Surgical Treatment for Thymoma (흉선종의 외과적 치료)

  • This report documents the clinical k pathologic features of 33 patients treated for thymoma for 11 years & 6 months between September 1977 and February 1989. At the Thoracic & Cardiovascular Surgery, Yonsei University, College of Medicine of the group, 31 patients treated with surgery were examined for the result of operation & prognosis. Mean age was 50 years. Thirteen were female and twenty were male. Dyspnea on exertion and chest discomfort were common in the patients without myasthenia gravis. Fourteen patients[42.6%] had myasthenia gravis and one patient had autoimmune thyroid disease. Four patients[12.1%] presented without symptoms attributable to their thymoma. Histologic review disclosed 12[36.4%] epithelial thymoma, 10[30.3%] mixed lymphoepithelial, 9[27.3%] lymphocytic, 1[3.0%] spindle cell and 1[3.0%] unknown cell thymoma. They were classified according to Masaoka`s clinical staging criteria; by these criteria, 8 patients were stage I, 5 patients were stage II, 15 patients were stage III and 3 patients were stage IV. Total excision of mass was possible in 20 patients. Partial excision of mass in 4 patients and biopsy in 7 patients were carried out during the operation. There was only one operation mortality. Follow-up was possible in 26 patients and follow-up ranged from 4 months to 10.5 years[mean 28.9 months]. One-year survival rates were 77.8% and eight patients expired during follow-up period. Eleven[78.6%] patients with myasthenia gravis were improved after the operation. This observation suggests that the most significant factor determining the survival is whether or not total surgical excision had been performed.

Mass Mortaliaty by Aeromonas hydrophila Inferction in the Production of the Korean Mandarin Fish Fingerling, Siniperca scherzeri (쏘가리 (Siniperca scherzeri) 치어 생산에 있어서 Aeromonas hydrophila 감염에 의한 대량 폐사)

  • A specific disease syndrome, which led to massive mortality on the Korean mandarin fish fingerling (Sinperca scherzeri) at Chongpyong Inland Fisheries Institute was atudied. The causetive agent isolated from the diseased fish was identified as Aeromonas hydrophila on the basis of biochemical and physiological characteristics. Infection experiments in the Korean mandarin fish fingerling, weighting 3-4 g with A. hydrophila were conducted by immersion, oral administration, intramuscular injection, and injection of the soluble extracellular products secreted from it. Motality rate was higher virulence in intramuscular injection group than other experimental groups. In injection group of the soluble extracellular products, all fish treated with high concentration ($8{\times}10^9$ cfu/ml) were repidly killed into 3-6 hrs. There results show that the Korea mandarin fish fingerling has high susceptibility to A. hydrophila.

Mechanical versus Tissue Aortic Prosthesis in Sexagenarians: Comparison of Hemodynamic and Clinical Outcomes

  • Background: The question of which type of prosthetic aortic valve leads to the best outcomes in patients in their 60s remains controversial. We examined the hemodynamic and clinical outcomes of aortic valve replacement in sexagenarians according to the type of prosthesis. Methods: We retrospectively reviewed 270 patients in their 60s who underwent first-time aortic valve replacement from 1995 to 2011. Early and late mortality, major adverse valve-related events, anticoagulation-related events, and hemodynamic outcomes were assessed. The mean follow-up duration was $58.7{\pm}44.0$ months. Results: Of the 270 patients, 93 had a mechanical prosthesis (mechanical group), and 177 had a bioprosthesis (tissue group). The tissue group had a higher mean age and prevalence of preoperative stroke than the mechanical group. The groups had no differences in the aortic valve mean pressure gradient (AVMPG) or the left ventricular mass index (LVMI) at 5 years after surgery. In a sub-analysis limited to prostheses in the supra-annular position, the AVMPG was higher in the tissue group, but the LVMI was still not significantly different. There was no early mortality. The 10-year survival rate was 83% in the mechanical group and 90% in the tissue group. The type of aortic prosthesis did not influence overall mortality, cardiac mortality, or major adverse valve-related events. Anticoagulation-related events were more common in the mechanical group than in the tissue group (p=0.034; hazard ratio, 4.100; 95% confidence interval, 1.111-15.132). Conclusion: The type of aortic prosthesis was not associated with hemodynamic or clinical outcomes, except for anticoagulation-related events.

A 12 Year Follow-up Study on the Factors Related with the Death of Hypertensives in a Korean Rural Area (일부 농촌지역 고혈압자의 사망에 미치는 요인 -12년 추적 연구-)

  • In order to find the factors associated with the death of hypertensives, 12 year follow-up study for 267 hypertensives whose average blood pressure were 140/90 mmHg or above during their first health screening in 1979-1980 at YongJin Township, Wanju Country, North Cholla Province by the Community Health Team of Chonju Presbyterian Medical Center. The study results are as follows : 1. Initial general characteristics of hypertensives were studied. The age distribution of studied hypertensives showed 27.3% in 40-49 years, 25.8% in 50-59 years, 29.6% in 60-69 years and 17.2% In 70 + years old group. Marital status showed that 82.8% of hypertensives had their wife or husband. 74. 5% were employed on agriculture. 56.5% of hypertensives were illiterate. 2. Among the hypertensives, 91.6% of male and 22.8% of female reported that they were smokers. 82.6% of hypertensives had body mass index lower than 25 Kg/$m^2$. 46.8% of average systolic blood pressure were 160mmHg or above and 54.3% of average diastolic blood pressure were 95mmHg or above. 3. Twenty percent of hypertensives reported that they were treating hypertension at the beginning of follow up, while 68.5% reported that they were not treated. 28.1% reported that they were treating hypertension within 6 months before last follow-up. but 69.3% reported that they were not treated for hypertension within Ii months before last follow up. So 50.6% were classified as never treated group and 41.2% as treated group. 4. Average blood pressure for initial 3 years were calculated. The change of average systolic blood pressure was observed as $161.3{\pm}19.4mmHg$ at the first year, $145.6{\pm}28.0mmHg$ at the second year and $141.4{\pm}37.2mmHg$ at the third year. Average diastolic blood pressure were changed from $96.2{\pm}14.4mmHg$ at the first year to $90.6{\pm}18.6mmHg$ at the second year and $86.4{\pm}22.9mmHg$ at the third year. 5. By the follow-up of hypertensives, 54 hypertensives (46.2%) among 117 male hypertensives and 50 hypertensives (33.3%) among 150 female hypertensives died for 12 years. 42.6% of male death and 52.0% of female death were caused by cerebrovascular diseases. 6. Through univariate statistical test about the association between general characteristics or cardiovascular risk factors of hypertensives and mortality for 12 years, age variable among male and among female age, marital status, occupation. educational level. systolic blood pressure and treatment status were shown as significant variable to influence upon the mortality. 7. By multiple logistic regression analysis, among male age and systolic blood pressure were selected as significant variable to be associated with the total mortality for 12 years. Among female age, systolic blood pressure and treatment status were selected as statistically significant variable to be associated with the total mortality for 12 years.

Toxicity and Behavioral Changes of Medaka (Oryzias latipes) by Brine Exposure (송사리(Oryzias latipes)를 이용한 고염해수의 생태독성 및 단기적 행동변화에 관한 연구)

  • Acute toxicity test and behavioral change analysis of seawater acclimated Japanese medaka were conducted to identify the brine effects on fish by seawater desalination. 7 day acute toxicity test of brine revealed linear concentration-response relationship from 40.0~80.0 psu treatment groups. There was no significant brine effect for 30-40 psu groups and mass mortality was observed from >50 psu exposure (7-day $LC_{50}$=51.4 psu). Images from the real time camera system were analyzed to observe the changes in behavioral patterns of medaka exposed to various salinity. 40.0 and 50.0 psu exposed groups were stabilized in behavioral patterns after 3.1 and 4.6 hours, respectively and 60.0 psu group showed sharp increase in activity during first 12 hours and 50% mortality thereafter. Similar patterns were observed to 70 and 80 psu groups and both experimental groups showed 100% mortality within 12 hours. Acute toxicity test and behavioral patterns showed very similar toxicity results which revealed the increases in mortality and behavioral activities from 50.0 psu. This critical salinity for fish impacts must be implemented to brine discharge strategy by seawater desalination into the coastal area. Also, we recommend that real time camera monitoring system must be a useful tool for early warning of fish toxicity for other applications. A Case of Direct Invasion of the Parotid Gland by Cutaneous Squamous Cell Carcinoma (이하선으로 직접 전이한 피부 편평상피세포암)

  • Cutaneous squamous cell carcinoma is the second-most common skin cancer and represents 20% of all skin cancers. Cutaneous squamous cell carcinoma often spreads to the parotid gland through lymph nodes, but, direct invasion of an adjacent organ may also occur. We present the case of 78-year-old man with ulcerated mass on the right infra-auricular area. The histopathologic finding was squamous cell carcinoma. There was no evidence of distant metastasis, but the mass was found to invade the superficial lobe of the right parotid gland. The mass was widely excised and superficial parotidectomy was performed while preserving the facial nerve. The defect was covered by primary closure. Postoperative radiotherapy was performed. At 20 months after surgery, our patient had no facial palsy, local recurrence, or metastasis. Cutaneous squamous cell carcinoma involving the parotid gland is an aggressive, rapidly advancing lesion, which if not recognized and treated early will result in high morbidity and mortality. Squamous cell carcinoma of the parotid gland has shown that patients who receive adjuvant radiotherapy have a lower recurrence rate and a higher survival rate than patients treated with surgery alone. The role of elective neck dissection remains controversial.

Investigation of interference current distribution in a long line scallop cage aquaculture (수하식 큰가리비 양식의 채롱간 조류의 흐름 간섭현상 규명)

  • The mass mortalities have been occurring of Korean scallop Patinopecten yessoensis from 1997's to now in Korea east sea. Cages behavior and interference effect (common name; curtain effect) between scallop cages were investigated in culture grounds on the eastern coastal waters of Korea for understand to mechanism of rising about mass mortalities of Korean scallop quickly. The first experiment was carried out in circulating water channel to assess inclination angel from relationship between velocity and cages interval, velocity with culture cages position. An angle of inclination of scallop culture cages were 94.6 to 92.3 degree under a several velocity which were from 0.1 m/s and 131.9 to 118.1 degree under 0.5 m/s with cages interval were 1 m, 94.3 to 91.0 degree under velocity is 0.1 m/s and 133.2 to 122.4 degree under 0.5 m/s with cages interval were 1.5 m and 94.6 to 96.4 degree under velocity is 0.1 m/s and 131.7 to 131.8 under 0.5 m/s with cages interval were 2 m. The second experiment was designed to prove the tank test. Velocities were measured inside and outside of the scallop culture ground at eastern sea of Korea. The velocity of inside of the culture was the slowest as 0.1m/s. In this result, interference between former cage and after cage was occurred.

Clinical Prognostic Factors and Survival Outcome in Renal Cell Carcinoma Patients - A Malaysian Single Centre Perspective

  • Background: This study concerns clinical characteristics and survival of renal cell carcinoma (RCC) patients in University Malaya Medical Centre (UMMC), as well as the prognostic significance of presenting symptoms. Materials and Methods: The clinical characteristics, presenting symptoms and survival of RCC patients (n=151) treated at UMMC from 2003-2012 were analysed. Symptoms evaluated were macrohaematuria, flank pain, palpable abdominal mass, fever, lethargy, loss of weight, anaemia, elevated ALP, hypoalbuminemia and thrombocytosis. Univariate and multivariate Cox regression analyses were performed to determine the prognostic significance of these presenting symptoms. Kaplan Meier and log rank tests were employed for survival analysis. Results: The 2002 TNM staging was a prognostic factor (p<0.001) but Fuhrman grading was not significantly correlated with survival (p=0.088). At presentation, 76.8% of the patients were symptomatic. Generally, symptomatic tumours had a worse survival prognosis compared to asymptomatic cases (p=0.009; HR 4.74). All symptoms significantly affect disease specific survival except frank haematuria and loin pain on univariate Cox regression analysis. On multivariate analysis adjusted for stage, only clinically palpable abdominal mass remained statistically significant (p=0.027). The mean tumour size of palpable abdominal masses, $9.5{\pm}4.3cm$, was larger than non palpable masses, $5.3{\pm}2.7cm$ (p<0.001). Conclusions: This is the first report which includes survival information of RCC patients from Malaysia. Here the TNM stage and a palpable abdominal mass were independent predictors for survival. Further investigations using a multicentre cohort to analyse mortality and survival rates may aid in improving management of these patients.

Effects of Dietary Energy and Protein Levels on the Performance of Brown Layer in Summer (산란계 사료의 에너지 및 단백질 수준이 하절기에 있어서 갈색산란계의 생산성에 미치는 영향)

  • Total 720 brown layers were fed 9 rations differing in metabolizable energy (2500, 2700 and 2900 kcal/kg) and crude protein (13, 15 and 17%) levels for a period of 12 weeks in order to study the effects of dietary energy and protein levels on the performance of brwon layer in summer. As metabolizable energy level increased from 2500 to 2900Kcal/kg of feed egg production, daily feed and protein intake were decreased, but daily energy intake, energy requirement and feed cost per kilogram of egg mass were increased. There were no significant difference in egg weight, feed conversion and motality among dietary energy levels. On the other hand, as dietary Protein level increased from 13 to 17% egg Production, egg weight, daily protein intake and protein requirement per kilogram of egg mass were increased, but feed and energy requirement and feed cost per kilogram or egg mass were decreased. However daily feed intake and mortality were not affected by dietary protein level.

The Impact of Obesity on the Use of a Totally Laparoscopic Distal Gastrectomy in Patients with Gastric Cancer

  • Purpose: Since a patient's obesity can affect the mortality and morbidity of the surgery, less drastic surgeries may have a major benefit for obese individuals. This study evaluated the feasibility of performing a totally laparoscopic distal gastrectomy, with intracorporeal anastomosis, in obese patients suffering from gastric cancer. Materials and Methods: This was a retrospective analysis of the 138 patients, who underwent a totally laparoscopic distal gastrectomy from April 2005 to March 2009, at the National Kyushu Cancer Center. The body mass index of 20 patients was ${\geq}25$, and in 118 patients, it was <25 kg/$m^2$. Results: The mean values of body mass index in the 2 groups were $27.3{\pm}2.2$ and $21.4{\pm}2.3$. Hypertension was significantly more frequent in the obese patients than in the non-obese patients. The intraoperative blood loss, duration of surgery, post-operative complication rate, post-operative hospital stay, and a number of retrieved lymph nodes were not significantly different between the two groups. Conclusions: Intracorporeal anastomosis seemed to have a benefit for obese individuals. Totally laparoscopic gastrectomy is, therefore, considered to be a safe and an effective modality for obese patients.
