End Stage Renal Disease caused by IgA Nephropathy : One Case Report (IgA 신증에 의한 말기 신질환에 대한 증례보고)

  • This case is to report the effect of renal function of chronic kidney disease(CKD) caused by IgA nephropathy. A 37-year-old man visited a Korean medicine hospital, who has been diagnosed with end stage renal disease(ESRD), 5 stage of CKD, caused by IgA nephropathy, has had no improvement of western medical treatment, and wanted to be treated using Korean medicine before renal transplantation. The decrease of creatinine value, the increase of glomerular filtration rate(GFR), and the decrease of CKD stage (5 to 4) was observed after combination treatment of Ikkigeonbiisuhwalhyeoltang and saam acupuncture was applied. This case report is suggested that combination treatment of acupuncture and herbal medicine could be effective to renal function of CKD in spite of a single case.

Effect of Arachidonic Acid on Renal Function of Dog (Arachidonic Acid의 개 신장기능에 미치는 영향)

  • Arachidonic acid which is precursor of prostaglandins, when administered ($100.0\;{\mu}g/kg$, or $100.0\;{\mu}g/kg/min$) intravenously, did not influence on renal function of dog. Arachidonic acid, when infused ($10.0\;{\mu}g/kg/min$) into a renal artery, produced marked diuretic action accompanied with augmentation of renal plasma flow and with little changed glomerular filtration rate, and exhibited the increased clearances of osmolar substance and free water, and the decreased reabsorption rates of sodium and potassium in renal tubules in only experimental kidney, but did not influenced at all in control kindey. The diuretic acition of arachidonic acid infused into a renal artery was not affected by pretreatment of indomethacin (10.0 mg/kg. i.v) which is inhibitor of cyclooxygenase. Above results suggest that arachidonic acid infused into a renal artery produced diuretic action through direct renal hemodynamic changes, that is mediated by reduction of postglomerular resistance being caused by dilation of vas efferense.

  • The action of serotonin (5-Hydroxytryptamine) on the excretory function of the kidney was investigated in the dog, utilizing the clearance method and the stop-flow technique. It was shown that serotonin, $10{\mu}g/kg/min$, i. v., exerts a marked antidiuretic effects and elicits a marked hemodynamic changes in the kidney: a highly significant decrease of the glomerular filtration rate and a tendency of decrement in the renal plasma flow. Little change in the systemic blood pressure was noted, and the participation of the antidiuretic hormone in the antidiuretic action was ruled out by adding vasopressin to the infusion fluid. The stop-flow analysis showed that there is no evidence of altered activity in the tubules by serotonin. It was thus concluded that serotonin elicits anti diuresis in the dog by decreasing glomerular filtration rate, which results from the constriction of Vas afferens in the glomeruli.

Influence of Bradykinin on the tubular function of the dog kidney (신세뇨관기능(腎細尿管機能)에 미치는 Bradykinin의 영향(影響))

  • In this study the influence of Bradykinin, a biogenic polypeptide, on the excretory function of the dog kidney was investigated, utilizing the clearance and stop-flow method. The results are summarized as follows; 1) Bradykinin administered intravenously elicited a marked antidiuresis. 2) When given into the renal artery, however, prompt increase of the urine flow, sodium excretion and free water clearance without significant change in the glomerular filtration rate ensued. 3) It was also effective during an osmotic diuresis with 10% Mannitol infusion, though the response was not so marked. 4) The stop-flow experiment showed that sodium reabsorption in the proximal tubules is inhibited by the infusion of bradykinin into the renal artery. It was thus concluded that the diuretic effect of bradykinin given intra-arterially results from the inhibition of sodium transport in the proximal tubules.

  • "Jinaik(津液)" is essential nutritious body fluid for human to be alive, and it can be divided various kinds according to it's location, character, and function. Five Fluids -sweat, snivel(mucus), slobber(saliva), tear, spittle(saliva)- are kinds of "Jinaik(津液)", and all of them belong to kidney. They are also related with five organs, such as sweat belongs to heart, snivel(mucus) belongs to lung, slobber(saliva) belongs to spleen, tear belongs to liver, spittle(saliva) belongs to kidney. We can find out five organs' physiological and pathological conditions by means of observations on the Five Fluids. Five organs' function comes out upper seven openings -two eyes two ears, nose, mouth and tongue. The Five Fluids provide all the nutrition upper seven openings need, so they can help for human to hear, see, smell, and taste properly.

  • Cyperus rotundus L. (Cyperaceae) has been used as an analgesic, antiinflammatoty agent, diuretic and emmenagoga in folk remedies. Cyperi Rhizomata, processed and unproces sed, were extracted with MeOH and fractionated with petroleum ether (Pet. Ether), $CHCl_3$, BuOH, water. In order to investigate the effects of their fractions on kidney function of acute renal failure rats induced by $HgCl_2$ urinary volume, BUN, Creatinine, Uric acid were determined. The diuretic effect of processed Cyperi Rhizoma was significantly increased in renal failure rats, on serum chemical parameters, the significant inhibition of BUN (blood urea nitrogen) of processed Cyperi Rhizoma was revealed.

Antioxidant Effects of Ascorbic Acid on Renal-Ischemia Reperfusion Injury in Rabbit Model

  • Renal ischemia-reperfusion (I/R) injury is great clinical important because viability of the organ depends on the tolerance to ischemia-reperfusion injury, an inevitable processing during surgery. The purpose of this study was to investigate the effects of premedicated ascorbic acid alone in I/R injury model induced by cross-clamping of renal vessels. In the rabbit models, 2-4 kg New Zealand white rabbits were subjected to 30 minutes of warm unilateral renal ischemia followed by removal of contralateral kidney and then divided into five groups, control (2) arid treatment groups (3). In control group 1, the rabbits only received right nephrectomy. In control group 2, the rabbits received I/R on left kidney after the right nephrectomy. In treatment group 1, the rabbits received ascorbic acid 50 mg/kg IV before the operation. In treatment group 2, the rabbits received ascorbic acid 100 mg/kg IV before the operation. In treatment group 3, the rabbits received ascorbic acid 200 mg/kg IV before the operation. Blood samples were collected from these rabbits for measurement of kidney function tests at the 0, 1 st, 3rd and 7th day and antioxidant enzyme( SOD, GSHPx, CAT) at 24 hours. Kidney function tests (serum creatinine and BUN) showed a significant difference between group 2 and group 4, 5. Activity of antioxidant enzymes in plasma were significant decrease in group 4, 5 compare to group 2. The result of this study suggested that the exogenous ascorbic acid had a role of attenuation of renal I/R injury in rabbit model.

  • Objectives This study was conducted to report the safety of vomit-inducing treatment (Tu Fa, 吐法) by Melonis Calyx, particularly with respect to clinical features and liver and kidney functions.Methods Patients treated with Melonis Calyx powder between September 2013 and September 2014 were assessed retrospectively.Results Less than 3 grams of Melonis Calyx powder can induce mild abdominal discomfort and produce no harmful effects on liver and kidney functions.Conclusions Through this study, a vomit-inducing treatment using less than 3 grams of Melonis Calyx can be considered safe enough to be used in clinics, with a cautious approach with respect to dehydration.

Effects of Fermented Milk Feeding on Function and Suppression of Aluminum Accumulation in Kidney of Rat (젖산균 발효유 섭취가 흰쥐의 신장내 알루미늄 축적억제와 신장기능에 미치는 영향)

  • This study was performed to evaluate the effects of the supplementation of lactic acid bacterias (LAB; S. thermophilus, L. acidophilus) aluminum accumulation and function kidney in rats treated with 250 g/g aluminum sulfate for 4 weeks. Fifty male Sprague Dawley strains were divided into five groups according to the types of supplement. the control, S. thermophilus culture group(A), autoclaved S. thermophilus culture(B), L. acidophilus culture group(C) and autoclaved L. acidophilus(D). The weight gain was increased by supplementation of S. thermophilus culture(A), autoclaved S. thermophilus culture(B), L. acidophilus culture(C), autoclaved L. acidophilus culture(D), especially by supplementation of S. thermophilus cultured(A) as compared to control group. The amount of water intake was increased in control group as compared to the LAB supplemented group. The content of aluminum were decreased 19.57%, 31.25%, 37.10%, and 32.40% in kidney after supplementation of non autoclaved culture group (A,C), and autoclaved group(B,D) respectively, as compared to control group. Water balance, urine volume, and excretion of sodium and potassium decreased in LAB supplemented group and control group. Excretion of creatinine increased in control group and tendency to increased in LAB supplemented group. In conclusion, the effect of suppression of aluminum accumulation was more effective in sup plementation of S. thermophilus culture(A), and L. acidophilus culture(C) than autoclaved S. thermo philus culture(B), and L. acidophilus culture(D).
