과학 실험 보고서를 이용한 불일치 사례에 대한 학생들의 반응 분석 (The Analyses of Students' Responses Toward Discrepant Events using Science Laboratory Reports)

  • 이 연구에서는 불일치 사례에 대한 학생들의 생각을 확인하기 위한 방안으로서 과학 실험 보고서의 활용가능성에 대해 조사했다. 과학 실험 보고서는 이 연구에서 개발되었으며, 불일치 사례에 대한 학생들의 반응과 관련된 정보를 수집하는 데 사용되었다. 분석은 기체의 용해도와 연소, 전기전도도에 대한 총 839개의 실험 보고서를 대상으로 했다. 실험 보고서에 나타난 학생들의 반응은 거부, 재해석, 배제, 판단불가, 주변이론 변화, 이론 변화, 과학적 개념 유지, 퇴행적 이론변화의 8가지 유형으로 분류되었다. 또한, 재해석과 이론 변화는 몇 개의 하위 유형으로 세분화되었다. 이 연구에서 나타난 학생들의 반응 특징들을 선행연구에서와 비교하여 논의했다.

계통도 분석법을 이용한 실험보고서 평가의 효과 분석 (A Network Analysis Approach to the Evaluation of Students' Laboratory Reports)

  • The purposes of this study were; 1) to apply the network analysis to evaluation of students' laboratory reports, 2) to test the applicability of the systematic network analysis. For this purposes, 7th grade 88 students from 22 middle schools in Inchon city of Korea were selected and their laboratory reports were evaluated by a evaluation criterion network developed in this study. The evaluation criterion consisted of 4 categories and 10 subcategories. The results of this study suggested that the network analysis approach was suitable one for the evaluation of students' laboratory reports. This was supported by total score distribution, means/weight, Cronbach's ${\alpha}$ coefficient(0.70), and the subcategories' reliabilities. The resluts of the factor analysis indicated that the evaluation criterion network of this study had two factors.

일개(K) 병원의 누락 조직검사결과지에 관한 조사연구 (A Study on Loose Laboratory Reports in A Hospital)

  • Background : The medical record is a compilation of pertinent facts of a patient's life and health history, including past and present illness and treatment. It is written by the health professionals contributing to that patient's care. And the medical record is the permanent, legal document which must contain sufficient information to identify the patient, justify the diagnosis and treatment, and record the results. As such, it must be accurate and complete. So we try to analyze the medical record especially a kind of incomplete record, loose laboratory reports. Methods: During the one-year period(from January to December 1988), a medical record practitioner examine and analyze the record of laboratory reports at K Hospital in Seoul. A total of 320 loose laboratory reports for 3,818 admitted laboratory reports. And a medical record practitioner and a physician review and analyze the influencing factors for the various reasons of clinical and laboratory aspects. Result: The loose percentage by department is the highest in obstetrics(40.4%) but the highest loose rate is in pediatrics(25.0%). The most of omission is occurred in operation room(80.3%) than OPD(19.7%). The change of diagnosis is according to duration of laboratory and more changable in cancer patient. Conclusion : Regular analysis of the documentation in the medical record so it fulfills its purposes of communicating patient care information. So it serves as evidence of the patient's course of illness and treatment for various legal, reimbursement, and peer evaluation review. And it is very important aspect of quality assurance in medical activities.

Errors in Surgical Pathology Reports: a Study from a Major Center in Pakistan

  • Background: Errors in surgical pathology diagnosis can have serious consequences for the patient. Since the final product of a surgical pathology lab is the report, errors can be picked by reviewing reports of cases. Aim: To determine the frequency and types of error in surgical pathology reports of cases signed out in 2014 in a laboratory in Karachi, Pakistan. Materials and Methods: All surgical pathology reports in which changes were made in the original report after sign out and an amended report was issued were included. Errors included: (1) misinterpretations; (2) missing critical information; (3) erroneous critical information; (4) misidentification; and (5) typographic errors. Results: Errors were identified in 210 cases (0.37%). These comprised 199 formalin fixed specimens and 11 frozen sections. The latter represented 3.8% of a total of 2,170 frozen sections. Of the 11 frozen section errors, 10 were misinterpretations. Of the 199 permanent specimens, 99 (49.7%) were misinterpretations, 65 (32.7%) belonged to missing critical information category, 8 (4%) belonged to erroneous critical information category, 8(4%) were misidentifications, 16(8%) were typographic errors while 3 cases (1.5%) were other errors. Most misinterpretations occurred in the gastro intestinal, liver and pancreato biliary tract (23.2%) and breast (13.1%). Another 87 cases were reviewed on the clinicians' request. However diagnosis after review remained the same as the original diagnosis. In 49 out of these (56.3%), additional workup was performed at the time of the review. Conclusions: Our findings were similar to other published studies. We need to develop documented procedures for timely review of cases to detect errors.

An Automated Draft Report Generator for Peripheral Blood Smear Examinations Based on Complete Blood Count Parameters

  • Background: Complete blood count (CBC) results play an important role in peripheral blood smear (PBS) examinations. Many descriptions in PBS reports may simply be translated from CBC parameters. We developed a computer program that automatically generates a PBS draft report based on CBC parameters and age- and sex-matched reference ranges. Methods: The Java programming language was used to develop a computer program that supports a graphical user interface. Four hematology analyzers from three different laboratories were tested: Sysmex XE-5000 (Sysmex, Kobe, Japan), Sysmex XN-9000 (Sysmex), DxH800 (Beckman Coulter, Brea, CA, USA), and ADVIA 2120i (Siemens Healthcare Diagnostics, Eschborn, Germany). Input data files containing 862 CBC results were generated from hematology analyzers, middlewares, or laboratory information systems. The draft reports were compared with the content of input data files. Results: We developed a computer program that reads CBC results from a data file and automatically writes a draft PBS report. Age- and sex-matched reference ranges can be automatically applied. After examining PBS, users can modify the draft report based on microscopic findings. Recommendations such as suggestions for further evaluations are also provided based on morphological findings, and they can be modified by users. The program was compatible with all four hematology analyzers tested. Conclusions: Our program is expected to reduce the time required to manually incorporate CBC results into PBS reports. Systematic inclusion of CBC results could help improve the reliability and sensitivity of PBS examinations.

과학 고등학교 학생들의 실험 보고서 작성에 대한 학년별 인식 조사 연구 (Study on Perception of Science High School Students about Composing Laboratory Reports by Grade)

  • 본 연구는 과학 고등학교 학생들의 실험 보고서 작성에 대한 인식과 실험 보고서 작성이 과학 개념 이해와 과학 탐구 능력에 미치는 영향 등에 대한 학년별 인식을 조사하고자 하였다. 이를 위해 서울과 경기에 위치하고 있는 과학 고등학교 4곳에서 260명의 학생이 조사에 참여하였다. 과학 고등학교 학생들은 실험 보고서를 작성하는 것에 그다지 긍정적이지 않았으나 실험 보고서를 작성하는 것이 과학 개념 이해와 과학 탐구 능력 향상에는 도움을 준다고 생각하는 것으로 나타났다. 이러한 경향은 학년에 따라서 많은 차이를 나타내었다. 과학 고등학교 1학년 학생들이 2학년에 비해 실험 보고서 작성, 과학 탐구 능력 향상, 과학 개념 이해 등 모든 영역에 대해 훨씬 더 긍정적인 견해를 나타내었다. 이는 과학 고등학교 학생들의 실험 보고서 작성에 있어 학년에 따라 교육적 접근이 달라져야 함을 나타낸다.

An inhibitory role of NEK6 in TGFβ/Smad signaling pathway

  • The NEK6 (NIMA-related kinases 6) is reported to play po-tential roles in tumorigenesis. Although it is suggested to function in several cellular pathways, the underlying mechanism in tumorigenesis is still largely unknown. In the present study, we discovered interaction of NEK6 with Smad4, a key member of transforming growth factor beta (TGFβ) pathway. Over-expression of NEK6 in hepatocellular carcinoma (HCC) cell lines suppresses TGFβ-mediated transcription activity in a kinase activity-dependent manner. In addition, NEK6 suppresses the cell growth arrest induced by TGFβ. Mechanically, NEK6 blocks nuclear translocation of Smad4, which is essential for TGFβ function. Moreover, we identified that NEK6 could be regulated by TGFβ and hypoxia. Our study sheds new light on the roles of NEK6 in canonical TGFβ/Smad pathway and tum-origenesis. The microRNA expression profiles of mouse mesenchymal stem cell during chondrogenic differentiation

  • MicroRNAs are potential key regulators in mesenchymal stem cells chondrogenic differentiation. However, there were few reports about the accurate effects of miRNAs on chondrogenic differentiation. To investigate the mechanisms of miRNAs-mediated regulation during the process, we performed miRNAs microarray in MSCs at four different stages of TGF-${\beta}3$-induced chondrogenic differentiation. We observed that eight miRNAs were significantly up-regulated and five miRNAs were downregulated. Interestingly, we found two miRNAs clusters, miR-143/145 and miR-132/212, kept on down-regulation in the process. Using bioinformatics approaches, we analyzed the target genes of these differentially expressed miRNAs and found a series of them correlated with the process of chondrogenesis. Furthermore, the qPCR results showed that the up-regulated (or down-regulated) expression of miRNAs were inversely associated with the expression of predicted target genes. Our results first revealed the expression profiles of miRNAs in chondrogenic differentiation of MSCs and provided a new insight on complicated regulation mechanisms of chondrogenesis.

대학생들의 물리실험에서 측정 활동 분석틀 개발 및 적용 (Development and Implementation of the Analysis Frame for Measurement Activity in Undergraduate Physics Laboratory)

  • 본 연구에서는 탐구 기능 가운데 자료 수집, 자료처리, 자료 해석과 관련하여 실험보고서 분석틀을 개발하였다. 또한 개발한 분석틀을 대학생들의 보고서 분석에 적용하여 오차와 불확도 개념을 중심으로 대학생들이 보고서 작성에서 어려움을 겪고 있는가에대해 알아보았다. 물리실험 수업을 수강한 30명의 학생들에게 실시한 설문지와 학생들이 작성한 실험보고서를 분석하였다. 학생들은 자료 수집과 관련하여 반복 측정에 대해 인식을 하고 있으나 보고서에서는 이를 명확하게 표현하지 못하였다. 극값이나 기울기가급하게 변하면 더욱 정밀한 측정을 해야 하지만 학생들은 이 부분에서도 어려움을 지니고 있었다. 특히, 오차와 불확도와 관련하여 학생들은 많은 어려움을 표출하고 있었다. 오차와 불확도의 전파에 대한 이해도가 매우 낮은 상태에서 단순한 오차 전파공식도 이용하지 못하는 학생이 많았다. 오차와 불확도를 분석을 통해 자료를 해석하는 과정에서도 많은 어려움을 겪고 있었는데, 오차의 원인들이 결과에 어떻게 영향을 주는지를 정량적으로 분석할 수 있는 학생은 거의 없었다. 반면에 학생 대부분이 오차와 불확도 분석이 중요하다고 응답하였다. 학생들이 중요성을 인식하고 있으나 올바른 개념을 지니지 못하여 보고서 작성에서 어려움을 겪고 있다는 결론을 내릴 수 있었다. 실험 수업에서 보고서를 통해 과학적 의사소통을 해야하는 학생들에게 물리 개념뿐만 아니라 자료를 수집하고 처리하고 해석하는 데 필요한 기본 개념에 대한 교육이 필요할 것이다.

The Intelligent Clinical Laboratory as a Tool to Increase Cancer Care Management Productivity

  • Studies of the causes of cancer, early detection, prevention or treatment need accurate, comprehensive, and timely cancer data. The clinical laboratory provides important cancer information needed for physicians which influence clinical decisions regarding treatment, diagnosis and patient monitoring. Poor communication between health care providers and clinical laboratory personnel can lead to medical errors and wrong decisions in providing cancer care. Because of the key impact of laboratory information on cancer diagnosis and treatment the quality of the tests, lab reports, and appropriate lab management are very important. A laboratory information management system (LIMS) can have an important role in diagnosis, fast and effective access to cancer data, decrease redundancy and costs, and facilitate the integration and collection of data from different types of instruments and systems. In spite of significant advantages LIMS is limited by factors such as problems in adaption to new instruments that may change existing work processes. Applications of intelligent software simultaneously with existing information systems, in addition to remove these restrictions, have important benefits including adding additional non-laboratory-generated information to the reports, facilitating decision making, and improving quality and productivity of cancer care services. Laboratory systems must have flexibility to change and have the capability to develop and benefit from intelligent devices. Intelligent laboratory information management systems need to benefit from informatics tools and latest technologies like open sources. The aim of this commentary is to survey application, opportunities and necessity of intelligent clinical laboratory as a tool to increase cancer care management productivity.
