Clinical Application and Limitations of the Capsular Pattern (관절낭 패턴의 임상적 적용과 한계)

  • A normal range of motion is essential for performing activities of daily living. The capsular pattern is the proportional motion restriction in range of motion during passive exercises due to tightness of the joint capsule. Although the capsular pattern is widely referred to in clinical practice, there is no scientific evidence to support the concept. In this review, the appropriateness of the capsular pattern for evaluation of joint pathology was assessed. In the Textbook of Orthopaedic Medicine written by Cyriax, the capsular pattern did not specify how much reduction in angular motion is considered motion restriction. As the definition proposed initially was unclear, different methods have been used in previous studies investigating capsular pattern. In addition, the capsular pattern described all the major joints of the human body, but only the hip joint, knee joint, and shoulder joint were studied in experimental studies. Sensitivity and specificity were reported in one study and were meaningful in specific pathologies (loss of extension to loss of flexion). There was no consensus on the reliability and validity. In summary, the capsular pattern suggested by Cyriax or Kaltenborn is not supported or applies only to certain conditions. Various components around a joint complement each other and provide stability to the joint. It is recommended that the therapist perform multiple assessments rather than rely on a single assessment when evaluating joints.

Inference on the Joint Center of Rotation by Covariance Pattern Models

  • Objective: In a statistical linear model estimating the center of rotation of a human hip joint, which is the parameter related to the mean of response vectors, assumptions of homoscedasticity and independence of position vectors measured repeatedly over time in the model result in an inefficient parameter. We, therefore, should take into account the variance-covariance structure of longitudinal responses. The purpose of this study was to estimate the efficient center of rotation vector of the hip joint by using covariance pattern models. Method: The covariance pattern models are used to model various kinds of covariance matrices of error vectors to take into account longitudinal data. The data acquired from functional motions to estimate hip joint center were applied to the models. Results: The results showed that the data were better fitted using various covariance pattern models than the general linear model assuming homoscedasticity and independence. Conclusion: The estimated joint centers of the covariance pattern models showed slight differences from those of the general linear model. The estimated standard errors of the joint center for covariance pattern models showed a large difference with those of the general linear model.

Money Management and Control of Family Finances Patterns of Urban Dual-Earner Couples. (도시 맞벌이부부의 금전관리와 경제생활주도권 유형의 측정방법 및 실태)

  • The purpose of this study was to examine patterns ofmoney management and control of family finances. The data for this study were collected from 311 wives who were currently employed in professional, managerial, and clerical occupation living or employed in Seoul. Frequency, percentage and cross-tab analysis were used to analyze the data. The results were as follows: 1) money management patterns were classified into six types; wife-management pattern, wife-management/husband-alienation pattern, wife-management/husband-involvement patter, joint management pattern, joint/separate mixed pattern, and multi management pattern. Among them wife-mannagement pattern was used most frequencly by the couples. 2) Four family finance control patterns were found and named by joint control, wife control, husband control, and self control. The joint control pattern was found to be the most prevalent one. 3) Money management patterns influenced how family finances were controled.

  • The purpose of this study is to present the research data to improve the quality of triangular ward in general hospital in Korea. Drawing documents analysis were used to examine the condition of ward. This study in focused on patient room and atrium according to connection type of triangular ward, allocation of NS, module and direction of patient room. The results of this study is as follows; Triangular ward has two joint pattern(conner and conner, side and side). Conner and conner joint pattern is good at planning of lighting than side and side joint pattern. Triangular ward can has different module system in each side. Allocation of NS is important factor which is focused on patient relationship or ward control.

Unintentional lumbar facet joint injection guided by fluoroscopy during interlaminar epidural steroid injection: a retrospective analysis

  • Background: An epidural steroid injection (ESI) is a commonly administered procedure in pain clinics. An unintentional lumbar facet joint injection during interlaminar ESI was reported in a previous study, but there has not been much research on the characteristics of an unintentional lumbar facet joint injection. This study illustrated the imaging features of an unintentional lumbar facet joint injection during an interlaminar ESI and analyzed characteristics of patients who underwent this injection. Methods: From December 2015 to May 2017, we performed 662 lumbar ESIs and we identified 24 cases (21 patients) that underwent a lumbar facet joint injection. We gathered data contrast pattern, needle approach levels and directions, injected facet joint levels and directions, presence of lumbar spine disease as seen on magnetic resonance images (MRI), and histories of lumbar spine surgeries. Results: The contrast pattern in the facet joint has a sigmoid or ovoid contrast pattern confined to the vicinity of the facet joint. The incidence of unintentional lumbar facet joint injection was 3.6%. The mean age was 68.47 years. Among these 21 patients, 14 (66.7%) were injected in the facet joint ipsilaterally to the needle approach. Among the 20 patients who received MRI, all (100%) had central stenosis and 15 patients (75%) had severe stenosis. Conclusions: When the operator performs an interlaminar ESI on patients with central spinal stenosis, the contrast pattern on the fluoroscopy during interlaminar ESI should be carefully examined to distinguish between the epidural space and facet joint.

  • The purpose of this study was to analyze the kinematics variables of upper limb joint during forehand stroke by swings patterns. Eight high school tennis players were chosen for the study, who have never been injured for last six months, in Busan. They performed horizontal swing and vertical swing that it was done each five consecutive trial in the condition of square, semi-open and open stance. It was filmed by 6 video camera and used with 3-dimensional motion analyzer system. The following kinematic variables were analyzed in relation to angle of joint(shoulder, elbow and wrist joint). The conclusion were as follow: 1. The angle of right shoulder joint represented all event that both swing were shown similar pattern in swing type and stance pattern. 2. All event in the angle of elbow joint had consistent with that except E2, horizontal and vertical swings in square stance. 3. All event in the angle of wrist joint was show to similar pattern except E2, horizontal and vertical swing in open stance.

  • In this paper, we present the new method for joint aspect inspecting system. We use the image processing and laser maker for light source. We can find the matrial joint status through processing the line pattern which is made by laser maker. To get the line pattern, in first, we did the preprocess of threshold. If the shape of line had over two segments, then the joint status is abnormal. We show our system efficency by experiment on tire facility.

The effects of elbow joint angle and resistance point on muscle activation of the contralateral shoulder muscles while performing the ulnar thrust PNF pattern exercise

  • PURPOSE: This study researched the effects of the changes of elbow joint angle and of arm position in PNF pattern on muscle activation of the contralateral shoulder muscles while performing PNF pattern exercise. METHODS: The research subjects were 16 male physical therapists who had no neuromuscular or neurological disorders. To measure the muscle activation of the contralateral shoulder muscles, EMG electrodes were attached to the muscle valley of the middle and posterior areas of the deltoid and triceps muscles of the arm. Muscle activation while performing the ulnar thrust PNF pattern exercise was measured with the elbow joint positioned at angles of $30^{\circ}$, $45^{\circ}$, and $60^{\circ}$. Resistance points were at the initial, middle, and end ranges of PNF pattern exercise. RESULTS: Muscle activation of the middle and posterior portions of the deltoid muscle increased significantly according to the changes of elbow joint angle. In each resistance point the middle range was significantly higher than at other points. A significant difference on muscle activation was demonstrated throughout each range depending on the type of muscle. Muscle activation of the middle and posterior portions of the deltoid muscle was higher than muscle activation of the triceps. CONCLUSION: The results of this study demonstrate that the PNF pattern exercising method used in this study is a selective exercising method focusing on the deltoid muscle over the triceps muscle. In order to increase the muscle strength to the maximum level, it is necessary to provide the maximum level of resistance in the middle range of the elbow joint.

Numerical simulation of hydraulic fracturing in circular holes

  • For investigating the effect of the pre-existing joints on the initiation pattern of hydraulic fractures, the numerical simulation of circular holes under internal hydraulic pressure with a different pattern of the joint distributions are conducted by using a finite element code, FRANC2D. The pattern of hydraulic fracturing initiation are scrutinized with changing the values of the joint length, joint offset angle. The hydraulic pressures with 70% of the peak value of borehole wall breakout pressure are applied at the similar models. The simulation results suggest that the opening-mode fracture initiated from the joint tip and propagated toward the borehole for critical values of ligament angle and joint offset angle. At these critical values, the crack grow length is influenced by joint ligament length. When the ligament length is less than 3 times the borehole diameter the crack growth length increases monotonically with increasing joint length. The opening-mode fracture disappears at the joint tip as the ligament length increases.

Binary Nonlinear Joint Transform Correlator with Sinusoidal Iterative Filter in Spectrum Domain

  • The joint transform correlator (JTC) has been the best known technique for pattern recognition and identification. This paper proposes a new technique of fringe adjustment by adopting a sinusoidal amplitude-modulated iterative filter convolved with an interference fringe pattern in the joint power spectrum (JPS) domain. The comparison of our new technique and other techniques is presented to show that the newly proposed technique can successfully improve both the correlation peaks and the peak signal-to-noise ratio (PSNR). Simulated results of enhanced interference fringes are also presented.
