대장-직장암의 간전이에서 FDG PET과 MR의 진단 성능 (Diagnostic Accuracy of PET and MR for Detecting Liver Metastasis from Colorectal Cancer)

  • 목적: 대장-직장암에서 간전이 여부를 진단하기 위한 영상 검사 방법으로 기존에는 CT 촬영을 시행하여 왔다. 최근에는 MR 및 FDG PET을 간전이 진단에 적극적으로 활용하고 있는 추세이다. 이에 이 연구에서는 대장-직장암 환자에서 간전이에 대한 FDG PET의 진단적 가치를 MR과 비교하여 보고자 하였다. 대상 및 방법 대장: 직장암으로 확진된 환자들 중에 간전이를 진단 또는 배제하기 위하여 CT 이외에도 MR 및 F-18-FDG PET (conventional PET 및 fusion PET)을 모두 시행한 환자는 26명이었다. 이 연구에서는 26명의 환자들에게서 영상화 된 35개의 병변을 분석의 대상으로 삼고, FDG PET과 MR의 민감도 및 특이도를 간분엽절제술 및 종양절제술을 통해 얻어진 병리소견 및 임상소견 또는 추적 영상 소견을 표준으로 하여 각각 구하였다. FDG PET과 MR은 평균 7일 간격으로 시행되었으며, 두 검사 사이에 치료적 요법을 시행 받은 환자는 한 명도 없었다. 결과: 병리 결과, 35개의 병변 중에서 18개(51.4%)의 병변은 간전이로 판명되었으며, 나머지 17개(48.6%)는 낭종, 지방, 혈관종, 농양, 호산구성농양 등의 양성 병변으로 판명되었다. 35개의 병변 중 MR과 FDG PET의 진단이 일치한 병변은 간전이 17개(94.4%)와 양성 병변 13개(76.5%)로 전체적으로는 85.7%의 일치도를 보였다. FDG PET의 민감도는 94.4% (17/18), 특이도는 94.1% (16/17)였으며 MR의 민감도는 100% (18/18), 특이도는 82.4% (14/17)이었다. MR에서 위양성을 보였던 3개의 병변은 낭종과 호산구성농양이었고, FDG PET에서 위음성을 보였던 1개의 병변은 직경 8 mm의 작은 병변이었다. 직경 10 mm 이하인 병변들만 분석했을 때, 총 20개의 병변에 대하여 FDG PET은 민감도 85.7%(6/7), 특이도 92.3%(12/13), MR은 민감도 100%(7/7), 특이도 76.9%(10/13)를 보였다. 그 외에도 간전이 병변의 평균 maxSUV는 $6.7{\pm}3.8$로 간전이와 양성병변을 최적으로 감별할 수 있는 maxSUV의 cutoff value는 3.1 이었다. (AUC=0.897, p<0.001, 민감도 83.3%, 특이도 94.1%) 결론: FDG PET은 간전이를 진단하는데 MR과 대등한 민감도와 특이도를 보였다. 통계적으로 유의한 차이를 보이지는 않았지만 간전이에 대한 FDG PET의 음성예측도는 MR보다 높았으며, 직경 10 mm 미만의 작은 병변에 대해서도 FDG PET은 우수한 성적을 보였다. 향후 대장암의 간전이 진단에 FDG PET이 유용하게 이용될 것이다.

PET/CT planning during chemoradiotherapy for esophageal cancer

  • Purpose: To evaluate the usefulness of positron emission tomography/computed tomography (PET/CT) for field modification during radiotherapy in esophageal cancer. Materials and Methods: We conducted a retrospective study on 33 patients that underwent chemoradiotherapy (CRT). Pathologic findings were squamous cell carcinoma in 32 patients and adenocarcinoma in 1 patient. All patients underwent PET/CT scans before and during CRT (after receiving 40 Gy and before a 20 Gy boost dose). Response evaluation was determined by PET/CT using metabolic tumor volume (MTV), total glycolytic activity (TGA), MTV ratio (rMTV) and TGA ratio (rTGA), or determined by CT. rMTV and rTGA were reduction ratio of MTV and TGA between before and during CRT, respectively. Results: Significant decreases in MTV ($MTV_{2.5}$: mean 70.09%, p < 0.001) and TGA ($TGAV_{2.5}$: mean 79.08%, p < 0.001) were found between before and during CRT. Median $rMTV_{2.5}$ was 0.299 (range, 0 to 0.98) and median $rTGAV_{2.5}$ was 0.209 (range, 0 to 0.92). During CRT, PET/CT detected newly developed distant metastasis in 1 patient, and this resulted in a treatment strategy change. At a median 4 months (range, 0 to 12 months) after completion of CRT, 8 patients (24.2%) achieved clinically complete response, 11 (33.3%) partial response, 5 (15.2%) stable disease, and 9 (27.3%) disease progression. $SUV_{max}$ (p = 0.029), $rMTV_{50%}$ (p = 0.016), $rMTV_{75%}$ (p = 0.023) on intra-treatment PET were found to correlate with complete clinical response. Conclusion: PET/CT during CRT can provide additional information useful for radiotherapy planning and offer the potential for tumor response evaluation during CRT. $rMTV_{50%}$ during CRT was found to be a useful predictor of clinical response.

네거티브 경구 조영제를 이용한 PET/CT 촬영시 나타난 종양성 섭취와 유사한 생리적 장관 섭취 (Colon Cancer Mimicking Physiologic FDG Uptake : with Using of Negative Oral Contrast)

  • A 64-year-old female with glioblastoma multiforme (GBM) was assigned to our department for whole body PET/CT scan. She ingested 1 liter of pure water as negative oral contrast just before PET/CT examination. FDG-PET/CT images showed a very intense hypermetabolic, focal lesion in the abdominal cavity around descending colon. The SUVmax of the lesion was 17.2. But there was no abnormal lesion corresponded to the area of PET scan in the combined contrast enhanced CT scan. We suggested considering a malignant lesion due to very intense glycolytic activity. Conventional abdominal CT scan & colonoscopy were accomplished within one week after PET/CT evaluation. There was no abnormality in both examinations. We executed follow-up PET/CT evaluation after 1 month and couldn't find any abnormality around the corresponding area. So we concluded the hypermetabolism was colonic physiologic uptake. A colonic physiologic uptake is a well known cause of false positive finding. Nuclear physicians should be considered the possibility of malignancy when interpret focal colonic uptake, especially incidental finding. There are a few reports that using of negative oral contrast is able to reduce gastrointestinal physiologic uptakes. But as we can see in this case, although we used negative oral contrast, intense physiologic uptake is detected and maxSUV is able to up to 17.2. So, it is important to keep a fact in mind. Even though there is a colonic physiologic uptake in PET/CT image, it may be able to show very intense hypermetabolism regardless of using negative oral contrast.

ATAD2 expression increases [18F]Fluorodeoxyglucose uptake value in lung adenocarcinoma via AKT-GLUT1/HK2 pathway

  • [18F]Fluorodeoxyglucose (FDG) PET/CT imaging has been widely used in the diagnosis of malignant tumors. ATPase family AAA domain-containing protein 2 (ATAD2) plays important roles in tumor growth, invasion and metastasis. However, the relationship between [18F]FDG accumulation and ATAD2 expression remains largely unknown. This study aimed to investigate the correlation between ATAD2 expression and [18F]FDG uptake in lung adenocarcinoma (LUAD), and elucidate its underlying molecular mechanisms. The results showed that ATAD2 expression was positively correlated with maximum standardized uptake value ($SUV_{max}$), total lesion glycolysis (TLG), glucose transporter type 1 (GLUT1) expression and hexokinase2 (HK2) expression in LUAD tissues. In addition, ATAD2 knockdown significantly inhibited the proliferation, tumorigenicity, migration, [18F]FDG uptake and lactate production of LUAD cells, while, ATAD2 overexpression exhibited the opposite effects. Furthermore, ATAD2 modulated the glycometabolism of LUAD via AKT-GLUT1/HK2 pathway, as assessed using LY294002 (an inhibitor of PI3K/AKT pathway). In summary, to explore the correlation between ATAD2 expression and glycometabolism is expected to bring good news for anti-energy metabolism therapy of cancers.

판정행렬을 기반한 일체형 PET-MRI의 폐암 진단 유용성 평가 (Evaluation of Usefulness for Diagnosis of Lung Cancer on Integrated PET-MRI Using Decision Matrix)

  • The results of empirical researches on the diagnosis of lung cancer are insufficient, so it is limited to objectively judge the clinical possibility and utilization according to the accuracy of diagnosis. Thus, this study retrospectively analyzed the lung cancer diagnostic performance of PET-MRI (Positron Emission Tomography-Magnetic Resonance Imaging) by using the decision matrix. This study selected and experimented total 165 patients who received both hematological CEA (Carcinoembryonic Antigen) test and hybrid PET-MRI (18F-FDG, 5.18 MBq/kg / Body TIM coil. VIVE-Dixon). After setting up the result of CEA (positive:>4 ㎍/ℓ. negative:<2.5㎍/ℓ) as golden data, the lung cancer was found in the image of PET-MRI, and then the SUVmax (positive:>4, negative:<1.5) was measured, and then evaluated the correlation and significance of results of relative diagnostic performance of PET-MRI compared to CEA through the statistical verification (t-test, P>0.05). Through this, the PET-MRI was analyzed as 96.29% of sensitivity, 95.23% of specificity, 3.70% of false negative rate, 4.76% of false positive rate, and 95.75% of accuracy. The false negative rate was 1.06% lower than the false positive rate. The PET-MRI that significant accuracy of diagnosis through high sensitivity and specificity, and low false negative rate and false positive rate of lung cancer, could acquire the fusion image of specialized soft tissue by combining the radio-pharmaceuticals with various sequences, so its clinical value and usefulness are regarded as latently sufficient.

췌장암 환자의 양전자방출단층촬영(PET) 검사 시 혈당치와 영상에 관한 분석 (Comparison of Blood Glucose Level in Pancreatic Cancer on $^{18}F$-FDG PET)

  • 양전자방출단층촬영 검사 시 정상 췌장의 FDG 섭취가 낮으며, 췌장암은 높은 FDG 섭취를 보이는 종양이므로 PET을 이용하여 췌장 병변의 악성 여부를 진단할 수 있다. 그러나 췌장암 환자에서 당뇨병의 유병률이 높다는 것은 여러 보고에서 잘 알려져 있고, 혈당의 수치가 정상 범위 이상인 경우엔 FDG 섭취가 감소하게 된다. 그러므로 혈당의 수치를 정상 범위로 조절하기 위해 검사를 지연시켜 환자의 불편함 뿐만 아니라 검사 일정 조정에도 지장을 초래 한다. 본 연구에서는 PET 검사 시 췌장암 환자의 혈당치가 영상에 미치는 영향을 평가하였다. 본 연구는 PET 검사를 위해 본원 핵의학과에 내원한 100명의 환자를 대상으로 하였다. 검사 직전 공복 혈장 포도당농도를 측정하여 당뇨병을 진단받은 환자군과 진단받지 않은 환자군으로 분류하였다. 영상분석은 당뇨병을 진단받지 않은 환자군, 당뇨병을 진단받은 환자군 중 7.0 mmol/L (126 mg/dL) 이상 그리고 미만인 세 그룹의 간과 폐에 관심영역(ROI)을 설정하고 최대 표준화 섭취 계수 ($SUV_{max}$)를 구하여 SPSS ver. 17을 이용하여 비교 분석하였다. 100명의 환자 중 남자 53명, 여자 47명이었고 평균 연령은 $60.1{\pm}13.5$세였다. 췌장암과 당뇨병이 같이 있는 환자들이 36명(남 22명, 여 14명)으로 36%였다. 또 검사 직전 공복 혈장 포도당농도가 7.0 mmol/L 이상인 환자는 100명 중 15명, 미만인 환자는 85명이었다. 당뇨병을 진단받은 환자군 중 7.0 mmol/L 이상, 미만, 당뇨병 진단을 받지 않은 그룹 간의 간과 폐에서의 $SUV_{max}$는 유의한 차이가 없었다(p>0.05). 결론적으로 췌장암 환자의 당뇨병 유병율이 높았고, PET 검사 시 공복 혈당치가 정상 혈당치보다 높아도 검사 전처치가 잘되어 있으면 영상을 진단하는데 큰 어려움을 주지 않을 것이라 생각된다. 또한 혈당치 조절을 위한 검사실 일정 조정에 변화를 주지 않을 뿐만 아니라, 추가적인 공복 기간으로 오는 불편함을 줄일 수 있을 것이라 사료된다.

Imaging Anatomy of Waldeyer's Ring and PET/CT and MRI Findings of Oropharyngeal Non-Hodgkin's Lymphoma

  • Background: This study was conducted to analyze positron emission tomography (PET) / computed tomography (CT) and magnetic resonance imaging (MRI) performance with oropharyngeal non-Hodgkin's lymphoma (ONHL).Materials and Methods: The complete image data of 30 ONHL cases were analyzed, all patients were performed PET / CT and MRI examination before the treatment, with the time interval of these two inspections not exceeding 14 days. The distribution, morphology, MRI signal characteristics, enhancement feature, standardized uptake value (SUV) max value and lymph node metastasis way of the lesions were analyzed. Results: Among the 30 cases, 23 cases were derived from the B-cell (76.7%), 5 cases were derived from the peripheral T cells (16.7%) and 2 cases were derived from the NK/T cells (6.7%). 19 cases exhibited the palatine tonsil involvement (63.3%). As for the lesion appearance, 10 cases appeared as mass, 8 cases were the diffused type and 12 cases were the mixed type. 25 cases exhibited the SUVmax value of PET / CT primary lesions as 11 or more (83.3%). MRI showed that all patients exhibited various degrees of parapharyngeal side-compressed narrowing, but MRI still exhibited the high-signal fat, and the oropharyngeal mucosa was intact. 25 cases were associated with the neck lymph node metastasis, among who 22 cases had no necrosis in the metastatic lymph nodes, while the rest 3 cases exhibited the central necrosis in the metastatic lymph nodes. Conclusions: PET / CT and MRI have important value in diagnosing and determining the lesion extent of ONHL.
