The Effect of Isometric Hip Adduction and Abduction on the Muscle Activities of Vastus Medialis Oblique and Vastus Lateralis during Leg Squat Exercises (쪼그려 앉기(Leg Squat) 운동 시 등척성 고관절 내·외전이 내·외측광근의 근 활성도에 미치는 영향)

  • The purpose of this study was to investigate the effect of isometric hip adduction and abduction on the muscle activities of vastus medialis oblique(VMO) and vastus lateralis(VL) during leg squat exercises. This study consisted of 21 healthy subjects who had no medical history of anterior knee pain or lower extremity disorders. The ball and belt were used to isometrically adduct and abduct the hip joint during the leg squat exercise, respectively. The surface electromyograms of VMO and VL were analyzed, and the findings were used to calculate the VMO:VL ratio during 3 different quadriceps-strengthening exercises(leg squat, LS leg squat with isometric hip adduction, LSHD leg squat with isometric hip abduction, LSHB). The muscle activities of VMO and VL and the VMO:VL ratios were compared using the paired t-test with Bonferroni adjustment. The results showed that the muscle activities of VMO and VL during LSHD were greater than those during LSHB. The VMO:VL ratio was the highest during LSHD. This finding suggests that LSHD using a ball is more effective than LS and LSHB in selectively increasing the muscle activities of VMO. Therefore, we suggest that leg squat exercise with isometric hip adduction using a ball would be useful for maintaining correct patella tracking and for selectively strengthening VMO.

  • This study was conducted to compare the PNF and Trunk Exercise Program has effectiveness on the pain, disability, static and dynamic balance in chronic low back pain patients. To evaluate the effects of exercise, subjects were evaluated by using VAS for pain, Oswestry disability index for disability, One leg stand test for static balance and Balance System for dynamic balance test. Pain, Oswestry disability index, One leg stand test, and Dynamic balance test in PNF had statistical significance than Trunk exercise program(p<0.05). So this results were suggested that the lifting and chopping pattern of PNF was effective on pain, disability and balance in chronic low back pain patients.

The Effect of Cigarette Smoking on Physical Fitness and Depression of Patients with Chronic Low Back Pain

  • Purpose: This study was conducted to investigate the effect of cigarette smoking on physical fitness (dominant hand grip power, dominant isokinetic leg muscle strength, abdominal muscle endurance, flexibility, cardiopulmonary endurance) and depression in patients with chronic low back pain. Methods: This study was a cross sectional study and subjects consisted of 60 young males with chronic low back pain. The subjects were allocated to two groups following a self-report survey: cigarette smoking group (n=25) or non-smoking group (n=35). Physical fitness (dominant hand grip power, dominant isokinetic leg muscle strength, abdominal muscle endurance, flexibility, cardiopulmonary endurance) were measured using objective methods and depression in patients with chronic low back pain was measured using the Korean version of center for epidemiologic studies depression (CES-D) scale. Results: The results of this study were as follows: In physical fitness, the cigarette smoking group showed a significant decrease in abdominal muscle endurance, flexibility, and cardiopulmonary endurance compared with the non-smoking group. Depression index (CES-D scale score) was significantly higher in the cigarette smoking group than in the non-smoking group. Conclusion: These results suggest that cigarette smoking had a negative effect on abdominal muscle endurance, flexibility, and cardiopulmonary endurance in patients with chronic low back pain. In addition, depression of patients with chronic low back pain was affected by cigarette smoking. Thus, we suggested that cigarette smoking may play a significant role in the deterioration of physical fitness and depression of chronic low back pain patients.

  • Objectives: This study was to report the effect of Hyeolbuchukyeo-tang(HC) on complications of post-hysterectomy. Methods: Three women who were undertaken a hysterectomy within a month were enrolled in this study. They complained of abdominal pain, lumbago, bladder pain, tinnitus, both leg pain, headache, etc. And received oriental treatments to decrease pain and improve the symptoms. They took Hyeolbuchukyeo-tang 3 times a day. Results: A hysterectomy can lead to complications. They complained of various symptoms. After the treatment, abdominal pain, bladder pain, both leg pain, headache were rapidly decreased and lumbago, tinnitus were improved too. Conclusions: These clinical cases indicated that Hyeolbuchukyeo-tang was effective in the treatment of women who undertaken a hysterectomy.

Effect of Single-injection Femoral Nerve Block Using Real-time Ultrasound on the Postoperative Pain after Bilateral Total Knee Arthroplasty: 0.25% vs 0.5% Levobupivacaine (초음파 유도 하 일회성 넙다리신경블록이 슬관절 전치환술 후 통증에 미치는 영향: 0.25% Levobupivacaine과 0.5% Levobupivacaine 주입의 비교)

  • Background: The purpose of this study was to compare the analgesic effect of 0.25% and 0.5% levobupivacaine for real time ultrasound guided single-injection femoral nerve block for the patients who are undergoing bilateral total knee arthroplasty (TKA). Methods: Femoral nerve block was done to all patients with 20 ml of 0.9% normal saline on one leg and 20 ml of 0.25% levobupivacaine on the other leg for group I (n = 16) and 0.5% levobupivacaine for group II (n = 15) with 1:200,000 epinephrine and using real-time ultrasound and a nerve stimulator. The data concerning the verbal numerical rating scale (VNRS) for each leg, the consumption of the intravenous patient-controlled analgesia (IV PCA) and the demands for the additional analgesics was collected at 0, 1, 6, 12, 24 and 48 hours after the operation. Results: The legs on which femoral nerve block was done with levobupivacaine showed a lower VNRS score than the legs with normal saline in either group I or group II. The VNRS scores between the two legs, the consumption of the IV PCA and the demand for additional analgesics showed no significant differences between the groups. Conclusions: Our results demonstrate that single-injection femoral nerve block using real-time ultrasound with either 0.25% levobupivacaine or 0.5% levobupivacaine 20 ml provides a good effect for the postoperative pain control after TKA.

  • Discal cyst is a very rare lesion that can result in refractory low back pain and leg radiating pain. Because they are so uncommon, their exact origin and pathophysiology are still unknown. A 31-year-old man visited our institute due to low back pain and severe left leg radiating pain. Magnetic resonance images [MRI] revealed spherically shaped extradural cystic lesion at L2-L3 level. Computed tomography [CT] discography demonstrated obvious communication between the intervertebral disc and the cyst. The patient underwent posterior decompression and excision of cyst. The symptoms were remarkably improved immediately after surgery.

Electrical Acupuncture Combined Bee Venom Therapy for Pain and Disability Induced Intervertebral Herniated Disc of L-spine;A Pilot Study

  • To examine the efficacy of electrical acupuncture combined bee venom in controlling low back with leg pain induced Herniated Intervertebral Disc (HIVD). 13 patients (men 8, women 5; age $41.30\;{\pm}\;17.27$) diagnosed as HIVD by CT or MRI had low back and leg pain as a chief complaint and diseases such as spinal tumor, infection and fracture were excluded. They were treated by electrical acupuncture (8 Hz) combined bee venom therapy three times a week in Wonkwang University Hospital, and pain intensity with VAS and functioning with Roland-Morris Disability Qestionnaire(RDQ) were measured at the baseline(B), and 4 weeks after treatment. The data were analyzed with Wilcoxon signed ranks test(SPSS12.0) and the level set for statistical significance was P<0.05. The pain was significantly reduced by two combined treatments (=0.002;$B7.69{\pm}2.06$,4Wks$4.70{\pm}2.72$). The function was also significantly improved by both treatments(=0.001;$B12.15{\pm}6.85$,4Wks$6.85{\pm}5.15$). The results suggest that a combination of acupuncture and bee venom therapy may be beneficial for low back pain from HIVD. In future, randomized clinical trials including follow-up are needed to examine the effects of these therapies.

  • Objectives : Yogaktong is the general term of lowback and sciatic pain. Simplely this is not for the syndrom or illiness but for the symptoms. In the field of western medicine, it's difficult to find out the cause and to be objective when the patient feel back and sciatic pain without HNP of L-spine. Methods : To get objective yogaktomg symptoms I did this study. This is the case study of 103 patients who felt yogaktomg, and had taken DITII(Digital Infrared Thermal Imaging) at department of acupuncture and moxibustion in Conmaul Oriental medical hospital from september 1, 1998 to august 30, 1999. Results : The temperature raised 53.4% at center of lowback. There is a temperature differance on both legs, in 70.73% of patients the pain side of leg is lower temperature than the other side. Conclusions : Generally, DITI of patients who have lowback and sciatic pain is shown increasing temperature in center of lowback, decreasing temperature in pain side of leg.

Long-term consistency of clinical sensory testing measures for pain assessment

  • Background: Understanding the stability of quantitative sensory tests (QSTs) over time is important to aid clinicians in selecting a battery of tests for assessing and monitoring patients. This study evaluated the short- and long-term reliability of selected QSTs. Methods: Twenty healthy women participated in three experimental sessions: Baseline, 2 weeks, and 6 months. Measurements included pressure pain thresholds (PPT) in the neck, upper back, and leg; Pressure-cuff pain tolerance around the upper-arm; conditioned pain modulation during a pressure-cuff stimulus; and referred pain following a suprathreshold pressure stimulation. Intraclass correlation coefficients (ICC) and minimum detectable change (MDC) were calculated. Results: Reliability for PPT was excellent for all sites at 2 weeks (ICC, 0.96-0.99; MDC, 22-55 kPa) and from good to excellent at 6 months (ICC, 0.88-0.95; MDC, 47-91 kPa). ICC for pressure-cuff pain tolerance indicated excellent reliability at both times (0.91-0.97). For conditioned pain modulation, reliability was moderate for all sites at 2 weeks (ICC, 0.57-0.74; MDC, 24%-35%), while it was moderate at the neck (ICC, 0.54; MDC, 27%) and poor at the upper back and leg at 6 months. ICC for referred pain areas was excellent at 2 weeks (0.90) and good at 6 months (0.86). Conclusions: PPT, pressure pain tolerance, and pressure-induced referred pain should be considered reliable procedures to assess the pain-sensory profile over time. In contrast, conditioned pain modulation was shown to be unstable. Future studies prospectively analyzing the pain-sensory profile will be able to better calculate appropriate sample sizes.

Clinical Outcomes of Percutaneous Endoscopic Surgery for Lumbar Discal Cyst

  • Objective : Discal cyst is rare and causes indistinguishable symptoms from lumbar disc herniation. The clinical manifestations and pathological features of discal cyst have not yet been completely known. Discal cyst has been treated with surgery or with direct intervention such as computed tomography (CT) guided aspiration and steroid injection. The purpose of this study is to evaluate the safety and efficacy of the percutaneous endoscopic surgery for lumbar discal cyst over at least 6 months follow-up. Methods: All 8 cases of discal cyst with radiculopathy were treated by percutaneous endoscopic surgery by transforaminal approach. The involved levels include L5-S1 in 1 patient, L3-4 in 2, and L4-5 in 5. The preoperative magnetic resonance imaging and 3-dimensional CT with discogram images in all cases showed a connection between the cyst and the involved intervertebral disc. Over a 6-months period, self-reported measures were assessed using an outcome questionaire that incorporated total back-related medical resource utilization and improvement of leg pain [visual analogue scale (VAS) and Macnab's criteria]. Results : All 8 patients underwent endoscopic excision of the cyst with additional partial discectomy. Seven patients obtained immediate relief of symptoms after removal of the cyst by endoscopic approach. There were no recurrent lesions during follow-up period. The mean preoperative VAS for leg pain was $8.25{\pm}0.5$. At the last examination followed longer than 6 month, the mean VAS for leg pain was $2.25{\pm}2.21$. According to MacNab' criteria, 4 patients (50%) had excellent results, 3 patients (37.5%) had good results; thus, satisfactory results were achieved in 7 patients (87.5%). However, one case had unsatisfactory result with persistent leg pain and another paresthesia. Conclusion : The radicular symptoms were remarkably improved in most patients immediately after percutaneous endoscopic cystectomy by transforaminal approach.
