Comparison of the Level and Side Effects of Spinal Anesthesia with Hyperbaric Bupivacaine in the Supine, Lateral, and Prone Positions (고비중 Bupivacaine 척추마취 후 체위에 따른 마취수준과 부작용의 비교)

  • Purpose: This study attempted to test whether there are differences in the level and hemodynamic side effects (blood pressure, heart rate, $O_2$ saturation), and nausea & vomiting of spinal anesthesia using hyperbaric bupivacaine according to position (supine, lateral, and prone positions) in orthopedic surgery patients who received podiatric surgery under spinal anesthesia. Methods: This study was conducted with 53 patients who had received orthopedic surgery under spinal anesthesia at I General Hospital. Data were analyzed using SPSS 20.0 through repeated-measures ANOVA, post-hoc test, Chi-test, and Fisher's exact test. Results: The change of position after spinal anesthesia with hyperbaric bupivacaine caused a change in the level of spinal anesthesia (F=12.768, p<.001). However, no difference of blood pressure, heart rate, $O_2$ saturation and nausea and vomiting caused by the change in anesthesia level was observed, and in prone position, drug was administered for the correction of side effects. Conclusion: As expected, recognizing that there can be a change in the level of spinal anesthesia after the change of position in surgical patients, nurse anesthetists should monitor their conditions carefully and continuously.

  • In cases with maxillary lateral incisor missing, the problems were usually treated by the choice between space opening for prosthetic lateral incisor replacement and space closure with cuspid sub stitution for the alteral insicors. The decision of the choice could be perplexing in evaluation of the individual case. Therefore, various diagnostic criteria, such as occlusion, dental esthetics, canine position and inclination, tooth size relationship, maxillary lip length, and skeletal relation had to be evaluated. On the basis of this diagnostic information, the treatment planning could be established. A case was shown to illustrate the treatment of patient with congenitally maxillary lateral incisor missing.

  • Pulmonary bullae were diagnosed in 2 dogs with respiratory distress at Veterinary Medical Teaching Hospital, Seoul National University. Radiographically, thin-walled, fluid-gas leveled bullae in standing lateral positioning and moderately thick-walled bullae in lateral positioning were identified in case 1 and 2, respectively.

  • An interferometric microscope with an improved lateral resolution is presented. The nanometer resolution XY stage is integrated into standard temporal phase shifting interferometer. The nanometer resolution XY stage is used to position specimen in subpixel of CCD detector, therefore CCD detector's sampling is improved. Two scanning algorithms and those simulation results are also presented. The simulation results show that scanning algorithms improve CCD detector's sampling significantly, and interferometeric microscope's lateral resolution is improved also.

Pain of Industrial Workers with Chronic Lateral Epicondylitis and Changes in Joint Position Sense according to Proprioception Improvement Training (고유수용성감각 증진 훈련에 따른 산업체 만성 외측상과염 환자의 통증 및 관절위치 감각의 변화)

  • This study performed a proprioception improvement training for recurrence prevention of industrial workers with chronic lateral epicondylitis and analysed their pain and proprioception. The subjects of the study were thirty chronic lateral epicondylitis patients working for K company and were divided into elbow exercise program Group made of 15 subjects and proprioception improvement training Group made of 15. As variables for pain, pressure pain, and grip strength were measured and for the propioception, joint position sense of each group was measured before and after the exercise program. To compare differences before and after the program, a paired t-test was used and to compare differences between both groups, an independent samples t-test was used. As a result of the test, it was discovered that pressure pain, grip strength and joint position sense of the both groups were improved statistically significantly. These were statistically differential effect between each group on pressure pain, grip strength and joint position sense. In conclusion, it is suggested that proprioception improvement training should be considered as a method chronic and recurrence prevention lateral epicondylitis of industrial workers.

AN ELECTROMYOGRAPHIC STUDY OF THE MASSETER MUSCLES IN CHILDREN WITH SPACE MAINTAINER (보극장치(保隙裝置) 장착아동(裝着兒童)의 교근활성도(咬筋活性度)에 관(關)한 근전도학적(筋電圖學的) 연구(硏究))

  • Electromyographic studies were performed on the action of the masseter muscles. Among the children aged from 6 to 8 years, ten children with normal occlusion and another ten with space maintainer were selected. The children were divided into 3 groups. They were: 1) A group: Children who removed space maintainer 2) B group: Children who inserted space maintainer 3) C group: Children with normal occlusion. The electromyogram was recorded with 4 channel polygraph. (Grass model VII) Electrodes which were cup-typed gold disks, 9 millimeters in the diameter, were located on the superficial layer of masseter muscles. The electromyogram was recorded in the physiologic rest position, molar occlusion, chewing movement, protraction, left lateral movement, and right lateral movement. The conclusions were as follows. 1. In the physiologic rest position, lateral movement, the electrical potentials of the masseter muscles were not changed clearly in each groups. 2. In molar occlusion, chewing movement, The electrical potentials of the masseter muscles of the B group were almost 10% higher than those of A group, and were almost 60-70% in comparision with C group. 3. In protraction, the electrical potentials of the masseter muscles of the B group were almost 40% higher than those of A group, and were almost equal to C group. 4. The electrical activities of the masseter muscles in the mandibular movements were in the following order: (1) Molar occlusion (2) Chewing movement (3) Protraction (4) Lateral movement.

  • Purpose : To evaluate the reliability and clinical usefulness of ultrasonography in the temporomandibular joint (TMJ). Materials and Methods : Parasagittal and paracoronal 1.5 T MR images and 7.5 MHz ultrasonographs of 40 TMJs in 20 asymptomatic volunteers were obtained. Disc position using MR imaging was evaluated and the distance between the lateral surface of mandibular condyle and the articular capsule using MR image and ultrasonograph of 27 TMJs with normal disc position was measured and compared. Intraobserver and interobserver measurements reliability was evaluated by using interclass correlation coefficients (ICC) and measurement error. Also, the distance measured on ultrasonographs was compared, according to mouth position and disc postion. Results : The normal disc position was found in 27 of 40 asymptomatic joints. At the intraobserver reliability of measurement, ICC at the closed and open mouth position were 0.89 and 0.91. The measurement error was 0.4% and 0.5%. At the interobserver reliability, ICC at the closed and open mouth position were 0.92 and 0.81. The measurement error was 0.4% and 0.7%. At the TMJ with normal disc position, the distances between the lateral surface of mandibular condyle and the articular capsule measured on MR images and ultrasonographs were $2.0{\pm}0.7mm,\;1.8{\pm}0.5mm$, respectively (p<0.05). On the ultrasonographs, the distances at open mouth position were $1.2{\pm}0.5mm$ (p<0.05). At the TMJ with medially displaced disc, the distances at the closed and open mouth position were $1.3{\pm}0.3\;mm\;and\;0.9{\pm}0.2\;mm$ (p<0.05). Conclusion : The results suggest ultrasonography of TMJ is a reliable imaging technique for assessment of normal disc position.

  • This paper describes position estimation algorithm using neural network for the navigation of the vision-based Wheeled Mobile Robot (WMR) in a corridor with taking ceiling lamps as landmark. From images of a corridor the lamp's line on the ceiling in corridor has a specific slope to the lateral position of the WMR. The vanishing point produced by the lamp's line also has a specific position to the orientation of WMR. The ceiling lamps has a limited size and shape like a circle in image. Simple image processing algorithms are used to extract lamps from the corridor image. Then the lamp's line and vanishing point's position are defined and calculated at known position of WMR in a corridor To estimate the lateral position and orientation of WMR from an image, the relationship between the position of WMR and the features of ceiling lamps have to be defined. Data set between position of WMR and features of lamps are configured. Neural network are composed and teamed with data set. Back propagation algorithm(BPN) is used for learning. And it is applied in navigation of WMR in a corridor.

A STUDY ON THE LATERAL CEPHALOMETRIC TOMOGRAPHY OF TMJ ARTHROSIS (악관절증의 측방두부계측 단층방사선학적 연구)

  • The author obtained individualized lateral cephalometric tomograms from 23 young adults (46 of left and right normal TMJ) with normal occlusion and 20 patients (14 of patient asymptomatic TMJ and 26 of patient symptomatic TMJ) with clicking and painful TMJ after the analysis of submental vertex view. Individualized lateral cephlometric tomogram analysis and TMJ space analysis were performed after tracing each film. All data from these analysis was recorded and statistically processed with CYBER computer system. 1. The results were obtained as follows. In submental vertex view, the mean condylar angulation of Rt. side in normal group was 20.348°±6.358°, Lt. side was 18.870°±7.777° and Rt. side in patient group was 19.350°±7.576° Lt. side was 17.750°±6.146° respectively. The mean condylar angulation of Rt. side was larger than Lt. side in normal and patient group. 2. When the mandible was moved from centric occlusion to centric relation, condylar position relating to the glenoid fossa was placed posteriorly and superiorly in normal TMJ group and patient symptomatic TMJ group. 3. In centric relation position, the proportion of anterior space to posterior space was 1.593 for normal TMJ group, 1.604 for patient asymptomatic TMJ group and 1.671 for patient symptomatic TMJ group. In centric occlusion position, 1.390 for normal TMJ group, 1.539 for patient asymptomatic TMJ group and 1.196 for patient symptomatic group. Normal TMJ group, patient asymptomatic TMJ group and patient symptomatic TMJ group and patient symptomatic TMJ group revealed significant difference in ∠C₂ measurement. (ANOVA-test, p<0.05) 5. Normal group and patient group revealed significant difference in Fh, ∠C₁and ∠C₂ measurement. (T-test, p<0.05) 6. There were strong positive correlation (0.8771) between Fp and Fm, and strong negative correlation (-0.9039) between ∠C₂ and ∠C₁ from the lateral cephalometric tomogram analysis.

Reliability of Lateral Deviation Measurement in the Hyoid Bone With Center Point and Lateral Motion Tests

  • Background: The hyoid bone is the only non-jointed structure among the skeletal tissues of the head and neck region, and its movement and posture depend on the attached muscle, ligament, and fascia. The location of the hyoid bone is important for airway maintenance, vocalization, chewing, swallowing, breathing, and head and scapular position. In general, the location of the hyoid bone is measured using radiographs and 3D computed tomography, and no studies have reported on clinical measurement methods. Objects: This study was performed to suggest clinical measurement methods for lateral deviation of the hyoid bone and to evaluate their reliability. Methods: In this study, 24 healthy volunteers (12 males, 12 females) in Cheongju-si participated. Two examiners performed the center point test and lateral motion test twice each to measure the lateral displacement of the hyoid bone. The reliability of the center point test was analyzed using intra-class correlation coefficients (ICC), and the reliability of the lateral motion test was analyzed using Cohen's kappa coefficient. Results: The intra-rater reliability of the center point test was good, and the inter-rater reliability was moderate. The intra- and inter-rater reliability of the lateral motion test showed substantial reliability. Conclusion: Based on these results, the center point test and the lateral motion test can be used as an alternative methods of the measurement of lateral deviation of the hyoid bone for people who have musculoskeletal disorders of the head, neck, and scapula.
