratification variables and colonies, descriptive statistics, Chi-square test, multiple logistic regression were used. Result: Flexibility exercises showed that it had an effect on stress and suicide intension of menopausal women. On the other hand, vigorous physical activity might be a stress factor of menopausal women. Conclusion: Menopausal women in 65 year and over appeared that they had more beneficial to low-intensity exercise to manage stress or suicide intension. This result will be used to develop the mental health promotion programs for menopausal women.

  • Objective: The purpose of this study is to evaluate the effects of Insamyang-yeongtang on menopausal disorder patients Methods : We surveyed 15 menopausal disorder patients who had been administered Insamyang-yeongtang. The patients of menopausal disorder had been estimated twice with Kupperman's index. Result : When patients had been not administered Insamyang-yeongtang. the mean of Kupperman's index score are $42.73(SD{\pm}19.16)$. When administered Insamyang-yeongtang, the mean of kupperman's index score are $25.87(SD{\pm}19.16)$. The difference mean of first and last score are 16.87(p-value 0.001). Conclusion : This study suggests that Insamyang-yeongtang is significantly effective on menopausal disorder patients.

Survey on Clothing Worn by Middle-aged Women Undergoing Menopause and Their Satisfaction with Garments According to Menopausal Symptoms (폐경기 증상 경험 유무에 따른 중년여성의 의복 구매 요인 및 만족도 조사)

  • This study investigated the garments of middle-aged women undergoing menopause and their satisfaction with them. Data were collected through a survey of 288 middle-aged women and analyzed using SPSS 17.0. As a result (in terms of degree of symptoms) backache, arthralgia, muscular pain and heat sensation were indicated as significantly more outstanding in the menopause group rather than the menopausal transition group. As a countermeasure to physiological symptoms, a change of temperature (warming or cooling) was applied for backache and arthralgia as well as taking medicine for stomachaches and muscular pain. Body parts suffering from heat sensations were the face, back neck, chest center, waist and hip. Body parts suffering from arthralgia and muscular pain were the shoulder and hip joints. The most significant purchasing factor for menopausal women was the fit of outerwear and antibiosis in underwear; however, women not undergoing menopause selected comfortableness when moving as the most important factor. In terms of body shaping and thermoregulation function, the satisfaction degree in underwear was significantly different between the two groups. Menopausal women wanted functional underwear that provided functions such as thermoregulation, absorption of secretions, and antibiosis.

The Effects of Line Dance Program on Muscle Strength, Balance, Flexibility, Pain, and Mood State in Post-menopausal Women (라인댄스가 폐경 이후 여성의 근력, 균형감, 유연성, 통증 및 기분상태에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of line dance program on muscle strength, balance, flexibility, pain, and mood state in post-menopausal women. Methods: A quasi-experimental research design (one group pretest-posttest) was employed. A total of 13 post-menopausal women completed 12-week line dance program(60min/day, 2days/wk) in S city. Descriptive statistics, Wilcoxon signed rank test were used in the data analysis by SPSS 18.0. Results: Muscle strength and balance were higher, and pain level were lower than before the line dance program. However, flexibility and mood state were not significantly changed after the line dance program. Conclusion: Findings of this study indicated that the 12-week line dance program were effective on muscle strength, balance, and pain relief in the post-menopausal women. Future study is needed to explore the effect of the line dance program on flexibility and mood state with the post-menopausal women.
