교정이나 악교정 수술 환자에서 치료계획 수립과 치료결과의 평가를 위해서 지금까지 많은 수평기준선이 사용되고 있으나 정확성과 재현성 등의 문제가 존재하고 있으며, 더욱 객관적인 수평기준선이 설정 될 수 있다면 교정학 분야에서 많은 발전이 기대될 수 있을 것이다. 이에 본 연구에서는 한국 성인의 부정교합군과 성별에 따른 Sella-Nasion(SN) 평면과 Frankfort-Horizontal(FH) 평면이 이루는 각도 및 FH평면과 다른 수평면 간의 상관 관계를 조사하기 위하여 먼저 임상검사를 시행하여 총 600 여명을 대상으로 측모두부규격 방사선사진을 촬영하였다. 계측결과를 바탕으로 연구대상을 골격성 I급 부정교합군 또는 정상교합군(남 50, 여 50), 골격성 II급 부정교합군(남 50, 여 65), 골격성 III급 부정교합군(남 50, 여50)으로 분류한 후 10개의 항목을 평가하여 다음과 같은 결론을 얻었다. 1. SN평면과 FH평면이 이루는 각도는 부정교합에 따른 차이는 없었으나 성별 간에는 세 부정교합군 모두에서 차이를 보여 남자는 $7.47{\pm}2.40^{\circ}$, 여자는 $8.93{\pm}2.72^{\circ}$ 였다. 2. SN 평면 또는 FH 평면과 Mandibular Plane이 이루는 각은 모든 부정교합군에서 여자가 남자보다 컸으며, 골격성 II급 부정교합군과 III급 부정교합군이 I급 부정교합군보다 큰 각도를 나타내었다. 3. FH평면과 Palatal Plane이 이루는 각은 부정교합군 및 성별에 따른 차이를 보이지 않고 비교적 일정하였다. 4. Gonial angle은 남녀 모두 III급 부정교합군이 다른 두 부정교합군보다 큰 각도를 보였다.
본 연구에서는 II급 부정교합자의 교합평면의 형태적 특징을 관찰하고, 안면의 수직고경과 연관지어 정상교합군과 비교하여 교합평면의 발현에 영향을 미치는 골격성, 치성요인의 기여정도를 알아보고자 했다. 성장과 연령군의 변이를 배제하기 위해 성장이 완료된 성인만을 연구대상으로 정하였다. 정상교합자 50명과 서울대학교 치과진료부 교정과에 내원한 50명의 Angle class II 환자의 측모 두부방사선 계측사진분석을 통하여 다음의 결과를 얻었다. 1. II급 부정교합자군에서 정상군보다 유의하게 크게 관찰된 교합평면의 경사도는 다음과 같다. ; SN-FOP, FH-BOP, MP-BOP, AB-BOP, AB-FOP, Facial plane-BOP, FP-FOP 2. II급 부정교합자군에서는 정상군에 비해 후안면 고경, 특히 후하안면고경이 유의한 차이를 보였다. 3. 교합평면과 상하악 전치가 이루는 각은 II급 부정교합군에서 통계적으로 유의하게 컸고, 대구치와 이루는 각은 상악 대구치와 해부학적 교합평면이 이루는 각만이 통계적으로 유의한 차이를 나타냈다. 4. 교합평면의 위치를 결정하는 요소중에서 하악절치에서 하악평면에 이르는 거리만이 II급 부정교합군에서 유의하게 컸다. 5. III급 부정교합군에서는 수직피개도와 교합평면 경사도의 상관관계가 존재하지만, 정상 교합군에서는 수직피개도와 교합평면 경사도간에 상관관계가 존재하지 않았다.
성장기 아동의 상기도 협착은 두개안면 성장에 영향을 미칠 수 있다. 본 연구는 3차원 cone-beam computed tomography (CBCT)를 이용하여 제II급 부정교합 아동의 상기도 부피 및 단면적 계측치와 측모두부계측치를 비교하여 상기도와 안면 형태와의 연관성을 알아보고자 하였다. 37명의 아동(남자 17명, 여자 20명, 평균 연령 11.02세)의 CBCT를 촬영하였다. 연구 결과, 상기도의 PNS-posterior plane에서의 단면적($S_{PP}$)은 제II급 부정교합군에서 유의하게 작았다 (p < 0.05). 상기도 부피 계측치는 제II급 부정교합군에서 제I급 부정교합군보다 작았으나, 두 군 간 통계적으로 유의한 차이는 없었다. 제II급 부정교합군은 후안모고경(PFH), mandibular body length (p < 0.05), Pog to Nperp (p < 0.01)에서 제I급 부정교합군보다 유의하게 작은 값을 보였고, FMA (p < 0.05), ANB, facial convexity (p < 0.01)는 유의하게 큰 값을 보였다. 제II급 부정교합군의 PNS 전방의 상기도 부피($W_N$)는 ANB difference 각도와 음의 상관관계를 보였다 (p < 0.05). 제II급 부정교합 아동은 상대적으로 좁은 상기도 부피를 가지며 이에 따른 감소 된 후안모고경(PFH)과 하악골의 divergent한 안면 성장 양상을 보였다.
유치열기의 3급 부정교합은 조기 치료가 추천되지만 정확한 진단이 쉽지 않다. 3급 부정교합의 골격성 특성과 치아 발육상태 사이에 연관성이 있다면 골격성 3급 부정교합의 감별 진단을 위한 평가 항목으로 치아성숙도가 활용될 수 있을 것이다. 본 연구는 유치열기의 골격성 혹은 비골격성 3급 부정교합으로 진단받은 환아를 대상으로 치아 석회화도 및 제 1대구치의 맹출률을 비교, 분석하여 상하악의 치아성숙도 차이가 유치열기 3급 부정교합의 감별 진단시 평가 항목으로 사용될 수 있는지 알아보기 위한 목적으로 시행되었다. 전치부 반대교합을 주소로 부산대학교 치과병원 소아치과에 내원한 유치열기 아동 중 비골격성 3급 부정교합군 18명과 골격성 3급 부정교합군 34명을 연구대상으로 선정하였다. 파노라마 방사선사진상에서 치령 및 제1대구치 맹출률을 비교, 분석하여 다음과 같은 결과를 얻었다. 골격 및 성별에 따른 역령과 치령의 차이는 존재하지 않았으며, 두 군 모두 역령에 비해 치령이 높게 나타났다(p < 0.05). 비골격성군과 골격성군의 상하악 맹출률의 차이는 각각 16.53%, 18.91%로 통계학적으로 유의한 차이를 보여(p < 0.05), 3급 부정교합의 감별 진단을 위한 평가 항목으로서 제1대구치 치아성숙도의 활용 가능성을 시사한다.
The purpose of this study was to detect out the changes occured during orthodontic treatment. The sample was consisted of 77 orthodontic patients. For this study 13 linear lengths and arch area were measured in maxilla, mandible respectively and were analyzed statistically. The results were as follows 1 The sequence of changes in the form and dimensions of dental arches following orthodontic treatment was as follows Class I malocclusion, Class III malocclusion, Class II malocclusion. 2 Changes in the form and dimensions of dental arches were greater in extraction cases than those of non-extraction cases 3 In comparison with maxilla and mandible on the amount of changes following orthodontic treatment in each malocclusion group, significant differences were greatest in class III malocclusion 4 In comparison with maxilla and mandible on the amount of changes following orthodontic treatment in extraction and non-extraction cases, significant differences were greater in extraction cases than those of non-extraction cases 5. The amount of changes during orthodontic treatment in extraction and non-extraction cases in male was not different from female's.
The work was undertaken to evaluate the calcification of the second and the third molars in skeletal Class II and III malocclusions. The differences in the calcification stages between skeletal Class II and III malocclusion were evaluated and statistically analysed from panoramic radiographs of 202 males and females ranging in age from 11 to 15 years old. The results were as follows, 1. The calcification stages of the second and the third molars were not different between the skeletal Class II and III malocclusions in each age groups of both sexes. 2. The calcification stages of lower second and third molars of the skeletal Class III malocclusion are more advanced than those of the skeletal Class II malocclusion in male. 3. The clacification stages of upper second and third molars are more advanced than those of lower second and third molars in skeletal Class II malocclusion. 4. The calcification stages of lower second and third molars are more advanced than those of upper second and third molars in skeletal Class III malocclusion.
Early orthodontic treatment in growing children requires the removable orthodontic appliances. The removable orthodontic appliance can be used in the primary dentition and mixed dentition. The purposes of use of removable orthodontic appliance in prmary dentition or mixed dentition are the interception of skeletal or dental malocclusion, guiding the normal dentition, and retention after comprehensive orthodontic treatment. Therefore, it is needed to use the removable orthodontic appliance in children with malocclusion. This report presents cases of growing children with skeletal class II and III malocclusion treated with removable orthodontic appliance during mixed dentition and Multiloop Edgewise Arch Wire(MEAW) during permanent dentition. The results obtained through these cases were summarized as follows : 1. Removable orthodontic appliances guide normal dentition and skeletal growth in growing children. 2. Removable orthodontic appliances play an important role in intercepting malocclusion in mixed dentition before use of fixed orthodontic appliance. 3. MEAW can be applied to finishing stage of all cases and is effective in correction of occlusal plane, achievement of interdigitation, and control of dental inclination. 4. It is needed that removable appliances are used during mixed dentition through correct diagnosis and fixed appliance are used in permanent dentition.
This investigation was designed to compare the craniofacial morphology of Class III malocclusions with that of normal occlusions using PM line. The subjects consisted of forty-four normal occlusions (twenty-three males and twenty-one females) and sixty-nine Class III malocclusions (thirty males and thirty-nine females), aged eight through ten. Using the tracings of the standard lateral cephalograms, various angles, linear measurements and linear ratios of counter-part were recorded and analyzed by t-test. The following characteristics of craniofacial morphology of Class III malocclusion were obtained by this study. 1. Maxillary anteroposterior position was balanced with Nasion but was not balanced with mandible because maxillary bony arch was small and positioned posteriorly and mandibular corpus was large and positioned relatively anteriorly. 2. Upper and lover alveolar bony arch were not balanced each other in its size. 3. In counterpart analysis, Class III malocclusion was more horizontally unbalanced than normal occlusion. 4. Class III malocclusion was divided into 11 groups by maxillary and mandibular bony arch position, size and alveolar bony arch size. Unbalanced bony size of the maxilla and mandible was a major characteristics of Class III malocclusion.
The purpose of this study was to evaluate self perception of occlusal status and recognition for orthodontic treatment as well as the prevalence of orthodontic treatment need objectively and subjectively. 3979 (male 2107, female 1872) school students of age 7~18 were analyzed by questionnaire and clinical examination and the results were as follows, 1. Perception of occlusal status on one's own was more generous than that of orthodontist's and it was affected by the factors such as age, patterns of malocclusion. 2. Recogniuon of subjective need for orthodontic treatment was more generous than that of objective need for the treatment, and factors such as age and sex of patient, parents' age and rate of education, income, occupation, classification of malocclusion and regional discrepancy were influenced. 3. Negative aspect of orthodontic treatment was influenced by the environmental factors. 4. A survey of want for orthodontic treatment showed one's alteration in recognition of the occlusal status with age and esthetic component was much more emphasized than functional component. 5. The multivariate discriminanat analysis for orthodontic treatment group showed that mother's rate of education, classification of malocclusion, demographic characteristics were critical in the determination of treatment group. 6. Test of inter-examiner reliability showed moderate coincidence.
The purpose of this study is to observe the effect of high pull headgear on the craniofacial structures of mixed dentition with Class II malocclusion. The cephalometric headplates of 16 children treated by high pull headgeaar during 6 months and 18 children during 12 months were traced, digitized and statistically analyzed. The results were as follows. 1. Inhibition of foreward growth of maxilla was observed in both group. 2. Clockwise rotation of maxilla was observed in both group. 3. There were distal movement of maxillary 1st molar and inhibition of alveolar bone growth of maxilla. 4. There was compensatory extrusion on mandibular 1st molar. 5. The ratio of anterior facial height to posterior facial height was almostly not changed. In the treatment plan of C II malocclusion by high pull headgear, ire must prevent the mandibulasr 1st molar from extruding and for orthopedic effect, at least 6 months is needed.
