Synovial chondromatosis (SC) in temporomandibular joint is a rare benign disorder characterized by cartilaginous metaplasia of the mesenchymal remnants of the synovial tissue. The etiology of the disease is unclear but may be associated with trauma, overuse, local infection, and embryologic disturbance. SC does not spontaneously resolve and respond to nonsurgical treatment. SC should be differentially diagnosed with other temporomandibular joint disorders such as arthralgia or osteoarthritis because surgery should be done for managing it. However, primary diagnosis of SC is not easy because of nonspecific symptoms and signs. For the patients with unsuccessful conservative treatment response, especially accompanied by crepitus, preauricular swelling or posterior open bite, computed tomography/cone-beam computed tomography or magnetic resonance imaging should to be performed to exclude SC. We discussed the importance of the early diagnosis and surgical treatment of SC from this case.

The management of mandibular condylar fractures in children has long been a matter of controversy. The fracture, if not treated appropriately, may result in complications such as disturbance of mandibular growth and temporomandibular joint ankylosis. They are usually treated nonsurgically, which has been proved to be satisfactory in the long term results. Nineteen children with 25 condylar fractures experienced during their growth period (age at trauma from 10 months to 12 years, mean 7.0 years) were studied. All patients were treated by arch bars and intermaxillary fixation for $7{\sim}14$ days. They have been evaluated with clinical and radiographic examination. The maximum mouth opening and lateral movement of the mandible were within normal limits. There was no malocclusion or ankylosis. Beginning of remodelling was evident at postoperative $1{\sim}3$ months. Remodelling of the condyle was good in 21, while partial adjustment occured in the other 4 condyles. These results suggest that the conservative treatment of condylar fractures in growing children results in good functional results and good remodelling of the condyle.
A 21-year-old woman presented with facial asymmetry. Crepitus and clicking of the temporomandibular joint were noted. The midline deviated 5.5 mm to the left, and secondary malocclusion was observed. Panoramic and cone-beam computed tomographic images showed an irregular and exophytic bony mass on the anteromedial surface of the right mandibular condyle. A 3-phase bone scan revealed increased tracer uptake on the affected side. The lesion was treated with excision and reshaping under the diagnosis of osteochondroma confirmed by a histopathological examination. The lesion recurred after 3 years, and the patient underwent condylectomy. Mandibular condylar osteochondroma is often resected because it causes functional and aesthetic problems, but it rarely recurs. To the best of our knowledge, only 2 cases of recurrent osteochondromas of the mandibular condyle have been reported previously. Surgical treatment of the osteochondroma should be performed considering the possibility of recurrence, and long-term follow-up is recommended.
Purpose: This retrospective study aimed to evaluate the nasopharyngeal and oropharyngeal dimensions of individuals with skeletal class II, division 1 and division 2 patterns during the pre-peak, peak, and post-peak growth periods for comparison with a skeletal class I control group. Materials and Methods: Totally 124 lateral cephalograms (47 for skeletal class I; 45 for skeletal class II, division 1; and 32 for skeletal class II, division 2) in pre-peak, peak, and post-peak growth periods were selected from the department archives. Thirteen landmarks, 4 angular and 4 linear measurements, and 4 proportional calculations were obtained. The ANOVA and Duncan test were applied to compare the differences among the study groups during the growth periods. Results: Statistically significant differences were found between the skeletal class II, division 2 group and other groups for the gonion-gnathion/sella-nasion angle. The sella-nasion-B-point angle was different among the groups, while the A-point-nasion-B-point angle was significantly different for all 3 groups. The nasopharyngeal airway space showed a statistically significant difference among the groups throughout the growth periods. The interaction among the growth periods and study groups was statistically significant regarding the upper oropharyngeal airway space measurement. The lower oropharyngeal airway space measurement showed a statistically significant difference among the groups, with the smallest dimension observed in the skeletal class II, division 2 group. Conclusion: The naso-oropharyngeal airway dimensions showed a statistically significant difference among the class II, division 1; class II, division 2; and class I groups during different growth periods.

This report describes the case of an edentulous patient with an atrophic maxilla and severe class III malocclusion. Prosthetic rehabilitation was performed using CAD/CAM techniques for manufacturing an implant-supported overdenture with horizontal insertion. A vestibulo-lingual insertion overdenture is a precision prosthesis with a fixation system affording a good fit between the primary and secondary structure. Both structures exhibit passive horizontal adjustment. This treatment option requires the same number of implants as implant-supported fixed dentures. The horizontal assembly system prevents the prosthesis from loosening or moving in response to axial and non-axial forces. The technique was used to rehabilitate a patient presenting an atrophic upper maxilla, with the insertion of 8 implants. No complications were reported at follow-up 3, 6 and 12 months after fitting of the prosthesis. This system offers solutions to the clinical and laboratory complications associated with hybrid prostheses, concealing emergence of the chimneys and improving implant-prosthesis hygiene.
Objectives: We analyzed the correlation between physique and size of the dental arch of college students with normal occlusion. Methods: Ninety-nine college students filled out the prepared questionnaire. The length and width of the dental arch of the students was measured using a plaster model after taking an impression. The data were analyzed using the t-test and correlational analysis. Results: The maxilla arch length was 3.70 mm longer and the inter-molar width of the maxilla was 3.06 mm longer in men (both p<0.01) than in women. Additionally, the mandible was 3.66 mm longer in men as compared to those in the women (p<0.01). As height increased, there was increase in the body weight (α=0.683, p<0.01), maxilla arch length (α=0.373, p<0.01), molar width of the maxilla (α=0.214, p<0.05), and the mandible (α=0.280, p<0.01). The greater the weight increase, the greater the maxillary arch length increase (α=0.392, p<0.01), and the greater the molar width of the maxilla (α=0.336, p<0.01) and mandible (α=0.502, p<0.01) increase. Conclusions: As physical size (both height and weight) increased, the maxillary length and molar width also increased. Based on the results of this study, the cause of malocclusion, form and size of the dental arch, and stable occlusion can be used as basic data.
Activator is a removable functional appliance used for correcting the skeletal Class II malocclusion in children with the mandibular deficiency. Berlin standard activator modified from Andresen activator has following characters; do not cover the palatal surface for tongue space, relief on lingual surface of mandibular incisors and resin capping 1/3-1/2 of crown height on mandibular incisors for preventing labioversion of mandibular incisors, L-hook between maxillary lateral incisor and canine for anterior high pull headgear, relief on mandibular posterior bite block for differential eruption of posterior teeth. Two cases presented here had a mandibular deficiency and slight maxillary protrusion. First case (an 11-year-old girl) treated with Berlin standard activator and anterior high pull headgear for 13 months followed by fixed orthodontic appliance for another 29 months. Second case (a 12-year-old boy) treated with Berlin standard activator for 6 months followed by fixed appliance for another 24 months. Treatment results showed a significant improvement in sagittal skeletal and occlusal relationship without premolar extraction. Mandibular condyles were concentric in TMJ [ossa, and masticatory muscle activities were normalized after treatment. In the retention period facial harmony and occlusal stability was maintained.
Purpose: Surgical approaches to the condylar neck and subcondyle area can cause some morbidity such as, facial nerve injury, time-consuming nature and external scar etc. So many surgeons hesitate using open reduction and internal fixation for the treatment of subcondylar fractures. We report open reduction and internal fixation of subcondylar fractures in 13 adult patients via transmasseteric approach. Methods: From 2007 to 2009, 13 adults with subcondylar fracture of mandible were treated with open reduction and internal fixation via transmasseteric approach. A preauricular incision was extended downwards in a curvilinear fashion in the cervicomastoid skin crease. Skin flap was elevated above the SMAS layer. Masseter muscle was splitted at the anteroinferior edge of the parotid gland. After the fracture was reduced, fixed with appropriate plates and screws. All operation were performed under general anesthesia. Results: Mean follow-up period was 13.3 months. There were no signs and symptoms of facial nerve injury, difficulty in mouth opening, or malocclusion. Dissection time was roughly within 30 minutes. Conclusion: Transmasseteric open reduction and internal fixation of mandible subcondylar fracture can be performed with excellent visualization, and inconspicuous scar. It also offers swift access to the subcondylar area while substatially reducing the risk to the facial nerve and eliminating the complications associated with transparotid approaches.
The Twin Blocks technique was developed by Dr. William Clark of Scotland during the early 1980's. Twin Blocks are an uncomplicated system that incorporates the use of upper and lower bite blocks. These blocks reposition the mandible and redirect occlusal forces to achieve rapid correction of malocclusions. They are also comfortable and the patients wear them full-time-inducing eating time. Occlusal forces transmitted through the dentition provide a constant proprioceptive stimulus to influence the rate of growth and the trabecular structure of the supporting bone. The features of Twin Blocks mean easier and quicker treatment. The occlusal inclined plane is the fundamental functional mechanism of the natural dentition. Twin blocks are bite blocks that effectively modify the occlusal inclined plane to induce favorably directed occlusal forces by causing a functional mandibular displacement. Upper and lower bite blocks interlock at a $45^{\circ}$ angle and are designed for full-time wear to take advantage of all functional forces applied to the dentition including the forces of mastication. The patients who were treated with modified Twin Blocks, and following results were observed: 1. Large overjet and deep overbite were corrected. 2. Class II molar relationship was changed into Class I. 3. Labial inclination of upper incisors was corrected by adjustment of labial bow of upper bite block. 4. The profiles of two patients were improved by anterior displacement of mandible.
Purpose : The purpose of the present study was to evaluate postoperative stability and soft-tissue osseous relations of the long face syndrome patients. Methods : Twenty-five patients who had undergone bimaxillary surgery to correct long face syndrome at the Pusan National University Hospital were evaluated. The lateral cephalograms of preoperative, 1 week postoperative and at least 1 year postoperative were examined. Results : 1. The facial height of the long face syndrome patients were longer than normal Korean adults. 2. The most common malocclusion type of the long face syndrome patients in Korea was class III. 3. Horizontal postoperative skeletal relapses were $-0.64{\sim}0.80mm$ in the maxilla, and $-0.56{\sim}0.48mm$ in the mandible. 4. Vertical postoperative skeletal relapses were $0.20{\sim}0.56mm$ in the maxilla, and $-0.80{\sim}0.20mm$ in the mandible. 5. Postoperative soft tissue changes in long face syndrome patients were correlated with postoperative skeletal changes. So prediction schemes for postoperative soft-tissue changes were obtainable. Conclusion : It is hard to predict the exact direction and quantity of the postoperative skeletal relapse in long face syndrome patient's orthognathic surgery because of large standard deviation. But soft tissue change is predictable via prediction scheme.
