Surgical Correction of Atrial Septal Defect in Adult (성인 심방중격결손증의 수술교정)

  • Repairs of atrial septal defect utilizing cardiopulmonary bypass were performed in 50 adults, ranging age from 16 to 53 years, since April 1986 up to October 1994. They occupied 38.8% of all adult congenital heart disease operated in the same period. Preoperatively, 16 patients were functional class II, 12 patients class III and 4 patients class IV[New York Heart Association Classification , respectively. Five patients combined with atrial fibrillation and the remainders revealed regular sinus rhythm. Cardiac catheterizations were performed in 43 out of 50 patients, and revealed a systolic pulmonary arterial pressure in excess of 51 mmHg in 4 patients but none had reversed shunt. 40 patients[80% were repaired with patch closure and remainings were repaired with direct closure. 49 patients were followed up for 2 months up to 102 months[average 55 months . A comparison of the preoperative and postoperative functional class demonstrated a mean decrease of one NYHA functional level[2.5$\pm$0.63 to 1.4$\pm$0.56 . There was no operative mortality. One patient died during the follow-up period and the death was unrelated to heart disease. Operative treatment is indicated for repair of atrial septal defect with left to right shunt in the adult patient and a considerable clinical improvement can be anticipitated with low mortality.

Mesoscale Features and Forecasting Guidance of Heavy Rain Types over the Korean Peninsula (한반도 호우유형의 중규모 특성 및 예보 가이던스)

  • This study classified heavy rain types from K-means clustering for the hourly relationship between rainfall intensity and cloud top height over the Korean peninsula, and then examined their statistical characteristics for the period of June~August 2013~2018. Total rainfall amount of warm-type events was 2.65 times larger than that of the cold-type, whereas the lightning frequency divided by total rainfall for the warm-type was only 46% of the cold-type. Typical cold-type cases exhibited high cloud top height around 16 km, large reflectivity in the upper layer, and frequent lightning flashes under convectively unstable condition. Phenomenally, the cold-type cases corresponded to cloud cluster or multi-cell thunderstorms. However, two warm-type cases related to Changma and typhoon were characterized by heavy rainfall due to long duration, relatively low cloud top height and upper-level reflectivity, and the absence of lightning under the convectively neutral and extremely humid conditions. This study further confirmed that the forecast skill of rainfall could be improved by applying correction factor with the overestimation for cold-type and underestimation for warm-type cases in the Local Data Assimilation and Prediction System (LDAPS) operational model (e.g., BIAS score was improved by 5%).

  • As the existing noise measuring device was affected the flow-field and structural vibration directly, new experimental device was required. Two Hydrophone Boxes are designed and their performances are investigated. The noise level of the KRISO cavitation tunnel is compared with those of the other cavitation tunnels which have been designed for the noise study. The present experimental results show the possibility of the full-scale prediction for propeller cavitation noise and the improvement of the measurement performance at the range of low-frequency.

Pattern and Management of Dyslipidemia in Type 2 Diabetes Patients in Korea (제 2형 당뇨환자에서 지질이상 유형 및 관리)

  • Dyslipidemia is an important CHD risk factor in diabetic patients. We conducted this study to assess the pattern of dyslipidemia in type 2 diabetes patients, to examine the demographic and clinical factors associated with dyslipidemia and to evaluate attaining within the lipid target goals and treatment strategies. A retrospective analysis was conducted among patents diagnosed type 2 diabetes at outpatient clinic in endocrinology between January 2003 and December 2004. Clinical history and physical examination were reviewed and laboratory data including blood glucose, HbAlc, lipid levels were recorded sequentially at least 1 year. In 882 patients with type 2 diabetes, 437 patients (49.6%) have dyslipidemia and 73% of them (319 patients) received lipid-lowering agents. 244 patients (94 males, 150 females, mean age 60 years old) were susceptible to analyses. The most frequent pattern of dyslipidemia is high LDL level and high TG levels (28%). Metabolic syndrome and macrovascular complication were significant negative independent association with lipid levels within the target goals (p<0.05). Only 15.2% (19 males, 18 females) attained within the lipid tar- get goals. Patients with diabetic dyslipidemia need maximization of lipid-lowering agents, increasing the fibric acid derivatives prescription and the effort to correction of low HDL and/or high TG.

  • The solution for PFC(Power Factor Correction), as a regulation in energy Policy, is becoming a hot Issue in every country because of the shortage of electrical energy. Therefore, a new improved idea for PFC problem has been introduced in this study. A lot of merits, effective cost by simple circuit, reduced PCB size, lighter than reactor in the view of weight, lower level of screen noise by leakage inductance in CTV applications, have been stated by comparing to the earlier method of using a Reactor. All test results in this statement were done by using a power device of STR-G9600 series based on the real load condition of color television. furthermore, the study shows that the test results also meets the IEC-1000-3-2 class D, which regulates the PFC when input power of a set is more than 75watts. More improved PFC in other applications hopes to be implemented by using the proposed method.

A Case of Pseudo-Bartter's Syndrome Due to Hypertrophic Pyloric Stenosis (비후성 유문 협착증에 의한 가성 Bartter 증후군 1례)

  • We report a 3-month old boy admitted to our hospital with Bartter's syndrome like symptoms and laboratory findings, which were vomiting, failure to thrive, hypochloremic and hypokalemic metabolic alkalosis associated with hyperreninemia, hyperaldosteronism and normal blood pressure. However, the urine chloride level was low. Hypertrophic pyloric stenosis was diagnosed through abdominal ultrasonography. Fredet-Ramstedt operation was done after electrolyte correction. After surgery he made a good recovery and gained body weight. The electrolytes maintained within a normal limit without any potassium supplementations after surgery. Differential diagnosis from Bartter's syndrome was made on the basis of a decrease in urine chloride and the non-necessity for potassium supplementation after surgery. It is relatively rare for hypertrophic pyloric stenosis to induce pseudo-Bartter's syndrome. The importance of considering this diagnosis in such cases is discussed.

A Hybrid Approach to Information System Sizing and Selection using Simulation and Genetic Algorithm (시뮬레이션과 유전 알고리즘의 하이브리드 기법을 이용한 정보시스템 용량 산정 및 선택 방안)

  • The purpose of this paper is to develop a new method for information system sizing and selection based on a hybrid mixture of simulation and genetic algorithm, and to show its cost-effectiveness by applying it to a real world problem. To serve this purpose, we propose an operational model which identifies a set of system alternatives using simulation, and determines the optimal one using genetic algorithm. Specifically, with simulation, we generate probability distributions describing real data gathered from actual system, which can overcome the major weakness of the existing methodology that normally employs point estimates of the actual data and constant correction factors without theoretical rationale. We next search for the optimal combination of H/W, the number of CPUs, and S/W, which meets both of our business goals of incurring low TCO(total cost of ownership) and maintaining a good level of transaction processing performance. Experimental result shows the proposed method in this paper saves the cost while it preserves the system's capacity within allowable performance range.

Autonomous Real-time Relative Navigation for Formation Flying Satellites

  • Relative navigation system is presented using GPS measurements from a single-channel global positioning system (GPS) simulator. The objective of this study is to provide the real-time inter-satellite relative positions as well as absolute positions for two formation flying satellites in low earth orbit. To improve the navigation performance, the absolute states are estimated using ion-free GRAPHIC (group and phase ionospheric correction) pseudo-ranges and the relative states are determined using double differential carrier-phase data and singled-differential C/A code data based on the extended Kalman filter and the unscented Kalman filter. Furthermore, pseudo-relative dynamic model and modified relative measurement model are developed. This modified EKF method prevents non-linearity of the measurement model from degrading precision by applying linearization about absolute navigation solutions not about the priori estimates. The LAMBDA method also has been used to improve the relative navigation performance by fixing ambiguities to integers for precise relative navigation. The software-based simulation has been performed and the steady state accuracies of 1 m and 6 mm ($1{\sigma}$ of 3-dimensional difference errors) are achieved for the absolute and relative navigation using EKF for a short baseline leader/follower formation. In addition, the navigation performances are compared for the EKF and the UKF for 10 hours simulation, and relative position errors are mm-level for the two filters showing the similar trends.

Hardware implementation and error analysis of an algorithm for compensating the secondary current of iron-cored current transformers (철심 변류기의 2차 전류 보상 알고리즘의 실시간 구현 및 오차 분석)

  • The conventional method to deal with current transformer (CT) Saturation is over dimensioning of the core so that CTs can carry up to 20 times the rated current without exceeding 10% ratio correction. However, this not only reduces the sensitivity of relays as some errors may still be present in the secondary current when a severe fault occurs, but also increases the CT size. This paper presents an algorithm for compensating the distorted secondary current of iron-cored CTs under CT saturation using the magnetization (flux-current : .lambda.-i) curve and its performance is examined for fault currents encountered on a typical 345[kV] Korean transmission system, under a variety of different system and fault conditions. In addition, the results of hardware implementation of the algorithm using a TMS320C10 digital signal processor are also presented. The proposed algorithm can improve the sensitivity of relays to low level internal faults, maximize the stability of relays for external faults, and reduce the required CT core cross-section significantly. Influence of Slashpipe Exercise on Symmetrical Contraction of Trunk Muscle in Normal Adults

  • Purpose: This study examined the effects of slashpipe exercise on reducing the thickness of the left and right external oblique, internal oblique, transverse abdominis, erector spinae, and multifidus muscles. Methods: A total of 29 healthy adult men and women were included in the study. They performed trunk flexion in the supine position and trunk extension in the prone position with a slashpipe and weight bar. The external oblique, internal oblique, and transverse abdominis muscles were measured in the supine position, while the erector spinae and multifidus muscles were measured in the prone position. The data were analyzed using the SPSS ver 21.0 statistical program. The difference in thickness between the right and left sides of the trunk muscle was analyzed by repeated measures analysis. The statistical significance level was set to p<0.05. Results: The results showed that the slashpipe exercise reduced significantly the difference in thickness of the oblique internus and erector spinae muscles compared to the weight bar exercise. Conclusion: The chaotic fluidity of the fluid filled inside the slashpipe could be used as sensory feedback information on body mal-alignment, which would have positively affected the symmetrical contraction of the trunk muscles as a trigger for self-correction. Therefore, it will have a useful effect not only on the health of the general public, but also on low back patients and athletes with muscle asymmetry.
