The Change of IgG Antibody Titer to Measles, Mumps and Rubella According to Age (국내 홍역, 유행성이하선염 및 풍진 백신 접종 후 연령에 따른 항체양성율 변화)

  • Purpose : We investigated the change of antibody titer to measles, mumps and rubella according to age after vaccination. Methods : The IgG antibody titers to measles, mumps and rubella were tested on the residual serum from patients aged 7-20 years old after routine laboratory testing in the hospital with informed consent from the parents. Results : Antibody to measles was present in 275 cases out of 408 cases with a positive rate of 67.4%, the mean IgG titer was 2.77${\pm}$1.18 Index. Antibody to mumps was present in 112 cases out of 408 cases with a positive rate of 27.5%, the mean IgG titer was 2.08${\pm}$1.29 Index. Antibody to rubella was present in 367 cases out of 408 cases with a positive rate of 90.0%, the mean IgG titer was 60.46${\pm}$63.47 IU/mL. Conclusion : It is important to maintain a high rate of vaccination coverage in order to prevent an outbreak of measles, mumps, or rubella. It is also important to stress the maintenance of vaccination records for further reference.

A Study on Vitamin A Levels in Measles Patients (홍역 환아의 비타민 A치에 관한 연구)

  • Purpose : This study was done to compare the vitamin A levels between hospitalized patients of measles symptoms and of the other infectious diseases common in childhood (bronchiolitis, penumonia, gastroenteritis and etc.). Methods : We have set up a population of 133 hospitalized patients of measles symptoms in the pediatric department of Kangnam Sacred Heart Hospital between Nov. 2000~Apr. 2001 and compared them to a group of 30 hospitalized patients of other infectious diseases between Dec. 2001~Feb. 2002. The whole patients were divided into three groups by diagnosing measles-specific IgM antibody positive as measles group(n=104), negative as measles-susepcted group(n=29) and other infectious disease group. And we compared the vitamin A levels, clinical symptoms, and general hematological and biochemical results. Results : Among the 104 measles patients, the concentrated age groups were 10 12 months(n=24, 23.1%), 13~15 months(n=16, 15.3%) and 16 months~4 years of age(n=24, 23.1%). The mean levels of vitamin A of measles and measles-susepcted groups were $69.6{\pm}28.6{\mu}g/dL$ and $75.2{\pm}34.4{\mu}g/dL$, respectively, and that of other infectious disease group was $28.1{\pm}17.5{\mu}g/dL$. Conclusion : The mean levels of vitamin A of both measles and measles-suspected groups were significantly higher than that of other infectious disease group(P<0.05) and showed no significant correlations with clinical symptoms and hospitalization duration.

Comparative Epidemiologic Survey of Measles in Two Primary Schools (2000년 영덕군 홍역 유행시 유행 차이에 따른 2개 초등학교 일부 학생에 대한 역학 조사)

  • Objectives : During March-May, 2000, a measles outbreak occurred at Youngduk, Korea. This county is divided into two areas with different historical and socioeconomic background. The outbreak occurred in one of these areas. We conducted a comparative epidemiologic study on the two areas in order to evaluate the factors related to the epidemic. Materials and Methods : We selected two groups, grades 3 and 5 in a primary schools in each area. We investigated outbreak-related factors using parent-questionnaires, the vaccination history from the student's health record and the records concerning the recent measles-outbreak from the local health center. Serologic test on measles-IgG and -IgM antibody was done. Results : The infection rate was 31.5% for the epidemic area and 3.7% for non-the epidemic area according to clinical or serological criteria (p<0.001). No difference was seen in the measles vaccination rate, residence at the time of vaccination or past measles infection history between the two areas. In the epidemic area, the attack rate for the 4-6 year-old MMR booster group(20.5%) was higher than the non-booster group(32.4%), but was not found significantly. Vaccine efficacy was 29.6% in the epidemic area and 87.0% in the non-epidemic area (p<0.001). The IgG level and positive rate were significantly different between the two areas (median 10727 IU/ml, 98.9% in epidemic area; median 346 IU/ml, 85.9% in the non-epidemic area, p<0.001). However, the IgG level and positive rate between the measles-cases and non-cases were not significantly different. Conclusions : This outbreak took place in mostly vaccinated children. These results suggest that a reduction of herd immunity for immunity failure after vaccination may be one of the feasible factors related to the outbreak pattern in the two areas. The results of the IgG level and positive rate suggest that re-establishment of a normal value for IgG level and of a qualitative method for IgG are needed.

Seroprevalence of Measles Antibody and its Attributable Factors in Elementary Students of Routine 2-dose Schedule Era with Vaccination Record (예방접종력이 있는 초등학생의 홍역 항체 양성률 및 기여 요인)

  • Objectives : We investigated the seroprevalence of the measles antibody and its at tributable factors for the students who underwent routine 2-dose Schedule Era. Methods : The subjects were 996 students of the national measles seroepidemiologic study in December 2000 who had vaccination records. We conducted a questionnaire survey and we performed serologic testing for the measlesspecific IgG by using an enzyme linked immunosorbent assay. Results : The coverage for the first dose of the MMR vaccination at 12-15 months of age was 95.1% and the coverage for the second dose of MMR at 4-6 years of age was 35.0%. The proportion of subjects undergoing 2-dosesof MMR dec reased as the age of the subjects increased. The seropositive rate of the measles antibody was significantly high in the second dose vaccinees (93.5% in the second dose group, 84.7% in the non-second dose group, p<0.001) and it was 72.0% in the 0-dose group, 85.4% in the 1-dose group and 93.7% in the 2-dose group (p<0.001). Two point eight percent of the subjects had a past history of measles infection. On the multiple logistic regression analysis, the first and second dose (odds ratio, 8.54; 95% CI.=3.05-23.91), the first dose (odds ratio, 3.06; 95% CI.=1.20-7.81) and the outbreak in the year 2000 (odds ratio, 1.89; 95% CI.=1.24-2.88) were the significant factors for the seropositivity. Conclusions : Maintaining high coverage with a 2-dose vaccination program would be the decisive factor to prevent an outbreak of measles and to eliminate measles in Korea.

Clinical Studies of Measles Prevalence in Western Kyungnam District in 2000 (2000년도 경남 서부 지역에 유행한 홍역의 임상양상)

  • Purpose : To investigate the feature of the range of measles in 2000, we studied epidemics of the measles in western Kyungnam District. Methods : Among 489 patients under 15 years of age who visited or were admitted to in our hospital from January to December in 2000, we selected 344 patients whose measles IgM antibody was positive. We investigated age, monthly incidence, clinical features, vaccination history, and measles IgG antibody. Results : Patients under 12 months of age occupied the largest proportion(36.9%). Patients firstly occurred in May and geometrically increased from October. Fever, cough and rashes were observed in all patients but conjunctivitis in 54.9%, Koplik spot in 23.8%. The first vaccination ratio was 41.8%. Compared with the number of patients and the first vaccination ratio, the first vaccination ratio was the lowest under 12 months of age. After that age group, the number of patients was decreased as the first vaccination ratio was increased. Among 152 patients sampled for measles IgG antibody, 35 patients had received the first vaccination and 6 patients had received the second vaccination. Among 35 patients who received the first vaccination, 22(62.9%) patients were negative of measles IgG antibody. Among 6 patients who received the second vaccination, 3(50%) patients were negative. Conclusions : Measles patients under 12 months still dominated. Therefore, routine vaccination of single measles vaccine, is currently done at 6 month when measles are prevalent, should be considered. High first vaccination failure suggests problems of vaccines itself, transport and storage rather than vaccination methods. Therefore a thorough investigation should be made.

The Clinical Usefulness of Tc-99m ECD Brain SPECT in Acute Measles Encephalitis (홍역 후 발생된 급성 뇌염 진단시 Tc-99m ECD 뇌혈류 SPECT의 임상적 유용성)

  • Purpose: Since the prognosis of measles encephalitis is poor, early diagnosis and proper management are very important to improve clinical outcomes. We compared Tc-99m ECD brain SPECT (SPECT) with MR imaging (MRI) for the detection of acute measles encephalitis. Materials and Methods: Eleven patients(M : F=4 : 7, age range 18 months-14 yrs) with acute measles encephalitis were enrolled in this studies. All of them underwent both MRI and SPECT. The results of SPECT were scored from 0 (normal) to 3 (most severe defect) according to perfusion state. We compared two image modalities for the detection of brain abnormality in acute measles encephalitis. Results: Seven of 11 patients (63.6%) revealed high signal intensity in the white matter on T2WI of MRI, on the other hand all patients (100%) showed hypoperfusion on SPECT. Severe perfusion deficits above score 2 were located with decreasing frequencies in the frontal lobe (81.8%), temporal lobe (72.7%), occipital lobe (27.3%), basal ganglia (27.3%), and parietal lobe (9.1%). Conclusion: We conclude that SPECT is more useful than MRI for the detection of brain involvement in patients with acute measles encephalitis.

A Case of Measles Pneumonia Complicated by Subcutaneous Emphysema and Pneumomediastinum in an 18-year-old Patient (피하기종과 종격기종을 동반한 홍역성 폐렴 1예)

  • Background : A 18 year-old man presented with dyspnea and a swelling of the neck. On physical examination, maculopapular rashes were noted on the face and the whole body and crepitus was noted at the thorax and upper arms. His chest X -ray showed bilateral interstitial infiltrates of the lung, pneumomediastinum and subcutaneous emphysema. On serologic examination, measles IgM was positive. Under the diagnosis of measles pneumonia, the patient was treated with oral ribavirin, which resulted in a complete resolution of the pneumomediastinum, subcutaneous emphysema, pneumonic infiltrate, and subjective symptoms of dyspnea and swelling of the neck in 7 days. Here we report this case with a brief review of the relevant literature.

Identification of Amino Acid Residues Involved in the Interaction between Measles Virus Haemagglutin (MVH) and Its Human Cell Receptor(Signaling Lymphocyte Activation Molecule, SLAM)

  • Signaling lymphocyte activation molecule (SLAM; also known as CD150) is a newly identified cellular receptor for measles virus (MV). The interaction between MV Haemagglutin (MVH) and SLAM is an initial step for MV entry. We have identified several novel SLAM binding sites at residues S429, T436 and H437 of MVH protein and MVH mutants in these residues dramatically decrease the ability to interaction with the cell surface SLAM and fail to co-precipitation with SLAM in vivo as well as malfunction in syncytium formation. At the same time, K58, S59 and H61 of SLAM was also identified to be critical for MVH and SLAM binding. Further, these residues may be useful targets for the development of measles therapy.

A Chosonization of recuperation and contraindications of Measles in the Late Joseon dynasty (조선후기 마진 질환 조리(調理)와 금기(禁忌)의 조선화)

  • In this paper, medical books on measles written in the late Joseon dynasty were analyzed to examine Chosonization of recuperation and contraindications of Measles in the Late Joseon Dynasty. It was approached in terms of utilization of Chinese medicine knowledge and utilization of clinical experience in Joseon. Through this study, the following facts were newly discovered. 1) Alcohol was taboo according to Chinese doctors, but in Joseon it was considered good if used properly. 2) Beef was recommended by Chinese medical doctors, but it was taboo in Joseon. 3) Dried fish was a food specially recommended in Joseon literature. 4) Except for diet, the contents of the treatment follow the Chinese literature as it is, or there is no content. In conclusion, Korean medical doctors simply followed Chinese medicine knowledge at the beginning of the 18th century, but in the mid to late 18th century, according to the accumulation of clinical experience in Joseon, they had unique characteristics associated with medicine during the Joseon era.

A Small Outbreak of Measles in 2013: In a Single Hospital in Northern Gyeonggi-do (2013년 경기 북부 한 병원에서 확인된 소규모 홍역 유행)

  • Purpose: This study analyzed a small outbreak of measles at a single hospital located in northern Gyeonggi-do in 2013. Methods: We reviewed the medical records of measles patients at The Catholic University of Korea Uijeongbu St. Mary's Hospital from August to October, 2013. Results: Fifteen children were confirmed to have measles by RT-PCR and serum IgM test; 1 neonate, 11 infants, and 3 toddlers. None of the patients had received Measles-Mumps-Rubella vaccination. All patients showed B3 type in viral genotyping. Nine children (60%) had been exposed to measles during treatment for other diseases in the pediatric ward. Incubation period was between 8 and 15 days. Fever started at a median 10 days after exposure and persisted for a median of 8 days. Rash showed at a median 13 days after exposure. Respiratory complications were observed in 40% of patients. Diarrhea developed in 53% of patients. Conclusion: Although measles has been well-controlled due to the high rate of vaccination coverage, it is possible to have an outbreak at any given time, especially in infants. We must learn from this outbreak, and remain fully aware of the possibility of reemergence and provide proper management, including vaccination or immune globulin administration, to infants exposed to measles. Reevaluation of serum IgG titer of neonates, infants, and pregnant women may be the first step to prevent further outbreaks.
