하악교정 수술환자의 술전후 음향음성학적 연구 (A comparative acoustic study before and after orthognathic surgery for correction of patients with Class III malocclusion)

  • 본 연구는 악교정 환자 중 9명의 골격성 3급 부정교합 환자를 대상으로 음향학적 분석을 통하여 술전, 술후에 나타나는 발음상의 차이를 살펴보았다. 술전, 술후간 모음 조음상의 차이는 술후 혀의 후방, 하방 이동과 동시에 목젖의 후상방 이동으로 인한 구강 내 조음 공간의 극대화로 요약할 수 있다. Long-term average 스펙트라 분석 결과, /ㅅ/의 조음상의 차이는 술전에서는 마찰소음 에너지가 전 주파수대에 걸쳐 고른 분포를 보이나 술후에는 정상인과 같이 고주파수대에 집중되는 경향을 보였다. 동시에 마찰소음의 peak frequency는 술후에서가 술전에 비해 높게 나타났다.

  • In adult patients, the corrections of their malocclusion may be more difficult and require longer treatment time due to thicker layer of cortical bone and reduced blood supply. Recently, various methods such as surgery, implant for anchorage and corticotomy have been tried to overcome these problems. Corticotomy is a surgical technique in which a fissure is made through the cortical bone that surrounds a tooth so that the tooth is embedded within a block of bone that is connected to adjacent blocks through only the medullary bone. Technique of corticotomy has been widely used for correction of maxillary transverse deficiency, but hasn't actively in other fields of orthodontics. We applied corticotomies in many types of orthodontic treatment and had satisfactory results. We suggested clinical application of corticotomy in adult orthodontics to reduce treatment period and to achieve better stability after orthodontic treatment.

Minimum Presurgical Orthodontic Treatment with Two Jaw Surgery Combined with Anterior Segmental Osteotmy in Skeletal Class II Malocclusion: A Case Report

  • This case report describes the treatment of a 23-year-old woman who had lip protrusion with gummy smile and mentalis muscle strain. Orthognathic surgery was performed in conjunction with orthodontics. Minimum dental decompensation was performed with presurgical orthodontics followed by an anterior segmental osteotomy for the majority of dental decompensation. Counterclockwise rotation of the maxillomandibular complex was applied by LeFort I osteotomy, and bilateral sagittal split ramus osteotomies with anterior segmental osteotomy to achieve overall facial balance. The active treatment period was 15 months. Stable occlusion and skeletal relationship were observed after a 10-month follow-up period.

Bioprogressive Therapy에 의한 Angle II급 1류 부정교합의 교정치험예 (Case Reports of Angle's Class II, Division 1 malocclusions treated by Bioprogressive Therapy)

  • Three patients who had Angle's Class II Division 1 malocclusion were treated by Bioprogressive therapy. In spite of their occlusions, the 3 patients did not have any skeletal problems. Their skeletal patterns were within normal range. So headgear or functional appliance therapy were not considered. During the treatment procedure, the most noteworthy results of Bioprogressive therapy were the effect of the Utility arch to intrude 4 mandibular anterior teeth, the effect of the Cuspid retractor in cuspid retraction and the effect of the Double delta retraction arch in the retraction of 4 anterior teeth. The whole treatment results in these cases which were achieved by Bioprogressive therapy were very favorable and the efficiency of this therapy was very excellent.

교정과 부정교합에 대한 지식에 관한 조사 (KNOWLEDGE OF ORTHODONTICS AND MALOCCLUSION)

  • Orthodontists want why the patients want orthodontic treatment. Demand and need for orthodontics is very important factors of cooperation and management. Chief complaint would be different according to the race and culture. Our results show that in the control group, Korean female adolescents[chief complaint is cowding$(30.6\%)$, spacing$(26.5\%)$, upper anterior protrusion$(18.4\%)$, anterior cross bite$(8.2\%)$ in order. Korean female adolescents considered anterior cross bite is more nonacceptable than spaeing and anterior protrusion. The purpose of orthodontics seemed to enhance esthetic, dental health, and to improve oral function.

  • This study was undertaken to find out the characteristic craniofacial morphology of Class II Division 2 malocclusions in children by means of roentgenocephalometry. The subjects consisted of twelve boys and thirteen girls with Class Ii, Division 2, thirty seven boys and fifty three girls with Class II, Division 1, and forty six boys and eighty one girls with normal occlusion, ranged from 10 years old to 18 years old. The following results were obtained; 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 2 malocclusion similar to the normal occlusion, but the mandible was the posterior position in th the cranial anatomy. 2. There were no significant differences in the anteroposterior relationship of the maxilla and the mandible between Class II, Division 1 and Class II, Division 2 malocclusions. 3. In Class II, Division 1 the axes of maxillary incisors showed labial inclination, but lingual inclination in Class II, Division 2 malocclusions. 4. Overbite was prominent one in Class II, Division 2, on the other hand overjet was distinguished in Class II, Division 1.

Total intrusion and distalization of the maxillary arch to improve smile esthetics

  • This case report illustrates the successful treatment of a patient with skeletal Class II malocclusion and an unesthetic smile involving excessive gingival display and large buccal corridors. By applying dual buccal interradicular miniscrews, total intrusion of the maxillary dentition along with distalization was induced to improve both the occlusion and smile esthetics. In addition to the conventional cephalometric superimposition, three-dimensional superimposition was performed and evaluated to validate the treatment outcome.

증령에 따른 치고졸의 변화에 관한 방사선학적 연구(I) (Radiographic Study of the Alveolar Bone Changes with Aging)

  • The author measured the degree of alveolar bone resorption around mandibular central incisors in Korean female(254). Mandibulr central incisors were selected from the females who do not have periodontal disease or malocclusion. The selected radiograms were enlarged in the $5"\times7"$ printing papers for the precise measuring. The obtained results were as follows : 1. The average alveolar bone resorption around mandibular central incisors in Korean women were 1.91mm in 3rd decade, 2, 16mm in 4th decade, 2.51mm in 5th deacade, 2.70mm in 6th decade, 2.94mm in older age group. 2. Alveolar bone resorption and age were in positive correlation; there is a tendency that the alveolar bone resorption increase with aging. 3. The regression equation is as follows. 국민학교 학동을 대상으로 한 직접 및 간접 구강보건교육의 효과평가에 관한 조사연구 (A Study on Appraisal for School Oral Health Education)

  • The authors have examimed 349 primary school children with questionare in order to appraise the oral health educational levels, one month later after performing oral health education as direct education and indirect education with video film, and compared the data from 350 uneducated children. The obtained results were as followings ; 1. It was estimated that the direct and indirect oral health education for school children were effective for in creasing the knowledge levels of oral health. 2. It was revealed that such items of oral health education as preventive measure for caries, tooth brushing method, etiology of dental caries and etiology of malocclusion were more effective for increasing the knowledge levels, compared to uneducated group. 3. Tooth Brushing Method should be educated in practical, not only to school children but also school teachers. 4. It should be established the goals and items for oral health education in practical as national level.

심신장애자의 치과질환 실태보고 (A Survey of the Oral Status in Handicapped)

  • The purpose of this study was to make a comprehensive study and provide information about the oral status of handicapped. 874 (male: 540, female: 334) persons who are housed by 12 institution aged from 4 to 28 years in Seoul area was examined by considering the DMF rate, periodontal condition and occlusion. The obtained results were as follows: 1. Cerebral palsy, poliomyelitis, mental retardation, and blindness groups had a significant higher DMF rate than that occurring in any of the remaining groups. 2. Prevalence of periodontal disease in blindness, mongolism, cerebral palsy, poliomyelitis groups showed higher than that of the remaining groups. 3. Mongolism group had a significantly higher incidence of malocclusion (69.23%).
