의료보험 통합과 지역의료보험의 재정분석 (The Merge of the National Medical Insurance System and the Financial Analysis of the Medical Insurance Program for the Self-employeds)

  • In Korea, the institutional reform for the national medical insurance system is in process. Eventually, three kinds of the national medical insurance system, i.e., medical insurance program for the industrial workers, the govemment employees and the private school teachers, and the self-employeds, would be merged into an unifed system. In this study, I analyzed the annual trends of the finance in the medical insurance system in Korea, in which I found the financial instability especially in the medical insurance program for the self-employeds. The regression analysis was carried out to forecast the accumulated reserve at the end of this year for the medical insurance program for the self-employeds. I also analyzed the economic effect of the merge of the medical insurance program for the self-employeds by using the case of Japan and Korea. I found that the medical insurance for the self-employeds is expected to have financial deficit at the end of the year 1998 after the merge. In onclusion, it seems to be quite difficult to solve the financial instability in the medical insurance program for the self-employeds after it would be merged. That means that there would be a lot of problems on the way to the merge.

지역의료보험 통합전후의 계층간 보험료 이전효과 비교 (Comparison of the Effect of Income-Redistribution before and after the Mergence of Medical Insurance Program for Self-employeds)

  • This study compared and analyzed the effect of income-redistribution, collecting data on the basis of the estimated details of insurance contribution and individual money wage lists for each one year before and after the combination of medical insurance program for industrial workers, by systematic sampling, extracting 4,160 families(14,764 people) among people applied to medical insurance program for self employees in Taegu City on the basis of Oct. 1st in 1998 with 227 associations of medical insurance program for self employees and medical insurance program for government employees and private school teachers combined, comparing the effect of income redistribution of before and after the combination of medical insurance program for self employees. The insurance contribution by household after the combination of medical insurance program for self employees showed the increase rate of average 20.9%, among them households of 68.8% increased and 31.2% decreased. The effect of income-redistribution was more positive because the degree of inequality was more deepened from 0.64 of the before-combination to 0.45 of the after-one in decile distribution ratio, from 0.26 to 0.34 in Gini -coefficient. Decile distribution ratio on the basis of insurance benefits by household was from 0.09 in the before-combination to 0.14 in the after-one, Gini-coefficient from 0.16 in the before-combination to 0.57 in the after-one was a little lowered. And decile distribution ratio of insurance benefits on the basis of insurance contribution was higher from 1.08 in the before-combination to 1.23 in the after-one, concentration index was a little lowered from 0.14 to 0.11, the effect of income-redistribution was improved in the phase of insurance benefits. The income-transfer rate of medical insurance program for self employees (the occupied rate of insurance benefits/ the occupied rate of insurance contribution) showed a lower trend in all of the before and after-combination towards upper classes, it was known that the income-transfer rate was higher from 1st degree to 7th degree in the after-combination in comparison with the before-one, but the effect of income¬redistribution was high because the income-transfer rate was lowered from 8th degree to 10th degree. The rate of medical insurance benefits (insurance benefits/ insurance contribution) increased from 0.79 in the before-combination to 1.07 in the after-one, and showed over 1.0 under 3th degree before the combination, but all of it was higher than 1.0 under 7th degree after the combination, the after-combination was more improved than the before-one in view of the rate of insurance benefits. As the result of above, on the basis of Oct. 1st in 1998 that 227 associations of medical insurance program for self employees was combined into one, we could say that the equality of imposing medical insurance contribution was more re-considered in the after-combination than in the before-one. But this study analyzed with classes divided, anyway, on the basis of insurance contribution, we have limit in explaining the correct effect of income-redistribution, because it was not analyzed according to classes of income, though it helps to analogize the effect of income-redistribution. So there must be analysis about the effect of income-redistribution, on the basis of the system, building up the system to grasp the correct income of the insureds of medical insurance program for self employees.

의료보험의 급여형평성 및 운영효율성 개선을 위한 제도간 비교연구 (A Comparative Study on the Beneficial Equity and Operational Efficiency of the Medical Insurance Programmes)

  • This study attempts to evaluate the beneficial equity and operational efficiency of the three Korean medical insurance programmes and thereby suggest directions for their policy improvement. Concepts of the equity and effciency were reviewed to develop indicators for comparative analysis. For the analysis, statistical and financial accounting data for 1991, issued by the National Federation of Medical Insurance and the Korea Medical Insurance Corporation, on the operational status and performances of the programmes, were collected and rearranged to be suited to the purpose of the study. The analysis reveals that beneficial inequity exists between self-employed and employee programs. and that operational inefficiency is prominent in both programms for self-employeds and for Government employees and private school teachers. In order to improve the beneficial inequity of the self-employed program, it is suggested that policies be formulated and implimented toward increasing the program revenue through increasing subsidies from the Government, and through inter- program finance adiustment. For the operational inefficiency of the two programs, it is judged that, toghether with the administrative support and control from the Government and the insurance society bodies, self- efforts be initiated to improve the internal mangement styles and systems of the insurance societies. Finally, from the viewpoint of the structural efficiency, expansion of the preventive insurance benefits by the insurance soceties is recommended both for beneficial equity and operational efficiency.

노인 의료보장제도의 개선 방안 (The Medical Insurance Program for the Elderly Person's)

  • After providing the purpose, scope and methods of present study in the first chapter. the second chapter discusses a theoretical overview on the social implication of medical expenditure and the medical insurance program for the aged population. In conclusion, to realize the reduction of aged population's medical expenditures, some possible plans are conceivable. Firstly, the payment level of medical insurance should be upgraded and the insurance coverage in oriental medicine treatment and the drug-store protection program need to be implemented. Secondly, the medical facilities and man-power have be expanded for the sake of reducing the social and geographical distances the aged population has to overcome to receive the medical benefits. Thirdly, the expansions of medical services for the home-stay aged are all the more required. Finally, a wide range of programs needs to be augmented to promote health for the aged population.

노인 암환자의 건강보험과 의료급여 이용차이 분석 (Differences of Cancer Patient's Health Care Utilizations between Medical Aid Program and National Health Insurance in the Elderly)

  • 본 연구는 건강보험심사청구자료에 대한 공변량분석과 다중회귀분석을 통해 노인 암환자의 건강보험과 의료급여 이용차이를 분석하고, 의료보장유형이 노인 암환자의 의료이용량에 미치는 영향을 분석함으로써 의료급여 환자의 도덕적 해이가 존재하는지 파악하는 데 목적이 있다. 연구결과, 첫째, 노인 의료급여 암환자가 건강보험 환자에 비해서 장기간 입원으로 총진료비를 많이 사용하고 있었다. 그러나 진료건당 외래진료비와 진료일당 입원진료비는 건강보험 암환자가 많아서 비용이 많이 소요되는 의료서비스를 더 많이 이용하고 있었다. 둘째, 노인 의료급여 암환자일수록 의료기관을 자주 외래방문하고 장기간 입원하여서 총진료비를 많이 사용하고 있었다. 그러나 진료건당 외래진료비와 진료일당 입원진료비는 건강보험 암환자일수록 증가하였다. 노인 의료급여 암환자들의 의료이용량 증가원인이 높은 비급여본인부담을 피해서 본인 부담이 없는 보험급여 의료서비스를 많이 이용 것에 있음을 알 수 있다.

ARIMA모델에 의한 피용자(被傭者) 의료보험(醫療保險) 수진율(受診率), 건당진료비(件當診療費) 및 건당진료일수(件當診療日數)의 추이(推移)와 예측(豫測) (Trend and Forecast of the Medical Care Utilization Rate, the Medical Expense per Case and the Treatment Days per Cage in Medical Insurance Program for Employees by ARIMA Model)

  • 공무원 및 사립학교교직원 의료보험과 직장의료보험에서 입원, 외래별 수진을, 건당진료비 (1985년 기준 불변가격), 건당진료일수 등의 장래예측을 통해 의료보험 진료비 안정화 방안을 마련하는데 기초자료로 제시하기 위하여, 이들의 $1979{\sim}89$년간 월별 통계자료를 이용, Box-Jenkins model인 ARIMA 모델을 적용하여 1994년 까지의 수진을, 건당진료비 및 건당진료일수를 예측한 결과를 요약하면 다음과 같다. 수진을, 건당진료비 및 건당진료일수의 ARIMA 모형을 제시하면 다음 표와 같다. 상기의 ARIMA 모형을 기초로하여 향후 5년간의 수진율을 예측한 결과, 공교의료보험 입원의 경우, 1989년의 실측치는 0.068건 이었으며, 1990년과 1991년은 0.068건, 1992년과 1993년은 0.069건, 1994년은 0.070건으로 연평균 0.7%정도 증가될 것으로 예측되었으며, 외래의 경우, 1989년의 실측치는 3.487건이었으나 1990년은 3.530건, 1994년은 3.668건으로 연평균 1%정도 증가될 것으로 예측되었다. 직장의료보험 입원의 경우, 1989년의 실측치는 0.063건이었으며, 1990년부터 1994년까지 모두 0.063건으로 안정될 것으로 예측되었으며, 외래의 경우 1989년의 실측치는 2.984건이었으나, 1990년은 3.016건, 1994년은 3.154건으로 연평균 1.1% 정도 증가될 것으로 예측되었다. 건당진료비의 향후 예측치는 12월을 기준으로하여 1985년 불변가격으로 공교의료보험 입원의 경우, 1989년의 실측치는 332,751원이었으나, 1990년은 345,938원, 1994년은 354,511원으로 연평균 0.6%정도 증가될 것으로 예측되었으며, 외래의 경우, 1989년의 실측치는 11,925원이었으나, 1990년은 12,638원, 1994년은 12,904원으로 연평균 0.5%정도 증가될 것으로 예측되었다. 직장의료보험 입원의 경우, 1989년 실측치는 281,835원이었으나, 1990년은 282,524원, 1994년은 293,973원으로 연평균 1%정도 증가될 것으로 예측되었으며, 외래의 경우, 1989년 실측치는 11,599원이었으나, 1990년부터 1994년까지 11,585원으로 안정될 것으로 예측되었다. 건당진료일수의 향후 예측치는 12월을 기준으로 하여 공교의료보험 입원의 경우, 1989년의 실측치는 13.79일이었으며, 1990년은 13.82일, 1993년과 1994년은 13.85일로 거의 안정될 것으로 예측되었으며, 외래의 경우, 1994년까지 5일 정도로 안정될 것으로 예측되었다. 직장의료보험 입원의 경우, 1989년의 실측치는 12.23일이었으나, 1990년은 12.30일, 1994년은 12.85일로 연평균 1.1%정도 증가될 것으로 예측되었으며, 외래의 경우 1989년의 실측치는 4.61일이었으며 1990년부터 1994년까지 4.60일로 안정될 것으로 예측되었다.

노인의료보장 및 건강관리를 위한 장$cdot$단기 대책 (Long-term Policy Development for the Aged on Medical and Health Care Security)

  • The ageing problem of the population has been emerging in Korea since 1970's so that it is expecting the elderly 65 years and over among the total population from $4.5\%$ in 1988 to be $6.3\%$ in year 2000. This study was conducted to provide secure policy development in coming years for the aged on medical and health care aspects based on the examining current status of the aged problems and health care policies and systems. The study divided into four parts; The first part examined the medical insurance program and public assistance program of the health services in relation to the aged. The second part emphasized on reduction of medical care cost for the aged. The third part studied the regular health check-up program and health education for the aged. The fourth part examined the chronic disease management programs for the aged and strategies of the health care service quality improvement and specialized programs. The following recommendations made as the results of the study. 1. At present, the medical insurance program and public assistance program for the medically indigent is not appropriate to the elderly because it is a part of general medical insurance program so that Health Security Law for the Aged is proposed. 2. Medical cost will be increased due to the high occupancy rate of hospital beds and long stay of the elderly so that it is recommended to develop an early discharge program, home health care program, Health hospice and an althernative programs. 3. At present, a regular health check-ups for the elderly is not included in medical insurance program so that it is recommended to be included in the insurance program and at the same time health education program thoroughly developed for the aged. 4. To make proper medical and nursing services on chronic diseases for the elderly, it is recommended manpower development, specialized clinics or hospitals, nursing homes and an equivalent long term care facilitices should be established on the community based and a research institutions also to be related to supper the care programs.

의료보험 적용대상자의 질병예방 및 건강증진사업 활성화 방안 (A Study on Program Development for Disease Prevention and Health Promotion)

  • The Korean people are all the beneficiaries of medical insurance or medical aid. It is important and needful to develop the preventive health program such as health examination and health education for disease prevention and health promotion of the beneficiaries. This paper diagnoses the status and problems of the current preventive health services to the beneficiaries and recommends how to develop the preventive health program in the medical insurance. This paper suggests that the government should strengthen the political support and supervision in order to develop the preventive health program in the medical insurance. In addition to the above suggestion, the following are recommended ; 1) to designate the large number of qualified hospitals for health examination. 2) to use the supplementary methods such as the health questionnaires in order to give the accurate health examination services. 3) to combine the health examinations by both laws of medical insurance and industrial health. 4) to arrange the manpower in charge of health education and to establish the health promotion centers. 5) to develop the effective mass media and materials for health education by use of TV, radio, VTR and slide projector.

건강보험에 있어서 의사와 환자간의 법률관계 - 임의비급여 문제를 중심으로 - (Legal Standings of the Patient and the Doctor within the National Health Insurance - With its focus on the issue of arbitrary medical charge cover -)

  • In providing general medical treatments, the medical service contract between the patient and the doctor is the mutually responsible onerous contract. However, the nature of the mutually assumed contract standings of the patient and the doctor has been changing since the implementation of the national health insurance program. For instance, besides the cases of beyond excessive medical charges and medical negligence, if the doctor charged for his/her medical treatments violating the post-treatment/nursing cover criteria, the overpaid medical charge, regardless of being collected with the patient's consent, has to be refunded back to the patient. Medically needed aspects, treatment results, and unfair benefits favoring the patient are not at all taken into consideration in the health insurance scheme. This makes it easier for patients to get refunds for their share of the medical payments by involving the Health Insurance Review & Assessment Service or the National Health Insurance Corporation, without engaging in civil law suits (for reimbursement claim) against doctors. In other words, the doctor's responsibility to provide medical treatments and the patient's responsibility to pay for the medical treatment provided within the contractual realm are being demolished by the administrational arbitration of the National Health Insurance system. The basic rights of medical service providers, and the patient's right to choose are as important constitutional rights, as the National Health Insurance program, which is essential in the social welfare system. Furthermore, the development of the medical fields should not be prevented by the National Health Insurance system. If the medical treatment services can be divided into necessary treatments, general treatments, and high quality treatments, the National Health Insurance is supposed to guarantee the necessary and general treatments to provide medical treatments equally to all the insured with limited financial resources. However, for the high quality treatments, it is recommended that they should not be interfered by the National Health Insurance system, and that they should be left to the private contract between the patient and the doctor.

의료보험 성인병 건강검진율의 결정요인 (Factors Affecting the Participation Rate in the Health Screening Program of Medical Insurance)

  • Objective : To analyze the factors affecting the participation rate in the health screening program of medical insurance. Method : We investigated the factors associated with the participation rate in the health screening program in Korea. Data were collected at the aggregate level from 145 employee health insurance societies and 227 self-employed health insurance societies from 1995 to 1997 Data were also collected at the individual level from four health insurance societies. This study hypothesized that the participation rate of the health screening program was related to 1) the characteristics of its members and the size of the health insurance society; 2) the specifications of the health screening program; 3) the venue of the health screening institution and the interests of individuals in the health screening program; and 4) the activities of the health insurance society. We used bivariate and multiple regression models to examine the factors on the participation rate of the health screening program. Results : First, in the case of dependents of on employee health insurance society, the ratio of dependents 40 years old and over, the average monthly contribution per household, the interest and satisfaction level of individuals in health screening, and the level of refunds for over-payment were all associated with the participation rate in the health screening program, accounting for 54.4% of the participation rate. Second, in case of those insured by the self-employed health insurance society, the interest and satisfaction level of individuals in health screening, the level of refunds for over-payment, and the performance level of on-the-spot health screening were statistically significant, accounting for 40.1% of the participation rate. Conclusion : The factors concerning the participation rate in the health screening program of medical insurance, in both a health insurance society and for individuals, were closely related to the age and gender of individuals and household contributions.
