요양보호사의 직무환경에 따른 직무스트레스와 소진이 직무만족에 미치는 영향 (The Effect of Job Stress and Burnout by Job Environment of Care-givers on Job Satisfaction)

  • 본 연구는 노인장기요양기관 종사자인 요양보호사들의 직무환경과 관련된 속성 및 스트레스와 소진을 파악하고 이들 요인이 직무만족에 어떠한 영향을 미치는지 효과를 분석하여 요양보호사의 조직업무를 향상시켜 노인의 삶의 질과 노인복지 발전에 도움이 되는데 목적을 갖고 연구하였다. 연구대상은 전북 군산시에 소재한 노인장기요양기관(요양시설, 재가노인복지시설)에 근무하는 요양보호사 129명을 대상으로 하였다. 분석결과 요양보호사의 직무환경의 하위요인으로 인적환경과 보상체계, 업무전문성 등이 직무소진, 직무스트레스, 직무만족에 유의적인 영향을 미치는 요인임을 실증시켜 주었다. 이는 그만큼 인적환경을 위한 업무전문성 및 능력과 혁신적 사고를 가진 업적에 비례한 보상체계가 잘 되어 있을수록 긍정적인 관계 형성과 동시 직무소진과 스트레스의 감소 및 직무만족이 향상됨을 의미한다. 이는 최근 장기요양을 위한 노인성 환자의 급증과 장기요양기관에서의 요양보호사의 역할과 직무가 중요시되는 현실에 비추어 볼 때 전문직으로서의 다양한 복지 수혜적 봉사자로서의 요양보호사들의 직무환경 개선을 위한 직무전문화 보장과 이들 직무능력에 따른 적절한 보상시스템의 체계화가 필요시 됨을 시사한 것으로 볼 수 있다. 따라서 직업에 대한 자긍심이 높은 전문 인력의 배출과 참여로 요양보호사의 잦은 직무소진과 스트레스를 낮출 수 있는 경감요인을 찾아 제거함으로써 보다 많은 인력의 참여와 조직헌신을 통한 직무만족 극대화를 통해 조직몰입을 높일 수 있을 것이다.

  • 최근 한국에서는 고령화가 본격적으로 진행되어 가족이나 개인중심의 노인돌봄시스템이 한계에 도달하였다. 본 연구는 장기요양등급자로 재가서비스를 받고 있거나 등급외자로 돌봄서비스를 받고 있는 노인을 대상으로 하는 재활 및 건강증진, 삶의 질 향상을 위한 적절한 서비스를 개발하는 목적을 갖는다. 특히 공급자입장의 품질 관리 측면에서 접근하였으며 서비스관리공급자인 기관 및 서비스제공자인 요양보호사를 대상으로 FGI 조사를 실시하고, 지역사회복지사와 방문간호사 대상 심층면접을 실시하였다. 연구결과에서는 서비스 공급자 및 제공자 간 서비스 내용에 대한 의식차이가 나타났고 또한 적절한 서비스를 제공하기 위해서는 지역사회에서 영역별로 분리되어 있는 케어서비스를 융합하기 위한 통합 교육 및 팀워크 훈련 등이 필요한 것으로 나타났다.

가족, 간병인, 간호사가 인지하는 노인요양보호사 교육의 중요도 (The Level of Importance on Education of long Term Care Nursing Assistants Perceived by Caregivers of the Elderly)

  • Purpose: This study aimed to explore the level of importance perceived by caregivers of elderly on the education of long term care nursing assistants (LTCNAs) taking care of elders with dementia or stroke. Method: Data was collected from 296 participants (112 families, 98 NAs, and 86 RNs) from October 2006 to February 2007. A structured questionnaire was used for data collection. Result: The item of 'attitude while caring for the aged' was identified as most important. The family group reported that 'position change' and 'understanding of geriatric diseases' were the most important for education, while LTCNAs, highly identified, 'bathing', and 'bed sheet change'. The RNs group regarded 'position change', and 'bathing' as most important. Institutions demanding LTCNAs' activities were special facilities for elderly care and special hospitals for the aged, and the need was also high in families with an elder incapacitated by illness. Conclusion: Nurses need to take an interest in education for enhancing the quality of LTCNAs and thus advance the quality of nursing care as well as the quality of life for the aged.

요양보호사의 임파워먼트가 직무만족과 조직몰입 및 이직의도에 미치는 영향 (The Effect of the Long-Term Care Worker's Empowerment on Job Satisfaction, Organizational Commitment and Turnover Intention)

  • 본 연구는 요양보호사의 임파워먼트가 직무만족, 조직몰입과 이직의도에 미치는 영향을 밝히는데 목적을 갖고, 충청지역 소재 노인요양시설 내 요양보호사 200명을 대상으로 설문지 조사방법을 사용하였다. 설문조사 기간은 2011년 5월 16일부터 30일까지 실시하였다. 분석결과 요양보호사의 임파워먼트가 직무만족과 조직몰입의 향상과 이직의도 감소에 유의한 영향을 미치는 것으로 나타났는데 이는 요양보호사의 임파워먼트가 높을수록 직무만족과 조직 몰입 성향이 높고 긍정적인 직무만족과 조직몰입에 따른 이직의도 또한 낮아지는 것을 의미해 주었다. 따라서 요양보호사의 낮은 임파워먼트를 고려할 때 개개인의 성격과 적성을 고려한 특기와 장점을 살린 임파워먼트를 높여주고 당해 직무에 보다 관심을 갖고 배려하여 줌으로써 종국적으로 직무만족과 조직몰입 및 이직의도의 억제에 기여할 수 있음을 시사한 것이라 할 수 있다.

건강보험청구자료로 본 요양병원의 기능 유형 (A Taxonomy of Geriatric Hospitals Using National Health Insurance Claim Data)

  • Purpose: This study classified the actual functions of geriatric hospitals and examined the differences in their characteristics, in order to provide a basis for discussions on defining the functions of geriatric hospitals and how to pay for care. Methodology: This study used various administrative data such as health insurance data and long-term care insurance data. Cluster analysis was used to categorize geriatric hospitals. To examine the validity of the cluster analysis results, we conducted a discriminant analysis to calculate the accuracy of the classification. To examine cluster characteristics, we examined structure, process, and outcome indicators for each cluster. Findings: The cluster analysis identified five clusters. They were geriatric hospitals with relatively short stays for cancer patients(cluster 1; cancer patient-centered), geriatric hospitals with relatively large numbers of patients using rehabilitation services(cluster 2; rehabilitation patient-centered), geriatric hospitals with a high proportion of relatively severe elderly patients(cluster 3; severe elderly patient-centered), geriatric hospitals with a high proportion of mildly ill elderly patients with various conditions(cluster 4; mildly ill elderly patient-centered), and geriatric hospitals with a significantly higher proportion of dementia patients(cluster 5; dementia patient-centered). The largest number of geriatric hospitals were categorized in clusters 4 and 5, and the structure and process indicators for these clusters were generally lower than for the other clusters. Practical Implications: We have confirmed the existence of geriatric hospitals where the medical function, which is the original purpose of a geriatric hospital, has been weakened. It has been observed that the quality level of these geriatric hospitals is likely to be lower compared to hospitals that prioritize enhanced medical functions. Therefore, it is suggested to consider the conversion of these geriatric hospitals into long-term care facilities, and careful consideration should be given to the review of care-giver payment coverage.

가정간호: 위기와 전망 (Home care services: crisis and prospects)

  • The Korean government introduced Home Care Services System to cut medical cost and make efficient use of limited medical resources because of increasing chronic diseases and the growing population of the elderly. The Korean government established measures to control the use of insurance services by restricting the number of nurse's visits to patient's home and by asking the patients to shoulder the transportation fee of nurses during the visit. Factors such as oversupply of hospital facilities, low price of home care services, high insurance coverage for hospital services and increased nuclear family set up resulted in the limited use of home care nursing services. The introduction of long-term care insurance in 2007 brought the decrease in the number of home care agencies and these agencies are facing a crisis today. The increase in chronic diseases and growing population of the elderly recently resulted in the need to control the high medical cost. Home care services for early discharge patients and chronic-severe disease patients will contribute in the reduction of medical cost at the same time improves the quality of patient's life. To catch up with the demands of the nation, accessibility to home care services should be improved and policies such as the expansion of home care services insurance coverage and promotion of establishing home care agencies should be considered.

요양기관별 한국노인의 기능상태와 장기요양등급 비교 (The Comparison of Functional Status and the Level of Health Care Needs in Elderly Koreans in Health Care Institutions)

  • Purpose: The purpose of this study was to compare functional status and the level of health care needs in elderly Koreans in health care institutions. Methods: Data were collected from 2,521 elderly patients admitted in 50 health care institutions (hospitals, oriental hospitals, geriatric hospitals, and health care facilities) selected through proportional stratified sampling in 2008. We used a long-term care (LTC) assessment tool developed by the government, which consists of 52 items with 5 subscales. Results: Statistically significant differences were detected in functional status and the level of health care needs among the subjects in different health care institutions. Of the sample population in geriatric hospitals, 48.9% were eligible for LTC Category 1, 20.9% for Category 2, and 17.2% for Category 3, and 13.1% of the subjects were not eligible for any of the categories. Of the sample population in health care facilities, 29.9% were eligible for LTC Category 1, 20.5% for Category 2, and 21.8% for Category 3, and 27.0% of the subjects were not eligible for any of the categories. Conclusion: The findings of this study indicate the necessity of the development of an evaluation system that helps determine whether a subject is eligible for LTC.

노인의료전문병원의 급식서비스 인카운터에 대한 중요도-만족도 연구 (The Importance-Satisfaction Study of Hospital Foodservice Encounters at the Elderly Health-Care Facilities)

  • Institutional care is often necessary for the health and well-being of the elderly. Good quality foodservice provided at long-term care facilities not only includes patients satisfaction but also cares for good health, contributing to the stability of foodservice management. The purpose of this study was to assess the importance and satisfaction attributes of foodservice management by hospitalized elderly patients. The data were collected via questionnaire by a one-to-one interview with 194 hospitalized elderly patients in six different hospitals. According to the results of dependent t-tests, overall mean scores for the importance attributes (3.96) and satisfactory attributes (3.83) were significantly different (p<0.001). As indicated by the patients, the recognized importance attributes were the kindness of foodservice personnel (4.19), kind smiles by foodservice personnel (4.16), and kind speaking by foodservice personnel (4.12). The most recognized satisfaction attributes were kindness of foodservice personnel (4.36), bedside meal service by foodservice personnel (4.25), kind speaking by foodservice personnel (4.24), kind smiles by foodservice personnel (4.24), and sanitary uniforms worn by foodservice personnel (4.21). These results suggest that the above encounter attributes (importance-satisfaction) would be useful tools for hospital foodservices to adopt, in order to control foodservice quality and satisfy the nutritional needs of elderly patients.

A Program to Treat Hepatitis B in North Korea: A Model of Antiviral Therapy in a Resource-Poor Setting

  • Despite the well-proven, safe and effective therapies for hepatitis B infection, delivery of treatment remains a significant challenge in resource-poor settings. Geopolitical and economic restrictions present additional difficulties in providing care in North Korea. However, treatment of patients with chronic hepatitis B remains a top priority for both the North Korean Ministry of Public Health and international agencies working in North Korean hepatitis healthcare facilities. Working in partnership, a path was created to institute this much-needed program. A consortium of United States and Australian humanitarian non-governmental organizations along with generous individual and corporate donors working in concert with local and national health authorities have succeeded in establishing the first hepatitis B treatment program in North Korea. The essential elements of this program include renovation of existing hepatitis hospitals, access to antiviral medications, establishment of laboratory facilities, creation of medical documentation and record-keeping, training of local health care professionals, and quarterly visits by international volunteer physicians and laboratory experts. Management and treatment decisions are made bilaterally. To date, nearly 1,500 patients have been evaluated, and over 800 have been started on long-term antiviral therapy. It is envisioned that this program will eventually be managed and funded by the Democratic People's Republic of Korea Ministry of Public Health. This program's success demonstrates a potential model for delivery of antiviral therapy for patients suffering from hepatitis B in other developing countries.

노인요양시설 사회복지사의 직무스트레스, 스트레스 대처전략, 공감능력이 소진에 미치는 영향 (The Influence of Job Stress, Stress Coping Strategies, Empathy ability on Burnout of Social Workers in Long-term Care Facilities)

  • 본 연구는 노인요양시설에 근무하는 사회복지사의 직무스트레스, 스트레스 대처전략, 공감능력, 소진간의 관계와 소진에 영향을 미치는 요인을 파악하기 위해 시도되었다. 자료 수집은 3개 도시의 사회복지사 201명을 대상으로 구조화된 설문지를 이용하여 2020년 3월 2일부터 5월 31일 2개월간 시행되었다. 수집한 자료는 기술통계와 t-test, ANOVA, Pearson correlations, 위계적 회귀분석으로 분석하였다. 연구결과 건강증진행위는 일반적 특성 중 연령과 결혼상태, 근무경력에 따라 유의한 차이를 나타냈다. 주요 변수 간의 관계에서 소진은 직무스트레스와 유의한 정적 상관 관계를 보였으며, 스트레스 대처전략 및 공감능력과 각각 유의한 부적 상관관계를 보였다. 소진에 영향을 미치는 요인을 파악하기 위해 위계적 회귀분석을 실시한 결과 직무스트레스와 공감능력이 유의한 영향 요인으로 나타났으며, 설명력은 72.3%로 나타났다. 본 연구를 통해 사회복지사의 소진을 경감시키기 위해서는 직무스트레스를 줄이고 공감능력을 증진시키는 것 필요함을 알 수 있다. 이에 사회복지사의 직무스트레스를 경감시키고 공감능력을 향상시킬 수 있는 소진 예방 프로그램의 개발이 필요할 것으로 생각된다.
