Objectives This study aims to comprehensively review research utilizing ultrasonography for assessing manipulative treatment on spinal diseases, with the goal of promoting the wider integration of ultrasound imaging into clinical practice. Methods A systematic search was conducted on three international databases (Embase, PubMed, Cochrane) up to July 23, 2023. The search included key terms such as ultrasonography, manipulation, and skeletal muscle. The inclusion criteria narrowed down the selection to studies specifically related to lumbar and cervical vertebrae. Results Eleven studies were included in the review, with 10 focusing on lumbar vertebrae and one on cervical vertebrae, all employing spinal manipulation treatment. Among the 11 selected studies, nine primarily focused on ultrasound imaging to measure muscle thickness, while two utilized shear wave elastography to assess muscle stiffness. Also, rigorous measures were taken to ensure the reliability of the ultrasonography data. Conclusions This scoping review highlights the limited but growing evidence supporting the use of ultrasonography to assess manipulative treatment for spinal diseases. Despite a scarcity of studies in South Korea, it is crucial to recognize the potential of ultrasonography in becoming a widely used and practical tool for evaluating the effectiveness of manipulative treatments in the near future.

The purpose of this study is to analyze, durability of motor effort after finishing rehabilitation program and the influence from the isotonic lumbar region extension exercise after an operation according to the different operation way in invasive degrees. We selected randomly 80 patients who have no complication and musculoskeletal system diseases with finishing the 12 weeks' rehabilitation program after getting laser discectomy, but fail to conservative treatment, about Lumbar HNP, and divided into minimal invasive groups 38 and invasive groups 42 for study. As the results, in minimal invasive group, after finishing cure and 6 months later, the degree of hold muscle of women is much better than that of men. but in Invasive group, that of men is better than that of women, in case of women, the $0^{\circ}$ and $72^{\circ}$ in lumbar flexion angle have weaken or no improvement than before beginning exercise. and muscle force in $72^{\circ}$ in lumbar flexion angle increased to all men and women than before beginning exercise in Minimal invasive group. but in invasive group. it decreased. When analyze the lapsed time in Minimal invasive group, the groups who start the exercise within 3 months are better than the groups who start the exercise above 3 months after an operation the hold muscle degree all angles after finishing cure and 6 months later. Meanwhile, in invasive group, the groups who start the exercise within 3 months are better than the groups who start the exercise above 3 months after an operation about the hold muscle degree in 36-72 of lumbar flexion angle but 0-36 are not good after finishing cure and 6 months later. When analyze BMI in Minimal invasive group, the normal groups are better than the overweight groups about hold muscle degree in all angles after finishing cure and 6 months later.
Objective : To broaden our understanding on occupational disease of market workers and to evaluate the effect of TKM treatment focusing on acupuncture and herb medicine and to promote base studies and clinical trials on occupational disease. Materials and Methods : Analysis was done on 33 Ga-Rak market workers' chart which had been selected through investigation of 1508 outpatient's chart of Kyung Won University Hospital from Jun. 1st, 2002 to May. 31th, 2003. Results : 1. Out of 33 patients, Men had more occupational diseases than women had and Most people were in their forties. 2. Major cause of the disease include repetitve bending, heavy weight lifting and overwork. 3. Past History of patients mostly include frequent lumbar sprain, periarthritis of shoulder, lumbar HNP 4. The patients with occupational disease were diagnosed as lumbar sprain, periarthritis of shoulder, lumbar HNP, degenerative spondylosis, spinal stenosis and their chief complaints were low back pain, omalgia, back pain with radicular pain. 5. The duration of treatment was mostly within a week. 6. Acupuncture, Bee Venom Acupuncture, moxibustion, Herb-medicine, extract, taping therapy, physical therapy were used as treatment methods. 7. Applied herb medicine were composed of 8 kinds of prescriptions and extract were made up of 6 prescriptions 8. Applied acupoints belonged mainly to 14 meridians 9. The treatment of herb medicine combined with acupuncture proved effective in treating the ocupational disease. Conclusion : The occupational disease of patients working in Ga-Rak market were closely related with overwork, especially with repetitive bending and heavy weight lifting and TKM treatment focusing on acupuncture combined with herb medicine was effective in treating occupational disease.

Objective : The authors investigated the efficacy of the calcium sulfate(OsteoSet$^{(R)}$ pellets) as an autograft extender when used to perform posterolateral lumbar fusions. Patients and Methods : Twenty patients who underwent lumbar posterolateral arthrodesis for various spinal diseases between October 1999 and March 2000 were evaluated. Arthrodesis was performed by transpedicular screw fixation and bone grafting with a mixture of autograft + calcium sulfate in a 1 : 1 ratio. At time intervals of 1, 2, 3, and 6 months, postoperative radiographs were obtained to review the resorption of calcium sulfate and the evidence of fusion. A modified Lenke scale was used to assess the status of the fusion. Results : At 2 months after operation, the average modified Lenke scale score for the OsteoSet$^{(R)}$ pellets group was 3.8. However at 6 months after operation, the average modified Lenke scale score for the OsteoSet$^{(R)}$ pellets group was 1.8. Resorption of calcium sulfate pellets was revealed in all cases at 6 months after surgery. Conclusion : It is presumed that a combination of calcium sulfate and autograft can play a role as an effective autograft extender in the posterolateral spinal fusion.
Objective : According to the recent development of minimally invasive spinal surgery, direct lumbar interbody fusion (DLIF) was introduced as an effective option to treat lumbar degenerative diseases. However, comprehensive results of DLIF have not been reported in Korea yet. The object of this study is to summarize radiological and clinical outcomes of our DLIF experience. Methods : We performed DLIF for 130 patients from May 2011 to June 2013. Among them, 90 patients, who could be followed up for more than 6 months, were analyzed retrospectively. Clinical outcomes were compared using visual analog scale (VAS) score and Oswestry Disability Index (ODI). Bilateral foramen areas, disc height, segmental coronal and sagittal angle, and regional sagittal angle were measured. Additionally, fusion rate was assessed. Results : A total of 90 patients, 116 levels, were underwent DLIF. The VAS and ODI improved statistically significant after surgery. All the approaches for DLIF were done on the left side. The left and right side foramen area changed from $99.5mm^2$ and $102.9mm^2$ to $159.2mm^2$ and $151.2mm^2$ postoperatively (p<0.001). Pre- and postoperative segmental coronal and sagittal angles changed statistically significant from $4.1^{\circ}$ and $9.9^{\circ}$ to $1.1^{\circ}$ and $11.1^{\circ}$. Fusion rates of 6 and 12 months were 60.9% and 87.8%. Complications occurred in 17 patients (18.9%). However, most of the complications were resolved within 2 months. Conclusion : DLIF is not only effective for indirect decompression and deformity correction but also shows satisfactory mechanical stability and fusion rate.

The use of computers has increased greatly, and services have been developed very much for the convenience of users. As the use of computers increases, human activity has declined, and symptoms of deterioration in health are increasing. As the time to sit and live increased, humans are more exposed to diseases of low back pain, and low back pain includes pain related to the lower back centering on the herniated disc. Most of the back pain is caused by abnormalities in the muscles and ligaments that support the spine and trunk. In this paper, we present an overall design for developing an app that can obtain lumbar exercise effects to reduce low back pain or help treatment. In order to use this effectively, we propose a plan to obtain lumbar exercise effect, and aim to present an app design that can help effectively treat low back pain by using graphic-based guidance points.
In this study, a 3D printed assistive device was applied to X-ray examinations to diagnose human diseases. Based on the results of evaluating the device, statistical and regression analyses were conducted to evaluate its clinical utility and purchase intention, respectively. In the experiment, 90 radiologists performed X-ray examinations on patients who agreed with the use of the assistive device in oblique view X-rays of the lumbar spine, and then statistical analyses were undertaken with a traditional aid and factor analysis. The non-standardized coefficient values of the multiple regression analysis performed by setting the purchase intention of the 3D printed device as the dependent variable and the 3D printed device and traditional aid calculated by factor analysis as independent variables were 0.893 (p<0.001) and 0.269 (p<0.001), indicating statistically significant results. The results show that the 3D printed assistive device proposed in this study has higher clinical utility than traditional aids used in oblique view X-rays of the lumbar spine.
Background: Acupotomy is a type of acupuncture where a scalpel-shaped needle (miniscalpel needle) is used instead of a normal acupuncture needle to exfoliate adhesion sites or to relax entrapped regions. This study aimed to identify the descriptive characteristics of patients who received acupotomy treatment at a single Korean Medicine Clinic. Methods: This retrospective review analyzed the medical charts of patients who had received acupotomy at least once from August 2017 to December 2019 at a single Korean Medicine Clinic. The demographic characteristics, chief complaints, acupotomy treatment sites, and principal diagnosis codes were analyzed. Results: We identified 551 outpatients; the average age was 52 ± 14.26 years and 49.9% were male. The patients underwent an average of 8.47 sessions of acupotomy. Altogether, 35.91% of the acupotomy treatments were administered to the spinal regions, of which 60.01% were in the lumbar region. The codes related to the lumbar spinal condition/disease which were used most frequently. The chief complaints were dizziness, lumbar spinal stenosis, and Dupuytren's contracture in patients over 60 years of age. Conclusion: This is the 1st analysis of acupotomy treatment patterns in Korea to date. Acupotomy is primarily administered in the treatment of spinal conditions/diseases, especially for those involving the lumbar region. Future studies are necessary to determine the clinical outcomes of patients who receive acupotomy treatment and the safety of this treatment.

Most degeneating diseases and back pains in the orthopaedic disease are originated from the unbalance of stress distribution in the low-lumbar spine. Therefore the stress analysis of lowerback is indispensible to the clinical diagnosis for the developing reason and the developing process of diseases. Therefore the same model materials as following are eveloped to analyze the stress distributions of lower-back by photoelastic experiment. The verterbral body and the process are molded from epoxy resin(the weight ratio of Araldite and hardner is 10 to 3), models are geometrically identical to them in vivo respectively and the ratio of their elastic modulus to that of model material is 1 to 10. It is assured that KE-1300 Silicon(E=0.8MPa), TSE-3562 Silicon(0.5MPa) and the composite silicon(3MPa) (the weight ratio of KE-1300 silicon and Jioreal : 10 to 4) are respectively effective as the model materials of ligament, musles and intervertevral disc which is essential to the movement of low-lumbar spine. All the elements associated with the movement of the low-lumbar spine are molded through the molding method developed in this research and assembled with the angles between the verterbra and the disc in the normal human lumbosacral spine. The stress distributions of the assembled model are analyzed by photoelastic experiment. It is certified by comparing the results of photoelastic experimebt with the clinical situations that the loading dveice and the loading conceptions used in this paper are effective.
Objectives : The aim of this study was to investigate clinical features of the patients admitted to the department of acupuncture and moxibustion in an oriental medicine hospital in order to provide clinical preliminary data for advancement of acupuncture and moxibustion Methods : Total 526 patients were admitted to the department of Acupuncture and Moxibustion in Dunsan Oriental Hospital of Daejeon University from September 1st, 2011 to August 31st, 2012. Of these patients 11 were discharged on the day that they were admitted and the remaining 526 patients who were hospitalized for over 2 days were investigated this study. We analyzed inpatients by discharge progress notes and medical records, using PASW statistics 18.0 for statistical analysis. Results : 1. There were 361 females(62.9%) and 213 males(37.1%). The age distribution was skewed to 40s and 50s and the average age of the patients was 42.91 years. 2. The prevalence of the diseases groups was in the order of musculoskeletal disease, neuromuscular disease, neuro-psychological disease, and EENT disease. The musculoskeletal disease and neuromuscular disease together made up 92% of all diseases. 3. The prevalence of individual diagnosis was in the order of Bell's palsy, cervical sprain, lumbar sprain, herniated lumbar disc, herniated cervical disc, and lumbar spinal stenosis. 4. There were 261 patients with single diagnosis and 193 with two and 120 with three or more diagnoses. The days of hospitalization significantly increased with the number of diseases the patients were diagnosed with. Conclusions : With high concentration of patients seen in certain disease groups, continuous efforts needed to expand the usage of acupuncture and moxibustion treatments to other disease groups and to explore and develop other diverse treatment methods for better outcome of the disease treated.
