Outcome of Children with Severe Acute Malnutrition and Diarrhea: a Cohort Study

  • Purpose: Severe acute malnutrition (SAM) is an important public health problem which contributes to significant number of under five deaths. Protocol based management significantly decreases risk of deaths in children with medical complications. Methods: Outcome of children aged 2 months-5 years admitted and fulfilling definition of SAM having diarrhea (group A) was compared to children with SAM having medical complications other than diarrhea (group B). Both groups were managed according to standard recommended protocols and monitored and followed up for 12 weeks after discharge. Results: The average weight gain, defaulter rate, primary failure, secondary relapse rate and readmission rate were similar in both groups. Length of stay in group A was three days longer (p-value=0.039). Discharge rate was comparable with overall 68% of children successfully discharged and 50% of children reaching weight/height > -2 standard deviation at follow-up of 12 weeks. Conclusion: The current management protocol is equally effective for managing children with SAM having diarrhea. Good adherence to management protocol of dehydration and timely modification of therapeutic feeds in children with persistent diarrhea results in satisfactory weight gain.

입원초기 영양불량 환자의 TPN 지침에 따른 영양개선 평가 (Evaluation of Nutritional Improvement by Total Parenteral Nutrition Guideline in Early Malnourished Inpatients)

  • Background: Malnutrition of inpatients has been associated with higher morbidity, mortality, cost, and longer hospital stay. Total parenteral nutrition (TPN) therapy plays an important role in decreasing morbidity and mortality among critical inpatients in hospitals, and has been commonly used to improve clinical outcomes. However, only a few studies were conducted regarding patients' nutritional improvement by TPN. Method: This study therefore evaluated the changes in nutritional parameters by TPN therapy for early malnourished inpatients. Data from early malnourished inpatients who were treated with TPN therapy between January 2012 and June 2013 at the ${\bigcirc}{\bigcirc}$ university Hospital were studied retrospectively. Information regarding sex, age, underlying diseases, division, TPN (peripheral and central), and changes in nutritional parameters were collected by reviewing electronic medical records. The criteria for evaluation of the changes in nutritional parameters were included physical marker, body mass index (BMI), and biochemical markers, including albumin (Alb), total lymphocyte count (TLC), and cholesterol. Nutritional parameters were collected three times: pre-TPN, mid-TPN and end-TPN. A total of 149 patients (peripheral, 97; central, 52) was evaluated. Results: In all patients, the malnutrition number was significantly decreased following the complete TPN therapy (peripheral patients, pre-TPN: $3.33{\pm}0.12$, mid-TPN : $3.06{\pm}0.17$, and end-TPN: $2.85{\pm}0.21$ (p < 0.05); central patients, pre-TPN: $3.38{\pm}0.11$, mid-TPN: $3.06{\pm}0.13$, and end-TPN: $2.75{\pm}0.21$ (p < 0.05). The malnutrition number means number of nutrition parameters below normal range of malnutrition. In addition, all of the four nutritional parameters (BMI, Alb, TLC and cholesterol) were increased with duration of TPN periods for all patients, and the changes in the early stage were larger than in the late stage (p < 0.05). The nutritional parameters of non-cancer patients were increased to a greater extent compared to cancer patients with longer TPN therapy, but it was not significant. The nutritional parameters of younger patients (50-60 years) were also increased more than of older patients (70-80 years), but it was not significant. Conclusion: In conclusion, the TPN therapy decreases malnutritional status and improves nutritional parameters in malnourished patients, thereby decreasing morbidity and mortality. The combined evaluation of all four nutritional parameters is more accurate for nutritional assessment than a single one.

영양관리과정에 근거한 영양중재가 노인 영양불량 입원환자의 식사섭취량 증진에 미치는 효과 (Effect of nutrition care process-based nutrition intervention on improvement of intake in the elderly in-patients with malnutrition)

  • 본 연구는 대구광역시 소재 종합병원에 입원한 노인 영양불량환자의 병원식사 섭취량 증진을 위한 영양중재의 효과를 분석하고자 하였으며, 이를 통하여 입원한 노인 영양불량환자의 영양관리 방안에 대한 기초자료를 제공하고자 하였다. 본 연구에서는 병원식사 섭취량 감소의 원인을 파악하여 각 원인에 따라 개별적인 영양중재를 실시하였다. 그 결과 연구대상자의 에너지 및 단백질 섭취량을 유의하게 개선시켰고 단백질의 INQ를 높였다. 또한 단백질, 철, 비타민 $B_2$의 NAR이 개선되고 MAR도 유의적인 증가를 보였다. 따라서 본 연구는 환자의 식사섭취량 부족의 원인을 정확하게 진단하고 영양진단에 따른 개별적인 영양중재를 통하여 환자의 식사섭취 상태를 개선시킬 수 있었다는 점에서 의의가 있다고 여겨진다.

Nationwide "Pediatric Nutrition Day" survey on the nutritional status of hospitalized children in South Korea

  • BACKGROUND/OBJECTIVES: To evaluate the nutritional status and prevalence of malnutrition in hospitalized children at admission and during hospitalization in South Korea. SUBJECTS/METHODS: This first cross-sectional nationwide "Pediatric Nutrition Day (pNday)" survey was conducted among 872 hospitalized children (504 boys, 368 girls; 686 medical, 186 surgical) from 23 hospitals in South Korea. Malnutrition risk was screened using the Pediatric Yorkhill Malnutrition Score (PYMS) and the Screening Tool Risk on Nutritional status and Growth. Nutritional status was assessed by z-scores of weight-for-age for underweight, weight-for-height for wasting, and height-for-age for stunting as well as laboratory tests. RESULTS: At admission, of the 872 hospitalized children, 17.2% were underweight, and the prevalence of wasting and stunting was 20.2% and 17.3%, respectively. During hospitalization till pNday, 10.8% and 19.6% experienced weight loss and decreased oral intake, respectively. During the aforementioned period, fasting was more prevalent in surgical patients (7.5%) than in medical patients (1.6%) (P < 0.001). According to the PYMS, 34.3% and 30% of the children at admission and on pNday, respectively, had a high-risk of malnutrition, requiring consultation with the nutritional support team (NST). However, only 4% were actually referred to the NST during hospitalization. CONCLUSIONS: Malnutrition was prevalent at admission and during hospitalization in pediatric patients, with many children experiencing weight loss and poor oral intake. To improve the nutritional status of hospitalized children, it is important to screen and identify all children at risk of malnutrition and refer malnourished patients to the multidisciplinary NST for proper nutritional interventions.

일 지역 독거 노인의 간호요구에 영향을 미치는 요인 (Factors Influencing Health Care Needs of Solitude Elderly)

  • Purpose: This study was to identify factors that influence the health care needs of that over 65 years of age in Mokpo, Korea. Method: The data was collected from June 2002 to September 2002. The subjects were 120 homebound solitude elderly(age=76.8). Subjects were interviewed with structured questionnaire in order to identify the health care needs, health variables (perceived health status, risk of malnutrition, K-IADL), psychological variables(self-esteem, depression) and demographic variables. physiological health variables (height, weight, blood pressure, pulse, blood sugar)were assessed after the interview. Result: In general perceived health status was poor, risk of malnutrition was high, number of disease was 3 disease, self-esteem was low but depression was high and health care needs were relatively high. Among the elderly education & counseling needs topped the list. In regression analysis, health care needs were significantly influenced by IADL(23%), duration of solitudes(4%), sex(3%), and education(l%). These variables explained 31% of the variances in health care needs. Conclusion: The result identified that health care needs should be a considered in IADL, female, duration of solitudes and education for the solitude elderly.

Effects of an Anabolic Steroid, Nandrolone Phenylpropionate, on Reductions in Body and Muscle Proteins Under the Dietary Regimens of Feeding a Low-Protein Diet and of 50% Food Restriction in Rats

  • The aim of the present investigation was to see whether an anabolic steroid, nandrolone phenylpropionate (NPP), exerts protienanabolic effects under such adverse nutritional conditions as protein deficiency and protein-energy malnutrition in male rats. feeding on a low-protein (8% casein) diet resulted in a marked reduction in body weight gain that was associated with reductions in body protein and protein content of gastrocnemius muscle. Administration of NPP (4 mg/kg body weight) did not alter muscle and body protein depletion induced by a low-protein diet. 50% food restriction caused reductions in body protein and in protein content of gastrocnemius muscle. These reductions were partially prevented by NPP (4 mg/kg body weight). Food restriction did not affect plasma concentration of corticosterone, insulin, or tetosterone plus dihydrotestosterone. On the other hand, neither plasma concentration of corticosterone nor insulin were affected by NPP. The present results show that anabolic steroids do not express anabolic effects under conditions of protein deficiency, but in protein-energy malnutrition, anabolic steroids exert their anabolic effects even in male rats.

만성경증의 영양부족과 인지능력과의 관련성에 대한 연구 : 학령기 아동 경우 (Marginal Malnutrition and Cognitive Performance in School-Aged Children)

  • As part of the Nutrition CRSP Mexico project conducted in rural villages in highland Mexico, this study examined whether nutritional factors, family characteristics, and the duration of schooling were associated with cognitive performances in 107 schoolers(53 boys and 54 girls) aged 8~10. Food intake was measured by recall, direct observation and weighment twice monthly over the course of a year. Families were characterized in terms of socioeconomic status and parental cognitive performance, education and aspiration. Dietary quality, but not energy intake, and anthropometry (wt-for-age, ht-for-age, head circumference) were significantly associated with cognitive test results for both verbal and performance measures. When the effect of SES is controlled using partial correlation, the relationships were substantially weakened. Sex differences were also found in response to nutritional deprivation. Among boys, dietary quality was the most important indicator of cognitive performance, while household economic conditions were more importantly associated with cognitive performance among girls. Participation in schooling was important for cognitive skille as it related to nutritional status.

가미보중익기탕(加味補中益氣湯)과 자궁하수(子宮下垂)의 활용(活用)에 관한 고찰(考察) (Literary Study on the BojungiggiTang and prolapse)

  • Korea has recently seen reduction in Gihyeolbujog(氣血不足)-related woman disease due to low birth rate and elevated nutritional intake. In particular Uterine Prolapse usually result from malnutrition in days of need and also from earlier resumption of social activity after childbirth. Nowadays some women still develop Uterine Prolapse, of which the reasons are earlier resumption of social activity after childbirth, malnutrition due to harsh diet, contaminated food, and Gihyeolbujog by stress. This research has determined that GamibojungiggiTang would be effective for the treatment of those diseases. This research found that the combination of BojungiggiTang with Sugjihwang(熟地黃)and Noggaggyo(鹿角膠), which were used for Sinsubujog(腎水不足) and IghyeolbojeongJe(益血保定劑). Respectively, produced satisfactory treatment outcomes for prolapse.
