Research on Reimbursement of Therapeutic Medical Device through Delphi Method and Analytic Hierarchy Process (치료재료 별도산정 기준 개선을 위한 델파이 및 계층분석과정 조사)

  • Purposes: The objectives of this study present the direction of the criteria for the separately reimbursement of therapeutic medical device. Methodology: We summarized experts' opinion using Delphi survey and Analytic Hierarchy Process(AHP). 48 experts were gathered from Medical Insurance Review Nurses Association, medical device industry, academy and association, Medical Device Expert Evaluation Committee. Descriptive statistics, consistency index, content validity ratio were analyzed. Findings: Clinical utility, patient safety, infection control, cost-homogeneity, cost-effectiveness showed high feasibility and importance, but market contribution and functional utility showed low feasibility and importance in a relative sense. The results of functional utility differed between clinical and non-clinical experts. Measurability was low across the whole area. Among the criteria for the separately reimbursement of therapeutic medical device. Patient safety/infection control and clinical utility showed the highest relative importance values, analyzed using AHP. Practical Implications: Patient safety and infection control are needed to be considered as one of Value Assessment Criteria. It is important to find out how to improve the measurability of therapeutic medical device.

Voltage-Fed Push-Pull PWM Converter Featuring Wide ZVS Range and Low Circulating Loss with Simple Auxiliary Circuit

  • A new zero-voltage-switching (ZVS) push-pull pulse-width modulation (PWM) converter is proposed in this paper. The wide ZVS condition for all of the switches is obtained by utilizing the energy stored in the output inductor and magnetizing inductance. As a result, the switching losses can be dramatically reduced. A simple auxiliary circuit including two small diodes and one capacitor is added at the secondary side of a high frequency (HF) transformer to reset the primary current during the circulating stage and to clamp the voltage spike across the rectifier diodes, which enables the use of low-voltage and low-cost diodes to reduce the conducting and reverse recovery losses. In addition, there are no active devices or resistors in the auxiliary circuit, which can be realized easily. A detailed steady operation analysis, characteristics, design considerations, experimental results and a loss breakdown are presented for the proposed converter. A 500 W prototype has been constructed to verify the effectiveness of the proposed concept.

  • Defense M&S for weapons effectiveness is a realistic way to support virtual warfare similar to real warfare. As the war paradigm becomes platform-centric to network-centric, people try to utilize smartphones as the source of sensor, and command/control data in the simulation-based weapons effectiveness analysis. However, there have been limited researches on integrating smartphones into the weapon simulators, partly due to high modeling cost - modeling cost to accomodate client-server architecture, and re-engineering cost to adapt the simulator on various devices and platforms -, lack of efficient mechanisms to exchange large amount of simulation data, and low-level of security. In this paper, we design and implement Smartphone Adaptor to utilize smartphones for the simulationbased weapons effectiveness analysis. Smartphone Adaptor automatically sends sensor information, GPS and motion data of a client's smartphone to a simulator and receives simulation results from the simulator on the server. Also, we make it possible for data to be transferred safely and quickly through JSON and SEED. Smartphone Adaptor is applied to OpenSIM (Open simulation engine for Interoperable Models) which is an integrated simulation environment for weapons effectiveness analysis, under development of our research team. In this paper, we will show Smartphone Adaptor can be used effectively in constructing a Live simulation, with an example of a chemical simulator.

Devising a Training Method for Assembly Work by Employing Disassembly

  • Efficiency in work training is a perennial issue due to high-diversity low-volume production, particularly for manufacturers producing office machines which are manually assembled by workers. To reduce the training cost, parts used in training are usually reused; a trainer disassembles a product assembled by a worker in training. This paper proposes a training method that employs disassembly usually performed by a trainer. This method assigns both assembly and disassembly to a worker in training, in contrast to the conventional method. The effectiveness of the proposed method is experimentally discussed in terms of learning assembly motions and work procedure at each learning stage, namely, "undergoing learning," "immediately after learning," and "seven days after learning." The effectiveness of the training method is confirmed. The method improves the stability of work procedure recollection immediately after training. Furthermore, at seven days after training, it improves retention of the assembly motions and work procedure, and also promotes and maintains memory related to product structure.

Evaluation of strength characteristics of cement-stabilized soil using the electrical resistivity measurement

  • In this study, the compressive strength of cement stabilized soil was predicted using the electrical resistivity measurement. The effects of the water to cement (w/c) ratio and recovered Carbon Black (rCB) contents were examined. A series of electrical resistivity and compressive strength tests were conducted on two types of stabilized soil after 28 days of curing. Multiple nonlinear regression (MNLR) analysis was used to evaluate the relationship between the compressive strength and the electrical resistivity in terms of the rCB, Cu (uniformity coefficient), and w/c ratio. The results showed that the w/c ratio and Cu have a strong influence on the compressive strength and electrical resistivity of the cement stabilized soil compared to the rCB content. The use of a small amount of rCB led to a decrease in the void space in the specimen and was attributed to the increase strength and decrease electrical resistivity. A high w/c ratio also induced a low electrical resistivity and compressive strength, whereas 3% rCB in the cemented soil provided the optimum strength for all w/c ratios. Finally, a prediction equation for the compressive strength using the electrical resistivity measurement was suggested based on its reliability, time effectiveness, non-destructiveness, and cost-effectiveness.

Post-polypectomy surveillance: the present and the future

  • An appropriate post-polypectomy surveillance program requires the effectiveness of reducing colorectal cancer and safety. In addition, the post-polypectomy surveillance program should consider the burden of limited medical resource capacity, cost-effectiveness, and patient adherence. In this sense, a risk-stratified surveillance program based on baseline colonoscopy results is ideal. Major international guidelines for post-polypectomy surveillance, such as those from the European Union and the United States, have recommended risk-stratified surveillance programs. Both guidelines have recently been updated to better differentiate between high- and low-risk individuals. In both updated guidelines, more individuals have been downgraded to lower-risk groups that require less frequent or no surveillance. Furthermore, increased attention has been paid to the surveillance of patients who undergo serrated polyp removal. Previous guidelines in Japan did not clearly outline the risk stratification in post-polypectomy surveillance. However, the new colonoscopy screening and surveillance guidelines presented by the Japan Gastroenterological Endoscopy Society include a risk-stratified post-polectomy surveillance program. Further discussion and analysis of unresolved issues in this field, such as the optimal follow-up after the first surveillance, the upper age limit for surveillance, and the ideal method for improving adherence to surveillance guidelines, are warranted.

Performance Evaluation for the A/O Pure-Oxygen Biofilm (POB) Process on the Removal of Organics and TKN in the Industrial Wastewater (혐기/호기 순산소 생물막공법에 의한 산업폐수의 유기물 및 TKN 제거 성능평가)

  • For the treatment of wastewaters generated from beer industry and petrochemical company with high organic and nitrogen contents, laboratory scale of A/O Pure-Oxygen Biofilm (POB) process was developed and studied by means of the comparative economic analysis with extended aeration process. When the wastewater of beer company was initially treated by the A/O POB process in the ranges of 70 to 150 mg TOC/L diluted with tap water, higher than 92% of TOC removal was accomplished in the all ranges. In case of petrochemical wastewater, the initial TOC removal was as low as 52%, though, it increased to 86% after 32 days of operation and also the TKN removal marked 71% after 27 days. Continuous high removal rates were monitored in both the TOC and TKN parameters during the experimental period. Due to the cost for PSA (Pressure Swing Adsorption) setting and biomass supporting media installation, the initial construction cost of A/O POB process was 2.9 times higher than that of extended aeration process. However, the advantages such as low sludge production, no need for sludge recycling and low energy consumption allow the A/O POB process to have 2.5 times lower operation and maintenance costs. Consequently, in the long term of operation, it is likely that A/O POB process would show higher performance as well as cost effectiveness compared to extended aeration process.

Bidirectional DC-DC Converter Based on Quasi-Sepic for Battery Charging System

  • In order to satisfy the voltage levels of the low voltage battery side and high voltage DC bus, a high voltage gain with bidirectional operation is required. In this system, the cost effectiveness of the design is a critical factor; therefore, the system should be designed using a small number of components. This paper propose a novel bidirectional converter composed with a quasi-sepic and switched-indictor network. The proposed converter consists a small number of components with a high voltage gain ratio. Detailed analysis are made with respect to the operating mode, number of components, voltage and current ripple and efficiency. To verify performance of the proposed converter, simulation was performed is this paper. The simulation results are shown to verify the feasibility and performance of the proposed bidirectional converter.

Economic Analysis of Introduction of Third Party Logistics System in a Tertiary Hospital (상급종합병원 3자 물류외주시스템 도입의 경제성 분석)

  • Purpose: This study purposed to analyze the economic feasibility of introducing a third party logistics system as a strategic alternative to hospital management. Methods: Economic analysis was performed by measuring changes in costs and benefits before and after implement the third party logistics system and estimating the size of net benefits for the next five years for the target hospital. A questionnaire survey and in-depth interview with stake-holders were conducted to find out the satisfaction and effectiveness of the system. Findings: According to results, the cost-benefit ratio for the implementation of the system was 1.18. For the next five years, the cost-benefit ratio was 1.48, the net present value was about 1.7 billion won, and the internal rate of return was 64%. The satisfaction of internal stake-holders was relatively high, in terms of improving the concentration of unique tasks and increasing the efficiency of inventory management. Practical Implications: It was found that the increase in benefits had a greater effect on the change in the cost-benefit ratio than the increase in costs resulting from the expansion of logistics, and the increase in the present value of net benefits gradually decreased as the cost increased. In addition, overall job satisfaction and satisfaction with outsourcing companies were relatively low, which means that stabilization of the new system is important. Further study is needed for more accurate economic analysis.

Object Tracking using Adaptive Template Matching

  • Template matching is used for many applications in image processing. One of the most researched topics is object tracking. Normalized Cross Correlation (NCC) is the basic statistical approach to match images. NCC is used for template matching or pattern recognition. A template can be considered from a reference image, and an image from a scene can be considered as a source image. The objective is to establish the correspondence between the reference and source images. The matching gives a measure of the degree of similarity between the image and the template. A problem with NCC is its high computational cost and occasional mismatching. To deal with this problem, this paper presents an algorithm based on the Sum of Squared Difference (SSD) and an adaptive template matching to enhance the quality of the template matching in object tracking. The SSD provides low computational cost, while the adaptive template matching increases the accuracy matching. The experimental results showed that the proposed algorithm is quite efficient for image matching. The effectiveness of this method is demonstrated by several situations in the results section.
