The resolution of Earth images taken from a satellite has close relation with satellite's altitude. If a satellite has lower altitude, it gets a picture having better resolution. However the satellite will be exposed to heavier air drag and will spend more fuel to maintain its altitude for a desired mission. Therefore, in this study, the required fuel to maintain very low earth orbit(LEO) with severe air drag is analyzed using optimization method such as collocation method. The required fuel to maintain the low altitude has significantly increased as the mission altitude is lowered and the solar activity is maximized. This study also shows that the fuel reduced by increasing the period of the satellite maneuver is very small, and that slightly increasing the satellite's mission altitude is much effective in reducing the amount of fuel to maintain its altitude. The calculated fuel to maintain very low earth orbit in this study would give useful information in planning the budget of fuel and cost for LEO satellites.
A severe crushing injury of the chest produce a very striking syndrome referred to as traumatic asphyxia. This syndrome is characterized by bluish-red discoloration of the skin which is limited to the distribution of the valveless veins of the head and neck. And also if it is characterized by bilateral subconjunctival hemorrhages and neurological manifestations. But these clinical entities faded away progressively in a few weeks. Apporximately 90% of the patients who live for more than a few hours will recover from traumatic asphyxia when it occurs as a single entity. And so, death results from either severe associated injuries of from subsequent infection, rather than from pulmonary or cardiac insufficiency in traumatic asphyxia. We have experienced 4 cases of traumatic asphyxia with severe crushing thoracic injuries at department of the chest surgery, Captial Armed forces General Hospital during about 3 years from April 1977 to Aug. 1980. The 1st 22 year-old male was struct 2$\frac{1}{2}$ ton truck on the road and was transferred to this hospital immediately. He had taken tracheostomy due to severe dyspnea with contusion pneumonia and for removal of a large amount of bronchial secretion. The 2nd case was 23 year-old male who was got buried in a chasm. In this case, the heavy metal post tumbled over him back while at work. The 3rd case was 39 year-old male who leapt out of a window in 5th story while fire broke out in living room by oil stove heating. He had multiple rib fracture with right hemothor x and right colle's fracture and pelvic bone fracture. The last 22 year-old male was run over by a gun carriage. The wheel of this gun carriage passed over his thorax and right chin. He was brought to this hospital by helicopter. when he was first examined at emergency room, he was in semicomatose state and has pneurmomediastinum with multiple rib fracture and severe subcutaneous emphysema. As soon as he arrived, bilateral closed thoracostomy was performed and cardiopulmonary resuscitation was done. In hospital 8th weeks, chest series showed fibrothorax in right side even if chest wall stabilized. All 4 cases had multiple petechiae over their facees and chest and bilateral subconjunctival hemorrhages referred to as traumatic asphyxia. 3 cases except one case who received splenectomy, had been suffered from contusion pneumonia and had been treated with respiratory care. In these 3 cases, they had warning of impending injury before accident, and took a deep breath hold it and braces himself. And also, even if he had not impending fear in remaining one case, he had taken a deep breath and had got valsalva maneuver for pulling off the heavy metal post. Intrathoracic pressure rose suddenly and resulted to traumatic asphyxia in this situation. All these cases were recovered completely without sequelae except one fibrothorax, right.

Terror groups joining the criminal act is to use Islam illegal aliens who already acquire base in our nation or entry maneuver of specialized terrorists connected to Al-Qaeda. Pretext of such terror is withdrawal of our military and western allies stationed in Afghanistan. Therefore, publicity terror of Taliban against our people living overseas and security measure of international events such as the world nuclear security summit meeting, Yeosu EXPO, and etc in the first half of this year should be thoroughly prepared, domestic illegal aliens' movement should be comprehended, and measure blocking international terrorists' relating to Taliban infiltration into our nation should be sought for. Also, there is need of early announcement of government's political will on our military stationed in Afghanistan.

In this study, we built a thermal model for SNIPE 6U nano-satellite which has scientific mission for measuring science data in near Earth space environment and described thermal design based on the thermal model. And the validity of the thermal design was verified through the on-orbit thermal analysis. The thermal design was carried out mainly on the passive thermal control techniques such as surface finishes, insulators, and thermal conductors in consideration of the characteristics of the nano-satellite. However, the components with narrow operating temperature range and directly exposed to the orbital thermal environments, such as a battery and thrusters, are accomodated with heaters to satisfy the temperature requirements. On-orbit thermal analysis conditions are based on the basic orbital conditions of the satellite, and thermal analysis was performed for Normal mode, Launch & Early Orbit Phase (LEOP), Safehold mode, and Maneuver mode which are classified by the power consumption and the attitude of the satellite according to the mission scenario. The analysis results for each mode confirmed that every component satisfies the temperature requirement. In addition, the heater capacity and duty cycle of the battery and thruster were calculated through the analysis results of the Safehold mode.

Purpose: To evaluate the feasibility and safety of intracorporeal overlapping gastrogastrostomy between the proximal anterior wall and antrum posterior wall (PAAP; PAAP anastomosis) of the stomach in minimally invasive pylorus-preserving gastrectomy (PPG) for early gastric cancer (EGC). Materials and Methods: From December 2016 to December 2019, 17 patients underwent minimally invasive PPG with PAAP anastomosis for EGC in the high body and posterior wall of the stomach. Intraoperative gastroscopy was performed with the rotation maneuver during proximal transection. A longer antral cuff (>4-5 cm) was created for PAAP than for conventional PPG (≤3 cm) at the point where a safe distal margin and good vascular perfusion were secured. Because the posterior wall of the proximal remnant stomach was insufficient for intracorporeal anastomosis, the anterior wall was used to create an overlapping anastomosis with the posterior wall of the remnant antrum. The surgical and oncological outcomes were analyzed, and the stomach volume was measured in patients who completed the 6-month follow-up. The results were compared to those after conventional PPG (n=11 each). Results: PAAP anastomosis was successfully performed in 17 patients. The proximal and distal resection margins were 2.4±1.9 cm and 4.0±2.6 cm, respectively. No postoperative complications were observed during the 1-year follow-up esophagogastroduodenoscopy (n=10). The postoperative remnant stomach (n=11) was significantly larger with PAAP than with conventional PPG (225.6±118.3 vs. 99.1±63.2 mL; P=0.001). The stomach length from the anastomosis to the pylorus was 4.9±2.4 cm after PAAP. Conclusions: PAAP anastomosis is a feasible alternative for intracorporeal anastomosis in minimally invasive PPG for highly posteriorly located EGC.
Objectives The purpose of this study is to carry out a survey on satisfaction for Korean medicine treatments in traffic injury patients. Methods Satisfaction survey was carried out on the 237 traffic injury patients who had been treated by Korean medicine treatments in Department of Korean Rehabilitation Medicine, Wonkwang University Gwangju Medical Center from February, 2017 to August, 2017. The questionnaire form of preceding study was used to assess the satisfaction degree of patients. All data were coded by SPSS v20.0 (IBM Corp., Armonk, NY, USA) and analysed by descriptive maneuver, x2-test, T-test and correlation. Results Among the 237 cases, the first hospital to be selected for traffic injury treatment was Korean medicine hospital in 91 cases. And 97 patients were transferred from the western medicine hospital. The main reason for choosing Korean medicine hospital was that the patients prefer Korean medicine treatments than western medicine treatments. 85.2.% of patients answered that their symptoms were improved, and 75.1% of patients answered that they were satisfied with Korean medicine treatments. The satisfaction degree of cupping treatment and acupuncture was relatively high, but the degree of Herbal medicine and physical treatment was relatively low. There were no statistically significant differences in satisfaction degree with sex and age. The satisfaction degree, improvement of symptoms, intention to revisit and intention to recommend was significantly correlated each other. Conclusions The degree of improvement of symptoms and The satisfaction degree were revealed relatively high. Additional and repetitive studies are needed to understand the patient's position and improve the quality of Korean medicine treatments for traffic accident patients.
In order to establish predicted normal values of volume of isoflow($V_{iso}\dot{V}$), $V_{iso}\dot{V}$ were measured in a total 234 healthy adults, consisting of 78 male nonsmokers, 108 male smokers and 48 female nonsmokers. The volumes of isoflow were determined through analysis of maximal expiratory flow volume curves recorded with a mixture of 80% helium and 20% oxygen(He-$O_2$ gas) following one(1VC) and three vital capacity(3VC) maneuver and with room air. There is no well-stated practical guideline for mechanically ventilated patients with or without acute respiratory distress syndrome (ARDS). We generate strong (1) and weak (2) grade of recommendations based on high (A), moderate (B) and low (C) grade in the quality of evidence. In patients with ARDS, we recommend low tidal volume ventilation (1A) and prone position if it is not contraindicated (1B) to reduce their mortality. However, we did not support high-frequency oscillatory ventilation (1B) and inhaled nitric oxide (1A) as a standard treatment. We also suggest high positive end-expiratory pressure (2B), extracorporeal membrane oxygenation as a rescue therapy (2C), and neuromuscular blockage for 48 hours after starting mechanical ventilation (2B). The application of recruitment maneuver may reduce mortality (2B), however, the use of systemic steroids cannot reduce mortality (2B). In mechanically ventilated patients, we recommend light sedation (1B) and low tidal volume even without ARDS (1B) and suggest lung protective ventilation strategy during the operation to lower the incidence of lung complications including ARDS (2B). Early tracheostomy in mechanically ventilated patients can be performed only in limited patients (2A). In conclusion, of 12 recommendations, nine were in the management of ARDS, and three for mechanically ventilated patients.
Background: Although profound hypothermia with total circulatory arrest(TCA) is a valuable maneuver in cardiac surgery, its applications have been limited due to serious complications, especially cerebral damage. In this study, the possible role of creatinine kinase-BB(CK-BB), an index enzyme of ischemic cerebral damage, was assayed as a parameter for the assessment of the cerebral complications after TCA. Hemoglobin(Hb), ionized calcium(Ca++), and blood glucose levels were also assessed as clinical parameters involved in cerebral damage. Materials and methods: Among patients with congenital heart disease, 18 patients who had been operated on with TCA were randomly selected and divided into two groups: 6 with acyanotic and 12 with cyanotic heart disease. Arterial blood from each patient was collected before and after TCA at scheduled times(15 min., 30 min, 1, 2, 4, 8, and 12hr). The levels of CK-BB, Hb, Ca++, and blood glucose were assessed in each sample. Results: As a whole, correlation between CK-BB level and blood sampling time after TCA was not statistically significant. Also, the difference in the level of CK-BB after TCA was not significant between the acyanotic and cyanotic groups. The levels of Hb and CK-BB correlated significantly. Conclusions: The results, which showed no correlation between the alterations in CK-BB level and the TCA duration, suggest that the single assay of the CK-BB level is not a representative measurement for the assessment of cerebral damage after TCA. Also, the cyanotic congenital heart disease group is not more vulnerable to cerebral damage induced by TCA.
