Partial Necrosis of the Mandibular Proximal Segment Following Transoral Vertical Ramus Osteotomy

  • Transoral vertical ramus osteotomy (TOVRO) procedure can result in a variety of complications. Complications commonly reported include extensive bleeding due to major blood vessel injury, unpredictable fracture, postoperative infection, neurosensory deficit related Inferior alveolar nerve, insufficient osteosynthesis, and temporomandibular joint problem. The authors describe a case of partial necrosis of the mandibular proximal segment following TOVRO, a rarely reported complication. A 37-year-old otherwise healthy woman underwent Lefort l osteotomy and TOVRO to correct mandibular prognathism. Postoperatively, she developed pain and swelling in the right submandibular region and was found to have a partial necrosis of proximal segment.

  • Patients with Pierre Robin sequence exhibit varying degrees of airway obstruction and feeding difficulty. In some patients, airway obstruction may be profound, warranting surgical intervention to maintain a patent airway. The purpose of this article is to highlight the advantages of the tongue-lip adhesion procedure for the management of airway obstruction in such patients compared to the currently available options.

  • Maxillary sinus infection following Le Fort I osteotomy is rare in patients without a history of preexisting nasal symptoms. A case of a 19-year-old male patient who suffered from preoperative chronic non-allergic rhinitis and developed repetitive postoperative maxillary sinus infection after Le Fort I osteotomy is reported.

  • Genioplasy with osteotomy is a flexible and useful procedure. However, osteotomy can cause different types of morbidity. Chin augmentation with porous polyethylene ($Medpor^{(R)}$) has become popular in the recent years. Porous polyethylene ($Medpor^{(R)}$) is an excellent biomaterial for reconstructing facial deformities. $Medpor^{(R)}$ has a porous architecture, which prevents capsule formation and decreases the degree of foreign-body reaction. In addition, it can be easily cut with scissors and molded, and it also maintains its shape. We report here on a satisfactory case of chin augmentation with using porous polyethylene ($Medpor^{(R)}$).

  • The postoperative maxillary cyst develops as a delayed complication after surgical intervention or Caldwell-Luc operation in the maxillary sinus and was also reported that it could occur after Le Fort I osteotomy. This is also called as surgical cliated cyst because of its lining epithelium is usually lined by a pseudo-stratfied ciliated columnar epithelium. This report represents a case of postoperative maxillary cyst which developed within the anterior of maxilla and in association with nasal mucosa 6 years after a Le Fort I osteotomy. In 1989, 26-year-old male complained of his mandibular prognathism and underwent orthogmathic surgery, Le Fort I osteotomy, bilateral sagittal split ramus osteotomy, partial glossectomy.

  • We experienced a case of rhinocerebral form of mucormycosis in a 9-year-old male suffered from acute lymphocytic leukemia (FAB $L_2$). On 15th day of induction chemotherapy (Hospital day 23) pain, tenderness and swelling on left maxillary area of face were noticed. We confirmed mucormycosis by biopsy of mass in left maxillary sinus. He expired on Hospital day 47.

  • IN 1956, Peer presented a very comprehensive account of free fatty tissue transplantation. The use of buccal fat pad had been usually used to reconstruct the perioral defects. The buccal fat pad is a special fatty tissue which is markedly different from subcutaneous fat. And it is a easily accessible fat tissue in oral and maxillofacial region. The buccal fat pad can be used as a free graft or pedicled flap. Recently, buccal fat pad is interested in cosmetic surgery because its' removal for cosmetic purpose has been favorable result. We used the buccal fat as an unlined, pedicled graft for closure of large oroantral fistula. We present a case of report and review of literature.

  • When the midfacial fractures involve the upper maxilla (LeFort I, II, III), there is a chance that the nasolacrimal duck may have been injured. When this suspected, We must observe the presence of epiphora carefully. If the epiphora was not improved following conservative treatment, dacryocystorhinostomy would be the treatment of choice. We performed two cases of DCR and got the favorable results.

  • The coronal approach is a versatile surgical technique. This method becomes particulary useful for exposure and internal fixation of midfacial fractures and the harvest of calvarial bone graft to manage the complex facial bone fractures. The rectrospective clinical study on the use of this technique in 10 patients was performed. The result shows that this technique provides the excellent exposure of fractures site, the ability to reduce the fragment accurately and good cosmetic results in incision area. We discussed with literatures review that anatomy, technique, indications, and potential complications of the coronal approach.
