  • In edentulous patients, the advantages of prosthodontic therapy using oral implants include both stability and comfort. Advantages suggested for this approach include the prevention of mandibular bone resorption and increased masticatory function. Implant hybrid prostheses place 4 to 6 implants between the mental foramens, and cantilevers are used to replace the posterior teeth at both ends. The 85 - year - old female patient visited our clinic with complete edentulism in the maxilla and mandible. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using removable complete denture in the maxilla and implant hybrid prosthesis in the mandible.

  • Peri-apical implant lesion, also known as 'retrograde peri-implantitis' can occur with multifactorial etiological factors. The purpose of this case report is to demonstrate resolution of periapical implant lesion by removal of causative factors and saving implant by regenerative therapy. A 54-year old male patient with mild dull pain around implant on the right mandibular second premolar area due to persistent peri-apical infection of the adjacent first premolar was treated. Extraction of tooth with symptomatic apical periodontitis and regenerative therapy on the buccal fenestration area of the implant and extraction site were performed. After 6-month reentry, notable regenerated bone tissue around implant was found, and implant placement on the previous extraction site was performed. After 14-month follow-up from the regenerative therapy, neither biological nor mechanical complication could be found around the implant, evidenced by high implant stability, normal clinical probing depth, and absence of discomfort spontaneously and during masticatory function. In conclusion, surgical intervention including regenerative therapy using bone graft and barrier membrane on periapical implant lesion can be suggested as one of the treatment options considering the extent of periapical lesion.

  • Cerebrovascular Accident(stroke) is that a sudden, nonconvulsive loss of neurologic function due to an ischemic or hemorrhagic intracranial vascular event. If stroke happens at the portion of trigeminal motor nucleus or its control part of cerebral cortex, masticatory muscles will be atrophy or paralyzed. So it is possible that dental occlusion changes after stroke. A 74-aged woman recurred mild stroke 2 month ago, who had experienced severe stroke 2 years ago. After recurrence, suddenly her upper full denture was dropped when lower denture contacted upper one. According to the her occlusion exam, her lower jaw moved back slightly compared with the occlusion of old denture. And her face had asymmetry and lower jaw dislocated to paralyzed side. A 50-aged man was treated because many cervical caries, which would occur because of an aftereffect of stroke, long-herm hospitalization and limits of self oral-care. 6 years ago he had cerebral hemorrhage and he claimed that he cannot bite exactly. Just two pairs of teeth was contact on biting, his lower jaw was located back, too. This two case suggests that dental occlusion can change after stroke.

  • Early loss of a primary second molar can cause mesial drift or tilting of the permanent first molar. We present a case of the early loss of the mandibular primary molars in a 5-year-old girl. The patient required extraction of the mandibular left primary first and second molars due to folliculitis on the successors. A flexible denture was used for eruption guidance of the permanent first molar and the recovery of masticatory function simultaneously.

  • This research examined an immediate loading capable of providing a masticatory function and financial affordability while satisfying every requirement of a useful immediate loading. In this research, numerical value analysis was performed on couple-type posterior teeth implants developed by making up for the problems of extant implants. As a result of examining the effects of external impacts on extant implants and their deformation, relatively lager deformations separated from a molar were found concerning molar deformation implant with the maximum deformation level being approximately 1.657mm. In this research, the improved implant showed much improvement in terms of impact analysis in its deformation dispersion status after unloading the impulsive load from a punch. As for the case of hepf, about 1.657mm occurred but the improved model showed about 0.9217mm, exhibiting 55.6% advancement.

  • In the treatment of mandibular condyle fracture, conservative treatment using closed reduction or surgical treatment using open reduction can be used. Management of mandibular condylar fractures remains a source of ongoing controversy in oral and maxillofacial trauma. For each type of condylar fracture, the treatment method must be chosen taking into consideration the presence of teeth, fracture height, patient'sadaptation, patient's masticatory system, disturbance of occlusal function, and deviation of the mandible. In the past, closed reduction with concomitant active physical therapy conducted after intermaxillary fixation during the recovery period had been mainly used, but in recent years, open treatment of condylar fractures with rigid internal fixation has become more common. The objective of this review was to evaluate the main variables that determine the choice of an open or closed method for treatment of condylar fractures, identifying their indications, advantages, and disadvantages, and to appraise the current evidence regarding the effectiveness of interventions that are used in the management of fractures of the mandibular condyle.

  • The goals of lip reconstruction are to provide oral competence, adequate support for the lower lip, contour restoration, adequate lip sulcus, and adequate oral aperture. The composite radial forearm palmaris longus free flap is thin enough that it can be folded onto itself without a significant increase in bulk. The flap is easy to dissect, the pedicle contains long vessels of large diameter, and the skin is a good color and texture match for the perioral region. Moreover, the vascularized tendon can be used for lower lip reconstruction. This makes the flap ideally suited for total lower lip reconstruction. We experienced the case of total lower lip excision and reconstruction with the radial forearm free flap including palmaris longus tendon, so we reported that case with literature. The patient has a lower lip squamous carcinoma(T3NIM0), and performed a total lower lip excision with right modified radical neck dissection and left extended supraomohyoid neck dissection, and a reconstruction with radial forearm free flap includng palmaris longus tendon. The oral competence and masticatory function were nearly normalized and cosmetical result was very acceptable.

  • Dental porcelains are widely used for restorative material because of its excellent esthetic property. But according to contact with natural teeth or metal in oral cavity, the porcelain may be worn and may effect on masticatory physiology and prosthetic function. The purpose of this experiment was to study on wear of porcelain surface which treated in different method. Using the abrasion device which was designed and constructed by myself, the different porcelain surface were abraded by gold alloy, nickel-chrome alloy and natural teeth in order to compare roughness. Results were as follows. 1. The group of porcelain abraded by gold alloy showed less surface roughness change (t=2.92, p<0.05), and the group of porcelain abraded by natural teeth had high surface roughness. change (t=6.84, p<0.05). 2. According to the method of surface treatment, the surface roughness were very significant (F=9.12, p<0.05). 3. After abrading, the porcelain surface roughness change was very significant (F=54.49, p<0.05). 4. There was no significant between surface treatment method and the kind of abrading materials. (F=1.01, p>0..05). 5. The group of natural glazing had the most smooth surface ($2.1{\pm}1.13{\mu}m$).

  • In this report, a case is presented in which resected mandible was reconstructed immediately with vascularized bone graft and adjunctive implantation of osseointegrated dental implants. The primary was central odontogenic myxoma of mandible extending from symphysis to the left condylar neck. The hemimandibulectomy defect was restored with free fibular flap. Three months after 1st surgery, the transplant received five $IMZ^{(R)}$ implants. The masticatory function was restored with the implant borne denture. The result including facial appearance was very satisfactory.

  • Loss of tooth results in remodeling and resorption of surrounding alveolar bone which causes atrophic edentulous ridge and gradually decreasing gingival attachment. As a result, edentulous patients face difficulty in using dentures due to pain, decrease of support, decline of masticatory efficiency of complete denture. To improve this, overdenture with implant in the mandible and attachment are considered as a treatment of choice as a favorable treatment. In this case, a patient with edentulous ridge for long period is rehabilitated by complete denture in maxilla and implant overdenture using Locator$^{(R)}$ attachment in mandible.
