미국 보건의료시설 가이드라인의 체계 특성 연구 (A Study on the Characteristics of the System of the US Health Care Facility Guidelines)

  • Purpose: At the time of consensus on the necessity of appropriate guidelines for health care facilities suitable for Korea, the paper aimed to analyze the characteristics in the aspect of the system of the US Health Care Facility Guidelines(FGI Guidelines) and to suggest implications. Since the system is significant for the guidelines to be socially accepted and operated, this study focuses on the system. Methods: Literature studies focusing on the analysis of prior studies and the 2018 edition of the FGI Guidelines were conducted. Results: As a result of reviewing the history and outline of the FGI Guidelines, the derived characteristics of the system were governance, procedure, and composition. First, it had multi-layered organization and multidisciplinary members. Secondly, the focus is on the procedure of gathering opinions from industry and the public. Third, the Guidelines have been continuously evolving to reflect the needs of the times and changes in the medical environment, and the content framework and method of writing are user-friendly in order to prevent confusion caused by vast contents. Implications: First, we need to consider the composition of a private organization to prepare Korean health care facility guidelines. Second, it needs a system that can induce the participation of multidisciplinary experts. Third, a comprehensive and user friendly guideline needs to be constructed.

  • As part of alternative of elderly problems in the future super-aging society, this study aims to extract an improvement plan of interior environment design of Seoul-type day care centers to expand in future by observing day care centers as medical and healing environments, from the perspective of user experience. For this study, after selecting major activity areas of representative cases of Seoul-type day care centers, user behaviors in the space were observed and 4 seniors of baby-boom generation who are potential uses were chosen as personas by reflecting characteristics of healthy elderly, physical aging, mental aging and infirm elderly, from the perspective of user experience. Based on previous studies on healing environments, these characteristics and demands of personas were classified into environmental aspects, physical aspects and mental aspects to extract an improvement plan. The study results are as follows. First, interior environment design reflecting persona characteristics helps the elderly adapt to the environment and provides a comfortable environment like home for stability, treatment and recovery of memory. Second, interior environment design reflecting persona characteristics helps psychological healing through light entering the large windows and natural environment elements. Third, for interior environment design reflecting persona characteristics, finish materials with bright and soft colors should be used to change the images of facilities positively. Fourth, spatial composition should be remedied through division of space and built-in furniture depending on the needs of users for loitering of extra time and programs.

우리나라 학교보건사업(學校保健事業) 변천에 관(關)한 연구 (A Study on the Analysis of School Health Program by the Chronological Events in Korea)

  • The purpose of this study was to identify and name clusters of school health program, and to describe some of the characteristics of administratie supports. The literature, materials and public documents were analysed by the chronological events from 1945 to 1989. The result of this study is as follows : 1. A brief summary of the history of school health program was included as an introduction to the analysis of the current programs of school health. Five current school health-program clusters were identified from findings of a study of programs : 1) physical assessment, laboratory examination and health services for the students, 2) health instruction 3) healthful living condition (environmental health), 4) health clinic management, 5) administrative supports. 2. The earliest school- based efforts focused on communicable disease pevention by the ministry of health and social affairs. Annual medical inspection (health assessment) for school children for eyes, ears, nose, and throat were mandated nation-wide in 1951 by physical assessment Act. 3. In 1979, the health instruction of schools to improve the health status of students was improved by health department in the Ministry of Education 4. Experiences in healthful environment were basic components of the school health program. However, without careful planning and supervision these experiences were not contributed to the goal of school health. The formal program of school health environment were initiated in 1979 5. In 1980, the guidelines of school health clinic management were prepared by Ministry of Education such as guidance of essential degrees and facilities in school health clinic. 6. Two patterns of administration of school health programs existed in Korea. In one the school health department operated its own health program and in the other the physical education department operated the health program within the school system. The school health department was established in Ministry of Education from 1979 to 1982. Improved school health programs will be a key element in the comprehensive national child health policy which I will ask the Ministry of Education to develop for the Department.

  • Continuously motivating people to exercise regularly is more important than finding a way out of barriers such as lack of time, cost of equipment, lack of nearby facilities, and poor weather. Our proposed system presents practicable methods of motivation through a diverse exercise aid system. The Health Improvement and Management System (all-in-one system which saves space and maintenance costs) measures and evaluates a diverse body shape analysis and physical fitness test and directs users to automated personalized exercise prescription which is prescribed by the expert system of three types of exercise templates: aerobics, anaerobics, and leisure sports. Automated personalized exercise prescriptions are built into XML based documents because the data needs to be in the form of flexible, expansible, and convertible structures in order to process various exercise templates, BIOFIT, a digital exercise trainer, monitors and provides feedback on the physiological parameters while users are working out in the gymnasium. If these parameters do not range within the prescribed target zone, the device will alarm users to control the exercise and make the exercise trainer adjust systemically the proper exercise level. Numeric health information such as the report of the physical fitness test and the exercise prescription makes people stay interested in exercising. In addition, this service can be delivered through the Internet.

  • Recently, appearance is recognized as competitiveness as well as self expressing means, so understanding of general people have been rapidly changed. Also, since modem people have interest in an operation as well as various cosmetic treatment fields, a plastic surgery clinic is expanding its region to laser treatment and skin care for modem people. However, the plastic surgery clinic is not located in a building for only hospitals, but located in general neighborhood living facilities or an office building, so medical treatment is performed at the place. It is often found that a building plan can not conform to conditions that the hospital requires. This study is to understand a plane deciding factor of the plastic surgery clinic by analyzing it in a limit of building space and functional aspects of the plastic surgery clinic. A study method is first to investigate space composition according to the function, area allocation according to the function and a space privacy region after classifying study objects into large, middle, small scales so as to understand a functional role of the plastic surgery clinic, and secondly to analyze on the base of length of long and short edges of space and a moving line system after classifying common space types of the plastic surgery clinic through plane analysis of the study objects. As a result of the study, functional space difference according to the scale was shown, and the common space types were affected by length of the long and short edges, and it can influence space composition.

노인전문병원 내 재활치료 공간 구성 특성에 관한 연구 (A Study on the Characteristics of Rehabilitation for Space Configuration in the Special Clinic for the Elderly)

  • It requires great efforts to satisfy all functional characteristics of the rehabilitating treatments space for the specialized clinic for the elderly as the rehabilitating scope for treatment of the clinic has become more specialized and sectionalized. The specialization of the clinic for the elderly is saliently different in terms of rehabilitation treatment. The changed concept of rehabilitation for the elderly means that it places great importance in helping the elderly in their day-to-day life through a short-term or outpatient treatment, not a long term stay at hospital for retreat. The rehabilitation space in the clinic is essential in that it provided environment for efficient space placement in varied location. Thus, this study has been researched on 6 clinics for the elderly. It is because the space will have a variety of functions, which will be for the sake of the effective rehabilitation treatment of those aged patients who use that medical facilities, and not only for the physical therapy. It could have been designed to accommodate and realize not only the physical well-being, but also psychological, social and spiritual integrity for a variety of treatments, with which as the basis, this study theorizes the characteristics of the essential elements and the specialization factors by extracting spatial element through theoretical investigation. In conclusion the study here proposes the planned guideline for the rehabilitation treatment space to be designed inside the specialized clinic for the elderly, with renewed awareness of the importance on the spatial element in a recently established clinic.

문화(집단)마을과 기존농촌마을의 비교평가에 관한 연구 - 월암 1리와 우항 2리를 중심으로 - (A Study on the Comparative Evaluation between the Planned Distict(Munhwa-Maul) and the Existing District in Korean Rural Village : The Cases of Wolam-1-Ri and Woohang-2-Ri)

  • As a basic research for the reorganization of the rural villages and the enhancement of the rural residential environment this study intends to compare attitudes and perceptions of the residents in the planned districts(Munhwa-Maul) and the existing districts in the two rural villages, Wolam-1-Ri and Woohang-2-Ri The major fadings of this study can be summarized as follows 1) The three types of reorganization of the rural villages are identified $.$ addition of the new residential site to existing village, development of a new village in a new site, redevelopment of existing village 2) The residents have low degrees of satisEaction at medical facilities, park and green spaces, public open spaces 3) The rates of residents who think their village territory include both the planned and the existing district is higher in the Woohang-2-Ri than in the Wolam-1-Ri 4) 57.6% of the farm householders in the two planned districts think the lot sizes are small, But the majority of the residents think the sizes of their houses are adequate. 5) A large number of residents show positive response to the development of the planned district 6) Since the majority of the residents(about 70%) in the two planned districts are not farm householders, new strategies need to be deveolped to induce more farm householders to the planned district.

체형 안면 소증 및 성정 특성 기반 사상체질 진단 프로그램 개발 (A Development of Sasang Constitution Diagnosis Program Based on Characteristics of Body Shape Face Physiological Symptom and Personality)

  • 1. Objectives : In this study we derived constitutional characteristics both by using questionnaire on character and symptom and by collecting objective measurement data through face, body shape. Furthermore, by developing a program, we intend to help one's diagnosis of Sasang constitution. 2. Methods : Through November 2007 to July 2009, we obtained questionnaire, face, and body shape data of 958 constitution confirmed subjects within the age range of 10-80 from 19 oriental medical facilities in the country. According to sex, we divided the subjects into two groups as real constitution and non-constitution group respectively. We analyzed the questionnaire through chi-square test (p<.01) and facial and body shape data through unpaired T-test (p<.01). By using weight law for questionnaire and euclidean distance for body shape and face data, we expressed constitution possibility to develop a supplementary program. 3. Results : The concordance rates of constitutional diagnosis by using the program for Taeeumin, Soeumin, Soyangin are, 76.7%, 61.5%, 67.4% resulting in 69.6% in male and 64.1%, 64.7%, 65.1%, resulting in 64.7% In female respectively. New cases tested on the program showed concordance rate of 65% in male and 62% in female with real constitution. 4. Conclusion : The constitutional diagnostic program based on the face, body shape and questionnaire, may have significant meaning as a supplementary tool in the constitutional diagnosis for clinical expert.

사상체질별 안면의 거리 각도 비율 특성 (Characteristics of Distance, Angle and Ratio among the Face Point on Photo in Sasang Constitutional Medicine)

  • 1. Objectives: We aimed to derive constitutional facial characteristics with front and side picture of the people in this study 2. Methods: Through November 2007 to July 2009, we obtained front and side face photograph data of 715 male/female constitution confirmed subjects within the age range of 10-80 from 19 oriental medical facilities in the country. According to sex, we divided the subjects into two groups, real constitution group and non-constitution group. We analyzed significant variables of distance, angle and ratio of facial point through unpaired T-test. 3. Results: There are different significant variables according to Sasang Constitution and even though they are much different between sex, the interpretation of the meaning of those variables are similar with the written characteristics of ancient Writing. The face size of the Taeeumin is bigger than that of the non-Taeeumin, and especially they have long length from mouth to mandible angle. Soeumin have small jaw area, long philtrum and narrow facial feature like egg. Soyangin has developed forehead and glabella, short jaw, long eye side. Taeyangin has developed forehead, weak nose especially side end of nose, long eye side. 4. Conclusions: Sasang Constitutional facial characteristics are different between man and woman but there are much similarity with the facts as known on the interpretation of significant variables characteristics.

Diabetes Management and Hypoglycemia in Safety Sensitive Jobs

  • The majority of people diagnosed with diabetes mellitus are in the working age group in developing countries. The interrelationship of diabetes and work, that is, diabetes affecting work and work affecting diabetes, becomes an important issue for these people. Therapeutic options for the diabetic worker have been developed, and currently include various insulins, insulin sensitizers and secretagogues, incretin mimetics and enhancers, and alpha glucosidase inhibitors. Hypoglycemia and hypoglycaemic unawareness are important and unwanted treatment side effects. The risk they pose with respect to cognitive impairment can have safety implications. The understanding of the therapeutic options in the management of diabetic workers, blood glucose awareness training, and self-monitoring blood glucose will help to mitigate this risk. Employment decisions must also take into account the extent to which the jobs performed by the worker are safety sensitive. A risk assessment matrix, based on the extent to which a job is considered safety sensitive and based on the severity of the hypoglycaemia, may assist in determining one's fitness to work. Support at the workplace, such as a provision of healthy food options and arrangements for affected workers will be helpful for such workers. Arrangements include permission to carry and consume emergency sugar, flexible meal times, selfmonitoring blood glucose when required, storage/disposal facilities for medicine such as insulin and needles, time off for medical appointments, and structured self-help programs.
