Scar Revision in Patient with Secondary Cleft Lip Deformity Using Straight Line Repair-Report of Case (직선 봉합법을 이용한 편측 구순열의 반흔성형술 증례)

  • In spite of all fine methods developed for treating cleft lip, a certain percentage of cases might still need secondary correction. Generally, secondary revision of cleft lip is much difficult to produce esthetic outcome, because the cleft lip scar gives a variable symmetrical and anatomic defect that may not be possible to make inconspicuous of this scar and rebuild good esthetics. In this case report, a five year-old girl was underwent secondary cleft lip repair using straight line technique to correct unfavorable postoperative scar, peaking of Cupid's bow, notching of vermillion and shortened lip on cleft side with simultaneous repairing cleft palatal fistula. After operation, the secondary deformity was much improved, but, long term follow up is needed to evaluate the additional postoperative deformity might be happen during growth.

  • In this paper, we propose an efficient method for recognizing pupils and lip in a human face. Pupils are detected by a cost function, which uses features based on the eye's shape and a relation between pupil and eyebrow. The inner boundary of lip is detected by weighted vectors based on lip's shape and on the difference of gray level between lip and face skin. These vectors extract four feature points of lip : the top of the upper lip, the bottom of the lower lip, and the two corners. The experiments have been performed for many images and show very encouraging result.

  • The Author had tried to classify lip print patterne of 380 persons aged from 3 to 71 for detecting possibility of individual identification. The results were as follows: 1. The incidency of lip print patterne by means of the Tsuchihashi modifiation method was high in type I, type Ⅱ,typeⅢ,type I`, typeⅣ, typeⅤ, respectively. 2. Any of lip print had no same shape. 3. No same shape of lip print was found even family members. 4. The shapes of lip prints were unchangeable in the period of 5 months.

Tongue-Lip Adhesion Using an Alveolar Protector Appliance for Management of Pierre Robin Sequence (피에르 로빈 연속증의 치료로써 치조 보호 장치를 이용한 혀-하순 유착술)

  • Purpose: Pierre Robin sequence is a congenital malformation in which micrognathia causes glossoptosis and airway obstruction. If conservative treatment fails, surgical procedures such as tongue-lip adhesion can be performed. However, this procedure remains a subject of debate, with favorable results being countered by reports of complications. To overcome the above limitations, we revised the traditional method of tongue-lip adhesion using an alveolar protector. Methods: Between 1992 and 2011, a total of eight patients were identified with Pierre Robin sequence and were treated with tongue-lip adhesion. Two of these eight tongue-lip adhesion procedures were performed with an alveolar protector. The operative technique for tongue-lip adhesion was similar to that described in other published reports. The alveolar protector was inserted between the ventral surface of the tip of the tongue and the lower labial sulcus. Results: Tongue-lip adhesion failed in two patients because of wound dehiscence. The primary surgical success rate was 66.7%. In the two tongue-lip adhesion procedures performed with the alveolar protector, we observed no postoperative complications. Conclusion: Resistance to traction of the tongue can be encountered with nonunionized symphysis menti, causing loosening of the traction suture through the symphysis menti. This can lead to backward positioning of tongue, resulting in dehiscence of tongue lip adhesion. The alveolar protector is a good adjunct to tongue-lip adhesion because this method avoids postoperative loosening of the traction suture and wound dehiscence. It is a simple and effective auxiliary method that yields functional improvement.

Finite Element Analysis of the Contact Stress Characteristics in Scraper Seals (스크레이퍼 실의 접촉응력 특성에 관한 유한요소해석)

  • This paper deals with a numerical study of the tribological contact stress distributions of elastomeric lip seals for oscillating shafts when the sealing interference and band width between the lip ease or contact seals and the shaft are present. Using the finite element method the contact stress and band width of scraper seals are analyzed for the sealing interference including some nonlinearities such as geometrical nonlinearity, material nonlinearity and nonlinear contact boundary condition. The FEM results showed that the contact stress concentrated on the contacting lip zone between the contacting edge of lip and the shaft for the increased interference. In double lip scraper seals, ole maximum contact stress of the dust lip, which is used to exclude foreign contaminants is six times higher than that of the primary sealing lip, which is used to contain lubricants.

  • The harmonious lip length, residual scar and Cupid's bow width and peak with a normal side are the aims of a unilateral cleft lip repair. Also, long term amelioration without necessary of revisional surgeries may be the ideal conditions. No one method can satisfy the wide varieties of cleft lip deformities. Recently with rearrangement of paraoral muscle and some modifications, a straight line repair technique has been concerned again. Straight scar line, simplicity, and short learning curve are the advantages of the straight line technique. Here two cases of the simple straight line technique were presented and discussed for its usefulness and reliability with short reviews of previous reports.

Cloning, Expression, and Purification of a Lipase from Psychrotrophic Pseudomonas mandelii (Pseudomonas mandelii의 lipase 유전자 클로닝, 발현 및 정제)

  • A gene encoding a lipase, lipT, was cloned from the psychrotrophic bacterium Pseudomonas mandelii and sequenced. An open reading frame of 1,686 bp was found that encodes a polypeptide consisting of 562 amino acid residues. Sequence analysis revealed a Gly-His-Ser-Leu-Gly sequence, which matches the consensus Gly-X-Ser-X-Gly motif conserved among lipolytic enzymes. The recombinant LipT protein was predominantly expressed as inclusion bodies in Escherichia coli and subsequently purified by nickel-chelate affinity chromatography. A small fraction of LipT was refolded, and the subsequent LipT exhibited substrate preferences for p-nitrophenyl butyrate (C4) and p-nitrophenyl octanoate (C8).

One Stage Correction of the Severe Secondary Cleft Lip Nasal Deformities in Foreigners (외국인에서 발생한 심각한 이차 입술갈림코변형에 대한 한 단계 수술)

  • Purpose: It is accepted universally that correction of the cleft lip nasal deformity requires multiple stages of surgery. Following primary lip repair in infancy or early childhood, secondary surgery to improve the deformity of the lip and nose is frequently necessary. A suitable surgical procedure to correct the accompanying deformity, such as cleft palate and alveolus, must be carried out at an appropriate age. In developing countries, it is common for patients with cleft lip nasal deformity to present severe secondary deformities in adolescence, because of poor follow-up and inappropriate surgery. Methods: The first patient was a 12 year old Mongolian boy. He presented prominent lip scar, short lip, wide columella, asymmetric nostril, palatal fistula, cleft alveolus, and velopharyngeal incompetence. He underwent cheilorhinoplasty, transpositional flap, alveoloplasty by iliac bone graft, and sphincter pharyngoplasty. On follow-up, a bilateral maxillary hypoplasia and a class III malocclusion developed. He underwent LeFort I osteotomy and maxillary advancement at the age of 16 years. The second patient was an 18 year old Eastern Russian girl. She presented with a deviated nose, right alar base depression, short lip, protrusion on vermilion, large palatal fistula, and severe VPI due to short palate. She underwent the combined procedure of cheilorhinoplasty, corrective rhinoplasty, tongue flap for palatal fistula, and superiorly based pharyngeal flap. And the tongue flap was detached at postoperative 3 weeks. Results: The overall results have been extremely pleasing and satisfactory to patients. There were no postoperative complications. Conclusion: We discovered the one stage operation for radical correction was sufficient procedure to provide excellent clinical outcomes in patients with severe cleft lip nose deformity.

Support Vector Machine Based Phoneme Segmentation for Lip Synch Application

  • In this paper, we develop a real time lip-synch system that activates 2-D avatar's lip motion in synch with an incoming speech utterance. To realize the 'real time' operation of the system, we contain the processing time by invoking merge and split procedures performing coarse-to-fine phoneme classification. At each stage of phoneme classification, we apply the support vector machine (SVM) to reduce the computational load while retraining the desired accuracy. The coarse-to-fine phoneme classification is accomplished via two stages of feature extraction: first, each speech frame is acoustically analyzed for 3 classes of lip opening using Mel Frequency Cepstral Coefficients (MFCC) as a feature; secondly, each frame is further refined in classification for detailed lip shape using formant information. We implemented the system with 2-D lip animation that shows the effectiveness of the proposed two-stage procedure in accomplishing a real-time lip-synch task. It was observed that the method of using phoneme merging and SVM achieved about twice faster speed in recognition than the method employing the Hidden Markov Model (HMM). A typical latency time per a single frame observed for our method was in the order of 18.22 milliseconds while an HMM method applied under identical conditions resulted about 30.67 milliseconds.

A Study of the Pattern Kernels for a Lip Print Recognition

  • This paper presents a lip print recognition by the pattern kernels for a personal identification. A lip print recognition is developed less than the other physical attributes of a fingerprint, a voice pattern, a retinal blood/vessel pattern, or a facial recognition. A new method is proposed to recognize a lip print bi the pattern kernels. The pattern kernels are a function consisted of some local lip print pattern masks. This function converts the information on a lip print into the digital data. The recognition in the multi-resolution system is more reliable than recognition in the single-resolution system. The results show that the proposed algorithm by the multi-resolution architecture can be efficiently realized.
