재활프로그램이 슬관절 골관절염 환자의 등속성 근력에 미치는 영향 (The effects on rehabilitative program on isokinetic muscle power of patients with osteoarthritis of Knee joint)

  • The purpose of this study was to investigerate the changes of isokinetic muscular function in elderly people who have been to take from osteoarthritis in both knee joints after 36 weeks' rehabilitative therapy programs. In this study the subjects were 20 women residing in S-Tower(n=10) and H-welfare Town(n=10) respectively. The rehabilitative group(equal to experimental group) had taken part in exercise program 5 days per week. And then was performed by warm up, workout(involving aerobic exercise and weight training), cool down, physical therapy(cryotherapy, TENS, ultrasound). Also its programs were classified in conditioning phase($0{\sim}12$ weeks), improvement phase($13{\sim}24$ weeks), and maintenance phase($25{\sim}36$ weeks) respectively. The results of inspections were as followed: In the isokinetic muscular function, there were significant differences in right leg's flexor and extensor in $60^{\circ}$/sec. And there were significant differences in right leg's flexor and left leg's extensor in $180^{\circ}$/sec. At last, there were significant differences in right leg's flexor and left leg's extensor in $240^{\circ}$/sec. In other words, the rehabilitative programs for 36 weeks could increase the muscular function in elderly with OA. In conclusion, the rehabilitative programs of this paper has shown the positive results, which involved in the muscular function variables in elderly people with OA in both knee.

3주간의 고지대 스키훈련이 크로스컨트리 스키 선수의 등속성 근기능에 미치는 영향 (The effect of 3 weeks high altitude skiing training on isokinetic muscle function of cross-country skierst)

  • 본 연구의 목적은 3주간의 고지대 스키 트레이닝이 크로스컨트리 스키 선수의 근기능에 미치는 영향을 분석하여 향후 크로스컨트리 스키 선수들의 근기능 향상 및 과학적 훈련프로그램 구성을 위한 기초 자료를 제공하는데 있다. 연구대상자는 크로스컨트리 스키 남자 대학 선수 6명을 대상으로 실시하였으며, 주기별과 고지대 트레이닝이 크로스컨트리 스키 선수들에 미치는 영향을 확인하기 위하여 일반선형모델 반복측정(General Linear Model ANOVA with repeated measure)과 대응표본 t 검정(Paired Samples t-test)을 적용하여 분석하였다. 고지대 스키 트레이닝을 3주간 실시한 결과 신체조성은 변화가 나타나지 않았지만, 어깨관절, 허리관절, 무릎관절, 발목관절의 등속성 근기능이 감소되는 것을 확인할 수 있었다. 따라서 SP기간과 같은 스키 트레이닝 기간 중에도 지속적인 근력훈련을 실시하여야 할 것으로 사료되면 이에 대한 추가 연구가 필요할 것으로 판단된다.

골관절염 노인의 건강 관련 삶의 질 영향요인: 국민건강영양조사 자료를 이용하여 (Factors influencing Health-related Quality of Life in Older Adults with Osteoarthritis: Based on the 2010-2011 Korea National Health and Nutrition Examination Survey)

  • Purpose: This study was conducted to identify factors that influence health-related quality of life in older adults with osteoarthritis. Methods: This study used a cross-sectional design with secondary analysis of the Korean National Health and Nutrition Examination Survey 2011. A total of 362 participants aged 65 years and older who had osteoarthritis were selected. Health-related quality of life using EQ-5D, perceived health status, body mass index, numbers of chronic illness, smoking and alcohol use, exercise, activity limitation, joint pain and stiffness, depression, and perceived stress were measured. Descriptive statistics, $x^2$-test, t-test, ANOVA, Pearson's correlation coefficients, and multiple regression were conducted with SPSS/WIN 21.0. Results: The mean score of EQ-5D was 0.77 in older adults with osteoarthritis. The results of multiple regression analysis showed that age, income, subjective health status, restriction of activity, knee joint stiffness, depressive mood, and perceived stress significantly predicted health-related quality of life in older adults with osteoarthritis, explaining 42% of the variance. Conclusion: Older adults with osteoarthritis have low health-related quality of life. In oder to improve health-related quality of life in older adults with osteoarthritis, it is important not only to enhance physical function but also to provide emotional support.

고등부 씨름선수의 체급별 기초·전문체력 비교분석 (A Comparison of Physical Fitness of High School Ssirum Elite-players According to Their Weight Class)

  • 본 연구는 고등부 씨름 선수들을 대상으로 체급별 기초·전문체력의 차이를 알아보는데 목적이 있다. 측정기간은 2019년 11월 25일-12월4일이며, 총 28명의 고등부 씨름 선수(80kg이하 7명, 90kg이하 8명, 90kg이하 7명, 100kg이상 6명)를 대상으로 체급별 기초·전문 체력요인(근력, 근파워, 민첩성, 무산소성파워, 유산소능력, 유연성, 등속성근기능)을 측정하여 비교·분석하였다. 자료처리는 SPSS 23.0을 활용하여 one-way ANOVA와 Duncan을 실시하였다. 그 결과 첫째, 악력과 팔굽혀펴기, 서전트점프, 제자리멀리뛰기, 체후굴, 체전굴, 사이드스텝, 반응시간에서 체급별 차이가 나타나지 않았다. 둘째, 배근력에서 100kg 이하인 그룹이 100kg 이상인 장사급보다 유의하게 낮게 나타났으며, 윗몸일으키기와 20m 셔틀런에서 장사급보다 유의하게 높게 나타났다. 셋째, 무산소성 최고파워와 평균파워의 상대적인 값은 100kg 이하인 그룹이 100kg 이상인 장사급보다 유의하게 높게 나타났다. 넷째, 등속성근기능의 경우 허리 신전/굴곡력과 무릎 관절의 우측 신전근력, 그리고 좌우측 굴곡근력에서 체급별 차이가 없었다. 본 연구 결과에서 얻은 결론은 고등부 씨름 선수의 경우 근파워, 유연성, 민첩성 그리고, 체중당 무산소성 평균파워와 등속성 허리근력, 무릎굴근력에서 체급에 따른 차이가 없다는 것을 확인할 수 있었다.

전신진동자극을 동반한 점진적 균형훈련 프로그램이 무릎뼈 관절염 환자의 무릎관절 통증과 기능장애 수준, 심리사회적 수준 그리고 균형능력에 미치는 효과 (Effects of Progressive Balance Training Exercise Programs with Whole Body Vibration on Pain, Function, Psychosocial Status, and Balance in Patients with Knee Osteoarthritis )

  • PURPOSE: This study aimed to investigate the effect of a progressive balance training program with whole-body vibration stimulation on knee joint pain, dysfunction, psychosocial status, and balance ability in individuals aged ≥ 65 years with knee osteoarthritis. METHODS: A total of 40 individuals aged ≥ 65 years with osteoarthritis of the knees participated in the study. Using a randomization program, participants were assigned to an experimental group (n = 20) or a control group (n = 20). Both groups were assigned a knee strength training program, and a progressive balance training program with whole-body vibration stimulation was assigned to the experimental group. All interventions were conducted three times a week for four weeks. Participants were evaluated for the following: pain (numeric rating scale, NRS), knee dysfunction (Korean version of the Western Ontario and McMaster Universities Arthritis Index, K-WOMAC), fall efficacy (Korean Version Falls Efficacy Scale, K-FES), quality of life (Euro Quality of life 5 Dimension, EQ-5D), and advanced balance scale score (Fullerton advanced balance scale, FAB) before and after the intervention, and the effects of the intervention were compared accordingly between groups. RESULTS: Both groups showed significant differences in the results of the NRS, K-WOMAC, K-FES, and EQ-5D assessments before and after the intervention, and there was a significant difference in the amount of change between the two groups (p < .05). There was a significant improvement in FAB in all but items FAB 8 and FAB 9 after the intervention in the experimental group. In the control group, there was a significant improvement in FAB 1, FAB 2, FAB 7, and FAB total after the intervention (p < .05). In addition, there was a significant difference in the amount of change between the two groups in all items except FAB 8 and FAB 9 (p < .05). CONCLUSION: The progressive balance training program with whole body vibration stimulation is an effective intervention method with clinical significance in improving knee joint pain, knee disability index, psychosocial level, and balance ability in adults aged ≥ 65 with osteoarthritis of the knees.

트레드밀 경사도에 따른 발목 중량부하 걷기 후 신체 기능에 미치는 영향 (The Effect of Ankle Joint Gram Walk on the Physical Function According to Treadmill Gradient)

  • Purpose : The purpose of this study was to investigate knee muscle strength, balance, and proprioception by walking while giving weight to the ankle joint at a load of 0%, 10%, and 15% gradient on a treadmill. Methods : Twenty-four subjects were divided into three groups. Group A walked on a 0% gradient while giving weight to the ankle joint as a load. Group B walked on a 10% gradient while giving weight to the ankle joint as a load. Group C walked on a 15% gradient while giving weight to the ankle joint as a load. This was performed three times per week for four weeks. The walking speed was set at 4km/hour and the walking time was set at 20 minutes. Measurements were performed before and after the experiment. Biodex medical system and Balance system SD were used as measuring instruments. Results : There was no significant difference between the gradients in muscle strength, balance and proprioception. However, there were differences in pretest and posttest measurements for muscle strength, balance and proprioception in each gradient. In particular, the gradient of 10% showed a significant difference. Conclusion : According to the results of this study, to use a 10% gradient is effective when walking while giving weight to the ankle joint as a load.

전방십자인대 재건술 후 닫힌사슬운동이 슬관절 대퇴둘레, Lysholm 척도에 미치는 영향 (The Effects of Closed Kinetic Chain Exercises on Thigh Circumference and Lysholm Scale of the Knee Joints of Patients with ACL Reconstruction)

  • Purpose: This study compared the thigh circumference and Lysholm scale of a stable and unstable exercise group of patients who had undergone an anterior cruciate ligament reconstruction (ACL reconstruction). Methods: The subjects were patients more than 4 weeks after their ACL reconstruction and could stand on one leg. The patients were divided into a control group with 9 patients performing closed kinetic chain exercises on a stable floor and an experimental group with 10 patients performing closed kinetic chain exercises en an unstable floor. The degree of muscle atrophy was compared by measuring the circumference of the injured thigh before the exercise program, and 3 weeks and 6 weeks after the exercise program. The Lysholm scale was used to assess the function of the knee joint. Results: There was no significant increase in thigh circumference according to the exercise periods in the two groups. However, there was a statistically significant increase before exercises and 6 weeks after the exercises (p<0.05). There were statistically significant differences in the Lysholm scores between the two groups (p<0.05). Conclusion: There were no statistically significant increase in the two groups, but there was a significant difference between before the exercise program and 6 weeks after the exercise program (p<0.05).

슬관절 전치환술 후 기능 회복을 위한 치료법 (Treatment Methods for Functional Recovery after Total Knee Arthroplasty)

  • 슬관절의 퇴행성 관절염에서 슬관절 전치환술은 널리 시행되고 있고 그 빈도도 점점 증가되고 있다. 수술 후 일상생활로의 복귀, 근력 및 운동범위의 회복 등 기능 회복을 위한 노력이 이루어져야 하고 이는 환자 요인, 수술 술기 및 재활 등 다양한 관점에서 접근해야 한다. 환자의 연령이나 비만의 정도, 성별, 대퇴사두근의 근력 등이 수술 후 기능 회복에 영향을 줄 수 있고 환자의 기대치나 만족도 같은 정신적인 상태도 영향을 준다. 기능 회복을 위하여 수술 전부터 환자교육 및 통증 조절, 대퇴사두근 근력의 강화 같은 치료를 시행할 수 있다. 수술 후 냉찜질 및 압박, 경피적 전기 신경 자극 치료, 신경근육 전기 자극 치료, 저주파 저강도 자기장 치료, 대퇴사두근의 근력운동, 관절범위 운동과 같은 물리치료도 적용할 수 있다. 최근에는 수중에서 근력과 균형감각을 회복시키는 수 치료도 점점 시행되는 추세이다. 이런 기능 회복을 위한 치료들은 수술 후 단기적으로만 시행될게 아니라 장기적으로 꾸준히 시행되는 것이 중요하며 술자는 환자의 상태나, 순응도, 사회적, 심리적 상황 등을 고려하여 적절히 적용하여야 한다.

일지역 노인의 관절 건강 상태 (A Study on Health Status of Joints in Elderly People)

  • Purpose: The purpose of this study was to describe the subjective health status of joints, flexibility of joints, muscle strength and state of Joint exercise in elderly people living in the community. Method: The participants were 74 elderly people in a health program of the Gerontological Health Research Center at G University. The data were collected from March to July, 2003. The instrument for the study included items on general characteristics (5 items), subjective health status of joints (10 items), flexibility of Joints and strength of muscle (8 items), and state of joint exercise (7 items). Results: The score for subjective health status of joints was 3.70 (range 1-5). Knee and waist joints were in the worst state. Neck extension and flexion were $17.38{\pm}1.80$ and $.60{\pm}1.08(cm)$ respectively. Trunk extension and flexion were $68.24{\pm}4.47$ and $58.00{\pm}4.91(cm)$ respectively. Flexibility of the Rt./Lt. shoulder joints was $14.63{\pm}12.51/18.82{\pm}13.80(cm)$. Muscle strength of the Rt./Lt. leg was $30.47{\pm}19.68/29.67{\pm}21.22$ (sec.). Shoulder joints were more flexible for men and people in their sixties compared to women and people in their seventies. Hand grip was stronger for men compared to women. The score for state of joint exercise was 2.83(range 1-4). The state of handgrip correlated with the state of joint exercise [r=.423 (Rt.)/r=.273(Lt.)]. Conclusion: To prevent falls and disorders in neuromuscular function, it is important to develop systemic joint exercise programs for elderly people.

슬관절과 견관절의 초기재활과정에서 관절운동범위와 스트레칭, 그리고 유산소성 운동 프로그램 (Range of Motion, Stretching, and Aerobic Exercise in Accelerated Rehabilitation of Knee and Shoulder)

  • 본연구는슬관절과견관절의초기재활과정에서관절운동범위와스트레칭, 그리고유산소성운동프로그램에대한논문이다. 만약관절이손상후장기간동안고정된다면관절이굳어지고 근육은위축이 될것이다. 그러므로본 프로그램은다양한운동기술을포함하고있다. 즉관절의경직을예방하기위한운동범위확보와근육의이완을위한스트레칭, 심폐기능의감소를예방하기위한심혈관훈련(수영, 상체자전거, 고정식자전거)을포함하고있다. 초기재활과정에서는임상운동전문가와환자사이의상호관계를형성하는것이매우중요하다. 또한임상운동전문가는환자의통증과운동적응도, 체력수준, 프로그램의진행등에관해스포츠의학전문의와상의하는것이권장된다.
