Effectiveness of Cervical Cancer Screening Based on a Mathematical Screening Model using data from the Hiroshima Prefecture Cancer Registry

  • Here we assessed the effectiveness of cervical cancer screening using data from the Hiroshima Prefecture Cancer Registry regarding patient age at the start of screening and differences in screening intervals. A screening model was created to calculate the health status in relation to prognosis following cervical cancer screening and its influence on life expectancy. Epidemiological data on the mortality rate of cervical cancer by age groups and mortality rates from the Hiroshima Prefecture Cancer Registry were used for the model projections. Our results showed that life expectancy when screening rate was 100% compared with 0% was extended by approximately 1 month. Furthermore, when the incidence of cervical cancer was 0% compared with the screening rate was 100%, life expectancy was extended by a maximum of 3 months. Moreover, among individuals affected by cervical c ancer, a difference of 13 years in life expectancy was calculated between screened and unscreened groups.

  • 원자력시설 해체부지를 재이용하는 과정에서 유발될 수 있는 방사선학적 리스크를 사전에 선별하기 위한 목적으로 단순화된 방사선량 평가모델을 개발하고, 이를 Microsoft $Excel{\circledR}$ 스프레드시트와 내장된 Visual Basic 및 마크로 기능을 활용하여 기능별로 모듈화된 평가도구를 구현하였다. 이와 함께 부지 특성자료가 불충분할 경우 신속한 사전평가를 위해 적용할 수 있는 일련의 입력변수 값 목록을 제안하였다. 동일한 조건에서 이 연구에서 개발된 평가도구를 이용해 유도한 사전 선별준위가 RESRAD Ver.6.2를 이용해 계산된 유도농도지침한계 및 독일 방사선방호령에 규정된 핵종별 부지 재이용 기준농도를 합리적으로 근사할 수 있음을 확인하였다.

  • An important purpose of screening DSM(Demand Side Management) programs is to determine which programs or technologies seem to be moot worthy of further development, given the characteristics of the particular utility system. Complication found in screening DSM technologies is the consideration of costs and benefits from several perspectives : the utility, society, participants, free riders and nonparticipants. In this paper, a nation-wide DSM screening model is presented using a rigorous mathematical formulation on the financial basis and the concept of marginal coots considering all affected parties inside the nation is fully exploited.

Markov's Modeling for Screening Strategies for Colorectal Cancer

  • Economic decision models are being increasingly used to assess medical interventions. Advances in this field are mainly due to enhanced processing capacity of computers, availability of specific software to perform the necessary tasks, and refined mathematical techniques. We here estimated the incremental cost-effectiveness of ten strategies for colon cancer screening, as well as no screening, incorporating quality of life, noncompliance and data on the costs and profit of chemotherapy in Iran. We used a Markov model to measure the costs and quality-adjusted life expectancy of a 50-year-old average-risk Iranian without screening and with screening by each test. In this paper, we tested the model with data from the Ministry of Health and published literature. We considered costs from the perspective of a health insurance organization, with inflation to 2011, the Iranian Rial being converted into US dollars. We focused on three tests for the 10 strategies considered currently being used for population screening in some Iranians provinces (Kerman, Golestan Mazandaran, Ardabil, and Tehran): low-sensitivity guaiac fecal occult blood test, performed annually; fecal immunochemical test, performed annually; and colonoscopy, performed every 10 years. These strategies reduced the incidence of colorectal cancer by 39%, 60% and 76%, and mortality by 50%, 69% and 78%, respectively, compared with no screening. These approaches generated ICER (incremental cost-effectiveness ratios) of $9067, $654 and $8700 per QALY (quality-adjusted life year), respectively. Sensitivity analyses were conducted to assess the influence of various scales on the economic evaluation of screening. The results were sensitive to probabilistic sensitivity analysis. Colonoscopy every ten years yielded the greatest net health value. Screening for colon cancer is economical and cost-effective over conventional levels of WTP8.

비용 효과도 최적화 기반 양산 무기체계 환경 부하 선별 시험 설계 방법 (The Mass Production Weapon System Environmental Stress-Screening Test Design Method based on Cost-effective-Optimization)

  • Purpose: There is a difficulty in Environmental Stress Screening (ESS) test design for weapon system's electrical/electronic components/products in small and medium-sized enterprises. To overcome this difficulty, I propose an easy ESS test design approach algorithm that is optimized with only one environment tolerance design information parameter (${\Delta}T$). Methods: To propose the mass production weapon system ESS test design for cost-effective optimization, I define an optimum cost-effective mathematical model ESS test algorithm model based on modified MIL-HDBK-344, MIL-HDBK-2164 and DTIC Technical Report 2477. Results: I clearly confirmed and obtained the quantitative data of ESS effectiveness and cost optimization along our ESS test design algorithm through the practical case. I will expect that proposed ESS test method is used for ESS process improvement activity and cost cutting of mass production weapon system manufacturing cost in small and medium-sized enterprises. Conclusion: In order to compare the effectiveness of the proposed algorithm, I compared the effectiveness of the existing ESS test and the proposed algorithm ESS test based on the existing weapon system circuit card assembly for signal processing. As a result of the comparison, it was confirmed that the test time was reduced from 573.0 minutes to 517.2minutes (9.74% less than existing test time).

  • (가) 검시료수(檢試料數)가 방대하게 되면 관능검사에 장기간을 요하므로 파넬에 미치는 부담감과 권태 피로등의 효과(效果)때문에 실험결과에 오차(誤差)가 크게 발생할 우려가 있다. (나) 다수(多數)의 검시료(檢試料)일 경우 시험수(試驗數)를 축소시키는 반면에 검출력(檢出力)이 양호한 수법(手法)으로써 새로이 표준시험대(標準試驗對) 1대비교법(對比較法)을 고안(考案)적용한 결과 양호(良好)한 성과(成果)를 얻을 수 있었다. (다) 본수법(本手法)은 특(特)히 시료수(試料數)가 많을때 그중에서 $4{\sim}5$개를 우선 선발하는 Screening test로 활용(活用)할 수 있음을 알았다. (라) 본수법(本手法)으로 Screening한 검시료(檢試料)에 대해 3각(角) 1대비교법(對比較法)으로 정사(精査)하여 최종판단을 내리는 일련의 과정을 거치도록 하는 것이 연구실적인 품결연구에 있어서는 가장 능률적인 관능검사의 방법이라고 할수 있다.

Data Mining for Detection of Diabetic Retinopathy

  • The incidence of blindness resulting from diabetic retinopathy has significantly increased despite the intervention of insulin to control diabetes mellitus. Early signs are microaneurysms, exudates, intraretinal hemorrhages, cotton wool patches, microvascular abnormalities, and venous beading. Advanced stages include neovascularization, fibrous formations, preretinal and vitreous microhemorrhages, and retinal detachment. Microaneurysm count is important because it is an indicator of retinopathy progression. The purpose of this paper is to apply data mining to detect diabetic retinopathy patterns in routine fundus fluorescein angiography. Early symptoms are of principal interest and therefore the emphasis is on detecting microaneurysms rather than vessel tortuosity. The analysis does not involve image-recognition algorithms. Instead, mathematical filtering isolates microaneurysms, microhemorrhages, and exudates as objects of disconnected sets. A neural network is trained on their distribution to return fractal dimension. Hausdorff and box counting dimensions grade progression of the disease. The field is acquired on fluorescein angiography with resolution superior to color ophthalmoscopy, or on patterns produced by physical or mathematical simulations that model viscous fingering of water with additives percolated through porous media. A mathematical filter and neural network perform the screening process thereby eliminating the time consuming operation of determining fractal set dimension in every case.

Statistical Optimization of Biosurfactant Production from Aspergillus niger SA1 Fermentation Process and Mathematical Modeling

  • In this study, we sought to investigate the production and optimization of biosurfactants by soil fungi isolated from petroleum oil-contaminated soil in Saudi Arabia. Forty-four fungal isolates were isolated from ten petroleum oil-contaminated soil samples. All isolates were identified using the internal transcribed spacer (ITS) region, and biosurfactant screening showed that thirty-nine of the isolates were positive. Aspergillus niger SA1 was the highest biosurfactant producer, demonstrating surface tension, drop collapsing, oil displacement, and an emulsification index (E24) of 35.8 mN/m, 0.55 cm, 6.7 cm, and 70%, respectively. This isolate was therefore selected for biosurfactant optimization using the Fit Group model. The biosurfactant yield was increased 1.22 times higher than in the nonoptimized medium (8.02 g/l) under conditions of pH 6, temperature 35℃, waste frying oil (5.5 g), agitation rate of 200 rpm, and an incubation period of 7 days. Model significance and fitness analysis had an RMSE score of 0.852 and a p-value of 0.0016. The biosurfactant activities were surface tension (35.8 mN/m), drop collapsing (0.7 cm), oil displacement (4.5 cm), and E24 (65.0%). The time course of biosurfactant production was a growth-associated phase. The main outputs of the mathematical model for biomass yield were Yx/s (1.18), and µmax (0.0306) for biosurfactant yield was Yp/s (1.87) and Yp/x (2.51); for waste frying oil consumption the So was 55 g/l, and Ke was 2.56. To verify the model's accuracy, percentage errors between biomass and biosurfactant yields were determined by experimental work and calculated using model equations. The average error of biomass yield was 2.68%, and the average error percentage of biosurfactant yield was 3.39%.

  • In Korea, Public Research Institutes(PRIs) are today faced with the challenges of creating values by transferring technologies in store within themselves to private commercial sector. Recently, It has been increasingly pointed out that PRIs have the poor capability to valuate prospective technologies of their own, and don't run the reasonable technology transfer mechanism in terms of establishing royalty rate and initial payment, designing remuneration to inventor, screening qualified licensee, and controlling the moral hazard. This paper develops an enhanced mathematical model of technology transfer from a PRI to a private industrial firm with including the inventor as an important player. The model is made up of the main part which derives the optimal royalty rate by maximizing the social welfare and sharing risk fairly between players and some sub-parts. The one sub-part is a principal-agent model which makes it possible to control the moral hazard of inventors, and the other part provides the criteria for screening appropriate licensees. Moreover, the moral hazard between inventor and licensee is addressed by introducing the cost reduction function of efforts exerted by them. The model is able to relate the optimal royalty rate to the parameters that represent the environments under which the concerned parties operate. Especially, the ratio of initial payment over the value of transferred technology is calculated from the binding relation with the royalty rate. The paper shows that the model suggested here is more enhanced by comparing with the existing technology transfer mechanism. Finally, the paper allows us to find better strategies for effective technology transfer and further develop more sophisticated technology transfer model.

체다치즈의 숙성 전과정에 대한 수학식 (A Mathematical Model for the Whole Ripening Process of Cheddar Cheese)

  • 치즈의 숙성 전과정을 잘 설명하여 줄 수 있는 수학식을 만들었다. X(0)와 $e_2$(0) 및 $k_1$값들을 증가시킴으로써 숙성과정을 촉진시킬 수 있었고 낮은 $k_2$값과 높은 $a_2$값을 가지는 신 균주의 탐색도 필요도하다는 것을 알았다. 그러나, 치즈 숙성과정 중 나쁜 치즈맛의 생산을 피하기 위하여서는 낮은 단백질 분해요소 활동도를 갖는 균주가 절대적으로 필요하다. 따라서 이 제안된 수학적 모델식은 치즈덩어리 내에서 일어나는 효소반응들을 잘 묘사하고 있으며, 궁극적으로는 값싸고 질 좋은 치즈를 생산하는데 유용할 것이다.
