Study on Medicine Related Policies for Management Strategies and Performances of the Pharmaceutical Industry (제약산업의 경영전략, 경영성과에 의약품관련정책이 미치는 영향에 관한 연구)

  • Objectives : The aim of this study was to investigate the effect of various Republic of Korea policies, including the medicine cost rationalization policy implemented By investigating the management strategy/performance changes of the industry caused by such policies. Methods : Pharmaceutical companies listed on the national stock exchange and the KOSDAQ were examined for 10 years from 2004 to 2013. Their financial statements were collected and market changes before and after drug price policy implementations were compared. Results : From the result of this research, among the medicine related policies, the drug price rationalization policy was found to have a significant effect on business management strategies in terms of safety, profitability and growth potential after its implementation. After the drug pricing transparency policy was implemented, management strategies were affected significantly in terms of safety and profitability. Conclusions : As a result, all of the medicine related policies were found to have regulatory effects. Based on these findings, implications, research limitations and future study plans are presented.

Interbed Networks in la Patient Monitoring System (환자 모니터링 시스템에서의 통신 방식(II): 인터베드 통신망)

  • In this paper, we present the design and implementation of the interbed network communication protocol, which links patient monitors, central stations, DB servers, and clinical workstations together in a patient monitoring system. We describe the requirements to be met thor real-time patient monitoring, propose 2 services Patient Locator Service(PL:7) and Remote Patient Monitoring Service( RPMS). PLS provides the information about how many patients are currently being monitored and where they are located, while RPMS allows the doctors to monitor their patients'vital sign in real-time. The messages for the services, their formats and exchange scheme are also presented with a whole picture of how they are implemented. We adopted the object-oriented programming paradigm in all the analysis and design processes. In the experiment performed in a real clinical setting, the services turned out to meet all the requirements needed for real-time patient monitoring.

  • The one of the necessary technologies to live in the ubiquitous society that we can exchange some informations whenever it is and wherever we are, is the technology of context-awareness. The technology of context-awareness can be used various fields-medical science, national defense, national security and etc. and the technology can affect our society by and large. The technology of context-awareness is also applicable to many fields in the urban transit. In this paper, we describe the present condition of the technology of context-awareness and the examples of practical application of it and suggest some plans that are applicable to build an intelligent surveillance system for urban transit. We can connect the operating officer who is in the nearest to the place where the some events happened within the urban transit service area through the intelligent surveillance system for urban transit and we can eel a foothold in perceiving automatically various events within urban transit service area. So, the technology of context-awareness is the basic technology to realize an intelligent surveillance for urban transit, for the security of passengers and the protection of facilities in urban transit.

  • 'Supplement 23:DICOM SR' announced on April 2000 made possible co-relations between the clinical reports and human body parts by standardizing the exchange of clinical information. DICOM SR uses its own coding schemes and values to represent information, which convey hierarchically structured content items. This paper analyzes radiology reports produced in HIS(Hospital Information System) and categorizes content structures into three parts such as, finding, conclusion, and recommendation, and implements a system which can be used by clinicians. Digital X-ray images and their reports have been managed separately. Since the report made by DICOM SR can refer to images, the integrated information of reports and images is possible.

  • HL7 is well-Known standard protocol for text data generated in hospital information systems. Vital sign information web viewer systems is also the standard protocol for medical image and transfer. In order to embrace new technologies as telemedicine service, it is important to develope the standard protocol between different systems in the hospital, as well as the communication with external hospital systems. In this paper, we proposed integration method between vital sign web viewr systems and hospital information systems. Through the proper data exchange and modification of information management, HIS will offer better workflow to all hospital employee.

Molecular Pharmacological Interaction of Phenylbutazone to Human Neutrophil Elastase

  • Human neutrophil elastase (HNElastase, EC 3.4.21.37), a causative factor of inflammatory diseases, was purified by Ultrogel AcA54 gel filtration and CM-Sephadex ion exchange chromatography. HNElastase was inhibited by phenylbutazone in a concentration dependent manner up to 0.4 mM, but as the concentration increased, the inhibitory effect gradually diminished. Binding of phenylbutazone to the human neutrophil elastase caused strong Raman shifts at 200, 440, and 1194 $cm^{-1}$. The peak at 1194 $cm^{-1}$ might be evidence of the presence $of\;-N=N-{\Phi}$ radical. The core area of the elastase, according to the visual molecular model of human neutrophil elastase, was structurally stable. A deeply situated active center was at the core area surrounded by hydrophobic amino acids. Directly neighboring the active site was one positively charged atom and two atoms carrying a negative charge, which enabled the enzyme and the drug to form a strong interaction. Phenylbutazone may form a binding, similar to a key & lock system to the atoms carrying opposite charges near the active site of the enzyme molecule. Furthermore, the hydrophobicity of the surrounding amino acid near the active site seemed to enhance the binding strength of phenylbutazone. Binding of phenylbutazone near the active site may cause masking of the active site, preventing the substrate from approaching the active site and inhibiting elastase activity.

  • This study is the analysis of characteristics of hospital space for the holistic healing environment of children on the basis of the healing theory on emotion of Max L$\ddot{u}$scher and the healing theory on senses of Rudolf Steiner of the overseas children's hospitals. According to the theory of Rudolf Steiner, children make comprehensive advancement of physical, spirit and soul through the interaction of 12 senses. In addition, Max Luscher asserts that the psychological health can be maintained when the 4 emotional status in content, confidence, self-esteem and liberty make the balance. The 4 elements of Luscher have correspond with spatial expression elements for refuge, flow, prospect and void, and this is analyzed in conjunction with the eight kinds of behavior affordance of ying and yang (-, +) that helps our the advancement of 12 senses of Steiner. WORK & STUDY(+), REST(-), MOBILITY(+), VISUAL SEQUENCE(-), SOCIAL EXCHANGE(+), REFRESHMENT(-), COMMUNITY(+) and MEDITATION (-) would be the 8 kinds of behavioral affordances to make the healing environment as presented in this study and the case analysis for children's hospitals have been undertaken.

Radiochemical separation of 89Zr: a promising radiolabel for immuno-PET

  • $^{89}Zr$ with the favorable nuclear decay kinetics and chemical properties is an appealing radiometal for its application in immuno-PET using radiolabeled monoclonal antibodies. Rising demand of ultrahigh purity and high-specific activity $^{89}Zr$ has propelled the radiochemist worldwide to develop an overall efficacious method for its promising separation from the target matrix $^{89}Y$. The requirement of elevated radiochemical purity (${\geq}$ 99.99%) has accelerated the efforts since last two decades to achieve higher decontamination and separation factors of carrier free $^{89}Zr$ over $^{89}Y$ using several suitable separation techniques. However, each of the technique has its own pros and cons which prior to its actual medical application needs to be optimized and thoroughly scrutinized to avoid further complications during radiolabelling of the pharmaceuticals. In this short review article we will specifically consider as well focus on the historical development and the recent advances on the radiochemical separation of $^{89}Zr$ from $^{89}Y$ which will be helpful for the separation scientist involved in this area to understand the existing available means and plan the strategy to investigate and develop the novel techniques to overcome the problems involved in the present methods.

A Korean Family with Cholesterol Ester Transfer Protein Deficiency

  • A 32-year-old female patient and her sister show high levels of high density lipoprotein (HDL) cholesterol in regular health checkups, since female patient was 11 years old. The patient's serum total cholesterol was 285 mg/dL and HDL cholesterol was 113 mg/dL. Her sister's total cholesterol was 240 mg/dL and the HDL cholesterol measured to be 90 mg/dL. Lipoprotein pattern and cholesteryl ester transfer activity gene analysis were examined in these patients. We found c.1321+1G>A (IVS14+1G/A) hetero mutation in cholesteryl ester transfer protein (CETP) genes. Generally, CETP mediates transfer and exchange of triglycerides and cholesteryl ester between plasma lipoproteins. Also we investigated a key role of HDL-CE and Apo A-1 metabolism. Patients with low levels of CETP have increased serum HDL levels. We hereby report two Korean cases of CETP deficiency in a family. Brief literature review ensues with the cases.

Efficient Preparation of Radioiodine Labelled 3,5,3'-Triiodothyronine and Thyroxine for Medical Use

  • For isotopic exchange labelling of 3,5,3'-triiodothyronine (T$_3$) and thyroxine (T$_4$) with radioiodide in the presence of molecular iodine, T$_3$:I$_2$ or T$_4$:I$_2$ molar ratios, pH, and reaction time are. considered to be important factors. A modified labelling and separation method is proposed in present paper, by which T$_3$-$^{125}$ I and T$_4$-$^{125}$ I can be obtained with the mean labelling yields of 45%, and 50%, respectively. The whole reaction products can be separated by adoption of thin-layer chromatography technique using silica gel plate and the solvent system composd of chloroform, methanol and ammonia.
