The clinical manifestations, the short- and long-term outcomes of Bartter syndrome (Bartter 증후군의 임상 양상과 장단기 치료 결과)

  • Purpose : Bartter syndrome is a renal tubular defect in electrolyte transport characterized by hypokalemia, metabolic alkalosis and other clinical signs and symptoms. The aims of this study were to analyze the clinical manifestations and the short- and long-term outcomes of Bartter syndrome. Methods : We retrospectively reviewed clinical history, laboratory finding of blood and urine, renal ultrasonography, and hearing tests of five patients who were diagnosed and managed with Bartter syndrome at Asan Medical Center from April 1992 to May 2007. We also evaluated height and body weight periodically after institution of therapy. Results : All patients had poor oral intake, failure to thrive and polyuria. Three of them had maternal history of polyhydramnios and premature delivery. The mean age at diagnosis was 11.8 months. All children presented with hypokalemia, metabolic alkalosis, hyperreninemia. Their blood pressures were normal. One patient had nephrocalcinosis on renal ultrasonography and all of them had normal result in hearing tests. After treatment with indomethacin or other prostaglandin inhibitors and potassium supplementation,their clinical features improved with catch-up growth and improvement in the development during long-term follow-up. Conclusion : We emphasize that early diagnosis and proper treatment in patient with Bartter syndrome are related to better prognosis.

Effect of Thyroid Hormones on the Redox Balance of Broiler Chickens

  • In the present study, two trials were conducted to evaluate the effects of hyper- and hypothyroid status on the redox balance of broiler chickens. In Trial 1, 3 groups of broiler chickens were randomly subjected to one of the three treatments: subcutaneous administration of triiodothyronine (T3, $150{\mu}g/kg$ BW), methimazole (MMI, 150 mg/kg BW), or saline. The blood, liver and heart were sampled at 3 h after injection. In Trial 2, three groups of 20 broiler chickens were randomly fed with one of the three diets: control, dietary supplementation of T3 (1.5 mg/kg diet) or MMI (1 g/kg diet) for 7 days. In trial 1, the plasma concentrations of T3 and T3 to thyronine ratio (T3/T4) were significantly increased by T3 injection. Plasma levels of thiobarbituric acid reacting substances (TBARS) tended to be increased (p = 0.067) by both T3 and MMI treatments while the ferric reduced/antioxidant capacity (FRAP) was increased only by MMI treatment. Acute T3 treatment had no significant effect on the activities of superoxide dismutase (SOD) and the concentrations of FRAP and TBARS in either liver or heart tissue. In contrast, the hepatic activities of SOD were decreased (p<0.05) while the cardiac levels of FRAP were significantly increased (p<0.0001) by MMI treatment. In chronic treatments, the rectal temperature of chickens was significantly decreased (p<0.05) by MMI treatment. The circulating T3 levels were significantly increased (p<0.05) by long-term T3 treatment, and showed a trend to decrease in MMI treatment. The plasma concentrations of TBARS were significantly (p<0.05) increased by MMI treatment. All the redox parameters measured in either liver or heart were not significantly altered by either long-term T3 or MMI treatment except that the hepatic SOD activities were significantly augmented by T3 treatment. The result showed that neither acute nor long-term elevation of circulating T3 levels induced lipid peroxidation in broiler chickens. The enhanced enzymatic antioxidant system (SOD in cardiac tissue) may be involved in the protection of the bird to increased oxidative challenge. The responses of redox balance to changed thyroid state seem to be tissue specific.

Hypotriglyceridemic effects of brown seaweed consumption via regulation of bile acid excretion and hepatic lipogenesis in high fat diet-induced obese mice

  • BACKGROUND/OBJECTIVES: The present study aimed to further investigate the potential health beneficial effects of long-term seaweed supplementation on lipid metabolism and hepatic functions in DIO mice. MATERIALS/METHODS: Four brown seaweeds (Undaria pinnatifida [UP], Laminaria japonica [LJ], Sargassum fulvellum [SF], or Hizikia fusiforme [HF]) were added to a high fat diet (HFD) at a 5% ratio and supplemented to C57BL/6N mice for 16 weeks. Triglycerides (TGs) and total cholesterol (TC) in the liver, feces, and plasma were measured. Fecal bile acid (BA) levels in feces were monitored. Hepatic insulin signaling- and lipogenesis-related proteins were evaluated by Western blot analysis. RESULTS: Fasting blood glucose levels were significantly reduced in the LJ, SF, and HF groups compared to the HFD group by the end of 16-week feeding period. Plasma TG levels and hepatic lipid accumulation were significantly reduced in all 4 seaweed supplemented groups, whereas plasma TC levels were only suppressed in the UP and HF groups compared to the HFD group. Fecal BA levels were significantly elevated by UP, LJ, and SF supplementation compared to HFD feeding only. Lastly, regarding hepatic insulin signaling-related proteins, phosphorylation of 5'-AMP-activated protein kinase was significantly up-regulated by all 4 types of seaweed, whereas phosphorylation of protein kinase B was up-regulated only in the SF and HF groups. Lipogenesis-related proteins in the liver were effectively down-regulated by HF supplementation in DIO mice. CONCLUSIONS: Brown seaweed consumption showed hypotriglyceridemic effects in the prolonged DIO mouse model. Specifically, combinatory regulation of BA excretion and lipogenesis-related proteins in the liver by seaweed supplementation contributed to the reduction of plasma and hepatic TG levels, which inhibited hyperglycemia in DIO mice. Thus, the discrepant and species-specific functions of brown seaweeds provide novel insights for the selection of future targets for therapeutic agents.

Effect of the Magnetized Water Supplementation on Lymphocyte DNA Damage in Mice Treated with Diethylnitrosamine (Diethyl Nitrosamine (DEN) 처리 실험동물에 있어 기간에 따른 자화육각수의 임파구 DNA 손상 개선효과)

  • Water gets magnetically charged when it is contacted with a magnet. Although magnetic water products have been promoted since the 1930's, they have received very little recognition due to questionable effectiveness. Diethylnitrosamine (DEN) is a widely occurring nitrosamine that is one of the most important environmental carcinogens primarily inducing tumors of liver. In this study, the effect of magnetized water supplementation on lymphocyte DNA damage in ICR mice treated with DEN was evaluated using the Comet assay. Mice were divided into 3 groups: control, DEN, and DEN + magnetized water group. Fifteen mice were maintained in each group for the entire experimental period of 6, 12 and 18 weeks. Five mice in each group were sacrificed at 6, 12, and 18th weeks, followed by the Comet assay using the blood obtained from heart puncture of the mice. The level of lymphocyte DNA damage reflected by tail moment and other DNA damage indices of tail DNA (%) or tail length of the magnetized water group were significantly decreased after the 6th, 12th and 18th weeks of supplementation compared with the positive control, the DEN group. The relative DNA damage of the magnetized water groups compared to the DEN control group after 6th, 12th, and 18th weeks of supplementation were 42.2%, 40.8%, and 32.9% for DNA in tail, 31.2%, 32.6%, and 21.3% for tail length, and 33.8%, 33.8%, and 24.6% for tail moment, respectively. This is the first report demonstrating that magnetized water may be involved in the lowering effect of the DNA damage in DEN-treated ICR mice. This result suggests that the magnetized water might have minimized the DNA damage by improving the antioxidant status of the mice. However, further studies are needed to characterize the condition of the magnetization and examine the long-term effect of the water product.

Influences of Long-Term Feeding of Japanese Green Tea Powder on Laying Performance and Egg Quality in Hens

  • Influences of Japanese green tea powder (GTP) supplementation to commercial diet on laying performance and egg quality were studied by using 60 laying hens. The experimental diet with or without 0.6% GTP was given ad libitum to the birds during the period from 6 to 71 weeks of age. The birds started egg production from 21 wk of age regardless GTP feeding. Body weight, feed intake, egg weight tended to decrease with GTP supplementation, while egg production rate tended to increase. Haugh unit score was significantly increased with GTP, which accompanied with the increased albumen height. These were observed almost throughout the laying period over 50 wk. Gel proportion in thick albumen was decreased as storage time is prolonged, then higher values of the gel proportion were recorded in the eggs from GTP group. Thick albumen of the eggs from GTP-fed layers had more carbohydrate than that from control layers. All these indicate physical stability of thick albumen in the eggs from GTP group. Thiobarbituric acid content of egg yolk tended to remain lower in the eggs from GTP group during 5-10 days of storage at room temperature. Levels of egg yolk cholesterol and yolk lipid were significantly reduced by GTP feeding. There were no significant differences in eggshell weight, shell thickness and shell strength between the two groups. Thyroid gland and liver from hens slaughtered at 71 wk of age did not differ in weight irrespective of GTP feeding. The present results suggest that GTP could modify components of edible part of egg, leading to the characteristics favourable to consumers such as high durability of thick albumen and less cholesterol in yolk, without altering general performance of the layers throughout this year round experiment.

Effects of Isoflavones Supplementation on Bone Mineral Density and Sex Hormones in Postmenopausal Women (이소플라본 보충이 폐경 후 여성에서 골밀도와 성호르몬에 미치는 영향)

  • Soy isoflavones have been hypothesized to exert hormonal effects and to enhance bone mineral density in postmenopausal women. To test this hypothesis, we studied the effects of soy isoflavones supplements on bone mineral density and sex hormones (serum estradiol, sex hormone-binding globulin and testosterone) in 47 postmenopausal women. There were 24 participants in the treatment group and 23 in the control group. The treatment group consumed the isoftavones extract capsule daily (which contained 90 mg of soy isoflavones) for 12 weeks. The study compared pre- and post-isoflavones intake in the following areas: physical examination, diet survey, bone mineral density and serum sex hormone levels. The average age of the treatment group was 64.63 years and that of the control group was 66.48 years. There were no significant differences between the two groups in terms of height, weight, and body mass index. Both groups maintained regular diet patterns in terms of their average daily nutrient intake. There was no significant difference between the treatment group (18.49 mg) and the control group (21.27 mg) in terms of daily isoflavones intake based on diet. The 12-week analysis of bone mineral density change after taking isoflavones supplements demonstrated no significant differences in the following : lumbar spine BMD (0.82 g/$\textrm{cm}^2$ in pre versus 0.81 g/$\textrm{cm}^2$ in post), femoral neck BMD (0.58 g/$\textrm{cm}^2$ in pre versus 0.57 g/$\textrm{cm}^2$ in post) in the treatment group. There was no significant difference in serum estradiol in the isoflavones treatment group. The subjects indicated no significant difference in serum testosterone in the isoflavones treatment group. But the subjects indicated a significant difference in sex hormone-binding globulin (60.04 nmol/L in pre versus 52.39 nmol/L in post) in the isoflavones treatment group at the levels of p < 0.05. The significant decrease in sex hormone-binding globulin did indicate the need for long-term study on isoflavones supplementation as well as its positive effect on bone mineral density.

Effect of vitamin C on azoxymethane (AOM)/dextran sulfate sodium (DSS)-induced colitis-associated early colon cancer in mice

  • BACKGROUD/OBJECTIVES: The objective of this study was to investigate the effects of vitamin C on inflammation, tumor development, and dysbiosis of intestinal microbiota in an azoxymethane (AOM)/dextran sulfate sodium (DSS)-induced inflammation-associated early colon cancer mouse model. MATERIALS/METHODS: Male BALB/c mice were injected intraperitoneally with AOM [10 mg/kg body weight (b.w)] and given two 7-d cycles of 2% DSS drinking water with a 14 d inter-cycle interval. Vitamin C (60 mg/kg b.w. and 120 mg/kg b.w.) was supplemented by gavage for 5 weeks starting 2 d after the AOM injection. RESULTS: The vitamin C treatment suppressed inflammatory morbidity, as reflected by disease activity index (DAI) in recovery phase and inhibited shortening of the colon, and reduced histological damage. In addition, vitamin C supplementation suppressed mRNA levels of pro-inflammatory mediators and cytokines, including cyclooxygenase-2, microsomal prostaglandin E synthase-2, tumor necrosis $factor-{\alpha}$, Interleukin $(IL)-1{\beta}$, and IL-6, and reduced expression of the proliferation marker, proliferating cell nuclear antigen, compared to observations of AOM/DSS animals. Although the microbial composition did not differ significantly between the groups, administration of vitamin C improved the level of inflammation-related Lactococcus and JQ084893 to control levels. CONCLUSION: Vitamin C treatment provided moderate suppression of inflammation, proliferation, and certain inflammation-related dysbiosis in a murine model of colitis associated-early colon cancer. These findings support that vitamin C supplementation can benefit colonic health. Long-term clinical studies with various doses of vitamin C are warranted.

Effect of Dietary Iron Levels on the Cadmium Accumulation in Cadmium Poisoned Rats (카드뮴의 장기 중독시 철분의 섭취 수준이 흰쥐의 체내 카드뮴 축적에 미치는 효과)

  • This study was undertaken to investigate the effect of dietary Fe levels on Cd accumulation in Cd poisoned rats. Forty male weaning Sprague Dawley rats weighing 80-90g were divided into 4 groups(LFe : low Fe, LFeCd : low Fe and Cd, AFe : adequate Fe, AFeCd : adequate Fe and Cd) according to Cd administration(Cd : 0 or 50ppm in drinking water) and Fe levels (Fe : 6 or 40ppm in diet) for 12 weeks. The food intake and weight gain of LFe and LFeCd were significantly lower than those of AFe and AFeCd(p<0.01, p<0.001). The water intake was not affected significantly by Cd and Fe, therefore Cd intake was no significant difference between groups. The Cd accumulation of kidney in LFeCd was significantly higher than those of AFeCd(p<0.001). But the Cd accumulations of brain, liver and spleen were not significantly different between Cd groups and without Cd groups. The serum Cd content and urinary Cd excretion of LFeCd was significantly higher than those of AFeCd(p<0.01, p<0.01). But the fecal Cd excretion of LFeCd was significantly lower than that of AFeCd(p<0.001). The Cd retention, Cd retention rate, and apparent Cd digestibility of LFeCd were significantly higher than those of AFeCd(p<0.001, p<0.001, p<0.001). It was concluded that adequate Fe supplementation have protective effects on the long term Cd poisoning in rats.

Effects of the Addition of \beta-Glucanase to Barley-based Layer Diet (산란계 사료에 \beta-Glucanase 첨가가 보리의 이용성에 미치는 효과)

  • Effects of the addition of \beta-glucanase to barley-based layer diets were examined by feeding 200 Leghorn layers with corn-based (Control) and \beta-glucanase supplemented diets (Barley+ Enzyme). The results obtained are sumrrarized as follows. 1. There were no siginificant (P>0.05) differences in hen-day egg production(%) and average egg weight between two treatments, indicating that the \beta-glucanase supplemented barley could successfully replace the commonly used corn in the layer diets. 2. Although there was no statistical difference (P>0.05) between two treatments, the daily feed consumption was numerically high in layers fed the barly diet compared to the corn-based diet. 3. Availabilities of crude fat and crude fiber of the barley diet were significantly poor (P<0.05) as compared to corn diet. 4. The \beta-glucarase supplementation depressed the viscosity of barley diets and excreta from therm. 5. Both serum and egg yolk cholesterol were not significantly affected by the addition of \beta-glucarase in the barley based diet. Our data indicate that the barley grain supplemented with \beta-glucarase can be sucessfully used as an energy source of layer diet when there is a price advantage. Although some possibilities to produce low cholesterol egg were recognized in this study, further studies pertaining to long-term feeding experiment and elucidaton of the metabolic interrelationship between serum and yolk cholesterol, are required to confirm the result.

Effects of a Web-based Education Program on Cognitive Function, Depression, and dementia Preventive Behavior among Elders in Communities (웹 기반의 치매 예방 융합교육 프로그램이 지역사회 노인의 인지기능, 우울, 치매예방 행위에 미치는 효과)

  • Purpose: This study was conducted to identify the effects of a convergent web-based educational program on cognitive function, depression, and dementia prevention behavior among elders living in the community. Method: Participants were 16 elders over 65 years of age (8 in the experimental group and 8 in the control group). The educational program was given twice a week for 9 weeks. Results: Cognitive function and dementia prevention behavior improved in the experimental group. Cognitive function and dementia prevention behavior were reduced in the control group compared to the results of the pre-inspection; however, no statistically significant difference was found. As homogeneity test was not conducted previously for the assessment of depression, the assessment was conducted based on covariates, Although no significant difference found between the two groups, depression level did increase in the control group. Conclusion: Findings from this study indicate that the educational program prevented worsening of depression and was effective in reducing depression. For further assessment, it is necessary to conduct long-term research including a revision and supplementation of the contents and length of the program.
