  • Purpose: The purpose of this study was to investigate the level of self management of patients with poorly controlled type 2 diabetes, and to investigate the factors influencing self management. Methods: The subjects consisted of 117 diabetes patients who visited the outpatient department of a university hospital from March to August 2008. Data were collected by asking the subjects to answer a 54-item questionnaire and were analyzed using the SPSS/WIN 14.0 program. Results: The total mean score of the patients in self -management was 4.38 out of 7. Medication adherence obtained the highest score under self-management, and the self-monitoring of blood glucose obtained the lowest score. Family support, self-efficacy, severity, and depression were found to be significantly correlated with self-management. In stepwise multiple regression analysis a total of 44.5% of the variance in self management was accounted for by family support, self-efficacy, severity, and depression. Conclusion: Therefore, a diabetic intervention program should be designed and provided for increasing family support, self efficacy, and severity and for reducing depression of patients with poorly controlled type 2 diabetes.

  • The objectives of this study were to survey the adverse reactions by Taeeumjowi-tang and to compare the tendency of symptoms by Sasang constitutional type. The clinical data for this study were based on the medical records of 78 cases collected from an oriental clinic. The major results of this survey are following: The 5.8% of Taeeum-in who took Taeeumjowi-tang showed side effects. And the 52.4% of Soeum-in and 41.6% of Soyang-in also showed side effects against Taeeumjowi-tang. They complained of excitation, irritability, restlessness, hypersensitivity, tinnitus, insomnia, upper abdominal discomfort, poor appetite, thirst, perspiration, dizziness, chest pain, tremor and evacuation troubles in common. And these adverse symptoms were similar to that by Ma-hwang. Therefore if we make a suitable medication guide for Ma-hwang based on Sasang constitutional type, it will be able to maximize the efficacy and minimize the adverse effect.

  • Purpose: The purposes of this study were to (a) explore the prevalence of sleep patterns of middle-aged women; (b) identify the relationship between subjective sleep quality and its related variables; and (c) predict the possible explanations of how the related factors affect subjective sleep quality of middle-aged women. Methods: A total of 256 women aged 40-60 years were purposely sampled and given self-administered questionnaires in a cross-sectional, correlation coefficient design. The data was analysed using t-test, one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression. Results: The results were as follows; (a) a total of 40% of the women were poor sleepers as defined by a global PSQI score >5; (b) there were significant differences in quality of sleep related to type of family, number of present illnesses, and use of medication; (c) health promoting behaviors, family functioning, depression, and climacteric symptoms were significantly related to subjective sleep quality; and (d) 34% of variance in subjective sleep quality can be explained by climacteric symptoms, depression, type of family, and spouse. Conclusions: Our findings suggested a high proportion of poor sleeping in middle-aged women. Poor sleeping appeared to have poor health promoting behaviors, lower family functioning, worse depression, and higher climacteric symptoms. Climacteric symptoms, depression, type of family, and spouse were independent risk factors for poor sleeping. Nurse professionals should screen for sleep problems in middle-aged women with a biopsychosocial and behavioral aspect.

  • Objectives : To research the trend of studies related to rheumatoid arthritis and to indicate the hereafter direction for its study in oriental medicine and its treatment. Methods : I reviewed 75 domestic oriental medical journals, and 50 medical journals about rheumatoid arthritis published after 2004, and comparative analysis was made. And these theses were classified by college, year, field of study, subject. Results : The following are the results of this study. 1. Classified by oriental medical college, Dae-jeon college published the most theses, followed by Dong-guk, Kyung-hee, Se-myung and Dong-shin college. Han-yang college published the most theses among college of medicine. 2. Classified by type of thesis, experimental theses(70 pieces, 94%) showed higher rate than that of clinical theses(4 pieces, 5%) in oriental medical studies. However, in medical studies, clinical theses(34 pieces, 68%) showed higher rate than that of experimental theses(15 pieces, 30%). 3. Analyzed by subject, the most dealt subject in oriental medicine was herb medication, followed by herbal acupuncture, single herb, electroacupuncture, sasang & gene, acupuncture & moxibustion, complex accordingly. The most dealt subject in medical clinical journals was standards of diagnosis & prognosis, followed by medication, gene analysis, pathogenesis, clinical pattern, operative treatment and complication accordingly. 4. In theses related to herb medication, most of the subject was to evaluate anti-inflammatory and immunoregulatory effects of herb medication with geopungseup, jibitong, hwalhyeolgeoer function. The tendency of experimental methods was focusing on understanding anti-inflammatory and immunoregulatory mechanisms through molecular biologic methods by analyzing cytokine and gene. 5. Most of theses related to herbal acupuncture were experimental studies verifying ant-inflammatory and immnoregulatory effects through methods observing change of cytokine and immunoregulatory factors. Regarding remedies for herbal acupuncture, Ulmus davidiana Planch was most preferred, followed by bee venom. 6. In theses related to single herb, Boik-yak was most prefered, followed by Geopungseup-yak and Hwalhyeolgeoer-yak. Regarding methods of research, there were tendency of shifting from methods verifying travail, anti-inflammatory and anti-pyretic effects through a test of behavior, morphometry, serology and temperature measurement of the rectum and the skin into verifying anti-inflammatory and immunoregulatory effects through observing inflammatory cytokine in the joint and cells of spleen. 7. In theses related to electroacupuncture, ST36 and adjuvant were most preferred as acupoints and induced factor. The tendency of experimental methods was turning from verifying mechanism of travail effect to analyzing inflammation and pain inducing factors. 8. Diverse medical clinical studies were published. Subjects such as diagnosis and prognosis, medication, gene analysis, clinical pattern, operational treatment, complication and pathogenesis were published. Especially, studies about standards of early diagnosis, and research on possible parallel medications with methotrexate were actively inquired. 9. Most of theses related to medical experimental studies was about mediators and receptors related to inflammatory induction and osteoclasia mechanism. Also, it was presented blockage of them can be effective on rheumatoid arthritis. Conclusions : The oriental medicine studies have merit in its diversity of treatment, but it clinical studies is lacking compared to experimental studies. Also, more diversity of subjects is necessary. Therefore, complementary measures are needed. Hereafter, oriental medicine research about rheumatoid arthritis needs more clinical research verifying the effectiveness and safety in clinical field. Also, we require oriental medical standard of diagnosis and researches on pathological generation which would make early checkup and prognosis possible.

  • Objective : In western medicine, many medication therapies and non-medication therapies have been treated for dementia. But these methods did no more than symptomatic therapies, not basic treatment, which just can better subordinate symptoms. In fact, care or control became the very point of treatment of dementia. So, to try to find oriental medical treatments for Dementia, a clinical analysis was carried out for 84 patients who were diagnosed as Dementia through MMSE-K and treated in Dong-shin Oriental Medicine Kwangju Hospital and Karitaas Sanatorium from 10. Jan. 1998 to 20. May. 2000. Methods : Regardless of patterns of Dementia, the basic treatment(本治) is to supplement encephalon deficiency(腦髓不足). And the external treatment(標治) must be accompanied by through diagnosis according to symptoms(辨證). All patients were given Yukmi-jihwanghwan gami and acupuncture treatment. Results : Dementia of cerebral vascular type shows a high distribution(51 patients, 60.7%). Women also are distributed more widely(62명, 73.8%). In Age distribition, 70-79 years-32 patients(38.1%), over 80 years-21 patients(25%), 60-69 years-17 patients(20.2%), 50-59 years-13 patients(15.5%) and under 49 years-1 patient(1.2%). From this, Dementia occurs well over 60 years and the rate of occurrence of Dementia is high as people grow older. In past history of people with Dementia, hypertension is associated much(50 patients, 64.1%). Distribution of mental state by MMSE-K examination declined in orientaion, mathmatical faculty, memory faculty and composition faculty. In therapeutic effect by treatment duration, 14 patients(93.3%) out of 15 in sanatorium show a significant effect. And 52 patients(75.4%) out of 69 in the hospital show the same result. Especially, 11 patients with Alzheimer type in sanatorium showed an good effect when treated over 4 months. 40 patients(83.3%) with cerebral vascular type out of 48 in the hospital showed the stage of betterment. Conclusions : The basic treatment(本治) for Dementia is to supplement encephalon deficiency(腦髓不足). For promoting encephalon through supplementing kindney(補腎健腦), All patients were given Yukmi-jihwanghwan gami. And the external treatment(標治) through diagnosis according to symptoms(辨證) were be accompanied by. The result is quite effective. We consider there must be more research based on this study.
