Analysis of Factors Affecting Hand Hygiene Practices in Seafarers (선원들의 손위생 수행에 영향을 미치는 요인 분석)

  • Seafaring is an important occupation that requires stringent hand hygiene practices as a basic method for preventing food-borne illness and infectious diseases when the diseases occur on board. The purpose of this study is to provide fundamental data for the prevention of food-borne illness and infectious disease on the ship by investigating the level of hand hygiene practices and influencing factors. A total of 222 seafarers were surveyed at a seafarers' educational institution between July and August 2017. Their hand hygiene practice were examined by a modified method using the guidelines which are recommended by the World Health Organization, the Centers for Disease Control and Prevention, and Hand Hygiene Australia. The mean of hand hygiene practice was 47.97 out of 75 points. By category, the most frequent hand hygiene practice was measured as 4.04 on a 5-point scale as 'after working'. Factors affecting hand hygiene practices were ship tonnage relating to in job characteristics, exercise in health-related characteristics, and soap in relation to the characteristics of the hand hygiene environment on board. To improve hand hygiene among seafarers, it is necessary to raise awareness of hand hygiene among seafarers who work on small ships in particular, and to improve the systems of hand hygiene on ships with continuous education, hygiene practice evaluation and feedback.

A Study on the Effect of Caregiver Burden on Suicidal Ideation among Caregiver for the Elderly with Dementia (치매노인의 증상정도가 부양자의 자살생각에 미치는 영향에 대한 연구: 부양부담의 매개효과를 중심으로)

  • The purpose of the study is examining the mediation effect of caregiver burden's segmentalized sub factors between dementia caregivers on the relationship between Symptom extent of dementia patients and Suicidal Ideation of dementia caregiver, and suggesting social welfare intervention methods for dementia caregiver The survey is targeted to demented elderly people and caregivers, and currently using medical care institution and day care center in Seoul, Gyeonggi Province and Pusan city. As a result of the survey, 415 cases were collected for the final analysis. In data analysis process, we used SPSS 21.0 for the mediation effect of conversational satisfaction and its significance, and the results are following. First, 21% of the caregivers responded that they had thoughts of suicide in the past year. Second, Symptom extent of dementia patients was positively related to caregiver burden. Third, worse in family relationships, which is sub factors of mediate variable, has partial mediate effect on the model. Based on these outcomes, we suggest the importance and necessity of improved approach about dementia elderly and caregiver between elderly couple as way to reduce caregiver burden and proposed social work-based intervention program for enhancing this.

Guidelines and Status of Hospital Library in Korea: for Patients and Carers (국내 병원도서관 관련 기준 및 현황)

  • This study examined and analyzed the related study, Library Law, comparison of Korean Library Standards and IFLA guidelines and the status of domestic hospital library provision in order to grasp the regulations and status of domestic hospital library. As a result, the related study is very insufficient compared to overseas, and the Law and Standards for hospital libraries are not clearly and in detail in the Library Law. It is impossible to grasp the statistics and actual condition of the hospital library through the Yearbook of the Korean Library. The Korean Library Standards and IFLA guidelines also show differences in composition and key elements. According to the survey, 49 hospitals (1.3%), 8 hospitals (0.2%), 5 nursing hospitals (0.1%), 4 children's hospitals (0.1%), and 1 military hospital (0.0%) were among the 67 hospitals surveyed. Of the 49 general hospitals, 42 were advanced general hospitals, of which 17 were providing hospital libraries. The most urgent issue for providing hospital libraries is the revision of the Library Law and the Korean Library Standards, and the mandatory provision of hospital libraries, publicity through library magazines, and related research should be carried out.

Analysis of Existing Guidelines and Controlled Clinical Trials for Development of [Guideline of Clinical Trials with Herbal Medicinal Products for Colorectal Cancer] (결장직장암 한약제제 임상시험 가이드라인 개발을 위한 한약제제 대조군 임상시험 고찰)

  • Objectives: This study aimed to ascertain what should be considered in the "Guideline for Clinical Trials with Herbal Medicinal Products for Colorectal Cancer" by analyzing the existing guidelines and clinical trials. Methods: The development committee searched guidelines for herbal medicinal products for colorectal cancer that have already been developed. Then, clinical trials for colorectal cancer using herbal medicine were searched. The searched trials were analyzed in terms of inclusion and exclusion of participants, intervention, comparator, outcomes and trial design. Then, we compared the results of our analysis with the regulations and guidelines of the Ministry of Food and Drug Safety in order to identify the issues we will have to consider when making the "Guideline for Clinical Trials with Herbal Medicinal Products for Colorectal Cancer". Several guidelines for anti-tumor agents and clinical trials with herbal medicinal products were searched on the national institution homepage. In addition, 12 articles were searched using a combination of the following search terms: 'colorectal neoplasms', 'herbal medicine', 'Medicine, Korean traditional', 'Medicine, Chinese Traditional', 'medicine, East Asian medicine', 'medicine, Kampo', etc. Results: The characteristics of participants were various, such as people with medical histories of surgeries or recurrent cancers or who complained of chemotherapy-induced side effects. The types of interventions were also various and included decoctions, powders, intravenous fluids, intraperitoneal injections and gargles. Comparators used included placebos and conventional treatments. The outcome measurements used in the studies were quality of life, symptom score, tumor response, and survival duration, etc. Safety was evaluated by recording adverse events. Conclusions: Findings were made by reviewing existing guidelines and comparing them with clinical trials for colorectal cancer and herbal medicinal products. These results will be utilized in the development of the "Guideline for Clinical Trials with Herbal Medicinal Products for Colorectal Cancer".

A useful additional medial subbrow approach for the treatment of medial orbital wall fracture with subciliary technique

  • Background: To date, a variety of surgical approaches have been used to reconstruct the medial orbital wall fracture. Still however, there is still a controversy as to their applicability because of postoperative scars, injury of anatomical structures and limited visual fields. The purpose of this study was to introduce a useful additional medial subbrow approach for better reduction and securement more accurate implant pocket of medial orbital wall fracture with the subciliary technique. Methods: We had performed our technique for a total of 14 patients with medial orbital wall fracture at our medical institution between January 2016 and July 2017. All fractures were operated through subciliary technique combined with the additional medial subbrow approach. They underwent subciliary approach accompanied by medial wall dissection using a Louisville elevator through the slit incision of the medial subbrow procedure. This facilitated visualization of the medial wall fracture site and helped to ensure a more accurate pocket for implant insertion. Results: Postoperative outcomes showed sufficient coverage without displacement. Twelve cases of preoperative diplopia improved to two cases of postoperative diplopia. More than 2 mm enophthalmos was 14 cases preoperatively, improving to 0 case postoperatively. Without damage such as major vessels or extraocular muscles, enophthalmos was corrected and there was no restriction of eyeball motion. Conclusion: Our ancillary procedure was useful in dissecting the medial wall, and it was a safe method as to cause no significant complications in our clinical series. Also, there is an only nonvisible postoperative scar. Therefore, it is a recommendable surgical modality for medial orbital wall fracture.

Evaluation of the Appropriateness of Empirical Antibiotic Prescription after Implementation of Antibiotic Treatment Guidelines for Pneumonia in a Hospital (원내 폐렴 진료 지침 수립 후 경험 항생제 선택의 적절성 평가)

  • Background : The Antimicrobial Stewardship Program promotes interdisciplinary interventions and targeted recommendations for the proper utilization of antibiotics. In particular, the aim of the program is to avoid indiscriminate use of broad-spectrum antibiotics based on the documented literature on the significant impact of unsystematic usage of antibiotics on the distribution of antibiotic-resistant microorganisms. To improve the care process for pneumonia treatment using antimicrobial agents, institution-level guidelines were established and disseminated at the Seoul National University Bundang Hospital in April 2016. In this study, we evaluated changes in the physicians'antibiotic prescribing patterns both before-and after-the implementation of the guidelines. Methods : The electronic medical records of inpatients who were prescribed with one or more antibiotics in May 2014 (Group A) and May 2016 (Group B) were reviewed. Data on demographic characteristics, clinical outcomes, and antibiotic prescriptions were collected and the prescription records were compared both before- and after- the implementation of the guidelines. Results : A total of 180 patients were included in the study: 77 patients in group A and 103 patients in group B. The baseline characteristics of the patients were not significantly different between the two groups. Community-acquired pneumonia was the most common diagnosis in both the groups and the difference was not significant (68.8% vs. 67.9%; p=0.67). The type of antibiotic prescriptions used for empirical treatment was not different between the two groups. The most commonly prescribed empirical antibiotics were cephalosporins, with no significant difference (p=0.31). One of the most inappropriately used antibiotics was piperacillin/tazobactam and the rate of prescription was similar in both the groups (p=0.68). The rates of appropriate empirical selection of antibiotics remained unaltered between the two groups (67.5% vs. 71.8%; p=0.53). Conclusions : Implementation of the guidelines only exhibited no significant effect on the antibiotic prescribing patterns of physicians for the treatment of pneumonia. To improve the adequate use of empiric antibiotics, more active interventions and closer monitoring of the feedbacks should be additionally considered and evaluated in future studies.

Clinical Features and Outcomes of Pelvic Insufficiency Fractures (골반 부전 골절의 임상 양상과 치료 결과)

  • Purpose: The purpose of this study was to investigate the radiological and epidemiological characteristics, as well as the clinical course of pelvic insufficiency fractures in the elderly population. Materials and Methods: At a Haeundae Paik Hospital, we retrospectively reviewed patients with pelvic insufficiency fractures between March 2010 and May 2017. The demographic data of patients were analyzed, and bone mineral density and bone turnover markers were evaluated to estimate the metabolic status of the bone. The radiological characteristics were evaluated by comparing the simple x-ray images with the computed tomography images, and the types of fractures were classified via computed tomography images. For clinical course evaluation, we investigated comorbid complications, and compared the walking ability scale before and 6 months after the fracture. Results: A total of 42 patients were included, with an average age of 76.5 years. All were female except one case. In 5 cases where the initial medical examination was from another institution, the fracture was not found in 3 cases. All cases received conservative treatment. After the diagnosis of pelvic bone fracture using a simple x-ray imaging, additional fractures were found in 81.0% of the study population using a computed tomography. Initiation of gait occurred at an average of 2.8 weeks, and every case except 1 (97.6%) fully recovered their gait ability. Conclusion: We concluded that there was a limitation with diagnosing pelvic insufficiency fracture using only a simple x-ray imaging technique. In general, cases in this study showed conservative treatment yielded favorable clinical outcome with relatively less critical complications.

Characteristics and Death Risk Factors of Patients in Long-Term Care Hospital Connected to Special Hospital (일개 특수 요양병원 환자들의 특성 및 사망 위험요인)

  • The objective of this study is to provide high-quality medical service, by understanding the characteristics and death risk factors of patients at V long-term care hospital, that has established and operated the cooperative treatment system in connection with a public institution, the S veterans hospital. A follow-up survey targeting a total of 850 discharged patients was conducted for the year 2017. Average age of patients was 79.2 years, with 86.4% belonging to the 70 years or over group. The men of national merit were 75.4%, which was more than three times higher than general patients. Hospitalization for 180 days or more was 46.4%, which was higher than general long-term care hospitals (37%). The major death risk factors were age and the number of hospitalized days. Our data indicate that transfer to S veterans hospital results in providing quicker treatment and higher effectiveness of the numerous recovery therapies. We therefore propose that the cooperative treatment system should be expansively operated to increase the quality of happy life, by alleviating the health of patients with chronic diseases in a long-term care hospital.

A Study on the Appropriateness of Community Rehabilitation Job Knowledge and Education for Community Care : Focusing on the Job as an Occupational Therapist (커뮤니티케어를 위한 지역사회 재활 직무 지식과 교육의 적절성 조사연구 : 작업치료사 직무 중심으로)

  • This study was conducted to find out the appropriateness of knowledge and education of occupational therapists' community rehabilitation jobs for community care. A questionnaire was developed to find out the job knowledge level of occupational therapists working in the community and the appropriateness of school education and maintenance education, and the validity and reliability were verified through a preliminary survey. Based on the verified questionnaire, a survey was conducted on occupational therapists working in the community. As a result, overall, the average score was low in the level of knowledge about the job of occupational therapists engaged in the community and the appropriateness of school education and maintenance education. Through this study, we were able to know the necessary knowledge in the practice of community occupational therapy, understand the level of knowledge retention of occupational therapists working in the community, and learn about the appropriateness of school education and maintenance education. Referring to the results of this study, if a systematic education program related to community occupational therapy jobs is developed in existing medical institution-centered education, the job knowledge level of occupational therapists in the community can be raised. Amid the rapidly changing times and the pandemic of infectious diseases, additional research is needed on job-related knowledge and education corresponding to various occupations in charge of rehabilitation in the community.

Analysis of the Characteristics of Children and Adolescent Patients Received Sealant after National Health Insurance Coverage using Big Data (빅데이터를 이용한 치면열구전색 급여화 이후의 소아청소년 치면열구전색 환자에 대한 분석)

  • The purpose of this study was to analyze the characteristics of pediatric and adolescent patients who received sealant after health insurance coverage based on demographic factors such as gender, age, insurance type, care institution and region. Patient Sample Data from the Health Insurance Review and Assessment Service were used for this study. A total of 8,454,636 patients' data were obtained from 2010 to 2017. Of these, 114,680 patients got sealant treatment. Females were more likely to receive sealant treatment than males. 5 - 9 year age group showed the highest number of patients and proportion of treatment. Patients with health insurance were more likely to receive pit and fissure sealant treatment compared to patients with medical aid program. The number of sealant patients and the proportion of sealant treatment were the highest in dental clinics, followed by dental hospitals and public health centers. The number of sealant patients were the highest in Gyeonggi and proportion of sealant patients were the highest in Jeonbuk.
