Sural Nerve Entrapment and Tenosynovitis of Peroneus Longus by Hypertrophied Peroneal Tubercle: A Case Report (비후된 비골 결절에 의해 발생한 비복신경 포착 및 장비골건의 건막염: 증례 보고)

  • A hypertrophied peroneal tubercle can present as a bony prominence at the lateral aspect of the foot and a peroneal tenosynovitis or tear. We report a case of a 52-year-old man complaining of lateral foot tingling pain and numbness. The sural nerve entrapment and peroneus longus tenosynovitis by hypertrophied peroneal tubercle were confirmed. Good results were obtained after excision of the hypertrophied peroneal tubercle and sural nerve release.

  • A room-corner fire scenario of ISO 9705 with flame spread model developed by Quintiere is applied to the interior finish materials to show the sensitivity of properties derived from AST, E-1321 and ASTM E-1354 is investigated and various range of thermal properties by the author were analyzed in the model. There are including flame heat flux and thermal inertia, lateral flame spread parameter, heat of combustion and effective heat of gasfication. The time for total energy release rate to reach 1MW is examined. The Effects of Taping, AMCT, Combination Treatment on the Pain and Grip Strength in Patient with Lateral Epicondylitis (테이핑, AMCT, 복합치료가 외측상과염 환자의 통증과 악력에 미치는 효과)

  • Purpose : The Purpose of this study was to investigate the effects of taping and AMCT(activator methods chiropractic technique) on the pain and grip strength in patients with lateral epicondylitis. Methods : To study divided of three groups. Taping groups were apply to taping was attached from wrist joint to elbow joint lateral epicondly after maximun flexion. AMCT application was contact the posterior aspect of the proximal head of the radius. The line of drive is anterior and inferior. Next, contact the anterior aspect of the lunate by positioning the instrument on the volar aspect of the wrist. The line of drive is straight posterior. combination groups was treated using the taping and AMCT application. Result : 1. The pain was decreased on taping groups, AMCT groups and combination groups of all(p<.05). 2. Power grip was enhanced in taping groups, AMCT and combination groups(p<.05). but taping, AMCT, combination groups was no difference compared with after 1 week therapy(p>.05). 3. Compared with AMCT and taping therapy about pain decreased was AMCT groups better than taping groups(p<.05). 4. Compared with AMCT and taping therapy about power grip was AMCT groups better than taping groups(p<.05). 5. AMCT and combination groups was no difference compared with pain release and power grip after therapy(p>.05). Conclusion : AMCT groups therapy are more effect able than taping therapy for grip strength and pain reduction. Hence, AMCT groups therapy is most effect able for pain reduction with lateral epicondylitis patients.

Study on the improvement of microdialysis method for measuring brain amino acids in systemic morphine treated rats (Morphine을 전신투여한 랫드의 뇌에서 분비되는 amino acid 성 신경전달물질 측정을 위한 미세투석법의 개선에 관한 연구)

  • In the present study, we designed and constructed new microdialysis probe in order to improve the efficacy and accuracy of microdialysis method. In addition, extracellular concentrations of GABA, glutamate, aspartate and glycine were monitored with new designed probe in the lateral portion of the ventrocaudal periaqueductal gray using unanesthetized and unrestrained rats. Furthermore, the effect of opiates on release of these amino acids, especially GABA, was analyzed by measuring their concentration in PAG dialysates following veratridine administration in the presence of systemic morphine. The results were summerized as follow : 1. The damaging rates of 1.0mm or 1.5mm window probe were 12.5% or 42.8%, respectively. In the group using 1.5mm window probe, the damaging area was extended into mesencephalic aqueduct because of microdialyzing pressure. 2. Because of the unique design of our probes with an opening facing one side, dialysis occurs in a hemisphere($600{\mu}m$ in mediolateral direction and $100{\mu}m$ in opposite side of the dialysis probe) around the opening rather than in a spherical shaped configuration which is typical of most commercially available probe designs. 3. Glutamate, taurine and glycine were present in the highest concentration in the dialysate sample obtained before treatment with veratridine, whereas, aspartate and GABA were present in the lowest concentration. 4. The concentration of all 5 amino acids increased significantly following $75{\mu}m$ veratridine perfusion into lateral ventrocaudal PAG. 5. There was no significant difference between basal and peak amino acid concentrations according to window sizes. 6. Morphine had no effect on baseline concentrations of amino acids in dialysates obtained from the lateral PAG as compared to saline treated controls. However, following veratridine treatment, morphine selectively affected GABA release in the lateral ventrocaudal PAG as compared to saline treated controls. These results suggest that GABAergic interneurons in the PAG are inhibited by opioids. Therefore, endogenous enkephalins or endorphins may directly inhibit intrinsic GABAergic intemeurons and block their tonic inhibition of PAG-NMR projection neurons. Moreover, new designed probes demonstrate improved efficiency and accuracy in collecting samples as compared to commercial types of microdialysis probes.

Neuroprotective Effects by Nimodipine Treatment in the Experimental Global Ischemic Rat Model: Real Time Estimation of Glutamate

  • Objective: Glutamate is a key excitatory neurotransmitter in the brain, and its excessive release plays a key role in the development of neuronal injury. In order to define the effect of nimodipine on glutamate release, we monitored extracellular glutamate release in real-time in a global ischemia rat model with eleven vessel occlusion. Methods: Twelve rats were randomly divided into two groups: the ischemia group and the nimodipine treatment group. The changes of extracellular glutamate level were measured using microdialysis amperometric biosensor, in coincident with cerebral blood flow (CBF) and electroencephalogram. Nimodipine (0.025 ${\mu}g$/100 gm/min) was infused into lateral to the CBF probe, during the ischemic period. Also, we performed Nissl staining method to assess the neuroprotective effect of nimodipine. Results: During the ischemic period, the mean maximum change in glutamate concentration was $133.22{\pm}2.57\;{\mu}M$ in the ischemia group and $75.42{\pm}4.22\;{\mu}M$ (p<0.001) in the group treated with nimodipine. The total amount of glutamate released was significantly different (P<0.001) between groups during the ischemic period. The %cell viability in hippocampus was $47.50{\pm}5.64$ (p<0.005) in ischemia group, compared with sham group. But, the %cell viability in nimodipine treatment group was $95.46{\pm}6.60$ in hippocampus (p<0.005). Conclusion: From the real-time monitoring and Nissl staining results, we suggest that the nimodipine treatment is responsible for the protection of the neuronal cell death through the suppression of extracellular glutamate release in the 11-VO global ischemia model of rat.

Effects of High-frequency Diathermy Integrated into Suboccipital Release on Tenderness and Neck Mobility and Disability in People with Chronic Tension-type Headache (뒤통수밑근 이완기법과 병행된 고주파 심부투열이 만성 긴장성 두통 환자들의 압통과 목 가동성 및 기능장애에 미치는 영향)

  • Background: Active trigger points (TrPs) of the suboccipital muscles greatly contribute to the occurrence of chronic tension-type headache, with increased sensitivity of TrPs and facilitated referred pain. Objects: This study aimed to investigate whether the integration of high-frequency diathermy into suboccipital release is more beneficial than the use of suboccipital release alone. Methods: Thirty subjects were assigned to either experimental group-1 (EG-1) to undergo suboccipital release combined with high-frequency diathermy (frequency: 0.3 MHz, and electrode type: resistive electronic transfer), or EG-2 to undergo suboccipital release alone, or the control group (CG) with no intervention, with 10 subjects in each group. The assessment tools included the headache impact test 6 (HIT-6), perceived level of tenderness, neck disability index, and neck mobility. Intervention was performed for 10 minutes, twice per week, for 4 weeks, and measurements were performed before and after the interventions. Results: The between-group comparison of the post-test values and changes between pretest and post-test showed significant differences for all parameters at p<.05, except for the left-to-right lateral bending range. In the post hoc test, EG-1 showed significant differences for the parameters in comparison with the CG, while no significant differences in the perceived tenderness level, on both temporal regions, were found between EG-2 and CG. Furthermore, the HIT-6 score and perceived tenderness level, in the right temporal region, showed significant differences between EG-1 and EG-2. In the within-group comparison, EG-1 and EG-2 appeared to be significantly different between pretest and post-test (p<.05), except for the perceived tenderness level in the right temporal region, with significance for the EG-1 group only (p<.05). Conclusion: These findings suggest that the suboccipital release technique may be advantageous to improve headache, tenderness, and neck function and mobility, with more favorable effects with the incorporation of high-frequency diathermy.

Effect of Balance before and after Impact on the Velocity and Angle of Golf Club during Driver Swing (골프 드라이버 스윙 시 임팩트 전·후 신체 균형성이 클럽헤드의 속도와 각도에 미치는 영향)

  • The purpose of this investigation was to determine whether correlations exist between balance and impact velocity, angular position, and maximum velocity of a club during drive swing. Twelve skilled golfers were recruited in this study. They were asked to perform ten swing trials and two trials were selected for analysis. Balance parameters were calculated via the force platform while kinematic variables were determined by using the Qualisys system. The results of the present study demonstrated that the average of COP velocity was faster in the medio-lateral direction rather than the anterio-posterior direction. Also, left foot's COP velocity and free torque were greater than the right foot's before impact. The range of the right foot's COP in the anterio-posterior direction before impact were correlated with the club velocity and angular position at impact. There was a negative correlation between the left foot's COP velocity before the impact and the velocity at impact. Additionally, the range and RMS of the left foot's free torque affected on the club angular position at impact and the maximum velocity at release, respectively. Finally, a negative correlation existed between the range of the right foot's free torque after the impact and club's maximum velocity at release.

The Effect of Meridian Tendino-musculature Acupuncture and Release Therapy on acute lumbar Sprain (요부(腰部) 경근(經筋)의 급성(急性) 염좌(捻挫)에 경근자침(經筋刺鍼) 및 경근이완요법(經筋弛緩療法)이 미치는 영향(影響))

  • Objective : To broaden understanding about relationship between Meridian Tendino-musculature and muscles in a lumbar area and to evaluate the effect of Meridian Tendino-musculature acupuncture and release therapy on acute lumbar sprain. Materials and Methods : From Oct. 1st, 2000 to Mar. 31th, 2001, 692 outpatient's chart of Kyung Won University Hospital were reviewed. Out of them, 39 outpatients were selected. they had low back pain, were diagnosed with acute lumbar sprain, showed only straightened curvature on lateral view of lumbar spine X-ray and get the Tendino-musculature acupuncture and release therapy. Results : 1. On patients' first visit, 72% had GrIII and Gr.IV predominantly. 2. Major muscles related with low back pain were divided into two groups. One was Quadratus lumborum group and the other was Rectus abdominis group. In the correlation with Meridian Tendino-musculature, the former was mainly related with Chok-taeyang(B) and slightly related with Chok-soyang(G), Chok-taeum(SP), the latter was mainly related with Chok-taeyum(SP), Chok-yangmyong(S) and slightly related with Chok-taeyang(B), Chok-soyang(G). 3. In the evaluation of treatment effect, Exellent was 27(69%), Good was 10(6%), Fair was 2(5%) and Bad was 0(0%). Fair rate reached 100% eventually. 4. Most of Gr.Ⅲ, Gr.Ⅳ patients who had severe conditions that almost every R.O.M. was limited and Milgram test positive was shown on the physical examination, were fully recovered and lived normal daily life without admission by Meridian Tendino-musculature acupuncture and release therapy within 3 to 5, 4 to 7 days, respectively, since they had started to get their outpatient treatment. 5. Two patients was troubled with pain induced, by twitching response and acupuncure stimuli, which lasted around acupunctured muscle for about a day after treatment, so they coudn't endure the pain and quitted treatment in spite of Fair condition. conclusion : Meridian Tendino-musculature acupuncture and release therapy was found to be helpful to patients who wish to recover from their back pain induced by acute lumbar sprain as soon as possible, but the treatment actually had some problems to be overcome such as pain during or after the treatment. therefor, in order to make this treatment method more available, we should pay more attention to improving treatment appliance and acupuncture technique.

A Fire Hazard Assessment of Interior Finish Materials (건물 내장재의 화재위험성 평가 방법)

  • To propose a new fire hazard assessment criteria of interior finish materials, the properties and incident heat flux of interior finish materials in a compartment fires are investigated and compared by using flame spread model developed by Quintiere. The properties considered on which fire growth depend are including flame heat flux and thermal inertia, lateral flame spread parameter, heat of combustion and effective heat flux and thermal inertia, lateral flame spread parameter, heat of combustion and effective heat of gasfication. ISO Room Corner Test(9705) is applied in the model and the time for total energy release rate to reach 1MW is examined. The results are compared for the 24 different materials tested by EUREFIC. Dimensionless parameter a, b and ${\gamma}$b are used to develope a new method in which fire hazard of interior finish materials can be classified resulting from correlation between b and flashover time. Results show that if b greater than about zero, flashover time in the ISO Room-Corner Test is principally proportional to ignition time only.

  • The earth pressure coefficient at rest for clayey soils in the one-dimensional state, $K_0$ obtained from the triaxial test is not correct in principle because the seepage flow is radial and the displacement of soil elements is three-dimensional. Measurements of the earth pressure and the pore water pressure during one-dimension consolidation in the consolidometer ring are presented. The earth pressure and pore water pressure are measured directly by a circular part of the consolidometer ring of a floating type at its mid height. A plastic clay showed $K_0$=0.5 irrespective of pressure in the consolidometer ring.
