Clinical Application of Gamitaeeumjowee-tang for Overweight or Obese Hypothyroidism Patients: A Retrospective Chart Review (과체중 또는 비만을 주소로 내원한 갑상선기능저하증 환자에 대한 가미태음조위탕의 임상적 활용: 후향적 차트 리뷰)

  • Objectives: Weight gain is one of the symptoms of hypothyroidism and it could deteriorate thyroid function. Our objectives are to evaluate weight changes and analyze adverse events in overweight and obese women with hypothyroidism who were prescribed Gamitaeeumjowee-tang. Methods: A retrospective chart review was conducted using medical records of patients with hypothyroidism, who had body mass indexes (BMI) above 23 and were prescribed Gamitaeeumjowee-tang for 12 weeks between August 2017 and November 2022. Reported adverse events (AEs) were assessed by severity, causality and system-organ classes. Results: Thirty patients were included (mean±standard deviation, age 46.10±9.94 years, weight 70.52±11.18 kg, BMI 28.30±3.80 kg/m2). The mean total weight loss of hypothyroidism patients was 5.45±2.77 kg, while the mean weight loss rate and mean BMI change were 7.66±3.41% and 2.18±1.07 kg/m2, respectively. Among evaluated causality of adverse events, 'Unlikely' was predominant (80.8%) and severity assessment showed most of the symptoms were mild (91.7%). Conclusions: Gamitaeeumjowee-tang could be an option for hypothyroidism patients, who are overweight or obese, to lose weight and no serious adverse events occurred. Further well-designed clinical studies are recommended.

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment on a patient with atopic eruption of pregnancyMethods: Patient who diagnosed by atopic eruption of pregnancy was enrolled in this study. She received Korean medicine therapies such as herbal medicine, acupuncture and external application. Sopungchungyoung-tangs and Seungmahwangryeon-tangs were prescribed according to the patient’s constitution which was pungyeol. The severity of symptom was evaluated by the severity SCORing for Atopic Dermatitis index (SCORAD index).Results: Subjective symptom such as Systemic pruritus, sleep loss, and Dermatitis lesions was reduced by Korean medicine therapies.Conclusions: The present study suggests that Korean medicine therapies have a effect on atopic eruption of pregnancy.

Damage detection in jacket type offshore platforms using modal strain energy

  • Structural damage detection, damage localization and severity estimation of jacket platforms, based on calculating modal strain energy is presented in this paper. In the structure, damage often causes a loss of stiffness in some elements, so modal parameters; mode shapes and natural frequencies, in the damaged structure are different from the undamaged state. Geometrical location of damage is detected by computing modal strain energy change ratio (MSECR) for each structural element, which elements with higher MSECR are suspected to be damaged. For each suspected damaged element, by computing cross-modal strain energy (CMSE), damage severity as the stiffness reduction factor -that represented the ratios between the element stiffness changes to the undamaged element stiffness- is estimated. Numerical studies are demonstrated for a three dimensional, single bay, four stories frame of the existing jacket platform, based on the synthetic data that generated from finite element model. It is observed that this method can be used for damage detection of this kind of structures.

Efficacy of Korean Red Ginseng in the Treatment of Atopic Dermatitis

  • In order to determine the efficacy of functional foods, objective measurement of the severity of atopic dermatitis (AD) after taking foods is important. The aim of this study was to conduct an objective evaluation of whether Korean red ginseng (KRG) might be helpful for improvement of skin condition and serum IgE in patients with AD. Thirty atopic patients (18 females and 12 males) participated in this study. Patients took KRG for 16 weeks. Bioengineering methods, including the corneometer and evaporimeter, were used at the start of the study and after 8 weeks and 16 weeks. In addition, we assessed serum IgE levels and the severity scoring of the atopic dermatitis (SCORAD) index. Transepidermal water loss and skin hydration showed significant improvement after 16 weeks. A significant decrease in the SCORAD index, as well as in serum IgE level, was observed after 16 weeks. Our results demonstrated that KRG may be helpful as a functional food for patients with AD.

The Effect of Second Toe Valgus on Correction Loss of Hallux Valgus Angle in Surgical Treatment (무지외반증 수술에서 제 2족지의 외반 정도가 무지 외반각 교정소실에 미치는 영향)

  • Purpose: The purpose of this study is to determine the correlation between the correction loss of hallux valgus angle and the severity of valgus angle of the second toe. Materials and Methods: We selected 312 cases from 268 surgical patients with hallux valgus deformity receiving distal chevron osteotomy. For a radiological evaluation, we analyzed the changes in the hallux valgus angles, first to second intermetatarsal angles, and valgus angle of the second toe post index operations. All patients were women; the mean age was 46 years. The mean follow-up period was 17 months. Results: The mean hallux valgus angle was $33.6^{\circ}$ preoperatively (range, $25.7^{\circ}{\sim}44.8^{\circ}$), $13.1^{\circ}$ (range, $8.4^{\circ}{\sim}16.4^{\circ}$) after 4 weeks of postoperative period, and $17.1^{\circ}$ (range, $9.4^{\circ}{\sim}28.5^{\circ}$) at their final follow-up. The mean valgus angle of the second toe was $8.4^{\circ}$ preoperatively (range, $2.0^{\circ}{\sim}25.8^{\circ}$) and $8.3^{\circ}$ (range, $1.7^{\circ}{\sim}24.9^{\circ}$) at the final follow-up. Preoperatively, there was a positive correlation between the valgus angle of the second toe and hallux valgus angle (r=0.747, p=0.001). The correction loss of hallux valgus angle had a significant correlation with the severity of valgus angle of the second toe (r=0.802, p=0.001). Conclusion: The existence of the second toe valgus deformity may present itself as a cause of correction loss of hallux valgus angle. The preoperative measurement of the second toe valgus angle may be a good predictor of correction loss; therefore, thorough preoperative warning on the possibility of correction loss should be conducted to maximize patient satisfaction after the procedure.

  • Purpose: The evidence for an association between inflammatory bowel disease (IBD) and obesity is conflicting. Therefore, we set out to review the body mass index (BMI) at presentation of IBD to understand if the rise of the obesity rate in the general population, lead to an increase of obesity in patients with IBD at the time of diagnosis. Methods: Retrospective review of all patients with IBD seen at Children's Hospital and Medical Center from January 1st 2010 to December 31st 2014. From the initial visit and endoscopy, we obtained: age; sex; BMI; disease phenotype; disease severity. Results: We had a total of 95 patients, 35 patients were excluded due to incomplete data or referral being made after diagnosis was made. 28 were males and 32 were females, Age range was 2-17 years. A 37 had Crohn's disease, 19 ulcerative colitis, and 4 indeterminate colitis. Disease severity in 19 cases was mild, 29 moderate and 12 severe. BMI distribution was as follows-obese (5.0%), overweight (6.7%), normal weight (65.0%), mild malnutrition (8.3%), moderate malnutrition (15.0%), severe malnutrition (1.7%). Conclusion: Our data is consistent with other series. Showing most children had a normal BMI, regardless of disease severity or phenotypes. One confounding factor is the possibility of delay in referral to GI. This could mean some obese children may fall in the normal BMI range at the time of diagnosis due to ongoing weight loss. Future studies should include prospective cohort studies, comparing incidence of IBD in obese and non-obese patients, severity at presentation, duration of symptoms, and clinical outcomes.

  • The present study was performed to evaluate the relationship between the serotype or the genotype of Actnobacillus actinomycetemcomitans (A. a.) and the severity of periodontal disease. Total 64 A. a. clinical isolates were sampled from 46 sites of 20 subjects classified into the group I (1 periodontally healthy subject, 2 gingivitis patients, 5 ealry adult periodontitis patients), group II (3 moderatelly adult periodontitis patients) and group III (1 advanced adult periodontitis patient, 8 RPP patients). Southern bolt hybridization (fingerprinting) patterns of the five reference strains, A. a. strain ATCC 29523 (serotype a), ATCC 29522 (Serotype b), ATCC 43719 (serotype c), IDH 781 (serotype d) and IDH 1705 (serotype e), were used as the five basic genotypic patterns (A, B, C, D, E). NT type was designated as one which did dnot represent any of those five basic types. The serotypes were determined by ELISA technique with the serum samples from pre-immunized rabbit. Based on subject-based analysis, it was noted that genotypes A and C, NT, and B, D, E were significantly related to the disease groups I, II, and III, respectively. It was also noted that both the serotypes a and c were significantly related to the disease group I and II, while serotypes were significantly related bm), and serotypes b and nd were frequently found in sites with severe attachment loss (LA>6mm). The results indicated that the significant relationship can be delineated beteen the genotypes and the serotypes of Acinobacillus actinomycetemcomitans and the periodontal disease severity. The results also indicated that genotyping can provide more detailed information on its relationship with the disease severity based on both the patient-based and the site-based analyses.

Developing the Accident Injury Severity on a Field of Construction Work Using Ordered Probit Model (순서형 프로빗 모형을 적용한 공사장 교통 사고심각도 분석)

  • The traffic accidents at a construction site, which happen due to construction vehicles' frequent access to a construction site, its subsequent conflicts with ordinary vehicles and pedestrians, and inappropriate installation & management of traffic security facilities, have not many proportions in all traffic accidents, but obviously, the accident damage is quite serious when comparing the level of the fatal per one accident. This research conducted an analysis of traffic accident injury severity using Ordered Probit Model in relation to 241 traffic accident cases that occurred caused by construction sites among the traffic accidents that took place in Seoul and Gyeoggi-do region for two years from 2006 until 2007. As a result, the significant variables enough to explain traffic accident injury severity were analyzed to be the state of road surface, linear shape of an accident spot & whether the damaging car belongs to the vehicle for construction, and whether vehicles have access to a construction site at the time of an accident. Through this, this research found out some fact as follows: first, there need to be more aggressive management of the vehicles for construction and a year-round placement of the manpower who can control vehicular access to a construction site. Second, it is necessary to get drivers to recognize the fact that there exists a construction site on the construction section which is on the border of curved roads in advance to prevent a traffic accident, helping to reduce socioeconomic loss & costs incurred by a traffic accident.

Prognostic Factor Analysis for Management of Chronic Neck Pain : Can We Predict the Severity of Neck Pain with Lateral Cervical Curvature?

  • Objective : Although little is known about its origins, neck pain may be related to several associated anatomical pathologies. We aimed to characterize the incidence and features of chronic neck pain and analyze the relationship between neck pain severity and its affecting factors. Methods : Between March 2012 and July 2013, we studied 216 patients with chronic neck pain. Initially, combined tramadol (37.5 mg) plus acetaminophen (325 mg) was administered orally twice daily (b.i.d.) to all patients over a 2-week period. After two weeks, patients were evaluated for neck pain during an outpatient clinic visit. If the numeric rating scale of the patient had not decreased to 5 or lower, a cervical medial branch block (MBB) was recommended after double-dosed previous medication trial. We classified all patients into two groups (mild vs. severe neck pain group), based on medication efficacy. Logistic regression tests were used to evaluate the factors associated with neck pain severity. Results : A total of 198 patients were included in the analyses, due to follow-up loss in 18 patients. While medication was successful in reducing pain in 68.2% patients with chronic neck pain, the remaining patients required cervical MBB. Lateral cervical curvature, such as a straight or sigmoid type curve, was found to be significantly associated with the severity of neck pain. Conclusion : We managed chronic neck pain with a simple pharmacological management protocol followed by MBB. We should keep in mind that it may be difficult to manage the patient with straight or sigmoid lateral curvature only with oral medication.

A clinical analysis on glaucoma, cataract and causing-blindness-diseases by oriental medical therapy (內障과 眼盲疾患의 韓方治驗에 關한 小考)

  • The glaucoma, cataract and causing-blindness-diseases have high clinical importance by their severity of the symptoms and possibility of causing visual loss. Glaucoma is caused by an increase in the intraocular pressure, if severe enough, can cause field loss and eventually complete blindness. In cataract, the general clarity of vision is impaired due to a loss of transparency of the chrystalline lens of the eye. The causing-blindness-diseases can be divided to PokMaeng which cause sudden blindness and ChungMaeng, in which visual acuity is losed gradually. In glaucoma, cataract and causing-blindness-diseases, the visual acuity is affected and the patient may complain blurriness, fatigue of eye, visual field loss deficiency or scintillant feeling, vitreous floaters. In this study, 32 patients who have cataract, glaucoma or other causing-blindness-diseases was treated with oriental medicine or acupuncture from July, l996 to May. 1997, and was followed up by their symptoms and visual acuity. 1. In glaucoma patients, intraocular pressure decreased significantry and the symptoms improved positively. 2. In cataract patients, there was no improvement of visual acuity but $40\%$ of patients found their symptoms improved. 3. In causing-blindness-diseases, $57\%$ of patients reported improvement of their symptoms such as blurriness or fatigue of eye. 4. Through oriental medical treatment, the symptoms of glaucoma, cataract and causing-blindness-diseases can be treated effectively. Therefore oriental management seems to be possible to cure glaucoma, cataract and causing-blindness-diseases.
