Early Treatment of a Class II Malocclusion with the Trainer for Kids (T4K): A Case Report (Class II 부정교합환자의 Trainer for Kids(T4K)를 이용한 조기치료 : 증례보고)

  • $T4K^{TM}$(Myofunctional Research Co, Australia) is one of the myofunctional appliance developed to be used in children of mixed dentition. Myofuncitonal appliance stimulate the facial, masticatory and tongue muscle and help to balance the muscular force. Labial bow included in the device exerts strength in excessively labial inclineded upper jaw, Lip bumper blocks strength of the mouth to prevent abnormal strength exerted in lower jaw, Tongue tag secures proper position of tongue, and additional exercise is not required for child patients. For the more, simpler design and softer texture of device prmoted cooperation of patients during use. This case report is to present the satisfactory results gained by using $T4K^{TM}$ on Class II patients. Comment 1. $T4K^{TM}$ was applied in Class II malocclusion patients of mixed dentition with expected space insufficient to gain facial improvement. 2. Excessive overjet, overbite were improved. 3. Main effects are regarded to have been achieved by development of lingual slant of upper jaw, labial slant of lower jaw, and lower part of jawbone. 4. Bad habits, such as mouth breathing, can also be adjusted.

Partial Denture Prosthesis Implant and Necessity Thereof in Korean Elderly : Analysis of the Data from the 5th National Health Nutrition Survey(2010-2012) (한국노인의 가공의치 보철장착실태 및 필요도: 제5기 국민건강영양조사자료 분석(2010-2012))

  • The purpose of this study was to provide the data for discussion related to oral health promotion policies for the elderly by examining and analyzing the prosthesis conditions and necessity for the fixed and removable dentures among the Korean elderly. The data, obtained from 4,557 elderly aged at 65 or higher who were targeted for the 5th National Health Nutrition Survey, were analyzed through the complex sample frequency analysis, complex sample cross analysis, and complex sample logistic regression analysis. The results of analysis showed significant relevance of whole denture implant and the necessity thereof in older subjects, rural community, and subjects with lower education background, regarding the state of upper jaw/lower jaw prosthesis and the necessity for upper jaw/lower jaw fixed partial denture/whole denture. In addition, the necessity for prosthesis implant was found to have correlation with the income and subjective health condition, while the necessity for artificial teeth(denture) was found to have correlation with the gender, age, education, and subjective oral health condition. Therefore, it is considered necessary to map out the prevention and treatment policies designed to help maintain and promote oral health based on oral health education, along with the policies that aim to recover the neutralized oral health functions, in relation to the oral health of the elderly.

The Association between Facial Morphology and Cold Pattern

  • Objectives: Facial diagnosis is an important part of clinical diagnosis in traditional East Asian Medicine. In this paper, using a fully automated facial shape analysis system, we show that facial morphological features are associated with cold pattern. Methods: The facial morphological features calculated from 68 facial landmarks included the angles, areas, and distances between the landmark points of each part of the face. Cold pattern severity was determined using a questionnaire and the cold pattern scores (CPS) were used for analysis. The association between facial features and CPS was calculated using Pearson's correlation coefficient and partial correlation coefficients. Results: The upper chin width and the lower chin width were negatively associated with CPS. The distance from the center point to the middle jaw and the distance from the center point to the lower jaw were negatively associated with CPS. The angle of the face outline near the ear and the angle of the chin line were positively associated with CPS. The area of the upper part of the face and the area of the face except the sensory organs were negatively associated with CPS. The number of facial morphological features that exhibited a statistically significant correlation with CPS was 37 (unadjusted). Conclusions: In this study of a Korean population, subjects with a high CPS had a more pointed chin, longer face, more angular jaw, higher eyes, and more upward corners of the mouth, and their facial sensory organs were relatively widespread.

  • The patient, 21 years and 3 months female, complained of protrusion of lower face. There was severe procumbency of upper & lower anterior teeth. Cephalometric analysis revealed that the anteroposterior jaw relationship was normal, but the teeth was foreward on their respective basal bones, so diagnosed as bimaxillary dental prognathism. The patient underwent extraction of four Ist premolar and was treated with multibanded & direct bonding system. After 14 months, She gained good interdigitation of buccal segment and attractive facial profile.

Evaluation of Skeletal Stability Following Two-jaw Surgery via Surgery First Orthodontic Treatment in Class III Malocclusion (III급 부정교합에서 선수술 교정치료를 통한 양악 수술 후 안정성)

  • Purpose: The purpose of the present study was to evaluate the postoperative skeletal stability of two-jaw surgery (Le Fort I osteotomy and bilateral sagittal split ramus osteotomy) via surgery first orthodontic treatment (SFOT) in class III malocclusion. Methods: Thirty-two patients who had two-jaw surgery via SFOT were included in this study. Serial lateral cephalograms were obtained before (T0), immediately after (T1), and six months after (T2) surgery. Twelve variables were measured for horizontal and vertical skeletal stability as well as for dental change. All measurements were evaluated statistically by a paired t-test ($P$ <0.05). Results: The mean skeletal changes were $0.1{\pm}2.5$ mm at point A and $-12.0{\pm}7.4$ mm at the pogonion. The mean horizontal relapse was 11.6% at the pogonion, and the mean vertical surgical changes included an upward displacement of $2.1{\pm}7.1$ mm and a forward displacement of $1.4{\pm}4.6$ mm at the pogonion. Upper incisor inclination decreased after surgery and was maintained at T2, and lower incisors were proclined from T1 to T2 by postsurgical orthodontic treatment. Conclusion: Postoperative skeletal stability of two-jaw surgery via surgery first orthodontic treatment in class III malocclusion was clinically acceptable.

Effects of Bee Venom Herb-Acupuncture on the Jaw Opening Reflex in Rats (흰쥐에서 합곡혈(合谷穴) 봉독약침자극(蜂毒藥針刺戟)에 의(依)한 개구반사(開口反射)의 반응(反應))

  • The purpose of this study is to investigate the effects of Bee Venom Herb-Acupuncture on the jaw opening reflex evoked by tooth pulp stimulation. Rats were anesthetized with thiopental sodium given intraperitoneally in an initial dose of 80mg/kg. Maintenance doses of 5mg/kg thiopental sodium were given through a cannular in the femoral vein as required to maintain light anesthesia. To apply noxious stimuli, a pair of enameled wires were inserted into the tooth pulp of the lower incisor. The effects of conditioning stimuli were estimated as an indicator of the degree of suppression of the digastric muscle electromyogram(dEMG) in the jaw opening reflex. Bee Venom Herb- Acupuncture(0.2% solution 0.1ml/rat) was injected to Hapgok(LI4) loci. In addition, Normal Saline (0.1ml/rat) was injected to Hapgok loci so as to compare the degree of suppression elicited from Bee-Venom. By administration of Bee Venom Herb-Acupuncture, the amplitude of dEMG was maximally suppressed to $67.5{\pm}3.38%$ ipsilaterally, 73.33{\pm}8.00%$ contralaterally. Generally, the dEMG activities caused by electrical stimulation were gradually suppressed during the stimulation and maximal suppressive effect showed at 15min after its onset. However the dEMG activities by Be Venom Herb-Acupuncture were immediately suppressed after its onset and the suppressive effect continued for a long time compared to electrical stimulation. In conclusion, Bee Venom Herb-Acupuncture may have a different mechanism of analgesia from that of electro-acupuncture and contribute to the modulation of pain analgesia.

Gender Differences in Pressure Pain Thresholds during Sustained Jaw Muscle Contraction

  • Purpose: To determine whether a fatiguing clench significantly affects the changes in pressure pain threshold (PPT) in men compared to women. Methods: The changes of PPTs from before to after a sustained clench in 12 men and 12 women were obtained. We used a decrease in median frequencies of surface electromyography (EMG) power spectra from the start to the end of the sustained clench as evidence of fatigue. Endurance time for the clench was used as a covariate. Results: The median frequencies decreased after the clench in both the anterior temporalis and masseter muscles, did not differ with the muscle or the gender of the subjects, and none of the interaction terms were significant. The PPTs were lower for women for both muscles, were decreased after the sustained clench, but failed to show the hypothesized gender by time interaction. Conclusions: Our results show that women have lower PPTs than men, but do not respond differently than men to jaw muscle fatigue.

First Record of the Black-barred Halfbeak, Hemiramphus far (Beloniformes: Hemiramphidae) from Jeju Island, Korea (제주도 북부 연안에서 출현한 학공치과 한국미기록종, Hemiramphus far)

  • Based on a single specimen collected from the northern coastal waters of Jeju Island, we described Hemiramphus far as the first record from Korea. The specimen shows a elongated body with a prolonged lower jaw, 13 dorsal fin rays, 11 anal fin rays, 36 predorsal scales, and silvery white body with five black bars laterally and yellowish color on posterior part of lower jaw, tips of dorsal fin and upper caudal lobe. "Geom-mu-neui-hag-gong-chi" is proposed as a new Korean name for the species.

한국인의 치경부마모증 경험도에 관한 조사연구

  • In order to collect some necessary data to promote correcting the toothbrushing technic on the Korean public, the author had examined the number of present teeth on which had cervical abrasion and the most basic home dental care in 2,000 Korean male and female adults from 19 to 52 years in the age. Then, the cervical abrasion experience rate and the cervical abrasion experience teeth rate were calculated and evaluated. The obtained results were as follows: 1. Cervical abrasion experience rate was 32.45% 2. Cervical abrasion experience rate was gradually increased by ageing. 3. Cervical abrasion experience rate in male adults was higher than that in female adults. 4. Cervical abrasion experience rate in the lower jaw was higher than that in the upper jaw. 5. Cervical abrasion experience rate of the teeth at the right side was higher than that of the teeth at the left side. 6. Cervical abrasion experience teeth rate was 3.82%. 7. Cervical abrasion experience rate was highest on the first bicuspid, and followed in the sequence of the second bicuspid, canine, first molar, incisors, and 2nd and 3rd molars.
